
Objective To explore autophagy-related genes(ARGs)in lung adenocarcinoma and con-struct a prognostic model for lung adenocarcinoma based on ARGs.Methods RNA high-throughput tran-scriptome data of lung adenocarcinoma were obtained from The Cancer Genome Atlas(TCGA)database and HADb database to acquire ARGs.A prognostic model for lung adenocarcinoma was constructed and validated based on differentially expressed ARGs,followed by the construction of column line graphs and calibration curves to explore the clinical application value of the model.Gene ontology and Kyoto Encyclopedia of Genes and Genomes enrichment analyses were performed on differentially expressed ARGs.Lasso regression analysis was conducted on differentially expressed ARGs with prognostic significance to construct the prognostic model for lung adenocarcinoma.Kaplan-Meier survival curves were plotted.Results A total of 31 differentially expressed ARGs were obtained,and 10 differentially expressed ARGs with prognostic signifi-cance were selected.Patients in the high-risk group were significantly associated with poorer overall survival,with statistical significance(P<0.05).T stage,N stage and risk score was an independent prognostic factor for patients with lung adenocarcinoma.The global consistency of the calibration curve column line graph was 0.710,indi-cating a high level of agreement between the model's predicted results and actual outcomes.Conclusion The risk model constructed based on differentially expressed ARGs can serve as a prognostic feature for patients with lung ade-nocarcinoma or provide a reference for individualized treatment for patients with lung adenocarcinoma.
Objective To explore the effect of health education and nursing based on information-motiva-tion-behavior(IMB)model in inpatients with cerebral hemorrhage.Methods A total of 86 inpatients with intracerebral hemorrhage treated in Department of Neurosurgery,Hanzhong Central Hospital from January 2020 to December 2022 were selected as the research objects,and they were divided into a control group and a study group according to random number table method,with 43 cases in each group.Patients in the control group received routine health education and nursing,and those in the study group received health education and nursing based on IMB model.Before discharge,the compliance of rehabilitation exercise and the scores of Fugl-Meyer Motor Function Assessment(FMA),Self-rating Anxiety Scale(SAS),and Self-rating Depression Scale(SDS)before and after nursing were compared between the two groups.Results The compliance of rehabilitation exercise in the study group was 97.67%,significantly higher than 79.07%in the control group(P<0.05).Before nursing,there were no statistically significant difference in FMA scores of upper and lower limbs between the two groups(P>0.05);after nursing,the FMA scores of upper limbs and lower limbs of the study group were(40.30±3.91)points and(20.55±2.69)points,which were significantly higher than(35.26±3.27)points and(16.71±2.67)points of the control group(P<0.05).Before nursing,there were no significant dif-ference in SAS and SDS scores between the two groups(P>0.05);after nursing,the SAS and SDS scores of the study group were(32.34±7.92)points and(33.28±6.83)points,which were significantly lower than(46.36±8.47)points and(46.36±7.54)points of the control group(P<0.05).Conclusion The application of health education and nursing based on IMB model in inpatients with cerebral hemorrhage can improve patients'compliance with rehabilitation exercise,speed up the recovery of limb function,and effectively improve their psychological state,which is worthy of clinical pro-motion and application.
Objective To study the application of intravoxel incoherent motion(IVIM)imaging and diffusion kurtosis imaging(DKI)in the grading of glioma.Methods The clinical data of 82 patients with brain glioma diagnosed by surgery and puncture pathology in Xianyang Hospital of Yan'an University from May 2022 to April 2023 were ana-lyzed retrospectively,including 12 cases of grade Ⅰ,26 cases of grade Ⅱ,25 cases of grade Ⅲ,and 19 cases of gradeⅣ.All patients underwent conventional MRI plain scan,IVIM,and DKI before operation.IVIM sequence applied 8 groups of b values:0 s/mm2,50 s/mm2,100 s/mm2,150 s/mm2,200 s/mm2,400 s/mm2,800 s/mm2,and 10 000 s/mm2.DKI adopts 30 directions,and b values were 0 s/mm2,1 000 s/mm2,and 2 000 s/mm2.The original data of IVIM and DKI are processed and analyzed by post-processing software,and combined with plain scan and enhanced images,the true dispersion coeffi-cient(D)diagram,perfusion fraction(f)diagram,and false dispersion coefficient(D*)diagram,as well as the parameters of average dispersion coefficient(MD),anisotropic fraction(FA),average dispersion kurtosis(MK),axial kurtosis(AK),and radial kurtosis(RK)are obtained.Receiver operating characteristic(ROC)curve of IVIM and DKI was drawn,and the area under the curve(AUC)was calculated.Results The D*values of tumor areas in patients with gradeⅠ,Ⅱ,Ⅲ,and Ⅳ gliomas were(2.03±0.52)×10-3 mm2/s,(3.32±0.68)×10-3 mm2/s,(4.63±0.82)×10-3 mm2/s,(7.15±0.61)×10-3 mm2/s,respec-tively;the MK values were 0.71±0.14,0.86±0.19,1.03±0.18,and 1.25±0.23,respectively;the higher the grade of glioma,the higher the level of D*and MK,with statistically significant difference(P<0.05).The AUC of D*value and MK value in differentiating high-grade gliomas were 0.840 and 0.821,respectively,indicating that both D*value and MK value had high diagnostic efficiency in differentiating high-grade gliomas.Conclusion There are significant differences in D*value and MK value of patients with different grades of glioma.The tumor of glioma can be accurately graded by IVIM and DKI scanning and related parameters,which can provide important reference for clinical diagnosis and prognosis evaluation.
