The liver toxicity of alkylphenols (APs) has been demonstrated in animal studies. However, relevant epidemiological evidence is still lacking in humans, especially during pregnancy. We obtained the levels of biochemical indicators of liver function in early (<13 weeks, mean gestation=9.80±1.96 weeks) and late (≥32 weeks, mean gestation = 37.23±2.45 weeks) pregnancies from 219 pregnant women in the Guangxi Zhuang birth cohort from 2015-2017. We also examined the serum levels of APs in these pregnant women in early pregnancy. The present study aimed to investigate the correlations between the exposure of pregnant women to APs and their serum liver function indices. The results of the generalized linear model (GLM) in this study revealed that nonylphenol (NP) was positively correlated with total bilirubin (TBIL) (P=0.04) in early pregnancy, and 4-n-nonylphenol (4-N-NP) was negatively correlated with glutamyl transferase (GGT) (P=0.012). In late pregnancy, NP was positively associated with TBIL (P=0.002), and 4-tert-octylphenol (4-T-OP) was positively correlated with alanine aminotransferase (ALT) (P=0.02). Restricted cubic spline (RCS) results revealed doseresponse relationships between NP and TBIL (Poverall=0.011) and between 4-N-NP and GGT (Poverall=0.007) in early pregnancy. In late pregnancy, there were doseresponse relationships between NP and TBIL (Poverall=0.001) and between 4-T-OP and ALT (Poverall=0.033). There was also a doseresponse relationship between NP volume and GGT with an inverted 'U' shape (Poverall=0.041, Pnonlinear=0.012). Bayesian kernel machine regression modeling (BKMR) revealed that TBIL increased significantly (P<0.05) with increasing levels of coexposure to APs in both early and late pregnancy. Overall, exposure to APs during pregnancy affects maternal liver function to varying degrees. The present study provides new epidemiological evidence that exposure to alkylphenols in pregnant women interferes with liver function.
Evidence from animal experiments suggests that exposure to bisphenols during early life may lead to impaired eye development and visual functions. However, population-based study on the association remains very limited. To investigate the relationships between prenatal bisphenols exposure and visual impairment in preschool children. A total of 744 mother-infant pairs were extracted from the Guangxi Zhuang Birth Cohort in China. Maternal serum bisphenol A (BPA) and its alternatives were measured using ultra-high liquid performance chromatography-tandem mass spectrometer. Visual acuity in preschool children were followed up in the local maternal and child health information management system. The associations of prenatal exposure to bisphenols (BPs) with visual acuity were analyzed by multivariate linear regression models. Logistic regression model, Bayesian Kernel machine regression (BKMR), and quantile g-computation (Qgcomp) models were applied to examine the single and mixture effects of prenatal exposure to the five BPs on risk of visual impairment (VI).In logistic regression models, the moderate-level BPA-exposed group (OR = 0.474, 95% CI 0.211, 1.065) and high-level TBBPA-exposed group (OR = 0.451, 95% CI 0.184, 1.109) had a lower risk of VI than the low-level exposed group, both approaching a level of significance ( P = 0.071 and P = 0.083, respectively). When stratified analysis by child sex, the significant associations of moderate-level BPA exposure and VI risk were only found among boys (adjusted OR = 0.230, 95% CI 0.061, 0.873, P = 0.031). And a suggestive negative association of high-level TBBPA exposure and VI risk was only found among girls (adjusted OR = 0.330, 95% CI 0.091, 1.193, P = 0.091). Further analysis using BKMR and g-computation models showed that mixed effects of the five BPs were also associated with decreased risk of VI, with effects-driven primarily by BPA and TBBPA for boys and girls, respectively. Findings from this study do not support the hypothesis that prenatal BPs exposure is associated with increased risk of VI. Further epidemiological studies remain warranted when confirming their associations.
