
Zusammenfassung Die vorliegende Übersicht beleuchtet die Entwicklung und klinische Anwendung von skelettaler Verankerung, insbesondere von palatinalen Mini-Implantaten, zur unsichtbaren Kieferorthopädie im Erwachsenenalter. Die palatinale Insertion und digitale Workflows optimieren Präzision und Effizienz. Der Fokus liegt auf präprothetischen Maßnahmen wie Molarenintrusion und -aufrichtung sowie deren Kombination mit Alignern, um biomechanische Limitationen bei translationalen Bewegungen in einem Hybridprotokoll zu überwinden. Diese Behandlungen erlauben, dass komplexe Korrekturen ästhetisch und compliance-unabhängig realisiert werden können. Jedoch sollten individuell die Vorteile, wie geringe Invasivität und Unsichtbarkeit, gegen die Nachteile, wie Zeitaufwand oder Rezidivrisiko, abgewogen werden.
Zusammenfassung Mit zunehmendem Alter verändert sich das orale Mikrobiom durch Abnahme protektiver, kommensaler Mikroorganismen und Zunahme von Pathogenen (Risiko für orale Dysbiosen). Auch das Immunsystem altert, was sich durch das sog. „Inflammaging“ (Vorhandensein geringgradiger, chronischer „steriler“ Inflammationen) und „Immunseneszenz“ (progressive Abnahme der Immunfunktion) erklären lässt. Destruktive Parodontalerkrankungen entstehen nur dann bei Vorliegen oraler Dysbiosen, wenn die Immunreaktion dysreguliert ist. Mikrobiologische und immunologische Alterungsprozesse sind daher ein Risikofaktor für Parodontalerkrankungen. Die orthodontische Zahnbewegung ist ein komplexer Prozess, in dem durch die sog. Mechanotransduktion (Umwandlung orthodontischer, mechanischer Kräfte in biologische Signale) sterile Inflammationsgeschehen und kontrollierte Knochenumbauprozesse ausgelöst werden. In diesem Artikel werden die Zusammenhänge zwischen oralem Mikrobiom, (Osteo-)Immunologie und orthodontischer Zahnbewegung unter Berücksichtigung erwachsener Patienten mit mikrobiologischen und immunologischen Herausforderungen aufgezeigt. Zudem sollen leitlinienkonforme Behandlungsempfehlungen für verschiedene Therapiezeitpunkte bei erwachsenen Patienten mit parodontalen Erkrankungen vorgestellt werden. Hierbei sollen spezielle Evidenz-basierte, biomechanische Therapiekonzepte für erwachsene Patienten mit Proklination, Elongation und Auffächerung von Frontzähnen dargestellt werden.
Abstract Das obstruktive Schlafapnoe-Syndrom (OSAS) stellt die häufigste schlafbezogene Atmungsstörung dar und ist mit einer erheblichen kardiovaskulären, metabolischen und gesundheitsökonomischen Belastung assoziiert. Die wachsende Evidenz zu einer Vielzahl OSAS-assoziierter Komorbiditäten unterstreicht die zunehmende klinische Relevanz der Erkrankung. Eine frühzeitige Diagnostik und eine adäquate, individuell angepasste Therapie sind entscheidend, um das Risiko langfristiger Folgeerkrankungen zu reduzieren. Die chirurgische Therapie des maxillomandibulären Rotationsadvancement (MMA) stellt ein etabliertes Verfahren mit kurativer Intention dar. Durch die skelettale Vorverlagerung von Maxilla und Mandibula wird der posterior airway space erweitert und der Tonus der pharyngealen Muskulatur erhöht. Bereits mehrere multizentrische Studien des letzten Jahrzehnts zeigen eine signifikante Reduktion des Apnoe-Hypopnoe-Index (AHI) und eine mit der CPAP-Therapie vergleichbare Effektivität. Auswertungen an unserer Klinik zeigen, dass das MMA ebenfalls bei normognathen Patient*innen zu einem Behandlungserfolg führt. Digitale Planungsprozesse und patientenspezifische Implantate ermöglichen eine höhere chirurgische Präzision, verkürzte Operationszeiten und eine verbesserte Standardisierung der Operation. Daten aus der Berliner OSAS-MMA-Kohorte zeigen eine hohe Genauigkeit der skelettalen Verlagerung, sowie eine signifikante