Objective To investigate the incidence of primary aldosteronism(PA)in hypertensive patients with acute ischemic stroke as the first symptom in Yuhuan,Zhejiang Province,and to analyze its clinical characteristics.Methods A total of 150 hypertensive patients with acute ischemic stroke as the first symptom admitted to the Inpatient Department of the Second People's Hospital of Yuhuan from January 2018 to December 2022 were selected for PA screening.Non-recumbent position aldosterone and renin activities were detected from 8:00 to 10:00 in the morn-ing,and aldosterone renin ratio(ARR)was calculated.Patients with positive ARR were given saline load test,and patients positive in saline load test was confirmed as PA.Adrenal morphology was evaluated by CT scan in pa-tients with PA.Results Among the 150 patients,23(15.33%)were diagnosed with PA,8(34.78%)were combined with hypokalemia,4(17.39%)were diagnosed with adrenal adenoma by CT,13(56.52%)were diagnosed with bilateral adrenal hyperplasia,and 5(21.74%)were diagnosed with unilateral adrenal hyperplasia.There was no obvious adrenal CT lesion in 1 patient(4.35%).There were 20 patients(86.96%)with grade 3 hypertension and 3 patients(13.04%)with grade 2 hypertension in PA.Conclusion In Yuhuan area of Zhejiang Province,the prevalence rate of PA among hyper-tensive patients with acute ischemic stroke as the first symptom was 15.33%,the proportion of grade 3 hypertension in patients with PA was as high as 86.96%,and the proportion of hypokalemia was only 34.78%.The proportions of adeno-ma,bilateral adrenal hyperplasia and unilateral adrenal hyperplasia were 39.13%,34.78%,and 21.74%,respectively.
In recent years,many studies have found that the core antibody detection of hepatitis B virus is relat-ed to the response to antiviral drug treatment,the differentiation of different natural history periods of chronic hepatitis B,and Hepatitis B(HBV)reactivation after immunosuppressive treatment.It is expected to become an important refer-ence index for initiating antiviral therapy,predicting efficacy and judging prognosis,and has great clinical application po-tential.This paper summarizes the latest research progress and clinical significance of the detection of hepatitis B virus core antibody(anti-HBc),so as to provide some help for the early realization of accurate and individualized treatment of chronic HBV infected patients.
A woman with systemic lupus erythematosus developed an incision infection 10 days after cesare-an section and suffered late postpartum hemorrhage due to uterine artery pseudoaneurysm rupture.After conservative treatment,uterine artery embolization was performed and reached a good hemostatic effect.This case warns clinicians to pay close attention to maternal women with systemic lupus erythematosus.Clinicians should consider the possibili-ty of uterine artery pseudoaneurysm formation for patients with late postpartum hemorrhage,actively evaluate the risk of massive bleeding,and perform uterine artery embolization to stop bleeding.
