The inaugural Guangdong Province Ophthalmology Resident Clinical Skills Competition was jointly organized by the Ophthalmologists Branch of the Guangdong Medical Association and Zhongshan Ophthalmic Center, Sun Yat-sen University in August 2024. This event drew participants from 30 ophthalmology residency training programs throughout Guangdong Province, with second- and third-year residents taking part. The competition was structured around four standardized stations: image interpretation, case analysis, basic clinical skills (including ocular medication administration and bandaging), and Wet lab surgery (specifically corneal and scleral wound repair). Through these stations, it provided a comprehensive and systematic assessment of the participants' overall clinical competencies in key areas such as imaging analysis, clinical reasoning, basic procedures skills, and microsurgical techniques. Not only did the competition evaluate the individual abilities of the participants, but also served as a significant indicator of the quality of residency training at each participating institution. Based on the teaching practices and competition experiences of Zhongshan Ophthalmic Center, Sun Yat-sen University, this article conducts a systematic analysis of the main issues that emerged during the competition regarding trainees' performance in anterior segment image interpretation, clinical reasoning, basic skills, and microsurgical techniques. Accordingly, we propose that teaching reforms should be promoted through the model of “using competitions to drive teaching and learning”. On the one hand, efforts should be directed towards enhancing the development of ophthalmic disease image databases, optimizing clinical teaching methods, standardizing skill operation protocols, and implementing systematic faculty training. On the other hand, competitions can be used to stimulate trainees' learning motivation, foster exchanges among training programs, and encourage the regular and multi-level organization of such competitions. This study provides valuable insights and practical directions for the continuous improvement of the quality of ophthalmic residency training. It may also serve as a useful reference for residency training competitions and teaching reforms in other medical specialties.
Purpose: Keloid (KD) and hypertrophic scars are prevalent and result from excessive growth of dermal tissue after skin damage. This review focused on the clinical application of the ultra-pulsed CO(2 )fractional laser combined with recombinant human epidermal growth factor (rHEGF) gel in patients with eyelid KD. Methods: Patients (N = 98) with KD who underwent surgery were randomly divided into a study group (ultra-pulsed CO2 fractional laser combined with rHEGF gel therapy, N = 49) and a control group (ultra-pulsed CO2 fractional laser therapy, N = 49). Besides, 5 cases dropped out of the study, including 2 cases in the study group and 3 cases in the control group. Finally, 47 cases of the study group and 46 cases of the study group were included in the analysis. The clinical baseline data such as sex, age, body mass index, scar area, etiology, Vancouver Scar Scale score, Patient and Observer Scar Assessment Scale score, four-item itch questionnaire score, serum interleukin-6 (IL-6), IL-10, and tumor necrosis factor-alpha level expression were recorded in the study group (N = 47) and the control group (N = 46). Results: There was no significant difference in gender, age, body mass index, scar area, etiology, Vancouver Scar Scale score, Patient and Observer Scar Assessment Scale score, 4-item itch questionnaire score, IL-6, IL-10, and tumor necrosis factor-alpha levels between the patients treated with ultra-pulse CO2 fractional laser + rHEGF gel and those only treated with ultra-pulse CO2 fractional laser (p > 0.05). Vancouver Scar Scale scores, Patient and Observer Scar Assessment Scale scores, and four-item itch questionnaire scores of patients with eyelid KD decreased to a greater extent than those treated with ultra-pulsed CO(2 )fractional laser combined with rHEGF gel (p <0.01). Compared with ultra-pulsed CO2 fractional laser treatment, ultra-pulsed CO2 fractional laser combined with rHEGF gel was more efficacious in treating patients with eyelid KD, with a lower incidence of adverse effects and a 1-year recurrence rate. Conclusions: Ultra-pulsed CO2 fractional laser combined with rHEGF gel can significantly improve the scar status and scar itching in patients with eyelid KD, with an obvious therapeutic effect, a low incidence of adverse effects, a 1-year recurrence rate, and high safety, which is worthy of popularization and application.
