Objective There was interdigestive periodic migrating myoelectric or motor complex of the upper gastrointestinal tract in conscious dogs.This study selected the left gastric(LGA) and superior mesenteric(SMA) arteries the main arteries dominating the gastrointestinal tract,and investigated the hemodynamics of LGA and SMA during MMC.Methods Blood flow of the left gastric(LGA) and superior mesenteric(SMA) arteries were measured by ultrasound transit-time blood-flow meters in 5 conscious dogs.Motilin(12.5,25,50,or 100 pmol·kg-1· h-1) was infused intravenously 30 minutes after the end of duodenal phase Ⅲ activities.In another series of studies,motilin or VIP(5,10,20,40,and 80 pmol/kg) dissolved in 1 ml of saline was injected intravenously over 1 minute with or without atropine(25 μg·kg-1·h-1).Results Motilin(12.5,25,50,and 100 pmol· kg-1·h-1) induced LGA blood flow increased,without affecting the blood pressure,heart rate,and SMA blood flow.Motilin significantly induced larger gastric vasodilatation than the equivalent doses of VIP.Conclusion LGA blood flow showed periodic changes associated with interdigestive migrating myoelectric or motor complex of the gastrointestinal tract(MMC),SMA blood flow remained unchanged during the contracting phase of the stomach and duodenum.Motilin has a potent and selective gastric vasodilator effect,Motilin plays an important role in the regulation of interdigestive gastric blood flow in dogs.
目的了解2型糖尿病并发非糖尿病肾损害的临床特点,探讨2型糖尿病并发非糖尿病肾损害的病理原因。方法对30例2型糖尿病并发肾损害患者的临床资料进行回顾性分析。结果所有30例患者中,糖尿病性肾病患者为13例(占43.3%),非糖尿病性肾病患者17例(56.7%);就非糖尿病性肾病的病理类型而言,IgA肾病Lee’sⅡ级为4例,Ⅲ级为3例,分别占23.5%和17.6%,膜增生肾小球肾炎3例(占17.6%),阶段性肾小球硬化症2例(11.8%),膜性肾病2例(11.8%),骨髓瘤肾病2例(11.8%),膜增生性肾炎1例(5.9%)。结论 2型糖尿病并发肾损害大多是非糖尿病肾损害,在临床过程中,如果出现非糖尿病肾损害征兆,应当立即进行肾活检。
随着影像学检查的广泛应用,以及解剖学的发展,胰胆管合流异常(pancreaticobiliary maljunction,PBM)的检出率有增多趋势.因PBM与先天性胆管囊肿、胆道结石、非结石性胆囊炎、胆道肿瘤及胆源性胰腺炎等胆胰疾病关系密切,PBM得到了越来越多的重视.现结合文献对PBM的研究进展综述如下.
目的了解脑血栓的临床表现症状,探讨消栓肠溶胶囊对脑血栓的临床疗效。方法将60例脑血栓患者随机分为2组,每组各30例。观察组在实用阿司匹林和尼莫地平治疗的基础上加用消栓肠溶胶囊治疗,对照组仅单纯性使用阿司匹林和尼莫地平治疗,然后观察并比较2组疗效。结果观察组治愈23例,治愈率为76.7%,总有效率为93.3%,对照组治愈16例,治愈率为53.3%,总有效率为73.3%,2组相比具有显著统计差异,P<0.05;治疗前后2组的血液流变学变化存在显著差异,P<0.01。结论消栓肠溶胶囊能够有效的降低脑血栓患者的纤维蛋白原,因而疗效比较显著,值得临床推广。
Objective Aim The aim of this study is to clarify the regulatory role of motilin in interdigestive gastric blood flow in conscious dogs.Methods Blood flow of the left gastric(LGA) arterie was measured by ultrasound transit-time blood-flow meters in 5 conscious dogs.Motilin was infused intravenously with or without phe-cyclo [Lys-Tyr(3-tBu)-βAla-] ·trifluoroacetate(GM-109;motilin antagonist),granisetron(5-HT3 antagonist),atropine,hexamethonium(C6),phenoxybenzamine,propranolol,or cimetidine.Results Motilin(100 pmol·kg-1·h-1) induced LGA blood-flow increase.GM-109 completely abolished the LGA blood flow and motility responses to motilin(100 pmol·kg-1·h-1).Atropine,granisetron and C6 abolished motilin-induced gastric contractions,but failed to affect the sustained LGA blood flow.The LGA flow and gastric motility responded to motilin were not inhibited by α-adrenergic and β-adrenergic blockers.Conclusion Motilin has a potent gastric vasodilator effect,and appears to regulate interdigestive gastric arterial blood flow by the motilin receiver on gastric arterial wall.Motilin plays an important role in the regulation of interdigestive gastric blood flow in dogs.