Objective To study the fetal protection effect of phloroglucinol combined with ritodrine hydrochlo-ride in the treatment of threatened abortion over 20 weeks of pregnancy and its effect on serum progesterone(P),β-hu-man chorionic gonadotropin(β-HCG)and estradiol(E2)levels in pregnant women.Methods The clinical data of 86 pregnant women with threatened abortion over 20 weeks of pregnancy who were admitted to the Department of Obstet-rics and Gynecology,Chang'an Hospital from February 2019 to February 2022 were retrospectively analyzed.They were divided into a control group(n=42)and a study group(n=44)based on different treatment methods.On this basis of basic treatment,the patients in the control group were treated with phloroglucinol,while the patients in the study group were treated with phloroglucinol combined with ritodrine hydrochloride.Both groups were treated continuously for 7 days.The success rate of fetal protection,the regression time of uterine contraction,the regression time of clinical symptoms,the levels of serum P,β-HCG,E2,serum interleukin-6(IL-6),interleukin-10(IL-10)and interleukin-17(IL-17)before treatment and 7 days after treatment,and the incidence of adverse reactions were compared between the two groups.Results The success rate of fetal protection in the study group was 95.24%,which was significantly higher than 72.73%in the control group(P<0.05).The regression time of uterine contractions and clinical symptoms in the study group were(7.06±1.12)h and(52.06±7.21)h,which were significantly shorter than(9.28±1.02)h and(77.46±8.43)h in the control group(P<0.05).Before treatment,there was no significant difference in the levels of serum P,β-HCG,and E2 between the two groups of pregnant women(P>0.05).After treatment,the levels of serum P,β-HCG,and E2 in the two groups were significantly increased,and the serum levels of P,β-HCG and E2 were(78.24±9.13)ng/mL,(78 482.43±1 058.19)mIU/mL,and(669.56±45.21)pg/mL,which were significantly higher than(67.92±9.16)ng/mL,(66 854.64±1062.49)mIU/mL,and(615.46±41.38)pg/mL in the control group(P<0.05).Before treatment,there was no statistically significant difference in the serum levels of IL-6,IL-10,and IL-17 between the two groups of pregnant women(P>0.05).After treatment,the serum levels of IL-6,IL-10,and IL-17 in the two groups were significantly de-creased,and the levels in the study group were(11.54±2.03)pg/mL,(28.19±2.41)pg/mL,and(12.19±2.07)pg/mL,which were significantly lower than(15.27±2.43)pg/mL,(36.53±3.15)pg/mL,and(16.95±2.48)pg/mL in the control group(P<0.05).The total incidence of adverse reactions in the study group was 13.64%,which was slightly higher than 7.14%in the control group,but the difference was not statistically significant(P>0.05).Conclusion Phloroglucinol combined with rit-odrine hydrochloride can effectively improve the levels of serum P,β-HCG,and E2 in pregnant women with threatened abortion over 20 weeks of pregnancy,with high success rate and high safety,which has clinical application value.
Objective To explore the application value of nursing guidance based on information asymmetry theory in patients with acute myocardial infarction(AMI)complicated with heart failure.Methods A total of 128 pa-tients with AMI and heart failure who were treated at Zhengdong Hospital of the First Affiliated Hospital of Zhengzhou University from May 2021 to February 2023 were selected as the research subjects.All the patients were treated with percutaneous coronary intervention(PCI)and were randomly divided into a study group and a conventional group,with 64 patients in each group.The patients in the conventional group received conventional nursing intervention,while the patients in the study group received nursing guidance based on information asymmetry theory in addition to convention-al nursing intervention.The cardiac function[left ventricular end-systolic diameter(LVESD),left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD)]of the two groups before intervention and on the day of discharge(after intervention)was compared.The stress response(diastolic pressure,systolic pressure)and negative emo-tions[Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS)]of the two groups on the day of admission,at the time of admission to the operating room,and on the first day after surgery were compared.The incidence of adverse events and satisfaction with care were also compared between the two groups.Results The systolic blood pressure of the study group was(107.25±7.38)mmHg and(102.14±6.95)mmHg at the time of admission to the operating room and 1 day after surgery,respectively,which was significantly lower than(124.11±7.52)mmHg and(113.77±7.12)mmHg of the conventional group;the diastolic blood pressure was(72.36±5.03)mmHg and(67.52±4.11)mmHg,respectively,which was significantly lower than(83.65±5.27)mmHg and(71.86±4.28)mmHg of the conventional group;the differ-ences were statistically significant(P<0.05).After intervention,the LVESD and LVEDD of the study group were(45.14±2.63)mm and(44.26±1.83)mm,respectively,which were significantly lower than(50.32±3.89)mm and(49.07±2.96)mm of the conventional group;LVEF was(49.97±2.79)%,which was significantly higher than(43.06±1.08)%of the conventional group;the differences were statistically significant(P<0.05).The SAS scores of the study group at the time of admission to the operating room and 1 day after surgery were(54.14±5.78)points and(50.32±5.62)points,respectively,which were significantly lower than(63.42±6.12)points and(58.77±6.08)points of the conventional group;the SDS scores were(53.74±7.36)points and(47.13±6.58)points,respectively,which were significantly lower than(65.87±7.61)points and(53.25±6.80)points of the conventional group;the differences were statistically significant(P<0.05).The incidence of adverse events in the study group was 3.13%,which was significantly lower than 15.63%of the conventional group;the satisfaction rate of patients with nursing was 95.31%,which was significantly higher than 79.69%of the conventional group;the differences were statistically significant(P<0.05).Conclusion Nursing guid-ance based on the theory of information asymmetry can alleviate the physiological and psychological stress responses of AMI patients with heart failure,improve the cardiac function of patients,reduce the incidence of adverse events,and im-prove the nursing satisfaction of patients.
