BackgroundDietary guidelines, integrating the latest scientific evidence with traditional dietary culture, are regularly issued to provide dietary recommendations and promote health. However, the relationship between adherence to dietary guidelines and cognitive health has yet to be fully elucidated. In this study, we specifically examine the association between adherence to the Dietary Guidelines for Chinese (DGC) and cognitive performance among older adults.MethodsWe analyzed data from the China Health and Nutrition Survey (CHNS) (2011 and 2015), including participants aged ≥60 years. Adherence to the DGC was assessed using the Chinese Healthy Eating Index (CHEI). Cognitive performance was evaluated across the following three domains: (1) memory (immediate and delayed recall), (2) attention/executive function (counting backward from 20), and (3) processing speed (serial 7 subtraction). A composite z-score was derived to represent global cognition. Multivariate logistic regression and restricted cubic spline models were used to assess associations.ResultsAmong 2,174 older adults, higher CHEI scores were significantly associated with better performance at counting backward (attention/executive function, p < 0.05), serial 7 subtraction (processing speed, p < 0.01), and global cognition (p < 0.01). A significant interaction effect was observed between the CHEI and nationality in relation to cognitive performance (pinteraction <0.05). Among the 17 CHEI components, only adherence to the recommended intake of fish and seafood (OR: 0.55; 95% CI: 0.37, 0.82) was independently linked to better cognitive performance. Sensitivity analyses excluding individuals with diabetes or overweight or obesity, and treating cognitive performance as a continuous outcome yielded consistent results (p < 0.05).ConclusionGreater adherence to the DGC was associated with better cognitive performance, particularly in processing speed and attention/executive function. Promoting adherence to dietary guidelines may be a viable strategy for preserving cognitive health in aging populations.
Chikungunya virus (CHIKV) poses a significant public health threat, and its continuous evolution necessitates high-resolution genomic surveillance. Current methods lack the speed and resolution to efficiently discriminate sub-lineages. To address this, we developed CHIKVGenotyper, an interpretable machine learning framework for high-resolution CHIKV lineage classification. This study leveraged a comprehensive dataset of 6886 CHIKV genome sequences, from which a high-quality set of 3014 sequences was established for model development. A hierarchical assignment pipeline that integrated a probability-based sequence matching model, machine learning refinement, and phylogenetic validation was developed to assign high-confidence labels across eight CHIKV lineages, thereby constructing a reliable dataset for subsequent analysis. Multiple machine learning models were trained and evaluated, with the optimal Random Forest model achieving near-perfect accuracy (F1-score: 99.53%) on high-coverage whole-genome test data and maintaining robust performance (F1-score: 96.50%) on an independent low-coverage set. The E2 glycoprotein alone yielded comparable accuracy (F1-score: 99.52%), highlighting its discriminative power. SHapley Additive exPlanations (SHAP) analysis identified key lineage-defining amino acid mutations, such as E1-K211E and E2-V264A, for the Indian Ocean Lineage, which were corroborated by established biological knowledge. This work provides an accurate, scalable, and interpretable tool for CHIKV molecular epidemiology, offering insights into viral evolution and aiding outbreak response.
Despite the end of the global Coronavirus Disease 2019 (COVID-19) pandemic, the risk factors for COVID-19 severity continue to be a pivotal area of research. Specifically, studying the impact of the genomic diversity of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) on COVID-19 severity is crucial for predicting severe outcomes. Therefore, this study aimed to investigate the impact of the SARS-CoV-2 genome sequence, genotype, patient age, gender, and vaccination status on the severity of COVID-19, and to develop accurate and robust prediction models. The training set (n = 12,038), primary testing set (n = 4,006), and secondary testing set (n = 2,845) consist of SARS-CoV-2 genome sequences with patient information, which were obtained from Global Initiative on Sharing all Individual Data (GISAID) spanning over four years. Four machine learning methods were employed to construct prediction models. By extracting SARS-CoV-2 genomic features, optimizing model parameters, and integrating models, this study improved the prediction accuracy. Furthermore, Shapley Additive exPlanes (SHAP) was applied to analyze the interpretability of the model and to identify risk factors, providing insights for the management of severe cases. The proposed ensemble model achieved an F-score of 88.842% and an Area Under the Curve (AUC) of 0.956 on the global testing dataset. In addition to factors such as patient age, gender, and vaccination status, over 40 amino acid site mutation characteristics were identified to have a significant impact on the severity of COVID-19. This work has the potential to facilitate the early identification of COVID-19 patients with high risks of severe illness, thus effectively reducing the rates of severe cases and mortality.
