Objective:To explore the efficacy of unarmed respiratory function training combined with electric standing bed in postural sputum excretion in elderly stroke patients.Methods:60 elderly stroke patients admitted to the Second Affiliated Hospital of Guangxi Medical University from January 2017 to July 2019 were selected. Among them, 30 patients (control group) received conventional position drainage and sputum removal, and the remaining 30 patients (observation group) received hands-free respiratory function training combined with electric standing bed sputum removal. The sputum removal effect and lung function before and after treatment were compared between the two groups. T test was used for the comparison of measurement data between groups, and χ2 test was used for the comparison of counting data.Results:The total effective rate after treatment was 96.67% (29/30) in the observation group and 76.67% (23/30) in the control group, the difference was statistically significant (χ2=5.193, P < 0.05). Compared with before treatment, the longest expiratory time, maximum gas volume and forced vital capacity in 2 groups were significantly increased after treatment (observation group: t=37.826, 9.209, 13.362; control group: t=22.347, 2.606, 6.746; P < 0.05). After treatment, the maximum expiratory time, maximum gas volume and forced vital capacity in the observation group were significantly increased compared with the control group, with statistical significance (t=26.302, 7.077, 9.74, P < 0.05).Conclusion:Unarmed respiratory function training combined with electric standing bed can significantly improve the sputum excretion effect and respiratory function of stroke patients, which is worthy of application.
目的 分析有压板和无压板两种类型的导航工具对数字化种植导板手术备孔精度的影响.方法 回顾性分析有压板导航工具和无压板导航工具植入的16例患者,共40颗种植体,每种工具各20颗.术前通过六维牙种植设计软件对种植体位点进行三维设计,术后拍摄CT并在六维牙种植设计软件中进行重建,与术前方案进行配准后导入Rhino软件中,对术前术后种植体颈部中心点距离偏差、底部中心点距离偏差、颈部垂直向偏差和角度偏差进行测量比较.结果 有压板导航工具植入的种植体颈部中心点距离偏差均值为1.243 mm,底部中心点距离偏差均值为1.911 mm,颈部垂直向偏差均值为1.201 mm,角度偏差均值为5.077°,无压板导航工具植入的种植体依次为0.696 mm、1.167 mm、0.449 mm、4.582°.结论 无压板的导航工具备孔精度高于有压板的导航工具备孔精度.
目的 利用模拟牙颌模型实现正畸弓丝初始回复力的口外测量.方法 以1例临床病例作为研究对象,通过3D打印制作与患者真实牙列分布相一致的上颌牙颌模型,并以此为基础利用Nano17 F/T力学传感器搭建矫治力测量系统,测量直径为355.6μm标准圆镍钛弓丝佩戴在牙颌模型上后施加在患者2颗上颌中切牙上的初始回复力.结果 患者上颌中切牙21受到的初始矫治力较中切牙11更大,且2颗中切牙所受到的矫治力系均不利于牙齿的理想移动,需要添加辅助装置改善牙齿受力状况.在优化正畸治疗方案后,患者在治疗18个月后获得满意的矫治效果.结论 利用患者的模拟牙颌模型可以准确测量正畸弓丝作用在目标牙齿上的矫治力系,预估牙齿的移动形式,并以此为基础改进牙齿矫治方案.矫治力的口外测量技术为临床正畸治疗提供重要参考,为矫治方案的优化设计开创新思路.
目的 探讨不同种植设计软件对种植体位置偏差的影响.方法 选取16位患者的颌骨CBCT数据,导入不同种植设计软件中进行方案设计,分别设计53颗种植体,在软件中依据种植体位置生成导板,采用光固化成型技术加工导板.将通过SimPlant软件设计制造的导板分为A组,通过6D Implant软件设计制造的导板分为B组.导板佩戴在石膏模型上进行CBCT扫描,将获取的数据进行三维重建并配准到种植方案,依据实际的导板位置模拟确定实际的种植体位置,分析种植体实际位置与设计位置的偏差.实验获取的数据采用SPSS 11.0统计学软件进行统计分析,并进行单侧t检验.结果 A组所引起的种植体实际与设计位置颈部中心偏差为(0.582±0.216)mm,水平向偏差为(0.226±0.132) mm,垂直向偏差为(0.522±0.213) mm;种植体顶端中心偏差为(0.643±0.256) mm,水平向偏差为(0.332±0.222)mm,垂直向偏差为(0.524 ±0.214) rm;体积重合度为(88.7±4.6)%.B组种植体颈部中心偏差为(0.456±0.222) mm,水平向偏差为(0.193 ±0.123)mm,垂直向偏差为(0.388±0.238) mm;种植体顶端中心偏差为(0.515±0.233)mm,水平向偏差为(0.277±0.190) mm,垂直向偏差为(0.390±0.236) mm;体积重合度为(91.1±4.0)%.结论 不同种植设计软件对种植体位置偏差的影响差异具有统计学意义.
