Osteogenesis is tightly correlated with angiogenesis during the process of bone development, regeneration, and remodeling. In addition to providing nutrients and oxygen for bone tissue, blood vessels around bone tissue also secrete some factors to regulate bone formation. Type H vessels which were regulated by platelet-derived growth factor-BB (PDGF-BB) were confirmed to couple angiogenesis and osteogenesis. Recently, preosteoclasts have been identified as the most important source of PDGF-BB. Therefore, inhibiting osteoclast maturation, improving PDGF-BB secretion, stimulating type H angiogenesis, and subsequently accelerating bone regeneration may be potent treatments for bone loss disease. In the present study, aucubin, an iridoid glycoside extracted from Aucuba japonica and Eucommia ulmoides, was found to inhibit bone loss in ovariectomized mice. We further confirmed that aucubin could inhibit the fusion of tartrate-resistant acid phosphatase (TRAP)+ preosteoclasts into mature osteoclasts and indirectly increasing angiogenesis of type H vessel. The underlying mechanism is the aucubin-induced inhibition of MAPK/NF-κB signaling, which increases the preosteoclast number and subsequently promotes angiogenesis via PDGF-BB. These results prompted that aucubin could be an antiosteoporosis drug candidate, which needs further research.
BACKGROUND: Low cardiorespiratory fitness (CRF) is a risk factor for many chronic diseases. This study aimed to evaluate the CRF of a sample of adults with different glucose tolerance statuses to explore its relationship with early abnormal glucose metabolism according to sex.METHODS: A total of 93 participants were assigned to three groups, i.e. the normal glucose tolerance (NGT) group, impaired glucose regulation (IGR) group and new-onset type 2 diabetes mellitus (T2DM) group, through an oral glucose tolerance test. Cardiopulmonary exercise testing was performed to evaluate the participants’ CRF. The physical measurements (including height, weight, systolic blood pressure [SBP] and diastolic blood pressure) and laboratory test results (including fasting plasma glucose and two-hour plasma glucose [2h-PG]) of all participants were collected. Partial correlation, multiple linear regression (stepwise method) and logistic regression were used to analyse the data.RESULTS: Compared to the males with NGT, those with T2DM or IGR had a lower exercise time (P = 0.044), anaerobic threshold (AT) oxygen uptake (VO2) (P = 0.009), maximum VO2/kg (P = 0.041) and oxygen uptake efficiency slope (P = 0.002). The male participants with T2DM had lower AT power (P = 0.001) than those with IGR or NGT. Compared to the females with NGT, the AT heart rate (HR) (P = 0.003), AT SBP (P = 0.002) and maximum VO2/kg (P = 0.039) were lower in the female T2DM and IGR groups. The multiple linear regression (stepwise method) analyses showed that the maximum VO2/kg (β = -0.268, p = 0.026) and one-minute HR recovery (β = -0.239, p = 0.039) of the females improved the prediction of the 2h-PG when entered in the model. The logistic analysis results indicated that the VO2 max of the male participants was related to pre-diabetes (β = -0.003, p = 0.024) and that their AT power was associated with new-onset diabetes (β = -0.053, p = 0.010). Meanwhile, the AT SBP of the female participants was related to pre-diabetes (β = 0.120, p = 0.019), and their AT HR was related to new-onset diabetes (β = -0.102, p = 0.014).CONCLUSIONS: Low CRF is associated with abnormal glucose metabolism. The CRF is closely associated with the 2h-PG after glucose load and is an important risk factor for pre-diabetes and new-onset diabetes.
OBJECTIVE:To investigate a new risk score for acute chest pain with suspected non-ST-segment elevation acute coronary syndrome (NSTE-ACS).METHODS:Patients who suffered from Chest pain and suspected NSTE-ACS were enrolled as subjects. Predictor variables had been analyzed, and a bootstrap technique was used to evaluate the internal validity of the model, and external validation had been assessed for a prospective cohort study.RESULTS:Thousand five hundred and sixty-eight patients had been included in this study. Six predictor variables were found to be significant and were used to develop the model. The C-statistic of the model was 0.83, and internal validation revealed the stability of the model and the absence of over-optimism. Patients were given different triage recommendations, and the risk score was prospectively validated.CONCLUSIONS:A risk score may be a suitable method for assessing the risk of major adverse cardiac events and aiding patient triage in emergency departments among patients with suspected NSTE-ACS.
