Objective Research the influence of tomography imaging about sentinel lymph nodes( SLN) in breast cancer patients after injected specific radiopharmaceuticals. Methods 47 breast cancer patients were randomly divided into two groups and injected different concentration99mTc-sulfur colloid to mammary gland subcutaneous tissue of 3,6,9 and 12 positions,and analysed 17 hours later. Results The observation group imaging positive diagnosis rate was 95. 0%( 19 /20). Control group was 92. 6%( 25/27). The two group imaging positive rate is no statistical significance( 2= 0. 967,P 0. 05). Conclusions Diagnosis is almost not affected in mammary glands SLN tomography imaging when injected different concentrations radiopharmaceuticals.
目的:研究滤膜的应用对乳腺癌前哨淋巴结断层显像的影响。方法将确诊为乳腺癌的T1-2 N0 M0114例患者按初次门诊就诊单双日随机分为2组,试验组49例患者,对照组65例。试验组患者乳晕皮下注射经滤膜预处理99 m Tc-硫胶体显像剂1 m Ci,对照组注射未经滤膜处理相同活度显像剂,17 h后行局部断层显像并观察对诊断的影响。结果试验组患者断层显像阳性率为91.8%,对照组为92.3%,2组断层显像阳性率比较差异无统计学意义(χ2=0.067, P >0.05)。结论滤膜的应用对乳腺前哨淋巴结99mTc-硫胶体断层显像诊断无明显影响。
Objective To study the inhibition effects of chitosan-gelatin membrane on fibrous capsule of the expanded flap. Methods 60 Wistar rats were used to establish models of the expanded flap, gelatin- chitosan membrane was implanted, water was regularly injected.The fibrous capsule on the same part of the dilator was cut and made into pathological section. Capillary and collagen content of the fibrous capsule was measured. Results The fibrous capsule was mainly composed of collagen fibers and inflammatory cell infiltration and a small amount of capillary under the light microscope. The surface density in the experimental group at 6th weeks and 12th weeks was 0.22±0.08 and 0.27±0.05,respectively,which was lower than in the control group(0.37±0.07 and 0.34±0.07), showing statistically significant difference(P 0.05). Capillaries surface density(0.13±0.18)in the experimental group was lower than the control group(0.15±0.23)after 6th weeks, the difference was statistically significant(P 0.05); no statistical significant difference in capillary surface density(P 0.05)was observed between two groups at 12th weeks(t=1.98,P 0.05). Conclusions Gelatin-chitosan membrane can intervene the fiber capsule formation and reduced the collagen content in the membrane, and has inhibitory effect to the fiber coated but doesn't affect the flap nutrition.
Objective: To evaluate the performance of image fusion technology in location of sentinel lymph nodes(SLN) in breast cancer.Methods: The information of 110 patients with breast cancer about99mTc-Sulfur colloid imaging was analyzed retrospectively.Results: The detection rate of plane scan mode was 88.2%(97 /110),and tomography 93.6%(103 /110).The former can't locate the SLN accurately,the latter can not only show the number and accurate position of SLN,but also provide 3D information.Conclusion: The application of imaging fusion technology can increase the positive detection rate of SLN,and provide accurate 3D positioning information.
目的:制备壳聚糖膜并研究其对皮瓣的止血效果。方法:壳聚糖干粉经酸碱处理,制膜待用。30只Wistar大鼠,背部两侧各剥离2.0cm×2.0cm对称腔穴,制成渗血皮瓣。分别敷以壳聚糖膜和灭菌纱布止血,记录止血时间,以膜或纱布中血红蛋白吸光度值表示出血量。结果:壳聚糖膜与创面粘附良好,壳聚糖膜组和纱布组的出血时间分别为(261±77)s和(389±51)s,血红蛋白吸光度值为(643.46±57.76)和(1114.81±127.61),二组比较差异显著(P<0.01)。结论:壳聚糖膜对皮瓣有良好的止血作用。
Objective To investigate the clinical therapeutic effect of the vacuum-sealing drainage (VSD) combining with skin graft in a deep burn wound.Methods Sixty one patients were divided into two groups according to treatment method.Thirty tree patients were assigned as the VSD treatment group,which was treated by VSD after debridement; their wounds were transplanted by the split thickness skins binding up by VSD when they were fitted.Twenty eight patients were assigned as the routine treatment group,which was treated by means of traditional regular dressing change to remove necrotic tissue and promote granulation tissue; their wounds were transplanted by the split thickness skins binding up by packing when they were fitted.Results After treatment by VSD,there are no systemic hypersensitivity reactions and abnormal bleeding.Thirty three cases of VSD treatment group were treated by VSD for 7 ~ 10 days,the other 2 cases were treated by VSD for another 7 ~ 10 days because of granulation tissue badly.Until their granulation was fresh,skin grafting technique was used and the skin survived well.The treatment time before skin grafts,the pain score,and the cost of ablative therapy for wound were ( 8 ± 0.9) day,(5.7 ± 1.6) point,and ( 1.5 ± 0.4) ten thousand yuan in VSD group vs.( 14 ± 1.2) day,(3.1 ± 1.1 ) point,and (0.6± 0.2)ten thousand yuan in control group.The survival rate and the wound germiculture positive rate were 97.0% and 6.5% in the VSD group vs 75.0% and 28.6% in the control group.The differences between groups were statistically significant ( P < 0.05 ).Conclusions VSD is a simple method that is easy to grasp,and that can promote granulation tissue growth,reduces the wound germiculture positive rate,shortens the wound healing period,increases the survival rate of skin grafts,and relieves patients 'pain.It is worth popularizing and applying in the deep burn wound therapy.
目的观察应用中草药浸浴烧伤患者康复期微循环的变化。监测其对烧伤后期康复治疗、功能恢复、创面修复、改善微循环的治疗效果。方法采用WX-753B型微循环显微镜及图像处理系统进行观测,应用法国产REF1254型自动调温浴缸。对50例烧伤康复期患者应用中草药浸浴,使其渗出和渗血减少,血流速度加快(P<0.01)。结果浸浴前烧伤康复期患者的足甲襞微循环最显著的变化管襻畸形率高(P<0.01),汗腺导管增多(P<0.01),血管管襻周围渗出等。浸浴前与治疗组比较经过中草药浸浴的烧伤病人足甲襞微循环血管管襻畸形明显改善,管襻周围的渗出、渗血减少。结论虽然严重烧伤经治疗伤面愈合,但是烧伤后微循环的紊乱长期存在,影响病人的康复。中草药浸浴可直接作用于全身治疗,一定时间内发挥药物作用,迅速改善微循环。具有时间短、作用快、效果显著的独特作用,最适合烧伤康复期病人的治疗。浸浴、浸浴前后进行足甲襞微循环襻动态观测,患者本组既往平均烧伤面积为(54.59±17.30)%,平均年龄为17~45岁,伤面愈合时间1~6个月。
40例康复期烧伤患者,均予中草药(中药组方为荆芥、双花、野菊花、大黄、水蛭)浸浴。用微循环显微镜及图像处理系统观测其足甲襞微血管循环变化结果患者足甲襞微循环血管管襻畸形明显改善,管襻周围的渗出、渗血减少。认为用本方浸浴可迅速改善烧伤患者微循环,是烧伤患者康复治疗的有益补充。