To study the designing and fabrication processes of customized lingual brackets based on selective laser melting .Firstly ,select a appropriate case ,scan the plaster model of the patient and reconstruct the 3D model of the teeth . T hen arrange the teeth and the structure of the bracket is designed by 3D-software . Next , fabricate brackets by selective laser melting equipment .Finally ,the brackets should be polished and other relevant post process .The results show that the width of lingual brackets' slots after 3D printing is 0 .10 mm smaller than designed value ,the thickness of the base plate is 0 .16 mm larger than that of the designed value .After adjust the printing parameters ,the exact size is obtained and is finally applied to the orthodontic treatment of the patient successfully .Based on selective laser melting process ,the digital design and precise fabrication of the individual lingual brackets are come to be true .
OBJECTIVE To explore effects of peri-implantitis on proliferation and differentiation of mandibular oste-oblasts ,so as to provide references for clinical tretment .METHODS Seven cases of patients with implant loosening and were pulled out were collected as observation group ,and 7 cases of healthy pulled out third molars as control group .The jaw bone tissue of the two groups of patients were taken for isolation ,culture and purification of oste-oblasts .MMT was used to assay osteoblasts proliferation ,RT-PCR was used to assay osteoblasts Ⅰ collagen (Col Ⅰ) mRNA level ,and Western blot was used to detect Col I protein level of osteoblasts .RESULTS The OD value of osteoblasts of observation group was lower than that of control group at the 7th day (P<0 .05) ,the OD value of osteoblasts of observation group was lower than that of control group at the 3rd day ,but the difference was not significant .The expression of Col Ⅰ protein of osteoblasts of observation group was significantly lower than that of control group (P<0 .05 ) .CONCLUSION Peri-implantitis can inhibit the proliferation and differentia-tion of the mandibular osteoblasts .
PURPOSE:To evaluate the clinical effects of bolt-like post crown system in restoration of molar residual root and crown with low occlusogingival distance and large root bifurcation angle.METHODS:Thirty two molar residual roots and crowns from 25 patients with low occlusogingival distance (<4 mm) and large root bifurcation angle were involved in this study. First, the root canals were prepared according to their natural directions following the standard procedures. Accurate impressions were taken with silicone. The core crown and the post which would inserted the straight root canal were founded together,and the blot-like post which would inserted the root canal with bigger bifurcat were founded alone. Finally, the two parts were cemented together in patient after try-in with RelyX™ Unicem cement.RESULTS:Except 3 patients who were lost during the follow up period, the remaining 22 patients (29 restorations) were followed up for 1 to 5 years. No prosthetic loosening or root fracture was observed during the follow-up period. All the fixed restorations were found with good marginal fitness, well masticatory function and favorable proximal contact relationship.CONCLUSIONS:Bolt-like post crown system is an ideal method for restoration of molar residual root and crown with low occlusogingival distance and large root bifurcation angle. This approach has positive effect on the retention, function and aesthetic outcome of the fixed restorations of molar residual root and crown with low occlusogingival distance.
2005年以来,我院为200例(554颗牙)患者实施口腔固定正畸矫治,其中815颗牙发生釉质脱矿,经精心护理,效果满意.现报告如下. 1 临床资料 本组200例,男87例,女113例,12~14岁.术前已完成龋齿充填和牙周治疗,治疗过程中采取我科标准化的口腔健康教育及口腔护理操作.每次观察前清洁并干燥牙齿,观察牙面上出现不透明的白垩斑或小凹陷,探针探查表面粗糙或松软,则定为脱矿,记录牙位和脱矿位置.
