Iodine excess typically affects thyroid function in the human body and may damage carotid artery. Four investigation plots with different water iodine levels were selected in Shandong Province, China. These included a low, medium, and high iodine group and an iodine excess group whose water iodine content was < 10, 50–150, 150–300, and > 300 μg/L, respectively. Residents aged 20–65 years answered a questionnaire and underwent carotid artery ultrasonography, and their height, weight, and urinary iodine concentrations were measured. A total of 2026 individuals participated in the study. Urinary iodine concentration increased with increased water iodine levels. The medial thickening rate and intimal roughness rate in the iodine excess group were significantly higher than in the other three groups. After controlling for factors such as gender, age, and BMI, iodine excess remained as a risk factor for carotid intima-media thickening. Excess water iodine in the external environment is a risk factor for intima-media thickening of the carotid artery, suggesting that iodine excess may cause vascular injury and promote atherosclerosis.
Objective The aim of this study was to evaluate the effects of long-term excessive iodine intake on blood lipids in adults. Methods Three counties from Dezhou city and Liaocheng city in the Shandong province of China were selected as survey locations. Three to five villages were selected from each county and then categorized by the iodine concentration detected in the groundwater into Low (<10 µg/L), Medium (10–150 µg/L), High (150–300 µg/L), and Excessive (>300 µg/L) groups. A self-reported questionnaire was completed by each subject to provide demographic characteristics. Body height, weight, and blood pressure were recorded by trained staff. Blood lipids were measured. Results A total of 2156 subjects were recruited for the final analysis. The serum triglyceride (TRIG) was significantly higher in the Excessive group than in the other three groups ( P < 0.05). Total cholesterol (TCHOL) and low-density lipoprotein-cholesterol (LDL-C) showed downward trends with the increases in the water iodine concentration. A statistical significance of the crude correlation coefficient was detected between the water iodine concentration and the TRIG, TCHOL, or LDL-C ( P < 0.05). A significant correlation was also noted between the water iodine concentration and TCHOL or LDL-C after adjustment for covariates. High iodine concentration was a significant protective factor for TCHOL and LDL-C in adults, whereas elevated BMI and advancing age were risk factors for both variables. Conclusion An association was identified between iodine excess and low TCHOL or LDL-C. In areas with excessive iodine, iodine intake should be considered in studies examining the factors that influence blood lipids.

County-based spatial distribution characteristics and the related geological factors for iodine in drinking-water were studied in Shandong Province (China). Spatial autocorrelation analysis and spatial scan statistic were applied to analyze the spatial characteristics. Generalized linear models (GLMs) and geographically weighted regression (GWR) studies were conducted to explore the relationship between water iodine level and its related geological factors. The spatial distribution of iodine in drinking-water was significantly heterogeneous in Shandong Province (Moran's I = 0.52, Z = 7.4, p < 0.001). Two clusters for high iodine in drinking-water were identified in the south-western and north-western parts of Shandong Province by the purely spatial scan statistic approach. Both GLMs and GWR indicated a significantly global association between iodine in drinking-water and geological factors. Furthermore, GWR showed obviously spatial variability across the study region. Soil type and distance to Yellow River were statistically significant at most areas of Shandong Province, confirming the hypothesis that the Yellow River causes iodine deposits in Shandong Province. Our results suggested that the more effective regional monitoring plan and water improvement strategies should be strengthened targeting at the cluster areas based on the characteristics of geological factors and the spatial variability of local relationships between iodine in drinking-water and geological factors.