Abstract A number of staging systems have been developed to predict clinical outcomes in hepatocellular carcinoma (HCC). However, no general consensus has been reached regarding the optimal model. New approaches such as machine learning (ML) strategies are powerful tools for incorporating risk factors from multiple platforms. We retrospectively reviewed the baseline information, including clinicopathologic characteristics, laboratory parameters, and peripheral immune features reflecting T-cell function, from three HCC cohorts. A gradient-boosting survival (GBS) classifier was trained with prognosis-related variables in the training dataset and validated in two independent cohorts. We constructed a 20-feature GBS model classifier incorporating one clinical feature, 14 laboratory parameters, and five T-cell function parameters obtained from peripheral blood mononuclear cells. The GBS model–derived risk scores demonstrated high concordance indexes (C-indexes): 0.844, 0.827, and 0.806 in the training set and validation sets 1 and 2, respectively. The GBS classifier could separate patients into high-, medium- and low-risk subgroups with respect to death in all datasets (P < 0.05 for all comparisons). A higher risk score was positively correlated with a higher clinical stage and the presence of portal vein tumor thrombus (PVTT). Subgroup analyses with respect to Child–Pugh class, Barcelona Clinic Liver Cancer stage, and PVTT status supported the prognostic relevance of the GBS-derived risk algorithm independent of the conventional tumor staging system. In summary, a multiparameter ML algorithm incorporating clinical characteristics, laboratory parameters, and peripheral immune signatures offers a different approach to identify patients with the greatest risk of HCC-related death.
目的 研究HEART风险评分在有冠心病的急性胸痛人群中的应用价值.方法 利用已有数据库进行回顾性分析.连续入选有冠心病病史的急性胸痛人群,计算HEART风险评分.研究终点为3个月时主要不良心脏事件.结果 研究共入选377例急性胸痛患者,其中186例患者发生主要不良心脏事件.HEART风险评分的受试者操作特征曲线(ROC)下面积为0.73(95%CI:0.68~0.78).入选患者分为3组:低危组(评分2~3分),中危组(评分4~5分)和高危组(评分6~10分),主要不良心脏事件发生率分别为8.3%(1/12),38.3%(72/188)和63.8%(113/177),各组间差异有统计学意义(P<0.001).根据不同的危险分层,实施不同的分诊策略.结论 HEART风险评分可以用于有冠心病的急性胸痛人群的分诊.
Abstract PurposeHepatocellular carcinoma (HCC) patients with alpha-fetoprotein (AFP)-negative (<8.78 ng/mL) have special clinicopathologic characteristics and prognosis. The aim of this study was to apply a new method to establish and validate a new model for predicting the prognosis of HCC patients with AFP-negative.Materials and MethodsA total of 410 AFP-negative patients with clinical diagnosed with HCC as a primary cohort; 148 AFP-negative HCC patients as an independent validation cohort. In primary cohort, independent factors for overall survival (OS) by LASSO Cox regression were all contained into the nomogram1; by univariate and multivariate Cox hazard analysis were all contained into the nomogram2. Nomograms performance and discriminative power were assessed with concordance index (C-index) values, area under curve (AUC), Calibration curve and decision curve analyses (DCA). The results were validated in the validation cohort.ResultsThe C-index of nomogram1was 0.708 (95%CI: 0.673-0.743), which was superior to nomogram2 (0.706) and traditional modes (0.606-0.629). The AUC of nomogram1 was 0.736 (95%CI: 0.690-0.778). In the validation cohort, the nomogram1 still gave good discrimination (C-index: 0.752, 95%CI: 0.691-0.813; AUC: 0.784, 95%CI: 0.709-0.847) and good calibration. The calibration curve for probability of OS showed good homogeneity between prediction by nomogram1 and actual observation. DCA demonstrated that nomogram1 was clinically useful. Moreover, patients were divided into three distinct risk groups for OS by the nomogram1: low-risk group, middle-risk group and high-risk group, respectively.ConclusionsNovel nomogram based on LASSO Cox regression presents more accurate and useful prognostic prediction for patients with AFP-negative HCC. This model could help AFP-negative HCC facilitate a personalized prognostic evaluation.