Prenatal exposure to fine particulate matter (PM2.5) has been linked with increased neurodevelopmental disorders. However, the most detrimental component of PM2.5 and the most vulnerable exposure time windows remain undetermined, especially in areas with high PM2.5 levels. In a prospective cohort study involving 4494 mother-child dyads, we examined the associations of prenatal exposure to PM2.5 and its four main components with children's neurodevelopmental and behavioral problems (NBPs), separately in three pregnancy trimesters. Poisson regression and generalized additive models were used to depict the linear and nonlinear associations, respectively. Weighted quantile sum and Bayesian kernel machine regression models were applied to examine the effects of exposure to both mixed and individual components. Results showed that exposure to PM2.5 and its components throughout the three trimesters increased the risk of children's NBPs (Risk ratio for PM2.5: 1.16, 95 % confidence interval 1.14-1.18 per mu g/m(3) in the first trimester; 1.15, 1.12-1.17 in the second trimester; 1.06, 1.04-1.08 in the third trimester), with associations gradually diminishing as pregnancy progressed (P values for trends < 0.05). Among the four main components of PM2.5, exposure to SO42- posed the highest risks on children's NBPs, while organic matter contributed the largest proportion to the overall impacts of PM2.5 exposure. These results underscore the significance of mitigating PM2.5 exposure in pregnant women to reduce the risk of neurodevelopmental disorders in offspring. Our findings would inform risk assessment of PM2.5 exposure and facilitate the development of precision preventive strategies targeting specific components of PM2.5 in similar areas with high levels of exposure.
Abstract Background Glyphosate is a commonly used herbicide worldwide and is purportedly associated with multiple health effects. Research assessing the association of glyphosate concentrations with glycosylated hemoglobin (HbA1c) levels and the prevalence of diabetes is scarce. We sought to evaluate the association between urinary glyphosate levels and HbA1c levels and the prevalence of diabetes. Methods A total of 2,745 adults in the National Health and Nutrition Examination Survey from 2013 to 2016 were included in this study. Generalized linear models (GLM) were applied to evaluate the associations of glyphosate concentrations with HbA1c levels and the prevalence of diabetes. The dose–response relationship was examined using restricted cubic splines (RCS). Results Significantly positive correlations of urinary glyphosate concentrations with HbA1c levels (percentage change: 1.45; 95% CI: 0.95, 1.96; P < 0.001) and the prevalence of diabetes (OR: 1.45; 95% CI: 1.24, 1.68; P < 0.001) were found after adjustment. Compared with the lowest quartile of glyphosate levels, the highest quartile was positively associated with HbA1c levels (percentage change: 4.19; 95% CI: 2.54, 5.85; P < 0.001) and the prevalence of diabetes (OR: 1.89; 95% CI: 1.37, 2.63; P < 0.001). The RCS curves demonstrated a monotonically increasing dose–response relationship between urinary glyphosate levels and the prevalence of diabetes and HbA1c levels. Conclusions Urinary glyphosate concentrations are positively associated with HBA1c levels and the prevalence of diabetes. To verify our findings, additional large-scale prospective investigations are required.
Background After decades of rapid economic development, anemia remains a significant public health challenge globally. This study aimed to estimate the associations of sociodemographic, dietary, and body composition factors with anemia among the Zhuang in Guangxi Zhuang Autonomous Region, China. Methods Our study population from the baseline survey of the Guangxi ethnic minority Cohort Study of Chronic Diseases consisted of 13,465 adults (6,779 women and 6,686 men) aged 24–82 years. A validated interviewer-administered laptop-based questionnaire system was used to collect information on participants’ sociodemographic, lifestyle, and dietary factors. Each participant underwent a physical examination, and hematological indices were measured. Least absolute shrinkage and selection operator (LASSO) regression was used to select the variables, and logistic regression was applied to estimate the associations of independent risk factors with anemia. Results The overall prevalences of anemia in men and women were 9.63% (95% CI: 8.94 – 10.36%) and 18.33% (95% CI: 17.42 ─ 19.28%), respectively. LASSO and logistic regression analyses showed that age was positively associated with anemia for both women and men. For diet in women, red meat consumption for 5–7 days/week (OR = 0.79, 95% CI: 0.65 – 0.98, p = 0.0290) and corn/sweet potato consumption for 5–7 days/week (OR = 0.73, 95% CI: 0.55 – 0.96, p = 0.0281) were negatively associated with anemia. For men, fruit consumption for 5–7 days/week (OR = 0.75, 95% CI: 0.60–0.94, p = 0.0130) and corn/sweet potato consumption for 5–7 days/week (OR = 0.66, 95% CI: 0.46–0.91, p = 0.0136) were negatively correlated with anemia. Compared with a normal body water percentage (55–65%), a body water percentage below normal (< 55%) was negatively related to anemia (OR = 0.68, 95% CI: 0.53–0.86, p = 0.0014). Conversely, a body water percentage above normal (> 65%) was positively correlated with anemia in men (OR = 1.73, 95% CI: 1.38–2.17, p < 0.0001). Conclusions Anemia remains a moderate public health problem for premenopausal women and the elderly population in the Guangxi Zhuang minority region. The prevention of anemia at the population level requires multifaceted intervention measures according to sex and age, with a focus on dietary factors and the control of body composition.