Objective To observe the application effect of nursing measures based on integrated theory of health behavior change(ITHBC)in cancer-related fatigue of patients with colorectal cancer during chemotherapy.Methods A total of 106 patients with colorectal cancer who received chemotherapy in Department of Oncology,the Second Affiliated Hospital of Air Force Medical University from May 2020 to April 2022 were selected as the study ob-jects.According to random number table method,they were divided into a study group and a control group,with 53 pa-tients in each group.The patients in the control group were given routine care,while those in the study group were given ITHBC-based nursing measures.Both groups of patients received nusing until discharge.The scores of the Cancer Fa-tigue Scale(CFS),Cancer Quality of Life Questionnaire C30(QLQ-C30),and Self-perceived Burden Scale(SPBS)were compared between two groups before nusing and at 6 months of follow-up after discharge.Results Before nurs-ing,there were no significant difference in the scores of emotional fatigue,physical fatigue,and cognitive fatigue be-tween the two groups(P>0.05).After nursing,the scores of emotional fatigue,physical fatigue,and cognitive fatigue were significantly lower than those before nursing,and those indicators in the study group were(8.36±2.69)points,(18.53±6.14)points,(8.82±2.39)points,which were significantly lower than(10.65±3.50)points,(21.36±6.79)points,(10.92±3.41)points in the control group(P<0.05).Before nursing,there were no significant differences in the scores of physical function,emotional function,role function and social function between the two groups(P>0.05).After nursing,the scores of the two groups were significantly higher than those before nursing,and those indicators in the study group were(78.79±5.67)points,(81.35±7.84)points,(80.93±5.46)points,(78.42±5.64)points,which significantly higher than(71.42±5.48)points,(73.43±7.25)points,(75.24±5.86)points,(69.37±5.42)points in the control group(P<0.05).Be-fore nursing,there were no significant difference in emotional,physical,and economic scores between the two groups(P>0.05).After nursing,the emotional,physical,and economic scores of the two groups were significantly lower than those before nursing,and the scores in the study group were(11.16±2.94)points,(10.32±1.27)points,(16.15±2.36)points,significantly lower than(15.74±2.45)points,(12.67±1.56)points,(19.87±2.65)points in the control group(P<0.05).Conclusion The application of ITHBC-based nursing measures in patients with colorectal cancer during chemo-therapy can effectively improve their cancer-related fatigue and quality of life,and can also effectively reduce the burden of self-perception,which is worthy of clinical application.