目的 探讨早期尿激酶应用对慢性肾衰竭静脉置管长期血液透析患者导管纤维蛋白鞘形成的防治作用.方法 选取我院2017年3月—2018年10月收治的慢性肾衰竭静脉置管长期血液透析患者85例,根据防治导管纤维蛋白鞘形成方法的不同分为观察组44例和对照组41例.观察组置管首次透析后即开始采用尿激酶封管法和滴注法防治导管纤维蛋白鞘形成;对照组置管首次透析后先采用常规肝素封管,待出现导管功能不良时再开始采用尿激酶封管法和滴注法防治导管纤维蛋白鞘形成.观察比较两组透析6个月后导管功能不良发生次数和首次导管功能不良出现时间;两组透析1、3及6个月时的血流量及静脉压;两组透析前及透析6个月时凝血酶原时间(PT)、凝血酶时间、纤维蛋白原、部分凝血酶时间;两组治疗期间不良反应发生情况.结果 观察组导管功能不良发生次数明显少于对照组,首次导管功能不良出现时间明显晚于对照组,差异均有统计学意义(P<0.05或P<0.01).透析3、6个月时观察组血流量明显大于对照组,静脉压明显低于对照组,差异均有统计学意义(P<0.05).透析6个月时两组组内治疗前后比较,仅对照组PT降低(P<0.05);观察组透析6个月时PT明显长于对照组(P<0.05).两组治疗期间均无严重不良反应发生.结论 早期应用尿激酶可有效防治慢性肾衰竭静脉置管长期血液透析患者导管纤维蛋白鞘形成,且安全性良好,是一种防治长期血液透析留置导管功能不良的有效手段.
Background/Aims: Studies have shown that nucleos(t)ide analogue (NA) treatment can reduce the risk of hepatocellular carcinoma (HCC) in chronic hepatitis B (CHB) patients, but it is unclear which NA is most effective. We performed a meta-analysis and systematic review comparing the efficacies of NAs in CHB patients. Methods: We searched literature databases for randomized controlled trials (RCTs) and observational studies that analyzed the hepatic biochemical response, virological response, seroconversion rate, drug resistance rate, and HCC incidence rate in CHB patients treated with NAs. Meta-analyses were performed with RevMan and Stata/SE software. Results: Twelve cohort studies and one RCT were selected, in which entecavir (ETV), lamivudine (LAM), telbivudine (LdT), and/or tenofovir disoproxil fumarate (TDF) were evaluated in CHB patients. The meta-analysis showed that ETV was superior to LAM with regard to the HCC incidence (p<0.001), biochemical response (p=0.001), virological response (p=0.02), and drug resistance (p<0.001), and ETV was superior to LdT with regard to the virological response (p<0.001) and drug resistance (p<0.001). We found no significant difference between ETV and TDF with regard to the HCC incidence (p=0.08), biochemical response (p=0.39), virological response (p= 0.31), serological conversion (p=0.38), or drug resistance (p=0.95). NA-treated patients with pre-existing cirrhosis had a 5.49 times greater incidence of HCC than those without cirrhosis (p<0.001). Conclusions: ETV or TDF should be used for long-term first-line monotherapy in CHB patients according to the current guidelines. Standardized protocols are needed for future studies of ETV and TDF to facilitate conclusive comparisons. Patients with cirrhosis are at significantly elevated risk for HCC, despite the benefits of NA treatment.
The liver plays an important role in the metabolism of lipid and lipoprotein. Dyslipidemia has been demonstrated to be related with several cancers, but the association between serum lipid and hepatocellular carcinoma (HCC) in the absence of cirrhosis remains unclear.
