    1Vision Foundation Models Pretrained on Skin Lesion Images Can Classify Oral Lesions Too!

    Recognizing the critical limitations imposed by data scarcity in Oral Squamous Cell Carcinoma (OSCC) detection, this work demonstrates how the domain transferability of Vision Foundation Models (VFMs) provides a powerful solution. We present the first work to employ a VFM, Google's Derm Foundation, pretrained on dermatology images, as a frozen feature extractor for OSCC detection. By training only a shallow Multi-Layer Perceptron (MLP) classifier, we achieve high-performance with minimal computational load. This lightweight pipeline, validated across multiple datasets, offers robust performance comparable to fully trained Convolutional Neural Networks (CNNs), establishing an efficient paradigm for early screening in resource-constrained environments. An additional contribution of the work is the combination of multimodal data (image and non-imaging) into an integrated pipeline with improved results.

    2Outcomes of Conservative and Surgical Management of Trigger Thumb in Children Up to Five Years at a Tertiary Care Center in Kolkata

    INTRODUCTION:Pediatric trigger thumb is a developmental condition characterized by flexion deformity of the interphalangeal joint due to constriction at the A1 pulley. Although spontaneous resolution has been reported, optimal treatment timing remains controversial. This study aimed to compare outcomes between conservative and surgical management in children aged up to five years. METHODOLOGY:A prospective analytical cohort study was conducted at a tertiary care center in Kolkata over 18 months. Fifty-eight children with clinically diagnosed trigger thumb were enrolled. All participants initially underwent six months of conservative observation. Those who did not improve underwent open A1 pulley release. The primary outcome was complete resolution of deformity. Secondary outcomes included degree of improvement, recurrence, complications, and time to resolution. Statistical analyses included the chi-square test, the Mann-Whitney U test, the Cox proportional hazards model, and multivariable logistic regression. RESULTS:Nineteen children (32.8%) improved with conservative management, whereas 39 (67.2%) required surgery. Surgical intervention achieved 100% resolution with no recurrence or major complications. Mean improvement in deformity was significantly greater in the surgical group (50.8° ± 5.8°) compared with the conservative group (14.5° ± 3.7°; p < 0.001). The relative risk of complete resolution with surgery was 3.05 (95% confidence interval (CI): 2.05-4.55), with an absolute risk reduction of 67.2% and a number needed to treat of approximately two. Cox analysis demonstrated faster recovery with surgery (HR: 4.62; 95% CI: 2.71-7.88; p < 0.001). An initial deformity of >20° independently predicted conservative failure. CONCLUSION:Surgical release provides significantly superior and faster resolution compared to conservative management, particularly in children presenting with deformity greater than 20°.

    3Histological Approach and Differentiation of Crohn’s Disease and Gastrointestinal Tuberculosis: Recommendations from the Joint IAPM-ISG-CCFI Working Group

    BACKGROUND:The most common causes for ulcero-stricturing diseases of the ileo-cecal region and colon in Southeast Asia are Crohn's disease (CD) and gastrointestinal tuberculosis (GI TB). Diagnosing these conditions is challenging because they share several clinical, endoscopic, radiological and histological features on mucosal biopsies. Therefore, there is a need to standardize the sampling, processing and interpretation of mucosal biopsies to aid clinical decision-making. METHODS:Recognizing this challenge, core subject experts nominated by the Indian Association of Pathologists and Microbiologists (IAPM), the Indian Society of Gastroenterology (ISG) and the Colitis and Crohn's Foundation, India (CCFI), collaborated to formulate comprehensive recommendations for pathologists regarding optimal biopsy protocols, histological interpretation and reporting for differentiating CD from GI TB. A structured Delphi process was followed. RESULTS:The recommendations from the core domain expert groups were based on discussions, brainstorming sessions and extensive literature reviews conducted over three virtual group meetings, multiple online voting sessions and one physical meeting involving all experts. This document is expected to standardize the practice of luminal gastroenterology by providing a ready reference for budding specialists and pathologists, thereby promoting uniformity in practice. CONCLUSION:These multi-society, evidence-based and practically applicable recommendations developed by core subject experts aim to promote uniformity and confidence in pathology reports, facilitate timely patient management and prevent complications arising from erroneous treatment.

    4Knowledge, Attitudes and Practices of Palliative Care among Postgraduate Medical Students: A Multicentre Cross-sectional Study from Kolhapur, India

    Introduction: Palliative care is a multidisciplinary approach designed to enhance the quality of life of patients with life-limiting illnesses. Despite its growing importance, palliative care remains insufficiently represented within Indian postgraduate medical training, leading to gaps in knowledge, attitudes, and clinical practice. Aim: To assess the Knowledge, Attitude, and Practice (KAP) of postgraduate medical students regarding palliative care in selected medical Institutions across Maharashtra, Karnataka, and Goa. Materials and Methods: This multicentre, cross-sectional observational study was conducted between August 2021 and January 2022 in tertiary care medical colleges across Maharashtra, Karnataka, and Goa, India A structured, validated online questionnaire was distributed to 1st-, 2nd-, and 3rd-year postgraduate students using Google Forms and Survey Monkey. The survey captured demographic characteristics, levels of KAP, and previous exposure or training in palliative care. Data were analysed using EpiInfo version 7.2. Descriptive statistics summarised variables; Chi-square and Fisher’s exact tests assessed associations between categorical variables; Student’s t-test and one-way Analysis of Variance (ANOVA) were applied to continuous variables. Principal Component Analysis (PCA) and K-means clustering were used to identify learner profiles. A p-value <0.05 was considered statistically significant. Results: Of 500 invited students, 345 (69.0%) responded, and analysis was performed on these respondents (n=345). Most participants were aged 24-35 years, 87 (25.22%), with balanced gender distribution males, 179 (51.87%), females, 166 (48.13%). Internal medicine, dental sciences, and orthopaedics were the most represented specialties. Knowledge levels were predominantly average in 182 students (52.75%), attitudes were largely neutral in 194 students (56.23%), and practice levels were moderate in 168 (48.7%) of respondents. Misconceptions persisted, particularly regarding opioid use and the World Health Organisation (WHO) analgesic ladder. Practical exposure was limited, with only 46.96% having participated in palliative rounds. PCA revealed three distinct learner clusters varying in readiness and confidence. Conclusion: Although foundational knowledge was adequate, gaps in attitude, misconceptions, and limited clinical exposure highlight the need for structured, competency-based palliative-care training within postgraduate curricula.