AimsThis study evaluates the long-term efficacy and safety of carbon dioxide (CO₂) laser excision as a novel treatment for large xanthelasma palpebrarum.MethodsThe study included 295 patients diagnosed with xanthelasma palpebrarum, categorised as 246 patients with grade I, 16 with grade II, 22 patients with grade III and 11 with grade IV lesions. All underwent CO₂ laser excision and were followed up for a 12-month period. Treatment efficacy was assessed through clearance and recurrence rates, evaluated using digital photography and complications were recorded.ResultsThe participant group consisted of 66 males and 229 females, with an average age of 41.7±11.6 years. Clearance rates exceeded 99% for grades I–III and were around 95% for grade IV lesions. The recurrence rate was 6.8%, with higher recurrence rates observed in lesions over 2 mm in height compared with those under 2 mm (p<0.001). The main complications over the 12-month follow-up were scarring (4.4%), hyperpigmentation (8.1%) and hypopigmentation (8.5%), with no severe complications reported.ConclusionCO₂ laser excision emerges as a precise, minimally invasive and effective modality for treating xanthelasma palpebrarum, marking a significant advancement in treatment modalities. Further research is required to reinforce these results and refine treatment protocols.
PURPOSE. Eyelid infiltrative basal cell carcinoma (iBCC) is the most common malignant tumor affecting the ocular adnexa, but studies on metabolic changes within its microenvironment and heterogeneity at the tumor invasive area are limited. This study aims to analyze metabolic differences among iBCC cell types using single-cell and spatial metabolomics analysis and to examine metabolic environment at the tumor invasive area. METHODS. Single-cell transcriptomic data of human basal cell carcinoma (BCC) were clustered and visualized using Uniform Manifold Approximation and Projection. Metabolic reprogramming was analyzed with single-cell flux estimation analysis. Spatial metabolomics data were obtained with the Timstof Flex MALDI 2 system, and Bruker software was used for region selection. RESULTS. Eight cell types were identified within the iBCC microenvironment. Differences between inflammatory cancer-associated fibroblasts and myofibroblastic cancer-associated fibroblasts were analyzed. Metabolic flux analysis showed increased glycolysis, glutamine, heme, and glutathione fluxes in the iBCC microenvironment. Spatial metabolomics revealed high levels of taurine, deoxy-GMP, O-phosphoethanolamine, and pyrithione. Both tumor and invasive regions had significant upregulation of fatty acid pathways, with marked increases in oleic and arachidonic acids at the invasive area. Specific upregulation of UDP-glucuronic acid and high UDP-glucose 6-dehydrogenase (UGDH) expression in the tumor region suggest UXS1 as a potential therapeutic target for iBCC. CONCLUSIONS. This study establishes a metabolic microenvironment atlas of iBCC, revealing significant metabolic differences and the dominance of lipid and lysosome metabolism. Potential metabolic markers and characteristic substances in the invasive area offer new insights for immunotherapy and the exploration of BCC's metabolic mechanisms.
BACKGROUND:Hypertrophic and keloid eyelid scars cause functional and aesthetic problems. Traditional fractional carbon dioxide (CO2) laser treatments require multiple sessions and use uniform parameters regardless of scar characteristics, limiting their effectiveness for complex eyelid scars. OBJECTIVES:In this study, the authors evaluate the efficacy and safety of the Manual One-shot Learning Technique (MOST) with fractional CO2 laser for personalized treatment of eyelid scars. METHODS:A retrospective study involved 154 patients with hypertrophic and keloid eyelid scars treated with the MOST fractional CO2 laser. Functional and aesthetic outcomes were assessed utilizing degree of incomplete eyelid closure, ectropion, and Vancouver Scar Scale (VSS). Patients were followed up for a total of 12 months to evaluate outcomes and monitor complications. RESULTS:The study included 154 patients (98 males, 56 females; mean age 32.6 ± 14.2 years). A single treatment session resulted in significant improvements, with a mean VSS score reduction of 2.75 ± 1.50 (P = .021), and 57.8% of patients achieving >75% scar clearance. After 1 to 3 treatment sessions, all patients showed significant functional and aesthetic improvements, with incomplete eyelid closure and ectropion significantly reduced (both P < .001), VSS scores decreased by 3.02 ± 1.76 (P < .001), and 90.9% of patients achieved >75% scar clearance. Complications included recurrence (10.4%), hypopigmentation (5.2%), and skin atrophy (5.2%). CONCLUSIONS:The MOST laser technique significantly improves functional and aesthetic outcomes for eyelid scars with fewer treatment sessions. By personalizing treatment based on tissue response, this approach enhances efficiency while reducing both patient burden and resource use. LEVEL OF EVIDENCE: 4 (THERAPEUTIC):
Aims: To analyse the characteristics and trends of non-tumour-related eye removal in southern China from 2009 to 2018. Methods: This retrospective observational study reviewed inpatient data from the Zhongshan Ophthalmic Center between 2009 and 2018 to identify the characteristics and trends of nontumour enucleation/evisceration. The chi-square test was used to assess differences in age, sex, and aetiology, whereas linear regression was used to analyse the significance of trends from 2009 to 2018 across subgroups. The nonquadratic Mann-Kendall test was used to evaluate trends in the aetiology and types of ocular injuries. Results: A total of 1824 patients (mean age: 28 years, standard deviation: 19.2) underwent nontumour-related eye removal during the study period. The major causes were trauma (72.6 %) and perforation (7.7 %). Work-related injuries constituted 30.5 % of ocular trauma cases, representing a moderate increase over the past decade. Nevertheless, no statistically significant trends were identified in the aetiology or specific causes of ocular trauma. Conclusions: From 2009 to 2018, the rate of nontumorous eye removal remained stable, with ocular trauma being the leading cause in southern China. Translational relevance: The findings highlight the need for preventive measures against workrelated injuries leading to enucleation and the importance of rigorous childcare to mitigate the risk of eye removal due to trauma.