Objective To study the effect of ultrasound-guided iliac fascial space block(FICB)in elderly pa-tients undergoing hip replacement(THA).Methods Eighty elderly patients with THA admitted to Ankang Central Hos-pital from October 2021 to October 2022 were selected and randomly divided into group A and group B,with 40 patients in each group.Patients in group A were treated with ultrasound-guided medial approach FICB,while those in group B were treated with ultrasound-guided lateral approach FICB.The perioperative indicators of two groups[surgical time,in-traoperative bleeding volume,length of resuscitation room stay,postoperative extubation time,Visual Analogue Scale(VAS)score at 48 hours after surgery,sufentanil usage,time of first out-of-bed activity,length of hospital stay],and post-operative block indicators[puncture injection time,catheterization time,number of cases with re-fixation of catheter,depth of catheterization],stress response indicators before and after surgery[serum norepinephrine(NE),cortisol(Cor)levels,C-reactive protein(CRP)],pain mediators before and after surgery[serum substance P(SP),neuropeptide Y(NPY),prostaglandin E2(PGE2),and 5-hydroxytryptamine(5-HT)],as well as the occurrence of adverse reactions were compared between the two groups.Results The VAS score of group A at 48 hours after operation was(2.43±0.45)points,which was significantly lower than(3.48±0.52)points of group B(P<0.05).There was no statistically sig-nificant difference between the two groups in terms of surgical time,intraoperative bleeding volume,length of resuscita-tion room stay,postoperative extubation time,sufentanil usage,time of first out-of-bed activity,and length of hospital stay(P>0.05).The number of cases with re-fixation of catheter in group A was 5.00%,which was significantly less than 25.00%in group B;catheterization time was(63.29±8.36)s,which was significantly shorter than(155.40±9.25)min in group B;the depth of catheterization was(11.13±1.27)cm,which was significantly longer than(7.09±1.18)cm in group B;the differences were statistically significant(P<0.05).There was no statistically significant difference in the puncture injection time between the two groups(P>0.05).At 1 day after operation,the levels of NE,CRP,and Cor in group A were(236.16±16.33)pg/mL,(7.18±0.64)pg/L,and(193.62±24.19)pg/mL,which were significantly lower than(289.29±27.12)pg/mL,(12.40±1.41)pg/L,and(235.39±26.31)pg/mL in group B(P<0.05).At 1 day after operation,the levels of NPY,SP,PGE2,and 5-HT in group A were(68.68±7.51)pg/mL,(72.14±8.38)pg/mL,(28.99±5.32)ng/mL,(0.64±0.14)μmol/L,which were significantly lower than(81.29±8.45)pg/mL,(95.68±9.35)pg/mL,(39.48±4.80)ng/mL,(0.95±0.22)μmol/L of group B(P<0.05).After surgery,the total incidence of adverse reactions in group A was 2.50%,which was slightly lower than 7.50%in group B(P>0.05).Conclusion Both the ultrasound-guided medial approach and the lateral approach FICB have better analgesic effect in elderly patients with THA,but the medial approach has more advantages than the lateral approach in reducing stress response and pain stimulation.
Objective To compare the application effects of dienogest(DNG)and mifepristone(MFP)in long-term maintenance treatment after conservative surgery for endometriosis(EMs).Methods A total of 102 patients who underwent laparoscopic conservative surgery for EMs in Ankang Central Hospital from May 2020 to January 2021 were selected as the research subjects.They were divided into DNG group and MFP group by random number table method,with 51 patients in each group.After surgery,patients in MFP group were treated with MFP,and those in DNG group were treated with DNG.Both groups of patients were given 6 months of treatment.Clinical efficacy after treat-ment was compared between the two groups.Pain degrees before treatment and at 1 week,2 weeks,and 4 weeks after treatment were evaluated with the Visual Analogue Scale(VAS).The patients were followed up for 18 months after treat-ment.The status of recurrence and pregnancy was compared between the two groups.The quality of life was evaluated with the Endometriosis Health Profile-5(EHP-5)before and after treatment.The incidence rates of adverse reactions that occurred during treatment were calculated.Results The total effective rate of clinical treatment in the DNG group was 94.12%,significantly higher than 80.39%in the MFP group(P<0.05).The VAS scores of the two groups of patients at 1 week,2 weeks,and 4 weeks after treatment were significantly lower than those before treatment,and the VAS scores of the DNG group at different times were significantly lower than those of the MFP group,with statistically signifi-cant differences(P<0.05).After treatment,the EHP-5 scores of the two groups were significantly lower than those before treatment,and the EHP-5 scores were significantly lower in the DNG group than in the MFP group[(7.74±1.24)points vs(9.46±1.42)points],with statistically significant differences(P<0.05).During the 18 month follow-up period after sur-gery,the recurrence rate of patients in the DNG group was 23.53%,significantly lower than 45.10%in the MFP group,and the pregnancy rate was 68.63%,significantly higher than 43.14%in the MFP group,with statistically significant dif-ferences(P<0.05).The incidence of adverse reactions in the DNG group during treatment was 13.73%,significantly low-er than 39.22%in the MFP group(P<0.05).Conclusion DNG is more effective than MFP in long-term maintenance treatment after conservative surgery for EMs.The former can better relieve pain,significantly reduce recurrence risk,and improve the pregnancy rate and quality of life,with fewer adverse reactions and higher safety.
This article presents a 34-year-old woman with cervical small cell neuroendocrine carcinoma.Micro-scopic examination revealed diffuse distribution of small round cells,with scant cytoplasm,deeply stained nuclei,and in-conspicuous nucleoli.Immunohistochemistry demonstrated positive results for specific markers including insulinoma-as-sociated protein 1(INSM1)and CD56,and the pathological diagnosis is cervical small cell neuroendocrine carcinoma.This report underscores the potential diagnostic value of INSM1 in identifying small cell neuroendocrine carcinoma.