Objective To investigate the comprehensive ability and influencing factors of clinical medicine graduates from a medical college in Xi'an,and provide a scientific basis for improving the training model and comprehensive ability of clinical medicine students.Methods Based on the Chinese Undergraduate Medical Education Standard-Clinical Medicine Specialty(2016 Edition),we conducted a self-designed questionnaire,the Clinical Medicine Graduate Comprehensive Ability Evaluation Questionnaire,containing 62 items in 4 dimensions,selected the clinical medicine graduates as the research objects by random sampling method,used SPSS 22.0 to make data analysis,and performed multiple linear regression to analyze its influencing factors.Results Clinical medicine graduates had the highest score in the field of clinical competence(3.05±0.84),followed by the field of professionalism(2.48±0.81),and the lowest score was in the field of science and academics(1.89±0.68).The top three scores of each factor were clinical practice ability(3.39±0.76),theoretical knowledge mastery(3.55±0.48)and medical knowledge mastery(2.98±0.81).The three factors with the lowest scores were literature search ability(1.31±0.64),global health issues(1.49±0.82)and self-improvement willingness and ability(1.81±0.73).Using the multiple stepwise regression method to analyze the influencing factors of comprehensive ability,the optimal equation was obtained:Y=8.412+0.063X6+0.190X8+0.266X10+0.031X11+0.187X12(X6,working level;X8,academic performance ranking;X10,practical teaching satisfaction;X11,mode of teaching satisfaction;X12,Re-education satisfaction after graduation).Among them,the higher the practical teaching satisfaction,the satisfaction of re-education and the satisfaction of teaching mode after graduation,the higher the student's performance ranking,and the higher the comprehensive ability score of clinical graduates.Graduates whose employment level was in a general hospitals had higher comprehensive ability scores than those of grassroots hospitals such as community/township hospitals.Conclusions In the process of training clinical medicine students,we should improve the quality of practical teaching and the teaching model,increase the training of science and academic fields,increase the investment in re-education of primary medical institutions,and raise the satisfaction of graduates in education,so as to improve the clinical medicine profession comprehensive ability of graduates.
At present, lung cancer is very common, with the highest mortality rate. As for the pathology of lung cancer, we are still at the research stage. The main research of this paper is the analysis of lung cancer based on big data technology disease data management status. This article collected a total of 50 medical examination data. The field types include three types: numeric type, sub-type, and text type. The data mining model of this article is mainly realized by the data mining function of Clementine12.0. The feature selection component can calculate the importance of each field and the predicted field, and can select fields with greater importance for analysis. This paper extracts the features of the last convolutional layer of the model, and then performs batch quadratic superposition and fusion of the feature maps, and finally takes the average value as the input data for the secondary detection. Experiments have found that the parallel algorithm designed in this paper can increase the computational efficiency by about 9 times. The results show that big data technology improves the efficiency of lung cancer diagnosis.