Eyelid basal cell carcinoma (BCC), the most prevalent malignant tumor of the ocular adnexa, has seen a steady rise in global incidence, accompanied by a notable trend toward earlier onset. This trend not only exacerbates the public health burden but also imposes greater demands on clinical strategies for prevention and management. This article reviews the latest research progress in the diagnosis and treatment of eyelid BCC. In diagnostic practice, alongside conventional histopathological examination, the incorporation of novel technologies—such as dermoscopy, reflectance confocal microscopy (RCM), and optical coherence tomography (OCT)—has markedly enhanced diagnostic sensitivity and specificity, while offering distinct advantages in tumor subtype differentiation and margin assessment. Metabolomic analyses reveal pronounced metabolic reprogramming in eyelid BCC, with aberrant activation of lipid, NAD, polyamine, and glucose metabolism. Spatial metabolomics further supports the biomarker potential of metabolites such as taurine and pyrithione in tumor initiation and progression. Beyond conventional surgery, advances in radiotherapy, targeted Hedgehog pathway inhibitors, and immunotherapies have created new opportunities for advanced or metastatic eyelid BCC. This review highlights emerging strategies for prevention, diagnosis, and treatment, offering clinicians fresh perspectives for patient management.
Proper development of surface epithelium (SE) is a requisite for the normal development and function of ectodermal appendages; however, the molecular mechanisms underlying SE commitment remain largely unexplored. Here, we developed a KRT8 reporter system and utilized it to identify FOXO4 and SP6 as novel, essential regulators governing SE commitment. We found that the FOXO4-SP6 axis governs SE fate and its abrogation markedly impedes SE fate determination. Mechanistically, FOXO4 regulates SE initiation by shaping the SE chromatin accessibility landscape and regulating the deposition of H3K4me3. SP6, as a novel effector of FOXO4, activates SE-specific genes through modulating the H3K27ac deposition across their super-enhancers. Our work highlights the regulatory function of the FOXO4-SP6 axis in SE development, contributing to an improved understanding of SE fate decisions and providing a research foundation for the therapeutic application of ectodermal dysplasia.