Verbesserung funktioneller und ästhetischer Ergebnisse. Bereits eine mittlere anteriore Verlagerung von etwa 8.63mm (SD±1.12mm) führt zu einer deutlichen AHI-Reduktion von im Mittel 71,2% des präoperativen Ausgangswerts. Moderne softwarebasierte Strömungsanalysen auf Basis von DVT-Datensätzen erlauben zusätzlich die Visualisierung statischer und dynamischer Atemwegsparameter. Indices wie der Pharyngeal Resistance Index (PRI) ermöglichen eine weitergehende funktionelle Einordnung der Obstruktion über klassische Schlaflaborparameter hinaus. Das maxillomandibuläre Advancement stellt auch bei normognathen Patient*innen mit obstruktiver Schlafapnoe eine hocheffektive therapeutische Option dar. Durch moderne virtuelle Operationsplanung und die Verwendung patientenspezifischer Implantate kann die Übertragungsgenauigkeit erhöht, die Operationsdauer reduziert und die intraoperative Sicherheit verbessert werden. Bei adäquat gewählter Verlagerungsstrecke lassen sich zudem eine stabile knöcherne Heilung sowie ästhetisch ansprechende Ergebnisse erzielen. Softwarebasierte Analysen unter Einbeziehung aktueller DVT-Bildgebung und strömungsdynamischer Parameter ermöglichen darüber hinaus eine präzisere Lokalisation und Quantifizierung der Atemwegsobstruktion und tragen so zu einem vertieften Verständnis der zugrunde liegenden Pathophysiologie bei.
Zusammenfassung Die vorliegende Literaturübersicht untersucht den Einfluss einer unbehandelten anterioren Diskusverlagerung auf das im Wachstum befindliche Kiefergelenk sowie die Entwicklung der Gesichtsschädelmorphologie. Hierzu wurde anhand der PICO-Kriterien eine systematische Literaturrecherche in der Datenbasis PubMed durchgeführt. Diese ergab 56 Treffer für den Zeitraum 2010–2025, welche durch Deduplizierung und Screening anhand definierter Ein- und Ausschlusskriterien auf eine Anzahl von 15 Studien eingegrenzt werden konnten. Die primären Endpunkte waren die anatomische Höhe sowie die Lage des Kondylus in der Fossa mandibularis im MRT oder DVT. Die sekundären Endpunkte stellten Parameter zur Gesichtsschädelmorphologie in der Fernröntgenseiten- bzw. Fernröntgenfrontalaufnahme dar. Während es unter einer anterioren Diskusverlagerung mit Reposition zu einem verlangsamten Kondyluswachstum kam, fand beim Vorliegen einer anterioren Diskusverlagerung ohne Reposition sowie einer nicht genauer spezifizierten anterioren Diskusverlagerung ein kondylärer Höhenverlust statt. Folglich führte eine unbehandelte unilaterale anteriore Diskusverlagerung zu einer Verstärkung der Mittenabweichung des Kinns und eine unbehandelte uni- oder bilaterale anteriore Diskusverlagerung zur Zunahme einer mandibulären Retrognathie sowie eines vertikalen Wachstumsmusters.
Zusammenfassung Die demografische Entwicklung und der zunehmende Zahnerhalt bis ins hohe Lebensalter führen dazu, dass kieferorthopädische Behandlungen zunehmend auch bei Patient*innen mit Parodontalerkrankungen durchgeführt werden. Insbesondere die Parodontitis stellt aufgrund von klinischem Attachmentverlust und alveolärem Knochenabbau eine besondere Herausforderung für Planung, Biomechanik und Langzeitstabilität kieferorthopädischer Therapien dar. Ziel dieses Beitrags ist es, ein klinisch orientiertes, interdisziplinäres Steuerungskonzept für die Integration kieferorthopädischer Maßnahmen in die Parodontitistherapie darzustellen. Im Fokus stehen parodontale Risikoprofile auf Grundlage der Staging- und Grading-Klassifikation, biomechanische Besonderheiten der Zahnbewegung im reduzierten Parodont sowie die leitlinienbasierte Sequenzierung der Therapie. Eine angepasste Biomechanik und die enge interdisziplinäre Zusammenarbeit sind entscheidend für sichere und langfristig stabile Behandlungsergebnisse.

