BACKGROUND:In 2010 the Beijing Municipal Government promulgated a policy aimed at improving the quality of life and subjective well-being of elderly residents that included a component focused on mental health.AIM:Identify factors associated with subjective well-being in a representative sample of elderly residents of Xi Cheng District in Beijing.METHODS:This cross-sectional study administered a self-completion survey to a stratified random sample of 2342 residents of Xi Cheng District who were 60 to 80 years of age. The level of well-being was assessed using a validated Chinese version of the Memorial University of Newfoundland Scale of Happiness (MUNSH). Detailed socioeconomic variables were obtained using a questionnaire developed by the authors. Social support, anxiety, and depression were assessed using validated Chinese versions of the Social Support Rating Scale (SSRS), Self-rating Anxiety Scale (SAS), and Self-rating Depression Scale (SDS).RESULTS:Among the 2342 respondents, 1616 (69.0%) had a total MUNSH score of 32 or above, indicating a high level of happiness; 423 (18.1%) has a total SSRS score 32 or below, indicating poor social support; 201 (8.6%) had a total SDS score of 53 or above, indicating significant depression; and 126 (5.3%) had a total SAS score of 50 or above, indicating significant anxiety. In the multivariate regression analysis the self-reported level of depression was the most important factor related to well-being. Anxiety, social support, income level, the quality of family relationships, the ability to self-regulate emotions, and regular exercise were also significantly related to well-being; but gender, marital status, age and educational level were not associated with well-being.CONCLUSION:Among elderly urban residents in Beijing, self-reports of poor subjective well-being are closely associated with self-reports of depressive and anxiety symptoms and also associated with social factors such as social support, income level and family relationships. Prospective studies are needed to identify the causal relationships of these variables and, based on the findings, to develop targeted interventions aimed at improving the quality of life and well-being of elderly community members.
Objective:To investigate prevalence of dementia and related factors among people aged 65 and over in an urban of Beijing City based on two surveys carried out in the same area both in 1986 and 1997, and to analyze the dynamic change of dementia prevalence in the past two decades. Methods: Using cluster sampling, 1562 elderly aged 65 orand overabove were interviewed by a set of fully structured assessments in a one-stage process developed by the 10/66 group for dementia in an urban district of Beijing City. Dementia patients were diagnosed in accordance with ICD-10 criteria. Results:The prevalence rate of dementia among the elderly people in 2004 was 4.1%, which was similar to 1997's result (3.2%, ). There were no statistical differences among the three surveys on prevalence of moderate and severe dementia(2.8%,1.9%,and 1.8% respectively,P=0.192). The prevalence rates of dementia were increased with age.In 2004, the patients with AD were much more than those with VD (47.9%/45.8%). Conclusions:The three surveys in the same district during two decades show that the level of dementia prevalence is rather stable. The prevalence of dementia increased with age.
Objective: To evaluate feasibility of informant questionnaire of the Community Screening Instrument for Dementia(CSID) and related factors in a sample of community elderly population, and to investigate prevalence of suspectable dementia in the sampled people.Method: A total of 2267 people aged 65 and above in one rural area and one urban area in Beijing were recruited and their informants were assessed by the CSID informant questionnaire.Validity of CSID were evaluated according to the computerized diagnosis of 10/66 project and ICD-10.Result:The split-half reliability of CSID informant questionnaire was 0.85 and the consistency was more than 0.8.When the cutoff point of CSID was 6 or more, sensitivity was 86.2% and specificity was 93.2%.At meantime, the prevalence of suspectable dementia was 9.1%.Logistic regression analysis showed that the information obtained from informant as a spouse and from informant who lived in urban area was correlated with positive diagnosis of dementia. Conclusion:CSID informant questionnaire is valid as a screening instrument of dementia.Information from spouse is useful to identify the change of cognitive function of the elderly.
