总结全病种睡眠医学中心的建立及管理体会.合理的空间布局、舒适的环境设施、经验丰富的医护技团队、完善的工作制度和流程是全病种睡眠医学中心设立和管理的重点.全病种睡眠医学中心自2021年5月建立至2022年11月共收治患者2 615例,2021年第三季度患者满意度97.4分,护理质量检查各项目均在95分以上,2022年第三季度患者满意度99.1分,护理质量检查各项目均在97分以上.
Background To validate the accuracy of ultra-wideband (UWB) wireless radar for the screening diagnosis of sleep apnea. Methods One hundred and seventy-six qualified participants were successfully recruited. Apnea-hypopnea index (AHI) results from polysomnography (PSG) were reviewed by physicians, while the radar device automatically calculated AHI values with an embedded chip. All results were statistically analyzed. Results A UWB radar-based AHI algorithm was successfully developed according to respiratory movement and body motion signals. Of all 176 participants, 63 exhibited normal results (AHI <5/hr) and the remaining 113 were diagnosed with obstructive sleep apnea. Significant correlation was detected between radar AHI and PSG AHI (Intraclass correlation coefficient 0.98, P<0.001). Receiver operating characteristic curve (ROC) analysis revealed high sensitivity and specificity. High concordance in participants with varying gender, age, BMI, and PSG AHI was reached. Conclusions The UWB radar may be a portable, convenient, and reliable device for obstructive sleep apnea screening.
Free radical injury and oxidant/antioxidant imbalance are two mainstream pathological theories of asthma. The study was aimed to explore whether Lyciumbarbarum polysaccharide (LBP), which is one of active constituents of Chinese traditional medicine Lyciumbarbarum, can rescue human bronchial epithelial (NHBE) cells from cigarette smoke extract (CSE)-induced oxidative stress, and probe into the possibility of LBP as a prophylactic agent for asthma. Cell viability was detected using CCK-8 assay. ROS level and apoptosis rate were determined by the means of flow cytometry (FCM). Detection of mRNA and protein expression was carried out using real-time quantitive PCR (RT-qPCR) and Western blot. Our study found an CSE-induced elevation of ROS level in NHBE cells accompanied by inhibition of cell viablity and increase of apoptosis rate. In injured cells, oxidative stress increased the expression of Kruppel-like Factor 2 (KLF2), which results in the up-regulation of its downstream genes, including gamma-glutamyl cysteine synthetase (gamma-GCS) and Nuclear factor (erythroid-derived 2)-like 2 (Nrf2). KLF2 and Nrf2/Bach1 pathway interacted to increase compensatory expression of antioxidant gamma-GCS in oxidative stress. As a result, we detected that LBP pretreatment was able to mitigate ROS increase and apoptosis abnormality in CSE-induced damage, and narrowed the differences in related gene expression between CSE injury model and normal NHBE cells through regulating KLF2 and Nrf2/Bach1 expression. The results suggested that LBP has great potential to be a new natural prophylactic agent for asthma patients.
Afatinib, a second-generation irreversible epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI), has been approved as EGFR, HER2, HER3 and HER4 inhibitor for non-small cell lung cancer (NSCLC) treatment. However, acquired resistance to afatinib has been found in most EGFR mutant NSCLC patients. Bladder cancer associated transcript 1 (BLACAT1) is a novel long non-coding RNAs (lncRNA) that play a functional role as an oncogenic lncRNA and is associated with chemoresistance. We aimed to identify the role of BLACAT1 in afatinib-resistant NSCLC and underlying mechanisms. Afatinib-resistant NSCLC cells were established. MTT assay, colony formation assay, apoptosis analysis, qRT-PCR and western blot analysis, immunohistochemistry, and in vivo study were carried out. BLACAT1 was up-regulated in afatinib-resistant NSCLC cells. Knockdown of BLACAT1 reversed the resistance of afatinib to NSCLC cells by modulating STAT3 signalling. The results provided a potential strategy for afatinib-resistant NSCLC by targeting BLACAT1.