Objective To understand the status quo of the illness situation and the implementation of prevention and control measures on Kashin-Beck disease( KBD ) in Shandong Province , and provide a scientific basis for assessment and acceptance of the work on eliminating KBD. Methods In accordance with the requirements of the national “Implementation Scheme for Endemic Disease Control in 2012” and the “Shandong Provincial Technical Scheme for KBD Control in 2012”. In four towns of Qingzhou, three epidemic villages were selected as the surveying villages in each town in order to understand the implementation of measures for prevention and control of KBD; information such as population of investigating villages, per capita income, farming, food and the implementation of control measures, etc were included. In each investigating village, 7 - 12 years old children were selected to receive clinical and X-ray examination. When fewer than 50 people in a investigating village , one additional endemic village was included near the investigating village , and all children aged 7 to 12 of the village were checked. The number of cases of clinical degree Ⅰ, Ⅱ, Ⅲand the detection rates, X-ray positive detection rate, the number of cases of metaphysis, epiphysis, distal end of phalanx, carpal bone, triad and the detection rates were calculated, respectively. Clinical and X-ray diagnosis were based on “The Diagnostic Criteria of Kaschin-Beck disease”(WS/T 207-2010). Results A total of 14 epidemic villages were selected to survey in four towns of Qingzhou. In the recent three years in the epidemic areas, measures of supplying Se had not been implemented;measures of resettlement and off-site education had not been implemented either. In the epidemic areas , the main crops were wheat and corn; the staple food was flour which accounted for more than 80%, and the foreign food bought accounted for only 11%. The areas of returning farmland to forest (grass) accounted for 0.32%(36/11 151) of the original arable land, and the areas of replanting economic crops accounted for 1.17%(131/11 151) of the original farmland. A total of 536 children aged 7 to 12 were examined by clinical and X-rays. Patient of KBD was not detected clinically. Six positive patients with metaphyseal changes of KBD were detected by X-rays, and the average X-ray detection rate was 1.12%(6/536). Patients with abnormal epiphysis, distal end of phalanx and carpal bone were not detected by X-rays. Conclusions KBD in Shandong Province has been effectively controlled. Since the cause of KBD is still unknown, it is necessary to build a sustainable long-term control mechanism of KBD;further improve the monitoring system and dynamic monitoring work still need to be strengthened.
Objective To research the influence of high iodine on cellular morphology and the expression of a gene family about apoptosis(TRX-1 and ASK-1). Methods The A7r5 cells were treated with potassium iodide(KI) at the doses of 0, 750, 1 500, 3 750,6 000 and 7 500 mg/L for 24 h. MTT method was used to detect cell viability changes and Western blot assay was used to detect the protein expression of TRX-1 and ASK-1. Results Compared with the control group,A7r5 cell survival rate of 6 000-7 500 mg/L KI treated groups were significantly decreased(P0.01),and the survival rate of cells decreased with the increase of KI exposure dose. 750-7 500 mg/L KI decreased the expression of TRX-1 protein and increased the expression of ASK-1 protein compared with control(P0.01). With the increasing of KI exposure,TRX-1 expression showed a gradual decline trend and the ASK-1 expression showed a gradual upward trend. Conclusion Higher concentration of iodine can decrease the viability of A7r5 cells and the expression of TRX-1 protein, increase the expression of ASK-1 protein, and induce apoptosis of A7r5 cells.

Objective To observe the antagonism of selenium to arsenic-induced adverse effect in the activity of SOD and GSH-Px,the content of MDA,and the expression of ICAM-1 and VCAM-1 in human umbilical vein endothelial cells(HUECV). Methods The density of HUVEC in logarithmic growth phase was adjusted to 1.0 ×105/ml,then the cells were divided into two groups and were treated as follows, one group was treated with arsenic solution at different doses(0,0.05,0.1,0.5,1.5,3,6 and 12 μmol/L),the other group was treated with the same doses of arsenic plus sodium selenite(0.1 μmol/L). The medium were collected after 48 h-culture to determine the activity of superoxide dismutase(SOD) and glutathione peroxidase(GSH-Px) and the content of malondialdehyde(MDA) by spectrophotometry. The expression of intercellular adhesion molecule-1(ICAM-1) and vascular cell adhesion molecule-1(VCAM-1) were detected by enzymelinked immunosorbent assay(ELISA). Results With the increase of arsenic exposure, the arsenic group showed a gradual decline trend in the activity of SOD,GSH-Px,while an upward trend in the content of MDA and the expression of ICAM-1 and VCAM-1. Selenium showed a significant antagonism to lower level arsenic-induced adverse effect in the activity of SOD and GSH-Px,the content of MDA,and the expression of ICAM-1 and VCAM-1 in human umbilical vein endothelial cells. Conclusion Exposure to arsenic in a certain level can cause decrease of antioxidation of vascular endothelial cell(VEC) and increase of lipid peroxidation and up-regulated expression of ICAM-1 and VCAM-1. Selenium supplying may have antagonism to the adverse effect of arsenic.
