Magnetic resonance image-guided adaptive brachytherapy (MR-IGABT) is the standard of care for definitive radiochemotherapy in locally advanced cervical cancer, but radiotherapy departments with sufficient case volume and unrestricted access to MRI capacity remain limited. This study evaluated the feasibility and outcomes of a nationwide Austrian model combining decentralized external-beam radiotherapy (EBRT) with centralized MR-IGABT. This retrospective cohort study included patients with FIGO IB–IVA cervical cancer treated between 2018 and 2022 who received EBRT ± concomitant chemotherapy at external Austrian institutions and were subsequently referred to the Medical University of Vienna for brachytherapy. All patients received four fractions of MR-IGABT with MRI-based target delineation according to GEC-ESTRO recommendations. Primary endpoints were local control (LC) and overall survival (OS). Secondary endpoints included pelvic lymph node control (PLNC), recurrence-free survival (RFS), and grade ≥ 3 morbidity requiring medical intervention. Radiotherapy has traditionally been guided by the principle that uniform delivery of a tumoricidal dose across the entire target volume maximizes local control. However, this paradigm becomes increasingly constrained in the setting of large, bulky or anatomically complex tumours, in which non-malignant tissue tolerances often preclude homogeneous dose escalation. Spatially fractionated radiotherapy (SFRT) has emerged as a complementary approach that introduces intentional intratumoural dose heterogeneity as an alternative therapeutic strategy when uniform irradiation is not feasible. SFRT involves the delivery of radiation as high-dose 'peaks' interspersed with lower-dose 'valleys', creating spatial domains that combine focal tumoricidal exposures with partial preservation of vascular, stromal and immune-related functions within the tumour microenvironment. Preclinical investigations and early clinical studies suggest that such architectures might be associated with rapid volumetric tumour regression, acceptable toxicity profiles and modulation of the tumour immune microenvironment, although the underlying mechanisms, generalizability and durability of these effects remain incompletely defined. In this Review, we summarize the conceptual foundations, biological hypotheses and clinical activity of SFRT, spanning macroscopic approaches such as GRID and lattice radiotherapy, biology-guided strategies and submillimetric implementations using minibeams and microbeams. We also highlight key translational and methodological limitations and discuss various challenges relating to dosimetry, biologically meaningful response assessments, patient selection and multicentre reproducibility.
Population-level data on fourth column fractures and prognostic implications remain lacking. This study aimed to characterise the epidemiology of thoracic vertebral fractures in Germany, including the prevalence of fourth column injuries and geriatric patients, comorbidity burden and the in-hospital mortality across distinct fracture populations. A Nationwide cross-sectional administrative inpatient data analysis of all adult thoracic vertebral fracture cases (ICD-10-GM S22.0x/S22.1) from the 2024 German Diagnosis Related Groups database was conducted across three groups: thoracic spine group (n = 27,315), geriatric subgroup (≥ 75 years; n = 16,276), and fourth column group with concomitant sternal fracture (n = 499). Varianten des Tousled-like kinase 2(TLK2)-Gens sind selten und vorwiegend mit neuroentwicklungsbedingten Phänotypen assoziiert. Bei medizinisch komplexen Erwachsenen können solche genomischen Befunde die klinische Aufmerksamkeit auf somatische Ursachen lenken und dadurch diagnostische sowie therapeutische Entscheidungen fehlleiten. Die Borderline-Persönlichkeitsstörung (BPS) kann hingegen die Symptomatik, die Inanspruchnahme und die Teamdynamik dominieren. Wir berichten über eine Frau in ihren Zwanzigern mit ausgeprägter abdomineller Operationsanamnese (mehrfache Dünndarmresektionen, Ileostoma, PEG (perkutane endoskopische Gastrostomie-Sonde), hoher Opioidexposition, parenteraler Ernährung sowie rezidivierenden Katheterinfektionen und Thrombosen. Eine heterozygote TLK2-Frameshift-Variante (c.362del, p.Pro121Argfs*4) war dokumentiert, doch wiederholte Beurteilungen ergaben keinen fortschreitenden, lebenslimitierenden organischen Prozess, der den hohen klinischen Versorgungsbedarf erklären würde. Merkmale einer BPS (mit intermittierenden artifiziellen Elementen) bestimmten das Geschehen: wiederholte Vorstellungen, Spaltungsphänomene und die Ablehnung psychiatrischer Aufnahmen. Mehrere Fachdisziplinen zogen sich schließlich zurück, sodass eine palliativmedizinische Zuweisung als „letzter Ausweg“ erfolgte. Das Palliativteam legte den Schwerpunkt auf klare Indikationen, Abgrenzungen, koordinierte Kommunikation und strukturierte Analgesie (kontinuierliche subkutane Hydromorphoninfusion mit niedrig dosiertem Ketamin über eine ambulante Pumpe mit Sperrzeiten) und verzichtete auf nicht nutzenbringende Abklärungen. Zwischen 2022 und 2025 kam es zu wiederholten Aufnahmen wegen Schmerzen, Stomablutungen (endoskopische Hämostase im November 2024), katheterassoziierten Infektionen (mehrfache PICC (Peripherally Inserted Central Catheter-/Port-Entfernungen; Pneumothorax nach Hickman-Versuch im März 2025) sowie Kurzaufnahmen bei dissoziativen Episoden (Juli 2025). In diesem Kontext fungierte der TLK2-Befund als „red herring“. Wenn psychiatrische Faktoren das Versorgungsgeschehen bestimmen, sind eine frühe Zuweisung zu psychiatrisch geführten Versorgungsmodellen, klare Kriterien zur Abgrenzung palliativ- versus psychiatrisch-indizierter Betreuung und grenzschützende Behandlungsprotokolle entscheidend, um moralische Belastung und Ressourcenverbrauch zu begrenzen.