随着我国经济社会的发展和生育政策的改变,社会生态环境不断变化,生命早期不良暴露日益增加,我国孕产妇发生妊娠疾病及新生儿不良出生结局形势依然严峻.多哈理论认为,生命早期发生的不良事件可能会影响成年后各类疾病的发生,这种影响甚至会持续好几代人.出生队列主要是在孕前或孕期招募参与者,对孕产期母亲健康与子代近远期健康结局进行跟踪随访,进而发现影响全生命周期健康结局的多种因素,检验病因假说.目前国内已有不少具有相当规模的出生队列,但其大多数主要面向汉族人群.我国是一个多民族国家,许多疾病的发生具有地域和民族分布特点,壮族是我国人口最多的少数民族,但目前关于壮族人群的大型流行病学调查十分有限,无法系统概括壮族人群和壮族母婴健康特点,提出面向壮族人群不良健康结局的有效防治措施.因此,有必要建立一个以壮族人群为主体的大规模出生队列,系统描绘壮族人群和壮族母婴健康特点,探索生命早期暴露与孕妇及子代近远期健康结局的关系,为有效降低壮族人群不良健康结局的发生风险,提高出生人口素质和促进妇女儿童健康水平提供重要依据.本文系统介绍了广西壮族人群出生队列构建的背景、过程、主要研究进展、创新性以及未来展望.
BACKGROUND:Gestational anemia is a complication of pregnancy, and a low level of hemoglobin (Hb) has been linked to adverse pregnancy outcomes. Previous studies reported that PFASs were more strongly associated with Hb than red blood cells, indicating that Hb is more susceptible to the effect of PFASs. However, the evidences regarding the effects of per- and polyfluoroalkyl substances (PFASs) on gestational anemia are currently limited. Therefore, it is important to explore the effects of PFASs on anemia in Chinese pregnant women.METHODS:A total of 821 pregnant women were recruited between June 2015 and April 2019 in the Guangxi Zhuang Birth Cohort. The concentrations of PFASs were assessed in maternal serum before 12 gestational weeks. To determine both individual and combined associations of PFASs exposure with anemia in the three stages of pregnancy, binary logistic regression, Bayesian kernel machine regression (BKMR), and weighted quantile sum (WQS) regression models were employed.RESULTS:In single-pollutant analysis, maternal exposure to perfluorododecanoic acid (PFDoA) and perfluoroheptanoic acid (PFHpA) were associated with anemia in the first trimester, exposure to PFHpA and perfluorobutanesulfonic acid (PFBS) were associated with anemia in the second trimester, and exposure to perfluorodecanoic acid (PFDA) and perfluorononanoic acid (PFNA) were associated with anemia in the third trimester. Notably, perfluoroundecanoic acid (PFUnA) had a nonlinear association with anemia in the third trimester. In multiple-pollutant analysis, a positive association of PFDoA with anemia in the first trimester and a negative association of PFBS with anemia in the second trimester were confirmed by BKMR. Exposure to PFASs mixture was not associated with anemia in all three trimesters. In WQS, there was a significantly negative association between the PFAS mixture and anemia in the second trimester.CONCLUSION:Maternal exposure to PFASs is associated with gestational anemia in different trimesters.