Aim. To determine whether nucleot(s)ide analogs therapy has survival benefit for patients with HBV-related HCC after unresectable treatment.Method. A systematic search was conducted through seven electronic databases including PubMed, OVID, EMBASE, Cochrane Databases, Elsevier, Wiley Online Library, and BMJ Best Practice. All studies comparing NA combined with unresectable treatment versus unresectable treatment alone were considered for inclusion. The primary outcome was the overall survival (OS) after unresectable treatment for patients with HBV-related HCC. The secondary outcome was the progression-free survival (PFS). Results were expressed as hazard ratio (HR) for survival with 95% confidence intervals.Results. We included six studies with 994 patients: 409 patients in nucleot(s)ide analogs therapy group and 585 patients without antiviral therapy in control group. There were significant improvements for the overall survival (HR = 0.57; 95% CI = 0.47–0.70;p< 0.001) and progression-free survival (HR = 0.84; 95% CI = 0.71–0.99;p= 0.034) in the NA-treated group compared with the control group. Funnel plot showed that there was no significant publication bias in these studies. When it comes to antiviral drugs and operation method, it also showed benefit in NA-treated group. At the same time, overall mortality as well as mortality secondary to liver failure in NA-treated group was obviously lesser. Sensitivity analyses confirmed the robustness of the results.Conclusions. Nucleot(s)ide analogs therapy after unresectable treatment has potential beneficial effects in terms of overall survival and progression-free survival. NA therapy should be considered in clinical practice.

Significant improvement in hepatic fibrosis did not occur among the patients treated with TZDs alone or in those who underwent both lifestyle changes and TZD therapy. Our findings indicate that additional variables should be assessed to obtain a more comprehensive evaluation of the effects of TZD treatment on IR and comorbidity risk factors in NASH patients, and suggest that including lifestyle changes and additional insulin-sensitizing agents in TZD regimens might improve the benefits of TZD therapy for NASH.
目的:研究HEART风险评分在非急性心肌梗死性胸痛人群的应用价值.方法:这是一个源于前瞻性获得的数据库的回顾性研究.连续入选2014年9月至2015年2月,在北京安贞医院急诊胸痛中心就诊的胸痛患者.计算HEART评分.研究终点为3个月时主要不良心脏事件.结果:研究总计入选1041名胸痛患者.其中,332名患者发生了主要不良心脏事件.HEART评分的受试者操作特征曲线下面积为0.77(95% CI:0.74~0.80).随评分增加,主要不良心脏事件显著增加(P<0.001).入选的所有患者分成三组:低危组(评分0~2分),中危组(评分3~4分)和高危组(评分5~8分),主要不良心脏事件发生率分别为1.7%,17.1%和54.6%,各组间差异有统计学意义(P <0.001).不同的组别,给予不同的分诊方案.结论:HEART风险评分可以用于急诊科非急性心肌梗死性胸痛患者的分诊和预后评估.

Background: Nup88 is overexpressed in a number of types of carcinomas and is associated with myometrial invasion, but its exact expression pattern in endometrial cancer and premalignant lesions is unknown.Aims: To evaluate the role of Nup88 in endometrial cancers and atypical endometrial hyperplasia and its clinicopathological significance.Methods: Nup88 expression was examined by immunohistochemistry in samples from 104 endometrial cancers, 21 atypical endometrial hyperplasia lesions, and 40 normal endometria. All samples were from patients who underwent surgery at the First Hospital of Hebei Medical University (Shijiazhuang, China) between April 2006 and December 2009. Nup88 expression was compared between the groups and associations were assessed between Nup88 and clinicopathological characteristics of the subjects.Results: Nup88 expression in cancer (76% of samples) and atypical hyperplasia (91%) was significantly higher compared to normal endometrium (33%, both P < 0.001), but there was no significant difference between endometrial cancer and atypical hyperplasia (P = 0.237). The expression of Nup88 increased significantly with increasing exposure time to estrogen (P = 0.033).Conclusions: Nup88 may be related to the occurrence of endometrial cancers and premalignant lesions. To explore serum vaspin, leptin, and adiponectin levels and their correlation with insulin resistance (IR) in pregnant women with and without gestational diabetes mellitus (GDM) and healthy non-pregnant women. A total of 262 individuals, including pregnant women with GDM (n = 86), those without GDM (n = 92), and age-matched healthy non-pregnant women (n = 84) were enrolled in this case–control study. Vaspin, leptin, adiponectin, glucose, insulin, hemoglobin A1C (HbA1c), and lipid parameters were measured. The homeostasis model assessment–insulin resistance (HOMA–IR), quantitative insulin sensitivity check index, and body mass index were calculated. Data inferred higher concentrations of vaspin (2.72 ± 2.20 vs. 1.84 ± 1.57 vs. 0.81 ± 1.02) in GDM than during normal pregnancy and in non-pregnant women, higher leptin (23.42 ± 12.18 vs. 22.19 ± 10.55 vs. 12.10 ± 11.26), and lower adiponectin (4,164.83 ± 2,650.39 vs. 4,871.66 ± 2,803.51 vs. 7,202.85 ± 4,893.13) in GDM and normal pregnancy as compared to non-pregnant women (p < 0.05). Vaspin was positively correlated to leptin (r = 0.273, p = 0.012), HOMA–IR (r = 0.387, p = 0.000), and triglycerides (TG, r = 0.218, p = 0.046) in GDM. In addition, leptin was negatively correlated to adiponectin in GDM (r = −0.336, p = 0.002) and normal pregnancy (r = −0.256, p = 0.014). Furthermore, vaspin was significantly correlated to GDM and HOMA–IR, and the weight gain might play a vital role in the occurrence of GDM. During pregnancy, high vaspin concentration is significantly associated with IR in GDM.