Here, a new indium (In)-based coordination polymer [In(hip)](DMF)2(H2O)3 (1, DMF=N,N-dimethylformamide) was successfully prepared by a solvothermal reaction of In(NO3)3·6H2O and 5-hydroxyisophthalic acid (H3hip) in a mixed solvent of DMF and H2O with the presence of NaCl as a template. Complex 1 was characterized by elemental analysis (EA), single-crystal X-ray crystallography, and powder X-ray diffraction (PXRD), and the results reveal that complex 1 shows a two-dimensional (2D) grid-like network with considerable solvent accessible volume that was generated from the packing of the 2D layers via the AB pattern. Furthermore, complex 1 could be downsized into nanoscale particles with the aid of polyvinylpyrrolidone (PVP). In addition, the anticancer activities of 1 and the nanoscale 1 were probed via the 3-(4,5)-dimethylthiahiazo (-z-y1)-3,5-di-phenytetrazoliumromide (MTT) assay.
Multicentral Validation of automatic UWB Radar Algorithm Compared with Polysomnography in Adolescents To validate the accuracy of ultra wide band(UWB) wireless radar algorithm in the screening diagnosis of sleep apnea-hypopnea syndrome.A total of 291 participants from three sleep centers underwent technician attended polysomnography(PSG) and wireless radar device detection simultaneously. There were 115 participants in training group, and their results were used to develop and optimize the apnea-hypopnea index(AHI) algorithm. Other 176 participants were in validation group.AHI results from PSG were reviewed by physicians, while AHI results from radar device were automatically calculated with an embedding chip.UWB radar based AHI algorithm was successfully developed according to respiratory movement and body motion signals. Significant correlation was detected between radar AHI and PSG AHI (Intraclass correlation coefficient 0.983, P=0.000). ROC curve analysis revealed high sensitivity and specificity in all AHI severity groups. High concordance in participants with different gender, age or PSG AHI were reached. The ultra wideband(UWB) radar might be a portable, convenient and reliable device for sleep apnea-hypopnea syndrome screening.
OBJECTIVE:To investigate the efficacy and the value of CAUP in treatment of OSAHS.METHOD:CAUP and the dissection of palatopharyngeus muscle were performed by RFVTR . One hundred and sixty-eight patients with OSAHS treated by CAUP from July 2001 to July 2004 were summarized. Among them, 52 patients were analyzed by PSG after 1 year of the operation.RESULT:All the operative procedure were carried out smoothly with excellent patient tolerance. Post-operation complications were foreign body feeling in oral cavity. No velopalatal insufficiency occurred. The efficient rate was 94.2% after 1 year of the operation according to the PSG results.CONCLUSION:CAUP is a simple, safe,repeatable and acceptable surgical procedure and it was developing with excellent value in the treatment of OSAHS on the basic of accurate X-ray imaging.
儿童扁桃体和(或)下鼻甲肥大和腺样体增生可引起呼吸障碍,表现为睡眠时打鼾、呼吸暂停和低通气.射频组织减容(radiofrequency volumetric tissue reduction, RFVTR)治疗是一种利用射频对深部组织造成液化和凝固性坏死,并进一步使组织体积缩小的外科技术.
目的介绍鼻空肠管在维持重症颅脑外伤患者肠内营养的临床应用.方法对32例重症颅脑损伤的昏迷患者,无法经口进食而需要肠道内营养者,行鼻空肠管.结果14例曾有上消化道出血者不影响鼻空肠管管饲肠内营养的给予,并且在肠内营养治疗后应激性溃疡很快得到纠正.10例重度营养不良患者情况明显改善,15例中度营养不良和7例轻度营养不良患者恢复正常.13例治疗前有肺部感染,治疗后很快得到控制.结论对于需要长期营养支持的重症颅脑损伤患者,该方法是一种护理简便、管饲保留时间长、并发症少、有效且经济的方法.
目的:研究阻塞性睡眠呼吸暂停低通气综合征(0SAHS)夜间窦性心动过速的特征.方法:选择0SAHS患者24例,根据呼吸暂停及低通气指数(AHI)分为轻、@重度3组,正常对照组11例,全部病例均经多导睡眠呼吸监护仪监测记录窦性心动过速,并分析低氧、窦性心动过速及0SAHS严重程度的相关性.结果:在快动眼睡眠期,低氧中期窦性心动过速发生次数,重度组高于中度组和正常组(P<0.05);低氧晚期窦性心动过速发生次数,重度组高于中度组,且均高于正常组(P<0.05).在非快动眼睡眠期,低氧中期窦性心动过速发生次数,重度组高于中度组,且均高于正常组(P<0.05);低氧晚期窦性心动过速发生次数,重度组高于中度组和正常组(P<0.05).结论:中重度OSAHS患者低氧时窦性心动过速的出现明显"滞后".