Endemic fluorosis is prevalent in China, which can seriously impair the bones, teeth and cardiovascular system. The study was intended to explore antagonism of selenium (Se) on high dose fluorine (F) inducing plasma 6-Keto-prostaglandin F1α,Thromboxane B2 and Endothelin-1. Twenty male rabbits were randomly divided into 4 groups: High F group (NaF,100mg/L), Se group (Na2SeO3,1mg/L), High F + Se group (NaF,100mg/L; Na2SeO3,1mg/L) and Control group (without fluorine and selenium). The ear vein blood was collected for measurement of serum selenium and fluorine at the end of 0, 3rd and 6th months. At the end of 6th months, plasma 6-Keto-prostaglandinF1α (6-K-P), Thromboxane B2 (TXB2) and Endothelin-1 (ET-1) were detected. Serum fluorine was increased in high F group and serum Se was increased in Se group. Plasma 6-K-P was decreased, while the plasma TXB2 and ET-1 increased with the development of fluorosis. Compared with the high F group, the plasma 6-K-P was higher (p<0.01), whereas plasma TXB2 and ET-1 were lower (p<0.01) in high F + Se group. The interaction showed that there was a significant antagonistic action of Se on decreased 6-K-P and increased TXB 2 induced by high dose fluorine. Our study showed that the Se may antagonize the adverse effects of high F.

Objective To investigate the spatial distribution and clustering areas of Budd-Chiari syndrome in Heze City,Shandong Province, and to provide epidemiological information for further exploring the etiology and related risk factors of the disease. Methods Detailed residential addresses of 342 cases of patients (residents of Heze City) with diaphragm type Budd-Chiari syndrome diagnosed between 1995 and 2004 in Heze Municipal Hospital,Heze Shan County Central Hospital,Affiliated Hospital of Xuzhou Medical College,Shandong Provincial Hospital and Beijing Xuanwu Hospital were collected.Geographic information system (GIS) was used as a platform for data management and display. The nearest neighbor index,Ripley's K(d) function,Ripley's L(d) function and the nearest neighbor clustering method were applied to detect the spatial characters of Budd-Chiari syndrome in Heze City,Shandong Province.Crimestat 3.0 was used for spatial analysis. Results The nearest neighbor distance analysis showed that the nearest neighbor index was 0.6767 (Z =-11.4387,P < 0.01).That was an aggregation at the first-order spatial scale.Within the study area, the first clustering radius of Budd-Chiari syndrome was 6.66 km,and the first clustering strength was 5.40; the average radius of the strongest clustering area was 126.61 km,and the clustering strength was 12.52,while the biggest clustering radius was larger than 222 km.After corrected by population, the gathering strength was slightly higher than that before the correction.Ten first-order hot spots were formed, and 95% confidence interval aggregation number was 7,which meant the results were statistically significant(P < 0.05),main clustering areas are in Mudan District,Shan County and Juancheng. One secondorder hot spot was gathered based on the first-order hot spot. Conclusions Spatial distribution of Budd-Chiari syndrome in Heze City,Shandong Province has showed spatial aggregation and heterogeneity. This study has a great epidemiological significance for further exploring the cause of Budd-Chiari syndrome.
Objective To detect the spatial distribution characteristics of iodine in drinking water of residents in Shandong province with spatial autocorrelation analysis. Methods The county-based study set Shandong province as a research site.A total of 108 164 water samples from 140 counties were collected. The drinking water iodine data in county-level city between 2008 to 2010 were obtained from Shandong Institute of Prevention and Control for Endemic Disease and was merged with an electronic map to build a spatial database.Global and local Moran's I index wcrc calculated, respectively, and spatial autocorrelation and cluster range of iodine distribution in drinking water in Shandong province were studied by SaTScan software. Results All counties were further grouped according to the " criteria of delimitation for IDD endemic areas" and "determination and classification of the areas of high water iodine and the endemic areas of iodine excess goiter",and 90 counties were iodine deficiency (< 10 μg/L),31 were iodine suitable (10-150 μg/L),and 19 (> 150 μg/L) were high iodine. For the overall study area, the iodine distribution in drinking water in Shandong province existed spatial autocorrelation (Moran' s I =0.52,Z =7.4,P <0.01).For the local scale, the drinking water iodine in 18 counties of Dezhou,Liaocheng and Heze city in western Shandong province was clustered, the local Moran's I were between 0.22-1.00 (P < 0.01),which were all high-high clusters, indicating the positive spatial correlation.Spatial analysis using SaTScan software detected two cluster areas including 20 counties, which the centers located in Xiajin and Dingtao county, the cluster radiuses were 57.47 km and 65.58 km respectively. The analysis results were consistent with the results of local spatial autocorrelation. Conclusion There are apparent spatial autocorrelation and strong spatial heterogeneity existed in the iodine distribution in drink water in Shandong province.