目的:探讨孕妇孕早期血浆钼浓度与新生儿出生体重和身长的相关性.方法:选取2015年6月至2018年6月广西壮族出生队列纳入的2 031名孕妇及其新生儿作为研究对象,采用广义线性模型(GLM)和限制性立方样条(RCS)分析孕妇孕早期血浆钼浓度与新生儿出生体重和身长的关联强度和剂量—反应关系.结果:不同孕前体重指数、孕次、产次、新生儿性别的孕妇分娩的新生儿出生体重和出生身长比较,差异有统计学意义(P<0.05);GLM分析结果显示,钼中浓度组和高浓度组孕妇分娩的新生儿出生体重较低浓度组分别下降39.327 g(95%CI:-76.686,-1.969)、71.751 g(95%CI:-109.345,-34.156),钼中浓度组和高浓度组孕妇分娩新生儿出生身长较低浓度组分别短0.205 cm(95%CI:-0.368,-0.042)、0.213 cm(95%CI:-0.377,-0.049);RCS显示,孕妇孕早期血浆钼浓度与新生儿出生体重之间存在剂量—反应关系(Poverall=0.008),但不存在非线性关系(Pnonlinear=0.285).结论:孕妇孕早期血浆高水平钼浓度与较低的新生儿出生体重和出生身长有关.
目的 了解广西35~74岁壮族人群睡眠情况及其影响因素,为改善广西壮族人群睡眠质量提供参考依据.方法 通过"广西少数民族自然人群慢性病前瞻性队列研究"基线调查数据,选择11457名35~ 74岁的广西壮族人群为研究对象,应用匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)量表评价研究对象的睡眠情况,采用x2检验以及多因素log-binomial回归模型分析影响睡眠质量的因素.结果 共纳入研究对象11457例,平均年龄为(54.09±10.45)岁,PSQi总均分为(4.86±2.95)分,睡眠不良发生率为18.67%(2139/11457).多因素log-binomial回归分析结果显示,女性(PR=1.169,95%CI:1.059~ 1.291,P=0.002)、高龄(PR=1.214,95%CI:1.118~ 1.317,P<0.001)、低教育水平(PR=1.110,95%CI:1.022~ 1.206,P=0.013)、单身(PR=1.142,95%CI:1.019~1.272,P=0.019)、有夜宵习惯(PR=1.459,95%CI:1.294~1.636,P<0.001)以及没有午睡习惯(PR=1.616,95%CI:1.496~1.745,P<0.001)是睡眠不良的影响因素.结论 广西35~74岁壮族中老年人群睡眠不良发生率较高.性别、年龄、教育水平、婚姻状况、夜宵习惯和午睡习惯为主要影响因素.需进一步加大力度开展睡眠健康教育活动,针对睡眠影响因素进行干预,积极改善中老年人群睡眠质量.
Background Long non-coding RNAs (lncRNAs) are involved in the development of hepatocellular carcinoma (HCC). We aimed to investigate the function of LINC01146 in HCC. Methods The expression of LINC01146 in HCC tissues was explored via The Cancer Genome Atlas (TCGA) and Gene Expression Omnibus (GEO) databases and was verified using quantitative real-time polymerase chain reaction (qRT–PCR) in our HCC cohort. Kaplan–Meier analysis was used to assess the relationship between LINC01146 and the prognosis of HCC patients. Cell Counting Kit 8, colony formation assays, Transwell assays, flow cytometric assays, and tumour formation models in nude mice were conducted to reveal the effects of LINC01146 on HCC cells both in vitro and in vivo. Bioinformatic methods were used to explore the possible potential pathways of LINC01146 in HCC. Results LINC01146 was significantly decreased in HCC tissues compared with adjacent normal tissues and was found to be related to the clinical presentations of malignancy and the poor prognosis of HCC patients. Overexpression of LINC01146 inhibited the proliferation, migration, and invasion of HCC cells in vitro, while promoting their apoptosis. In contrast, downregulation of LINC01146 exerted the opposite effects on HCC cells in vitro. In addition, overexpression of LINC01146 significantly inhibited tumour growth, while downregulation of LINC01146 promoted tumour growth in vivo. Furthermore, the coexpressed genes of LINC01146 were mainly involved in the “metabolic pathway” and “complement and coagulation cascade pathway”. Conclusion LINC01146 expression was found to be decreased in HCC tissues and associated with the prognosis of HCC patients. It may serve as a cancer suppressor and prognostic biomarker in HCC.