Objective To investigate the molecular mechanism of Paeoniae-Glycyrrhizae decoction(PGD)in treating chronic urticaria based on network pharmacology and molecular docking technology.Methods Active com-pounds and targets of PGD were screened through the TCMSP database.The disease targets were retrieved from the Gen-eCards database using"Chronic urticaria"as the keyword.Venn diagrams were used to screen the common action sites of compounds and the disease.A"Drug-Common Target-Disease"network was constructed using Cytoscape 3.9.1 soft-ware,and the interactions between common targets were found through the PPI network.GO function enrichment analy-sis and KEGG pathway enrichment analysis were performed on the common target genes.Molecular docking and result visualization were done using Auto Dock and PyMol software respectively.Results By intersecting the 105 active com-pounds and 218 drug targets of PGD with the 961 disease targets of chronic urticaria,83 common targets were obtained.GO function enrichment analysis of these common targets indicated that their functions were mainly related to heterotyp-ic stimuli,lipopolysaccharide responses,oxidative reactions,and epithelial cell proliferation.In the KEGG pathway en-richment analysis,the 142 involved pathways primarily cover fluid shear stress and atherosclerosis,PI3K-Akt signaling path-way,AGE-RAGE signaling pathway in diabetic complications,human cytomegalovirus infection,chemical carcinogenesis-re-ceptor activation,and Kaposi's sarcoma-associated herpesvirus infection,etc.The molecular docking results showed good bind-ing forces between the main compounds and the main targets.Conclusion PGD acts on multiple targets to modulate immune function and alleviate inflammatory responses,thereby playing a therapeutic role in chronic urticaria.
Objective To investigate the predictive value of serum chemokine CCL21 level for adverse cardio-vascular events in patients with gestational hypertension.Methods The CCL21 levels in 45 patients with gestational hypertension(case group)and 45 pregnant women with normal blood pressure(control group)who were admitted to Bei-jing Fengtai Hospital during the period from March 2018 to January 2020 were determined by enzyme linked immuno-sorbent assay(ELISA)in the morning on the second day after admission.All subjects were followed up for 3 years after discharge.Cardiovascular risk factors[body mass index(BMI),total cholesterol(TC),triglyceride(TG),high-density li-poprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),fasting plasma glucose(FPG),systolic blood pressure(SBP),diastolic blood pressure(DBP)],and incidence of adverse cardiovascular events were compared between the two groups.Serum CCL21 levels was compared between mothers with and without adverse cardiovascular events.The predictive value of serum chemokine CCL21 level for adverse cardiovascular events in patients with gesta-tional hypertension was analyzed through receiver operating characteristic(ROC)curve.Results In the case group,se-rum CCL21 was(160.34±21.57)ng/L,and 3-year postpartum FPG,TG,SBP,and DBP levels were(5.60±0.75)mmol/L,(1.08±0.46)mmol/L,(123.89±12.93)mmHg,and(86.80±9.34)mmHg,respectively,which were significantly higher than(125.48±16.53)ng/L,(5.07±0.64)mmol/L,(0.76±0.34)mmol/L,(111.58±10.76)mmHg,and(78.81±8.42)mmHg in the control group(P<0.05).There was no statistically significant difference in BMI,TC,HDL-C,and LDL-C levels between the two groups at 3 years postpartum(P>0.05).The incidence of adverse cardiovascular events in the case group was 13.33%in the 3-year postpartum period,and there were no adverse cardiovascular events in the control group in the 3-year postpartum period,with no statistically significant difference between the two groups(P<0.05).The serum CCL21 level of patients with adverse cardiovascular events was(173.14±16.86)ng/L,which was significantly higher than(155.76±15.92)ng/L in those without cardiovascular adverse events(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of serum CCL21 for adverse cardiovascular events in patients with gestational hypertension in 3-year postpartum period was 0.80.When the critical value was 160.24 ng/L,the diagnostic sensitivity and specificity of CCL21 were 0.620 and 0.950.Conclusion Serum CCL21 level is of certain predictive value for adverse cardiovas-cular events in patients with gestational hypertension.