The aim of the present review of the literature was to evaluate the impact of an untreated anterior disc displacement on the growing temporomandibular joint as well as the development of craniofacial morphology. To answer the research questions a systematic research of the literature following the PICO criteria was conducted via the database PubMed. This led to 56 results for the span 2010-2025. Through deduplication and screening according to defined inclusion and exclusion criteria the results were narrowed down to 15. The primary endpoints were the anatomical condylar height and the condyle's position in the mandibular fossa in MRI or CBCT. The secondary endpoints were cephalometric parameters in the frontal and lateral view. While an anterior disc displacement with reduction resulted in reduced condylar growth, an anterior disc displacement without reduction and an anterior disc displacement that was not further specified led to condylar resorption. Consequently, an untreated unilateral anterior disc displacement was followed by an increase in the deviation of the chin and an untreated uni- or bilateral anterior disc displacement by an aggravation of mandibular retrognathism and a vertical growth pattern.
Demographic changes and the increasing preservation of natural dentition into advanced age have led to a growing number of orthodontic treatments being performed in patients with periodontal diseases. In particular, periodontitis poses a significant challenge for the planning, biomechanics, and long-term stability of orthodontic therapies due to clinical attachment loss and alveolar bone resorption. The aim of this article is to present a clinically oriented, interdisciplinary management concept for integrating orthodontic interventions into periodontal therapy. The focus is placed on periodontal risk profiles based on the staging and grading classification, biomechanical characteristics of tooth movement in the reduced periodontium, and guideline-based sequencing of therapy. Adapted orthodontic biomechanics and close interdisciplinary collaboration are essential for achieving safe and long-term stable treatment outcomes.
The aim of this article is to describe a digital in-office workflow for planning and fabricating a customized lingual fixed appliance. Based on a digital model, individualized setup creation, virtual bracket positioning, and the design of transfer trays for indirect bonding are presented. In addition, the export of an ideal archwire template for individualized archwire adjustment is described. Key workflow steps and clinically relevant design and process parameters are outlined.
Transverse maxillary expansion in adolescents is commonly performed conservatively using rapid maxillary expansion (RME). However, due to progressive obliteration of the midpalatal suture, RME is often performed with surgical assistance after growth is complete. This case report describes a 15.8-year-old patient who had passed his pubertal growth peak but still underwent successful conservative RME. Despite the unfavorable initial conditions, a clearly defined activation protocol produced a midline diastema within one week, after which the maxilla was transversely expanded to the desired width. Following a six-month retention phase, treatment was continued with aligner therapy. This case demonstrates that conservative RME without surgery remains possible even in later adolescence, provided that diagnosis, indication, and activation protocol are carefully selected.
Background The use of digital workflows allows the production of many orthodontic appliances to be modernized by making it more efficient, precise, and patient-friendly. This also applies to retention appliances. The presented workflow, consisting of intraoral scanning, CAD design and 3D printing, replaces traditional analogue steps nearly entirely and enables a model-free in-office fabrication of retention splints. Procedure Starting from the scan, the digital model is processed in a CAD software and the retention splint is individually designed. Parameters such as minimum thickness, spacer settings and digital blockout play an essential role in this step. The following 3D printing process enables material-efficient production, including the ability to fabricate multiple splints simultaneously. Particular attention is given to the orientation and placement of the splints on the build platform, as this significantly influences print quality, printing time and post-processing effort. A standardized post-processing protocol involving washing and UV curing ensures that the splints achieve their final mechanical and surface quality. Relevance The digital workflow substantially reduces manual labour and the required hands-on time. At the same time, patients benefit from a more comfortable and faster treatment experience. The retention splints can be fabricated with high accuracy and are fully comparable to conventionally vacuum-formed appliances. Thus, direct 3D printing represents a modern, practical alternative to traditional thermoforming techniques and can be easily integrated into everyday clinical practice.