目的 了解广西壮族老年人高血压患病情况及其影响因素,为针对该人群开展高血压综合防治提供科学依据.方法 采用方便抽样的方法,在2017-2019年期间对广西壮族自治区内年龄为60~74岁的4 251名壮族人群开展问卷调查、体格检查和血生化指标检测,了解广西壮族老年人的高血压患病情况.采用多因素非条件logistic回归分析高血压患病的影响因素,采用优势分析法计算影响因素对高血压患病的贡献度.结果 广西壮族老年人的高血压患病率为51.96%.多因素非条件logistic分析结果显示,年龄增长(OR = 1.05,95%CI:1.03~1.84)、男性(OR = 1.39,95%CI:1.23~1.58)、腰围异常(OR = 1.61,95%CI:1.39~1.84)、体质指数异常(OR = 1.19,95%CI:1.04~1.36)、甘油三酯(triglyceride,TG)异常(OR = 1.51,95%CI:1.30~1.75)和总胆固醇(total cholesterol,TC)异常(OR = 1.15,95%CI:1.02~1.31)是高血压患病的危险因素(P<0.05).优势分析结果显示,影响因素对高血压患病贡献大小的前三位分别为腰围异常(33.31%)、TG异常(24.18%)、年龄(18.12%).结论 在广西壮族老年群体中,超过一半的老年人患有高血压,高血压患病与年龄、性别、腰围、BMI、TG和TC有关.控制腰围和降低血脂是该群体高血压防治的重点干预措施.
Objective: To introduce and compare four analysis methods of multiple parallel mediation model, including pure regression method, method based on inverse probability weighting, extended natural effect model method and weight-based imputation strategies. Methods: For the multiple parallel mediation model, the simulation experiments of three scenarios were carried out to compare the performance of different methods in estimating direct and indirect effects in different situations. Dataset from UK Biobank was then analyzed by using the four methods. Results: The estimation biases of the regression method and the inverse probability weighting method were relatively small, followed by the extended natural effect model method, and the estimation results of the weight-based imputation strategies were quite different from the other three methods. Conclusions: Different multiple parallel mediation analysis methods have different application situations and their own advantages and disadvantages. The regression method is more suitable for continuous mediator, and the inverse probability weighting method is more suitable for binary mediator. The extended natural effect model method has better performances when the residuals of two parallel mediators are positively correlated and the correlation degree is small. The weight-based imputation strategies might not be appropriate for parallel mediation analysis. Therefore, appropriate methods should be selected according to the specific situation in practice.
目的 探讨孕妇分娩年龄和初潮年龄间隔年数与大于胎龄儿(large for gestational age,LGA)的相关性.方法 选取2015年6月-2018年4月在广西壮族自治区百色市5个县孕早期建卡产检2 032名孕妇为研究对象.分析孕妇间隔年龄与LGA发生的相关性,接收者操作特征(receiver operating characteristic,ROC)曲线分析孕妇间隔年龄在预测LGA发生中的作用.结果 队列随访至2 032名孕妇分娩,其中发生LGA 67例,发生率为3.3%.LGA组孕妇分娩年龄、间隔年龄和BMI较非LGA组大(均有P<0.05).相关性分析中,校正混杂因素后,孕妇间隔年龄与出生体重无相关(P=0.178),但间隔年龄≥30岁的孕妇LGA发生风险是间隔年龄<10岁的29.214倍(P=0.018).ROC曲线分析显示,孕妇间隔年龄预测总体、分娩女婴的AUC分别为0.585、0.644,孕妇间隔年龄预测LGA的最佳截断点均为13岁.结论 孕妇分娩年龄和初潮年龄间隔年数与LGA发生有关,间隔年数越大则LGA发生风险越大.分娩年龄和初潮年龄间隔在女婴LGA发生预测中效果更好.