Objective To investigate the correlation between the expression of intestinal flora,serum epitheli-al cell membrane-bound mucin 1(MUC1),and suppressors of cytokine signaling 3(SOCS-3)expression level and the severity of disease in children with ulcerative colitis(UC).Methods The clinical data of 110 children with UC admit-ted to the First Hospital of Yulin from January 2018 to October 2022 were retrospectively analyzed.According to the se-verity of ulcerative colitis,the children were divided into a mild group(35 cases),a moderate group(46 cases),and a se-vere group(29 cases).Another 30 healthy subjects were selected as the control group during the same period.The intesti-nal flora distribution in fecal samples and serum MUC1 and SOCS-3 levels of all subjects were detected,and the correla-tion between each index and the severity of UC was analyzed by Pearson correlation analysis.Results The serum MUC1 level were compared:severe group>moderate group>mild group>control group,the and the level of SOCS-3 were compared,severe group0.05).The results of Pearson correlation analysis showed that MUC1,Enterobac-ter,Enterococcus,and Clostridium were positively correlated with the severity of UC(r=0.639,0.221,0.420,0.316,P<0.05),and SOCS-3,Bifidobacterium,Lactobacillus,and Eubacillus were negatively correlated with the severity of UC in children(r=-0.481,-0.257,-0.252,-0.132,P<0.05).Conclusion The more severe the disease of UC in children,the more intestinal pathogens and the less probiotics.Serum MUC1 was positively correlated with the severity of UC in children's disease,and SOCS-3 was negatively correlated with the severity of UC.
Objective To analyze the clinical value of serum leptin(Leptin)and monocyte chemoattractant pro-tein-1(MCP-1)levels in predicting postherpetic neuralgia(PHN)in patients with acute herpes zoster.Methods A total of 168 patients with acute herpes zoster diagnosed in Anyang People's Hospital from January 2020 to January 2022 were prospectively chosen as the research subjects.The patients were followed up for 3 months after treatment,and they were divided into PHN group(78 cases)and non-PHN group(90 cases)according to whether PHN occurred.At the same time,96 health examinees from the hospital during the same time period were selected as the health group.Serum Leptin and MCP-1 levels of all subjects were detected and compared.Pearson correlation analysis was used to analyze the cor-relation between serum Leptin,MCP-1 and numerical rating scale(NRS)score.Receiver operating characteristic curve(ROC)and area under the curve(AUC)were used to evaluate the predictive value of serum Leptin and MCP-1 levels for PHN in patients with acute herpes zoster.Multivariate logistic regression analysis was used to explore the risk factors for PHN in patients with acute herpes zoster.Results Serum Leptin and MCP-1 levels in the PHN group were significant-ly lower than those in the non-PHN group and healthy group,while those in the non-PHN group were significantly lower than those in the healthy group,with statistically significant differences(P<0.05).Pearson correlation analysis showed that serum Leptin,MCP-1 were negatively correlated with NRS scores in patients with acute herpes zoster(r=-0.406,P<0.001;r=-0.452,P<0.001).The results of ROC curve analysis showed that AUC of serum Leptin for predicting PHN in patients with acute herpes zoster was 0.769,and the cut-off value was 16.8 μ g/L,with the sensitivity,specificity of 92.31%,50.00%.The AUC of PHN in patients with herpes zoster was 0.832,and the cut-off value was 101.4 pg/μL,with the sensitivity and specificity of 92.31%and 67.78%,respectively.The AUC of serum Leptin combined with MCP-1 for predicting PHN in patients with acute herpes zoster was 0.921,and the specificity,sensitivity were 85.56%,85.90%.Multivariate Logistic regression analysis showed that serum Leptin,MCP-1,and NRS scores were risk factors for PHN in patients with acute herpes zoster(P<0.05).Conclusion Serum levels of Leptin and MCP-1 are decreased in patients with acute herpes zoster,and both are negatively correlated with NRS score,which are expected to be effec-tive biomarkers for predicting PHN in patients with acute herpes zoster.