Background Considering the lack of valid and reliable food frequency questionnaires (FFQs) for nutritional epidemiological studies in China, it is necessary to develop an effective one for assessing nutrient intake among preschool children. The aim of this study was to evaluate the validity and reliability of newly developed FFQs for assessing food and nutrient intakes among preschool children in Northwest China. Methods Semiquantitative 67-item FFQs were developed and validated. In total, 326 children (aged 2–6 years) were recruited from three different cities in Northwest China. All subjects were asked to complete the FFQs twice with a six-month interval period for test-retest reliability. Apart from the FFQs, a 3-day food record (3-DFR) was also carried out to evaluate the validity of the FFQs. Results There was no significant difference in the nutrient intakes of preschool children between the two FFQs (P > 0.05), and these two FFQs demonstrated a positive correlation (P < 0.05). Spearman's coefficient correlation values ranged from 0.222 (“Selenium”) to 0.832 (“Energy”). The intraclass correlation coefficient values ranged from 0.282 (“Selenium”) to 0.882 (“Energy”). With regards to the validity of FFQs, nutrient intakes from FFQs were greater than 3DR dietary recalls (P < 0.05). After adjusting for total energy and intraindividual variation, all nutrient intakes showed a positive correlation (P < 0.05), and these correlations became stronger. According to the quartiles of nutrient intakes, the exact agreement between the FFQs and 3DR dietary recalls ranged between 40% (“Selenium”) and 70% (“Energy”), and grossly misclassified was low (12.5%). Conclusions The findings of this study indicate that the designed FFQs exhibit good test-retest reliability and moderate relative validity. Hence, the FFQs can serve as an important tool for the large-scale assessment of food and nutrient intakes among preschool children (in the mentioned areas of China).
目的 调查某医学院校大学生手机依赖对睡眠质量的影响,为有针对性地制定干预措施提供依据.方法 2020年10-12月,采用整群抽样方法抽取陕西省2所医学院校987名大学生作为研究对象,利用手机依赖量表和匹兹堡睡眠质量指数问卷调查大学生手机依赖程度和睡眠质量,并对数据进行分析.结果 医学院核大学生手机依赖发生率为54.71%,手机依赖程度由轻度到重度依次为29.69%、18.95%和6.08%.手机依赖程度与睡眠质量、睡眠时间、睡眠效率和睡眠障碍均呈正相关(r=0.069~0.126,均P<0.05).性别、学习排名、是否失恋、是否焦虑以及睡眠质量5个变量是影响大学生手机依赖程度的重要因素(均P<0.05).结论 医学院校大学生手机依赖检出率较高,手机依赖与大学生睡眠质量存在相关性.学校应加强对大学生科学使用手机的教育和宣传,形成合理科学使用手机的态度和习惯,提高大学生睡眠质量.
近年来,老年人HIV感染数及患者人数不断增长,越来越引起社会的广泛关注[1].2017年国务院发布《中国遏制与防治艾滋病"十三五"行动计划》中首次将老年人群列入艾滋病重点防治对象,并明确指出健康教育和行为干预是预防艾滋病的重要策略[2].为探索社区老年人群开展艾滋病健康教育与行为干预的有效方法,采用危害宣传的自省式健康教育模式和常规健康教育方式进行比较研究,分析其知识、态度、行为改变情况,为老年人社区干预方法选择提供依据.
Tuberculosis has been a serious threat to the health of people in all countries for many years. This study mainly explores the application of tuberculosis in the prevention and control of epidemics based on big data technology. First, collect data on tuberculosis epidemics. Tuberculosis epidemic information according to the statutory infectious disease reporting system in the China Disease Prevention and Control Information System, case data of reported cases of tuberculosis in mainland China in the past three years was collected according to the review date. Next, collect map data and statistics of demographic data. Finally, clean and organize the data. Check whether there are outliers, duplications, missing and logical errors in the case data, and unify the units of the variables. The reported incidence rate was the highest in the third year and the lowest in the first year, which were 104.90 cases per 100,000 and 71.89 cases per 100,000, respectively. The number of reported cases in the three years was basically similar to the reported incidence rate. This research will effectively improve the accuracy of tuberculosis diagnosis and provide decision support for tuberculosis diagnosis.