The association between co-exposure to multiple metals and renal function is poorly understood. We aimed to evaluate the individual and joint effects of metal exposure on renal function in this study. We performed a cross-sectional study including 5828 participants in Guangxi, China, in 2019. Urine concentrations of 17 metals were detected by inductively coupled plasma mass spectrometry (ICP-MS). Logistic regression model and restricted cubic spline (RCS) were applied to investigate the association of individual metal exposure with renal dysfunction. Weighted quantile sum (WQS) regression and Bayesian kernel machine regression (BKMR) were used to assess the co-exposure effects of the metals. Participants with the highest quartile of urinary Cu were at 1.84-fold (95% confidence interval (CI): 1.20-2.87) increased risk of renal dysfunction compared with the lowest quartile. The highest quartiles of urinary Sr, Cs, V, Ba, and Se were associated with 0.27-fold (95% CI: 0.17-0.43), 0.33 (95% CI: 0.19-0.53), 0.41 (95% CI: 0.25-0.65), 0.58 (95% CI: 0.36-0.90), and 0.33 (95% CI: 0.19-0.56) decreased risk of renal dysfunction compared with their lowest quartile, respectively. Furthermore, urinary Ba and Cu were non-linearly correlated with renal dysfunction. The WQS analysis showed that mixed metal exposure was inversely associated with renal dysfunction (OR = 0.47, 95% CI: 0.35-0.62), and Sr accounted for the largest weight (52.2%), followed by Cs (32.3%) in the association. Moreover, we observed a potential interaction between Cu, Cs, and Ba for renal dysfunction in BKMR model. Exposure to Se, Sr, Cs, V, and Ba is associated with decreased risk of renal dysfunction, whereas an increased risk is associated with Cu exposure. Co-exposure to these metals is negatively associated with renal dysfunction, and Sr and Cs are the main contributors to the associations.
目的:了解广西35~74岁壮族居民慢性肾脏病(CKD)患病现状及影响因素,探讨CKD与代谢综合征(MS)的关系,为CKD筛查和防制措施的制定提供依据.方法:基于广西少数民族自然人群队列对35~74岁壮族居民进行问卷、身体指标资料收集;采用Logistic回归分析CKD的影响因素,采用趋势性卡方检验分析CDK患病率与MS代谢紊乱因素个数的关系.结果:共纳入研究对象10703例,平均年龄(55.6±10.4)岁,CKD患病率为11.8%,男性患病率(13.7%)高于女性(10.3%);肾功能下降和蛋白尿阳性率分别为2.9%和8.9%.MS患者中CKD患病率为15.8%,CKD患病率、肾功能下降和蛋白尿阳性率随MS代谢紊乱因素个数增多而升高(P<0.001).多因素Logistic回归显示,男性(OR=2.031)、高年龄(OR=11.672)、高血压(OR=1.484)、高血糖(OR=1.476)、低HDL-C血症(OR=2.059)、高尿酸血症(OR=4.252)、高胆固醇血症(OR=2.216)是肾功能下降的危险因素(均P<0.05);吸烟(OR=1.392)、高血压(OR=1.212)、高血糖(OR=2.602)、低HDL-C血症(OR=1.171)、中心性肥胖(OR=1.199)、尿素升高(OR=2.639)是蛋白尿阳性的危险因素(均P<0.05).结论:广西35~74岁壮族居民CKD患病率较高,肾功能下降、蛋白尿阳性率与MS代谢紊乱因素个数密切相关,加强MS防控可有效降低CKD的发生.
目的:分析广西地区35~74岁壮族人群的高血压患病情况及其影响因素,为广西地区该年龄段人群制定高血压防治策略提供依据.方法:利用"广西少数民族自然人群慢性病前瞻性队列研究"基线调查数据,以35~74岁广西壮族人群为研究对象,分析该人群高血压患病情况,并采用χ2检验及多因素log-binomial回归模型探讨高血压相关影响因素.结果:共纳入35~74岁广西地区壮族居民10860名,其中高血压患者3991名,高血压粗患病率为36.75%,标准化高血压患病率为31.88%,男性患者的患病率为18.31%(1988/10860),女性患者的患病率为18.44%(2003/10860).经多因素log-binomial回归分析结果显示,高血压患病与年龄增长(45~54岁,PR=1.87,95%CI:1.68~2.07;55~64岁,PR=2.83,95%CI:2.56~3.14;65~74岁,PR=3.33,95%CI:3.00~3.69)、男性(PR=1.18,95%CI:1.11~1.24)、文化程度高(小学及初中,PR=0.92,95%CI:0.86~0.97;高中以上,PR=0.87,95%CI:0.80~0.95)、吸烟(PR=0.90,95%CI:0.84~0.97)、饮酒(PR=1.09,95%CI:1.02~1.16)、家庭年收入高(PR=0.93,95%CI:0.88~0.99)、高血压家族史(PR=1.23,95%CI:1.17~1.30)、甘油三酯(TG)过高(PR=1.16,95%CI:1.10~1.22)、低密度脂蛋白胆固醇(LDL-C)过高(PR=1.15,95%CI:1.06~1.25)、体重指数(Ibm)过大(PR=1.31,95%CI:1.24~1.38)和腰围过大(PR=1.12,95%CI:1.06~1.18)有关(P<0.05).结论:广西地区35~74岁壮族居民高血压患病率处于较高水平,年龄增长、文化程度、家庭年收入、吸烟、饮酒、高血压家族史、LDL-C、TG、性别、Ibm和腰围异常是广西壮族35~74岁人群发生高血压的主要影响因素.相关部门应当针对上述影响因素采取有效措施,为广西地区该年龄段人群制定高血压防治策略,降低高血压的发生率.