Objective To explore the application effect of the information-motivation-behavior(IMB)skill model in early rehabilitation nursing for patients with knee osteoarthritis after knee joint replacement surgery.Methods Eighty patients with knee osteosarcoma who underwent knee joint replacement surgery at the Department of Orthopedics,the Sixth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from July 2020 to July 2022 were studied.The patients were randomly divided into an observation group and a control group using a ran-dom number table method,with 40 patients in each group.The patients in the control group were given routine care after surgery,while those in the observation group were given IMB technique model care after surgery.The changes in Hospi-tal for Special Surgery knee score(HSS),knee range of motion(ROM),self-efficacy scale for rehabilitation(SER),and Visual Analogue Scale(VAS)scores were compared between two groups before care,after 1 month and 3 months of care.The incidence of complications and satisfaction rates were also compared between the two groups.Results Be-fore nursing,there was no statistically significant difference in HSS scores and ROM between the two groups(P>0.05);after 1 month and 3 months of nursing,the HSS scores,ROM of the two groups significantly increased,and the indexes of the observation group were(62.74±5.27)points and(91.28±6.28)points,(72.19±4.17)°and(118.34±12.09)°,which were significantly higher than(54.28±5.36)points and(82.19±6.81)points,(61.38±5.01)°and(106.27±9.27)°in the con-trol group(P<0.05).Before nursing,there was no statistically significant difference in SER between the two groups(P>0.05);after 1 month and 3 months of nursing,the task self-efficacy,coping self-efficacy,and total SER scores of patients in both groups significantly increased,and the scores in the observation group were significantly higher than those in the control group(P<0.05).Before nursing,there was no statistically significant difference in VAS scores between the two groups(P>0.05);after 1 month and 3 months of nursing,the VAS scores of both groups significantly decreased,and the scores of the observation group were(4.91±0.93)points and(2.78±0.41)points,which were significantly lower than(5.72±0.72)points and(3.92±0.51)points of the control group(P<0.05).The incidence of complications in the observation group was 5.00%,which was significantly lower than 22.50%in the control group;and the satisfaction rate of patients with nursing was 95.00%,which was significantly higher than 75.00%in the control group;the differences were statistical-ly significant(P<0.05).Conclusion The application of IMB technique model after knee joint replacement surgery for pa-tients with knee osteosarcoma can effectively alleviate pain,increase knee joint range of motion,promote knee joint func-tion recovery,improve patients'rehabilitation self-efficacy,reduce complications,and have a high patient satisfaction rate.
Objective To investigate the value of measurement of pubo-femoral distance(PFD)by ultrasound in different body positions in patients with developmental dysplasia of hip(DDH).Methods A total of 102 patients with DDH admitted to Chang'an Hospital from May 2019 to May 2022 were prospectively selected as the observation group,and 80 healthy subjects were selected as the control group.All subjects were examined by ultrasonography,and the PFD value,bone margin coverage,angle between baseline and the top line of the bone α were measured and com-pared between the two groups.The difference of left and right hip PFD(ΔPFD)was compared between the two groups.The ΔPFD of children with unilateral and bilateral DDH were observed and compared.Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of α angle,bone margin coverage,and PFD in DDH patients.Results In the observation group,there were 38 cases of bilateral diseases and 64 cases of unilateral diseases,involv-ing a total of 145 hips(121 hips for females and 24 hips for males;88 hips on the left and 57 hips on the right.Accord-ing to ultrasound examination,there are 42 hips of Graf Ⅱa,17 hips of Graf Ⅱb,18 hips of Graf C,20 hips of Graf D,25 hips of Graf Ⅲ,and 23 hips in Graf Ⅳ.The PFD of the observation group was(6.33±1.54)mm,significantly higher than(3.28±0.27)mm of the control group(P<0.05).The α angle and bone margin coverage rate of children in the obser-vation group were(51.25±6.87)°and(36.32±10.36)%,significantly smaller(lower)than(69.45±10.18)°and(65.45±9.15)%in the control group(P<0.05).ΔPFD of the children in the observation group was(3.43±0.78)mm,significantly higher than(0.58±0.17)mm in the control group(P<0.05).ΔPFD of the males and females in the observation group were(3.43±0.54)mm and(3.58±0.97)mm,respectively,which were significantly higher than(0.53±0.14)mm and(0.58±0.17)mm in the control group(P<0.05);there was no statistically significant difference in ΔPFD between males and females in the observation group(P>0.05).ΔPFD was(3.46±0.45)mm for children with unilateral DDH in the ob-servation group and(3.28±1.07)mm for those with bilateral DDH,both significantly higher than(0.58±0.17)mm in the control group(P<0.05);There was no statistically significant difference in Δ PFD between children with unilateral DDH and those with bilateral DDH(P>0.05).ROC curve analysis showed that the specificity of α-angle,bone margin coverage,and PFD in the diagnosis of DDH was 91.3%,88.2%,and 99.3%,respectively,and the sensitivity was 78.2%,69.6%,and 90.6%,with statistically significant differences(P<0.05).Conclusion Ultrasound measurement of PFD in different body positions contributes to the screening and diagnosis DDH patients,and the difference in hip PFD is inde-pendent of the gender of the child,which can provide objective guidance for the diagnosis and treatment of DDH.