目的 调查学龄前儿童膳食摄入情况,为研究学龄前儿童膳食模式及饮食特征、制定有针对性的膳食干预措施提供科学依据.方法 利用自行设计的《学龄前儿童半定量食物频率问卷》,在我国西北地区西安、兰州、西宁、银川和乌鲁木齐采用方便抽样的方法抽取541名3-7岁儿童为研究对象,对其膳食摄入情况进行调查,利用潜在剖面分析法提取膳食模式.结果 根据赤池信息量(Akaike information criterion,AIC)、贝叶斯信息量(Bayesian information criterion,BIC)、调整贝叶斯信息量(aBIC)值最小,且使罗梦戴尔鲁本校似然比(Lo-Medell-Rubin likelihood ratio test,LMR)和基于Bootstrap的似然比检验(Bootstrap likelihood ratio test,BLRT)检验均有统计学意义(P<0.05),确定3分类模型为理想模型.根据3个潜在类别的类别概率和20类食物的条件均数分布情况,将学龄前儿童膳食模式分别命名为高质量膳食模式、常规膳食模式和低质量膳食模式,其在学龄前儿童中的分布分别为9.22%,57.38%和33.41%.低质量的膳食模式中叶类蔬菜、蛋类及制品、奶类及制品、仁果类和核果类水果等食物摄入均较低,油炸类、甜食类和饮料类等食物的摄入较高(P<0.05).结论 潜在剖面分析可应用于居民膳食模式的提取,但其精确性仍需进一步验证.本研究所提取的低质量膳食模式的学龄前儿童应当加大蔬菜、蛋奶类及水果类食物的摄入,减少油炸、甜食以及饮料类食物的摄入,有针对性地进行儿童膳食干预,促进儿童身心健康.

目的:观察TP53、KRAS、KDR、APC、MLH1及PIK3CA等56个基因在消化道恶性肿瘤中的突变情况,探讨驱动基因的诊疗意义.方法:选择胃食管癌15例、结直肠癌21例患者组织石蜡标本,运用高通量基因测序技术检测56个肿瘤突变高频基因,并进行生物统计分析.结果:36例标本检出34例有已知基因突变,突变基因为TP53、KRAS、MLH1、PIK3CA、KDR、APC、SMARCB1、KIT、EGFR、NRAS、STK11、SMO、SMAD4、CDH1、ERBB2、JAK2和RET共17种.其突变率由高到低依次为:TP53(75.00%),KRAS(33.33%),MLH1、PIK3CA、KDR(均为13.89%),APC(11.76%),SMARCB1(8.82%),KIT、EGFR及NRAS(均为2.78%).KRAS在结直肠癌中的突变率(47.62%)显著高于在胃食管癌中的突变率(13.33%).TP53的突变率明显高于其他突变基因.在结直肠癌中KRAS的突变率显著高于KDR、APC、MLH1,未发现有统计学关联.结论:消化道恶性肿瘤TP53突变率较高,可能与病理组织分化程度低及淋巴结转移有关.KRAS在结直肠癌中较在胃食管癌中更易发生突变,存在部位差异.同时KRAS与KDR、APC、MLH1,在结直肠癌患者中存在共突变现象,提示结直肠癌的酪氨酸激酶通路和血管内皮生长因子受体基因在致癌过程中可能存在协同作用,导致肿瘤在结直肠中更易发生发展,选择靶向药物治疗时需考虑协同作用,以便科学精准用药.