目的:了解广西35~74岁壮族居民高尿酸血症(HUA)患病情况及影响因素,分析相关指标与HUA的剂量-反应关系,为制定有针对性的防治措施和病人管理提供科学依据.方法:基于广西少数民族自然人群慢性病前瞻性队列研究,对纳入对象开展问卷调查、体格检查并采集血液、尿液标本进行常规和生化检测,采用Logistic回归分析HUA的影响因素,并用限制性立方样条图分析相关指标与HUA的剂量-反应关系.结果:广西35~74岁壮族居民HUA的粗患病率和标化患病率分别为24.5%和29.6%;多因素分析显示,男性(OR=1.48,95%CI:1.31~1.68)、55~64岁(OR=1.20,95%CI:1.03~1.39)、65~74岁(OR=1.62,95%CI:1.38~1.90)、初中(OR=1.12,95%CI:1.00~1.26)、高中(OR=1.36,95%CI:1.16~1.58)、大专及以上(OR=1.73,95%CI:1.33~2.24)、长期用药史(OR=1.20,95%CI:1.06~1.36)、饮酒(OR=1.81,95%CI:1.57~2.09)、饮茶(OR=1.18,95%CI:1.04~1.35)、高血压(OR=1.18,95%CI:1.07~1.31)、超重(OR=1.85,95%CI:1.67~2.05)、肥胖(OR=2.85,95%CI:2.45~3.31)、高甘油三酯(TG)血症(OR=1.73,95%CI:1.52~1.96)、高总胆固醇(TC)血症(OR=1.42,95%CI:1.24~1.62)、低密度脂蛋白胆固醇(LDL-C)血症(OR=1.60,95%CI:1.39~1.83)是HUA发生的独立危险因素;限制性立方样条图显示,体重指数(Ibm)、收缩压(SBP)、舒张压(DBP)、TG、TC、高密度脂蛋白胆固醇(HDL-C)与HUA的发生均存在剂量-反应关系.结论:广西35~74岁壮族居民HUA患病率较高,多种与不良生活方式密切相关的指标与HUA患病风险呈剂量-反应关系,今后健康干预应针对这些指标制定具体的控制目标,减少HUA的患病风险.
目的 调查广西农村地区不同民族人群的肝功能状况及其影响因素,为制订相关预防措施提供依据.方法 基于"广西少数民族慢性病前瞻性队列研究"平台,于2017-2019年在广西南宁市良庆镇、那马镇等农村地区开展调查.抽取研究对象空腹静脉血5 mL,用于检测肝功能、血生化等指标.结果 本次纳入调查人数13258人,来自汉、壮、瑶、侗等多个民族,平均年龄53.93岁,其中男性6562人(49.49%)、女性6696人(50.51%).不同民族人群血清ALT(F=13.002,P<0.001)、AST (F=9.344,P<0.001)、ALP(F=7.498,P<0.001)、Tbil(F=87.426,P<0.001) 水平差异有统计学意义,壮族人群较低.不同性别、不同民族人群肝功能异常率差异有统计学意义,女性(x2=485.478,P<0.001,异常率为14.4%)和壮族人群(x2=228.487,P<0.001,异常率为20.8%)较低.肝功能异常者中,59岁及以下的中青年人群占76.55%.影响ALT的因素是年龄(OR=0.969,P<0.001)、性别(OR=2.903,P<0.001)、家庭收入(OR=1.065,P=0.029)、体脂率(OR=1.010,P<0.001);影响AST的因素是性别(OR=2.037,P<0.001)、家庭收入(OR=0.930,P=0.048);影响ALP的因素是年龄(OR=1.027,P<0.001)、民族(OR=0.664,P=0.010)、家庭收入(OR=0.903,P=0.011)、体脂率(OR=1.006,P=0.003);影响Tbil的因素是性别(OR=2.723,P<0.001)、民族(OR=0.813,P=0.010).结论 广西农村地区壮族人群的主要肝功能指标水平及异常率均低于汉、侗等民族;女性肝功能指标水平较低;肝功能异常者主要是中青年个体.年龄、性别、家庭收入、体脂率是影响广西农村地区人群肝功能的主要因素.
