ABSTRACT Accurate identification of P waves on electrocardiogram (ECG) is essential in supraventricular tachycardias because U waves may resemble retrograde P waves, producing a pseudo–long RP interval and hiding typical atrioventricular nodal reentrant tachycardia (AVNRT). A 40‐year‐old woman presented with regular narrow‐complex tachycardia with a positive deflection immediately following the T wave, creating the impression of a long RP tachycardia. The electrophysiological study revealed a short VA interval with an H‐A‐V activation sequence, characteristic of typical slow‐fast AVNRT. This case emphasizes the diagnostic challenge posed by U waves at higher heart rates, which can obscure the true arrhythmia mechanism.
This case study examines the implementation of open book accounting (OBA) within a medium sized construction company, operating in a strained relationship with a strategic client. The purpose of the research is to evaluate how financial transparency can enhance collaboration, reduce costs, and strengthen trust between partners. The adopted methodology follows a six stage qualitative analysis: initial diagnosis, definition of the transparency framework, establishment of governance mechanisms, technological implementation, piloting on a real project, and evaluation of outcomes. The results indicate that OBA significantly reduced contractual disputes, improved the accuracy of cost estimates, accelerated decision making processes, and reinforced the supplier–client relationship. The study confirms existing literature suggesting that OBA can generate competitive advantages by increasing trust, optimizing processes, and fostering innovation. The conclusions highlight the critical role of governance, digitalization, and organizational culture in the effective implementation of OBA.
Philosophical discussions are becoming increasingly important for understanding the impact of computer languages on the future evolution of our civilization. Much time has passed since software became an integral part of the environment in which we live. Through many observations over decades, clear conclusions have been drawn regarding software applications, which could not have been initially foreseen.
This paper examines the involvement of young people in the sustainable transformation of Romanian rural areas, using Ariniș commune, Maramureș County, as a case study. The originality of the research lies in its focus on youth participation as a key factor linking education, local initiative, entrepreneurship and com-munity development in a rural Romanian context affected by demographic decline and limited economic opportunities. The study is based on an exploratory quantitative approach, using data collected through an online questi-onnaire applied between April and May 2025 to 62 respondents aged 15–40 with current or previous resi-dence in Ariniș commune. The data were analyzed through descriptive statistics in order to identify per-ceptions regarding education, vocational training needs, youth involvement, local development opportuni-ties and perceived barriers to community progress. The results show that education is perceived as a strategic factor for rural development, although respon-dents indicate shortcomings in the capacity of the local school to support professional integration. The findings also reveal interest in vocational training connected to agriculture, administration, business, per-sonal care services and technical fields. Young people are seen as potential contributors to local develop-ment, particularly through entrepreneurship, cultural activities, ecological initiatives and participation in community decision-making. However, their involvement is limited by the lack of administrative vision, financial constraints, demographic aging and insufficient economic initiative. The scientific contribution of the paper consists in highlighting the relationship between youth retention, education, local participation and rural transformation. From a practical perspective, the study supports the need for youth-oriented rural policies focused on vocational training, entrepreneurship support, participa-tory governance and the creation of local opportunities capable of reducing migration and strengthening community resilience.
A 58-year-old woman with recurrent paroxysmal atrial fibrillation and a history of cryoablation 3 years prior underwent an electrophysiological study for an irregular narrow complex tachyarrhythmia. The electrocardiogram showed positive P’ waves in all leads except aVR, with the P’ waves separated by large isoelectric intervals, indicating focal atrial tachycardia rather than atypical atrial flutter. High-resolution 3D mapping revealed a focal atrial tachycardia originating from the postero-septal part of the superior vena cava (in close proximity to the right superior pulmonary vein). Three applications at the superior vena cava level terminated the tachycardia and restored sinus rhythm. There were no recurrences. This case underscores the need for high-resolution electrophysiological mapping to identify the arrhythmia origin, especially in patients with prior procedures.