Systemic diseases such as cardiovascular diseases, chronic kidney disease, and liver cirrhosis are often associated with chronic inflammatory responses in the body, which also affect the oral cavity. Periodontal diseases, particularly periodontitis, are frequent manifestations of this systemic inflammation and are closely linked to these conditions. A disrupted host response and dysbiosis of the oral microbiota are key mechanisms driving systemic inflammation, which in turn promotes the progression of cardiovascular, renal, and hepatic diseases.In orthodontics, it is crucial not to view periodontal diseases in isolation. These conditions can serve as early indicators of systemic diseases and should be routinely assessed as part of patient care. This is particularly important for patients with anorexia nervosa, who often experience periodontal changes due to impaired nutrition and micronutrient deficiencies. Research has shown that periodontal therapies not only improve oral health but also significantly reduce systemic inflammatory markers. Given the bidirectional relationship between periodontal health and systemic diseases, early detection and treatment of periodontal changes should be integrated into the preventive dental care provided in orthodontics.
Background With the increasing proportion of adult orthodontic patients, a growing number of individuals seek consultation after completion of orthodontic treatment due to persistent or recurrent esthetic concerns in the anterior tooth segment. These concerns are frequently related to treatment-associated relapses or incomplete three-dimensional correction, despite an otherwise successful initial therapy. The aim of this article is to present and biomechanically evaluate minimally invasive orthodontic correction strategies for esthetically relevant anterior segment relapses in adult patients. Materials Three clinical cases illustrating different types of anterior segment relapse are presented: (a) open gingival embrasures ("black triangles"), (b) isolated torque discrepancies of upper anterior teeth, and (c) isolated torque discrepancies of lower anterior teeth. Corrections were performed using aligner-based partial treatment with power ridges and attachments and reduced multibracket segmental mechanics. Treatment planning was based on an etiological analysis with particular emphasis on contact point position, root angulation, and periodontal conditions. Results In all cases, substantial esthetic improvement was achieved with limited appliance design and short treatment duration. Interproximal enamel reduction combined with coronal relocation and extension of the interproximal contact area proved particularly effective for the elimination of open gingival embrasures. Rotational and torque discrepancies were successfully corrected using localized, biomechanically controlled approaches without inducing periodontal complications. Conclusions Minimally invasive orthodontic corrections represent an effective and patient-oriented treatment option for esthetically motivated anterior segment relapses in adult patients when carefully indicated. These approaches constitute a valuable adjunct to the orthodontic treatment spectrum but do not replace comprehensive retreatment in cases of complex malocclusion.
Background Combined orthodontic and orthognathicthera-py represents a major and planning-sensitive subfield of adult orthodontictreatment. Therefore preoperative dentoalveolar decompensation and precise orthognathic planning are crucial for achieving a successful treatment outcome. Case Presentations These case reports describe a fully digital workflow for combined orthodontic and orthognathic treatment in two adult patients. Preoperative dentoalveolar decompensat ion was achieved using a fully individualized lingual appliance (WIN, DW Lingual Systems, Bad Essen, Germany), whereas orthognathic surgery was performed based on a three-dimensional virtual surgical planning software (Mimics Enlight, Materialise NV, Leuven, Belgium). Conclusion In both patients, the tooth positions defined in the digital setup were clinically achieved, and the preoperatively planned skeletal movements precisely matched the postoperative jaw position. The described workflow demonstrates a high level of concordance between virtual planning and clinical outcome and highlights the potential of fully digital concepts in the management of dentofacial deformities in adults.
Die Okklusionsebene steht im engen Zusammenhang mit der Entwicklung einer Malokklusion und dient als wichtiger Parameter bei der Erstellung kieferorthopädischer Behandlungspläne. Während eine tendenziell steile Okklusionsebene bei der Klasse II-Malokklusion beobachtet wird, ist die Klasse III-Malokklusion durch eine flache Okklusionsebene gekennzeichnet. Der Therapieansatz soll daher so festgelegt werden, dass durch gezielte Modifikation und Kontrolle der Okklusionsebene eine Autorotation der Mandibula möglich ist und das mandibuläre Wachstum in eine günstige Richtung gelenkt wird. In Fällen schwerwiegender skelettaler Dysgnathie stellt die kombinierte kieferorthopädisch-kieferchirurgische Behandlung mit gleichzeitiger Rotation der Okklusionsebene ein effektives Verfahren dar, um ein langfristig stabiles Behandlungsergebnis zu erzielen.