To study the improvement of the entrepreneurial performance of start-ups and achieve an organic combination of entrepreneurship education and entrepreneurial performance, the entrepreneurial group of college students was taken as the springboard to discuss the entrepreneurial performance of start-ups initiated by college students. First, through questionnaire design and scale selection, the results of the questionnaire survey and the reliability and validity of the scale tool were tested. Second, the variable of college students' entrepreneurial self-efficacy based on psychological factor analysis was introduced. The correlations among entrepreneurship education, entrepreneurial self-efficacy, and entrepreneurial performance were analyzed. Finally, the intermediary role of entrepreneurial self-efficacy was verified by using the hierarchical linear regression analysis method combined with the BARON three-part verification method. The results show that the response rate of the questionnaire is 92%, and the selection of research samples is scientific. The Cronbach's α reliability coefficients corresponding to each scale are all above 0.8, and the common factor variances are all above 0.7. Therefore, the reliability and validity of scale tools are good. Entrepreneurship education, entrepreneurial self-efficacy, and entrepreneurial performance are significantly correlated with each other. The college students' entrepreneurial self-efficacy has a good explanatory ability and plays an intermediary role; in the entrepreneurial innovation learning dimension, its corresponding β = 0.257; in the dimension of school organizational support, the corresponding β = 0.439; in the dimension of social interaction, the corresponding β = 0.238. The results have a reference value for studying entrepreneurship education and the improvement of entrepreneurial performance from a psychological level.
目的 调查西安市新城区社区学龄前儿童常见不健康饮食行为特征,并系统分析影响儿童饮食行为的因素,为规范学龄前儿童良好的进食行为、促进学龄前儿童合理膳食提供依据.方法 2017年8-12月选取西安市新城区6个社区卫生服务中心306名学龄前儿童作为研究对象,采用“学龄前儿童饮食行为量表”进行,并利用多元线性回归分析儿童饮食行为影响的因素.结果 西安市社区学龄前儿童最常见的不健康饮食行为是挑食、主动进食能力差、不良进食习惯等.不同性别的挑食,不良进食习惯及过饱响应得分差异均有统计学意义(t=3.650,2.024,3.707,P<0.05).抚养人教育背景不同,儿童挑食行为得分差异有统计学意义(F=3.126,P=0.026).抚养人身体状况不同,儿童主动进食能力得分差异有统计学意义(F=5.052,P=0.007).家庭常住人口数不同,学龄前儿童的挑食行为,不良进食习惯得分差异有统计学意义(t=6.442,3.515,P<0.05).经多元线性回归分析,影响学龄前儿童饮食行为的因素依次为是否独生子女、家庭常驻人口数、体重指数及抚养人教育背景.结论 加强抚养人知识储备和健康抚养观念的建立,家庭成员统一抚养观念,减少肥胖/超重儿童不良进食习惯,是培养学龄前儿童健康饮食行为,促进身心健康的有效措施.
Objective: To analyze the residents' satisfaction with family doctors contracted services in Xi'an, and provide scientific basis for government departments to strengthen the construction of community health services, improve the quality of community health services and residents satisfaction. Method: Through consulting literatures, expert consultation and other methods to screen 12 service indexes, and the weighted rank sum ratio method was used to evaluate the satisfaction of family doctor's contracted service quality in different types of communities and family residents in Xi'an. Results: According to the survey of residents' satisfaction in Xi'an, it was found that the top three weighting factors in the evaluation index system are medical equipment (0.27), propaganda method (0.10) and home service (0.10). Using the weighted rank sum ratio method, we found that traditional old communities, community units, low-income families and empty-nest families are more satisfied with family doctor's contract service. However, comprehensive community and urban-rural community as well as high-income families, families with reorganization and chronic disease are less satisfied with family doctor's contract service. Conclusion: The medical equipment of community institutions, propaganda methods of family doctors contract service and the quality of home service are important factors which affecting the development of the business. In the family physician contract services process of actively implementing, Xi'an city should take advantage of strengthening the construction of the general practitioner team and improving the professional quality of medical staff and the utilization rate of medical equipment. Using the Internet plus to build a family physician contract service platform, strengthen the publicity and promotion of this service, strengthen the construction of community health service system and improve the convenience and normativeness of family doctors' contracted services.