目的:探讨广西壮族35~74岁人群性别、年龄对体重指数(Ibm)与高血压患病风险关联的修饰效应.方法:选择"广西少数民族自然人群慢性病前瞻性队列研究"项目中35~74岁壮族人群12535名作为研究对象.采用多因素logistic回归分析基线Ibm、性别Ibm、年龄别Ibm与高血压患病风险的关系.结果:共有4623名(36.88%)患有高血压,其中男2339名(38.56%),女2284名(35.31%).以Ibm< 18.5 kg/m2为参照,总人群高血压患病风险随着Ibm的增加逐渐升高(趋势P< 0.05).<45岁人群的高血压患病风险随着Ibm的增加,呈现先下降后升高的J型曲线;45~59岁人群及≥60岁人群患高血压的风险随着Ibm的增加逐渐升高,与总人群趋势一致.各年龄段内Ibm为18.5~23.9 kg/m2、24.0~27.9 kg/m2、≥28.0 kg/m2的人群其高血压患病风险(OR值)分别为,<45岁:0.48、1.01、2.12;45~59岁:1.33、2.41、3.69;≥60岁:1.61、2.79、3.26.Ibm与高血压患病风险关联的性别差异则在45~59岁人群中最显著,且性别与Ibm存在交互作用(P=0.002),该年龄段女性与男性高血压患病风险(OR值)分别为18.5~23.9 kg/m2:1.09 vs.2.04;24.0~27.9 kg/m2:1.73 vs.4.27;≥28.0 kg/rn2:2.35 vs.7.61.结论:Ibm是罹患高血压的影响因素,其与高血压患病风险的关联受到年龄、性别因素的修饰,<45岁人群需注意Ibm过低可能对血压产生的不良影响,而45~59岁人群男性则更应注意Ibm增加所导致高血压患病风险的快速上升.
目的:了解广西35~74岁壮族人群腹型肥胖流行情况及其影响因素,为腹型肥胖的早期预防控制提供参考依据.方法:利用"广西少数民族自然人群慢性病前瞻性队列研究"基线调查数据,以11 031名35~74岁广西壮族人群为研究对象并分析该人群腹型肥胖的流行情况,采用x2检验及多因素log-binomial回归模型探讨腹型肥胖的影响因素.结果:11 031名35?74岁广西壮族人群中,腹型肥胖3 355名,腹型肥胖率为30.4%,女性(33.0%)高于男性(27.2%)(P<0.05).经多因素log-binomial回归分析结果显示,年龄≥55岁(PR=1.20,95%CI:1.13~1.27)、喜食水果(PR=1.08,95%CI:1.02~1.15)是广西壮族人群腹型肥胖的危险因素(P<0.05),高中及以上学历(PR=0.88,95%CI:0.78~0.98)、工作性质为体力劳动工作(PR=0.83,95%CI:0.78~0.89)、吸烟(PR=0.73,95%CI:0.66~0.80)是广西壮族人群腹型肥胖的保护因素(P<0.05).结论:广西壮族人群腹型肥胖率较高,年龄、文化程度、职业、吸烟及水果偏好是广西壮族人群发生腹型肥胖的主要影响因素,应针对上述影响因素制订合理的政策和措施,从而降低腹型肥胖率.