目的观察磺达肝癸钠和那曲肝素对急性ST段抬高型心肌梗死的临床疗效及安全性。方法急性ST段抬高心肌梗死患者120例,随机分为对照组和治疗组,每组60例。2组基础治疗相同,治疗组除基础治疗外,给予磺达肝癸钠2.5 mg/d,皮下注射,连用7 d。对照组给予低分子肝素0.1 ml/10 kg,皮下注射,每12小时1次,连用7 d。观察治疗7 d时及30 d内心脏事件发生情况。比较2组疗效及安全性。结果 7 d时,治疗组与对照组心血管事件(再发心绞痛、死亡、室速、室颤)及严重出血(脑出血、腹膜腔出血、呕血)的发生率比较,差异无统计学意义(P>0.05)。轻度出血(皮下瘀斑、牙龈出血、血尿、便血+++以下)并发症例数均较对照组明显减少(P>0.05)。30 d时,治疗组与对照组心血管事件的发生率比较,差异无统计学意义(P>0.05)。严重出血及轻度出血较对照组明显减少(P<0.05)。结论磺达肝癸钠治疗急性ST段抬高型心肌梗死安全有效,磺达肝癸钠出血发生率较那曲肝素明显降低。
In previous studies, the relationship between aspartate aminotransferase to alanine aminotransferase ratio(AST/ALT ratio) and liver disease has been evaluated. In viral hepatitis, alcoholic liver disease and primary biliary cirrhosis, AST/ALT ratio has been proven to be an indicator of liver cirrhosis.1-3 AST/ALT ratio was a potential value in differentiating nonalcoholic steatohepatitis from alcoholic liver disease.The value < 1 suggestted nonalcoholic steatohepatitis, a ratio of ≥ 2 was strongly suggestive of alcoholic liver disease.4
Objective To evaluate the relationship between obesity and serum soluble intercellular adhesion molectde-1 (sICAM-1) in adolescents. Methods Totally, 158 adolescents aged 13-15 years were divided into three groups based on their body mass index (BMI) , 80 in control group, 44 in overweight group and 34 in obesity group. Height, weight, waist circumference (WC) and blood pressure were measured for all of them and sICAM-1 was assayed from fasting venous blood collected. Results BMI, WC and systolic blood pressure were significantly higher in overweight group than those in control group (P < 0. 01). BMI, WC, systolic blood pressure and diastolic blood pressure were significantly higher in obesity group than those in overweight group (P<0. 05). Level of sICAM-1 was significantly higher in obesity group than that in control group (P <0.05). Their sICAM-1 showed positive correlation with BMI (r = 0.168, P=0.035) and WC (r=0. 179, P =0.025). Multiple linear regression analysis using sICAM-1 as a dependent variable showed that WC (β= 0.009, 95% CI 0. 001-0.018, P=0.025) was an independent predictors for sICAM-1 adjusted for age, sex, BMI and blood pressure. Conclusions Results mentioned above suggest that endothelial dysfunction associates with obesity, particularly with abdominal obesity. sICAM-1 can be used as an early marker of atherosclerosis in adolescents.