Obtaining a full understanding of the anatomical structure of the eyeball and the contents within the eye plays a crucial role in learning eye diseases. However, it can be challenging for beginners to quickly learn, memorize, and apply this anatomical knowledge. To improve the teaching of ocular anatomy and help students overcome learning difficulties in ophthalmology clerkship, Zhongshan Ophthalmic Center developed an integrated ocular anatomy course comprising three main components: 'online preview,' 'lecture,' and 'wetlab dissection'. Fifty-five fourth-year medical students from Sun Yat-sen University were recruited for this study. The impact of the integrated course was evaluated through three in-class quizzes (T1: pre-teaching assessment, T2: post-wetlab training, and T3: end of clerkship) and assessments of students' self-confidence levels. Additionally, students' course evaluations were gathered via a questionnaire survey. Statistical analyses were conducted via ANOVA and the Kruskal–Wallis test. The integration course significantly improved the mean scores of post-teaching tests (67.08 ± 10.27 vs. 74.36 ± 12.75 vs. 78.81 ± 12.70, P < 0.0001) and the students’ self-confidence scores (P < 0.0001). Fifty-four participants were satisfied with the integrated anatomy course (79.3
AIM: To evaluate refraction and ocular biological characteristics in children with unilateral congenital ptosis. METHODS: In this cross-sectional study, 200 Chinese children (3-15y) with unilateral congenital ptosis were evaluated. Cycloplegic refraction and ocular biological measurements were taken from Oct. 2020 to Aug. 2022. RESULTS: In patients with congenital ptosis, the prevalence of with-the-rule astigmatism and oblique astigmatism was significantly high. The cornea was flatter in ptotic eyes [K1 (42.37±1.62 vs 42.78±1.51 D), K2 (43.8±1.86 vs 44.2±1.64 D), corneal power (Km; 43.09±1.68 vs 43.49±1.53 D), all P<0.001]. Axial length (AL) was longer in ptotic eyes (22.55 vs 22.51 mm, P=0.012). The white-to-white (WTW) was significantly smaller in ptotic eyes (11.49 vs 11.65 mm, P<0.001). The central corneal thickness (CCT) was greater in ptotic eyes (553.50 vs 545.00 µm, P<0.001). No significant differences were found in anterior chamber depth (AD), lens thickness (LT) and vitreous thickness (VT) between ptotic and fellow eyes (P>0.05). In addition, the incidence of amblyopia in ptosis eyes was 32.0%, which was significantly higher than that in the normal population. CONCLUSION: Ptotic eyes have longer AL, flatter, thicker and smaller corneas than fellow eyes. The congenital ptosis increases the risk of amblyopia. The results suggest that regular examinations of refractive status and ocular biological parameters such as AL, are essential for children with unilateral congenital ptosis.
Understanding how embryonic progenitors decode extrinsic signals and transform into lineage-specific regulatory networks to drive cell fate specification is a fundamental, yet challenging question. Here, we develop a new model of surface epithelium (SE) differentiation induced by human embryonic stem cells (hESCs) using retinoic acid (RA), and identify BMP4 as an essential downstream signal in this process. We show that the retinoid X receptors, RXRA and RXRB, orchestrate SE commitment by shaping lineage-specific epigenetic and transcriptomic landscapes. Moreover, we find that TCF7, as a RA effector, regulates the transition from pluripotency to SE initiation by directly silencing pluripotency genes and activating SE genes. MSX2, a downstream activator of TCF7, primes the SE chromatin accessibility landscape and activates SE genes. Our work reveals the regulatory hierarchy between key morphogens RA and BMP4 in SE development, and demonstrates how the TCF7-MSX2 axis governs SE fate, providing novel insights into RA-mediated regulatory principles.
Graves' disease (GD) is an autoimmune condition that can progress to Graves' ophthalmopathy (GO), leading to irreversible damage to orbital tissues and potential blindness. The pathogenic mechanism is not fully understood. In this study, we conducted single-cell multi-omics analyses on healthy individuals, patients with GD without GO, newly diagnosed patients with GO, and treated patients with GO. Our findings revealed gradual systemic inflammation during GO progression, marked by overactivation of cytotoxic effector T cell subsets, and expansion of specific T cell receptor clones. Importantly, we observed a decline in the immunosuppressive function of activated Treg (aTreg) accompanied by a cytotoxic phenotypic transition. In vitro experiments revealed that dysfunction and transition of GO-autoreactive Treg were regulated by the yin yang 1 (YY1) upon secondary stimulation of thyroid stimulating hormone receptor (TSHR) under inflammatory conditions. Furthermore, adoptive transfer experiments of the GO mouse model confirmed infiltration of these cytotoxic Treg into the orbital lesion tissues. Notably, these cells were found to upregulate inflammation and promote pathogenic fibrosis of orbital fibroblasts (OFs). Our results reveal the dynamic changes in immune landscape during GO progression and provide direct insights into the instability and phenotypic transition of Treg, offering potential targets for therapeutic intervention and prevention of autoimmune diseases.