The purpose of this study is to investigate the mental health status of 1021 community residents in Xi'an, which provides a scientific basis for understanding the mental health status of urban residents in our country, establishing a psychoanalysis model and promoting residents' health. The community residents of 8 community health service centers in Xincheng District of Xi'an City were used as the research object from June 1, 2016 to December 31, 2016, using the SCL-90 psychological scale for their psychological status measuring. The result is that the scores of SCL-90 for community residents in Xi'an are between 1.24 and 1.67. There were significant differences between male and female in other factors except for hostility (t=1.111, P=0.267), phobic-anxiety (t=1.658, P=0.098) and psychotic (t=0.359, P=0.025), interpersonal relationship (t=5.132, P=0.000), depression (P<0.05), and women's scores were higher than men's score (t=2.730, P=0.006). The scores of female anxiety (t=2.722, P=0.007) and psychotic (t=3.302, P=0.001) in 2016 were higher than those in the year of 1986; the study showed that the highest score was between the ages of 18 and 44 (youth group). This study can reflect the mental health of urban residents in Xi’an. The study found that the general mental health of residents in China remains steady. The concrete manifestations of women’s psychological health problems are increasingly prominent, and there is an upward trend in obsessive-compulsive disorder, interpersonal relationship and psychosis. The Chinese residents have the most severe mental health problems between the ages of 18 and 44, especially in the age group of 30-39.
目的:探讨肿瘤相关成纤维细胞(carcinoma-associated fibroblasts,CAFs)条件培养液对人肝内胆管癌HCCC-9810细胞中survivin基因表达的影响.方法:采用MTT法检测CAFs条件培养液对HCCC-9810细胞增殖能力的影响,FCM法检测CAFs条件培养液对HCCC-9810细胞周期分布的影响,实时荧光定量PCR法和蛋白质印迹法分别检测CAFs条件培养液对HCCC-9810细胞中survivin mRNA和蛋白表达水平的影响.结果:CAFs条件培养液能够明显促进HCCC-9810细胞的增殖(P<0.05).CAFs条件培养液处理组G0/G1期HCCC-9810细胞所占百分比低于RPMI1640培养液培养的对照组(P<0.05),S期HCCC-9810细胞所占百分比高于对照组(P<0.05),G2/M期HCCC-9810细胞所占百分比与对照组间的差异无统计学意义(P>0.05).CAFs条件培养液能够上调HCCC-9810细胞中survivin mRNA和蛋白表达水平(P值均<0.05).结论:CAFs条件培养液能够促进HCCC-9810细胞的增殖和细胞周期进程,这一作用可能与上调survivinmRNA和蛋白表达水平有关.
To investigate the mobile phone dependence and its influencing factors among college students in 3 universities in Shaanxi Province Methods: We selected total of 838 undergraduates in 3 colleges in Shaanxi province as the research objects. We used "the standard of mobile phone dependence syndrome to collect information and analyzed the factors which affecting college students' mobile phone dependence syndrome by Multi - factor logistics Regression. Results: It can be seen from this study that the rate of mobile phone dependent syndrome in college students is 27.2%. The male mobile phone dependence syndrome detection rate was significantly higher than that in female, enrollment for the city students, per capita income high students, mobile phone dependence syndrome detection rate is high; the history of art majors(OR=1.437), junior college students (OR=1.583) their mobile phone dependence syndrome detection rate are high. Students with frequent anxiety disorder (OR=2.192) and grief free (OR=1.976) have more pronounced symptoms of mobile phone dependence. Conclusion:, The 3 university students of Shaanxi Province mobile phone dependence syndrome detection rate is higher, the school should strengthen students' scientific use of mobile phone publicity and education, college students should strengthen self supervision and management, to avoid the long time use of mobile phone, mobile phone use to form a reasonable scientific attitude and habits, so as to improve college students' mental health and quality of life.