Purpose: To evaluate the expression of sry-box transcription factor 9 (SOX9) in orbital fibroblasts (OFs) of thyroid eye disease (TED) and to find its potential role and underlying mechanism in orbital fibrosis. Methods: OFs were cultured from orbital connective tissues obtained from patients with TED (n = 10) and healthy controls (n = 6). SOX9 was depleted by small interfering RNA or overexpressed through lentivirus transduction in OFs. Fibroblast contractile activity was measured by collagen gel contraction assay and proliferation was examined by EdU assay. Transcriptomic changes were assessed by RNA sequencing. Results: The mRNA and protein levels of SOX9 were significantly higher in OFs cultured from patients with TED than those from healthy controls. Extracellular matrix-related genes were down-regulated by SOX9 knockdown and up-regulated by SOX9 overexpression in TED-OFs. SOX9 knockdown significantly decrease the contraction and the antiapoptotic ability of OFs, whereas the overexpression of SOX9 increased the ability of transformation, migration, and proliferation of OFs. SOX9 knockdown suppressed the expression of phosphorylated ERK1/2, whereas its overexpression showed the opposite effect. Epidermal growth factor receptor (EGFR) is one of the notably down-regulated genes screened out by RNA sequencing. Chromatin immunoprecipitation-qPCR demonstrated SOX9 binding to the EGFR promoter. Conclusions: A high expression of SOX9 was found in TED-OFs. SOX9 can activate OFs via MAPK/ERK1/2 signaling pathway, which in turn promotes proliferation and differentiation of OFs. EGFR was a downstream target gene of SOX9. SOX9/EGFR can be considered as therapeutic targets for the treatment of orbital fibrosis in TED.

Ptosis is a common ophthalmologic condition, and the diagnosis is primarily based on ocular appearance. The diagnosis of such conditions can be improved using emerging technology such as artificial intelligence-based methods. However, unified data collection and labeling standards have not yet been established. This directly impacts the accuracy of ptosis diagnosis based on appearance alone. Therefore, in the present study, we aimed to establish a procedure to obtain and label images to devise a recommendation system for optimal recognition of ptosis based on ocular appearances. This would help to standardize and facilitate data sharing and serve as a guideline for the development and improvisation of algorithms in artificial intelligence for ptosis.
Lutein (LU) is a carotenoid that has recently been implicated in multiple roles in fibrosis, inflammation, and oxidative stress. Thyroid-associated ophthalmopathy (TAO) is particularly relevant to these pathological changes. We thus aim to probe the potential therapeutic effects of TAO in an in vitro model. We used LU pre-treating OFs derived from patients with TAO or not, then treated with TGF-β1(or IL-1β)to induce fibrosis (or inflammation). We analyzed the different expressions of related genes and proteins, and the molecular mechanism pathway on TAO OFs was screened by RNA sequencing, which is identified in vitro. We found that LU attenuates fibrotic and inflammatory effects in TAO. LU inhibited ACTA2, COL1A1, FN1, and CTGF mRNA expression and suppressed α-SMA, and FN1 protein expression induced by TGF-β1. Besides, LU suppressed OFs migration. Besides, it is shown that LU suppressed inflammation-related genes, such as IL-6, IL-8, CXCL1, and MCP-1. Moreover, LU inhibited oxidative stress induced by IL-1β, which is analyzed by DHE fluorescent probe staining. RNA sequencing suggested ERK/AP-1 pathway may be the molecular mechanism of LU protective effect on TAO, which is identified by RT-qPCR and western-blot. In summary, this study provides the first evidence that LU significantly attenuates the pathogenic manifestations of TAO by inhibiting the expression of fibrotic and inflammation-related genes and ROS produced by OFs. These data suggested that LU may be a potential medicine for TAO.

The insights into how genome topology couples with epigenetic states to govern the function and identity of the corneal epithelium are poorly understood. Here, we generate a high-resolution Hi-C interaction map of human limbal stem/progenitor cells (LSCs) and show that chromatin multi-hierarchical organisation is coupled to gene expression. By integrating Hi-C, epigenome and transcriptome data, we characterize the comprehensive 3D epigenomic landscapes of LSCs. We find that super-silencers mediate gene repression associated with corneal development, differentiation and disease via chromatin looping and/or proximity. Super-enhancer (SE) interaction analysis identified a set of SE interactive hubs that contribute to LSC-specific gene activation. These active and inactive element-anchored loop networks occur within the cohesin-occupied CTCF-CTCF loops. We further reveal a coordinated regulatory network of core transcription factors based on SE-promoter interactions. Our results provide detailed insights into the genome organization principle for epigenetic regulation of gene expression in stratified epithelia.
Objective:This study is aimed at exploring the impact of eye model based on multichannel convolutional neural network (CNN) on eye plastic surgery and aesthetic effect, thus formulating methods to improve the effect of eye plastic surgery. Methods:A total of 64 patients who underwent pouch plastic surgery from January 2020 to March 2021 were selected as the research objects and were divided into observation group and control group by random number table method. The subjects in the observation group were evaluated by multichannel CNN-based eye model and doctors' experience, while those in the control group were evaluated by doctors' experience only, with 32 cases in both groups.
