Objective:To discuss the diagnosis and differential diagnosis value of MSCT on pleomorphic adenoma and ade-nolymphoma of parotid by investigating the imaging manifestations of CT. Methods:the CT characteristics of 25 cases of pleo-morphic adenoma and 18 cases of adenolymphomas in the parotid gland confirmed by histological pathology in our hospital were studied retrospectively. Results:25 cases of pleomorphic adenoma of parotid occurred in young patients,most lesions were located in the non-posteriorinferior quadrant of parotid gland,regular or irregular in shape with a clear boundary and heteroge neous or inhonmogeneous (cystosolid) density with large cysts. 18 cases of adenolymphoma occurred almost all in old men,lesions in unilateral sides or bilateral sides,most lesions were located in the posterior inferior quadrant,with a regular shape and clear boundary heterogeneous or inhonmogeneous (cystosolid) density with fracture cysts. Small vessels could be seen in or around the tumor. There were singificant differences (P<0.05),between pleomorphic adenoma and adenolymphoma of parotid in age,gender, number of morbidity,the incidence ratio quadrant,morphology,the ways of cyst and small vessels in or around the tumor. Con-clusion:There are some characteristics of CT in pleomorphic adenoma and adenolymphoma of parotid. Imaging characteristics combined with clinical materials can improve the diagnosis and differential diagnosis on pleomorphic adenoma and adenolym-phoma of parotid gland.
Objective To discuss the imaging manifestations of CT of in the parotid gland,in order to improve the understanding of the CT imaging features of parotid adenolymphomas before operation.Methods The CT characteristics of 18 cases of ademolymphomas in the parotid gland confirmed by histological pathology were studied retrospectively.Results There were totally 24 lesions in 18 cases,of which 14 cases with unilateral lesions and 4 cases with bilateral lesions.23 lesions were located in the superficial lobe of the parotid gland and one lesion involved the superfical and deep parotid lobe simultaneously.22 lesions were located in posterior and inferior quadrant of parotid and 2 were other sites.23 lesions were round or oval and 1 was lobulated in shape.The margins in 22 lesions were well-defined,2(8.3%) lesions were ill-defined with ipsilateral parotid gland were increased in density.The density of 17 lesions were homogeneous (solid) and 7 lesions were inhonmogeneous (cystosolid) on plain CT.2 lesions showed slightly enhanced,9 lesions showed moderate enhancement,13 lesions showed significant enhancement,and the cystic region within the lesions showed no enhancement.Small vessels could be seen in or around the tumor in 21 lesions and no enlarged cervical lymph node was noted.Conclusion There were some characteristics of CT in parotid adenolymphoma.Imaging characteristics combined with clinical materials can play a helpful role in the preoperative diagnosis of parotid adenolymphomas.
直肠癌是仅次于肺癌和乳腺癌的第三大恶性肿瘤,是世界发病率和死亡率较高的常见恶性肿瘤,在我国其发病率和死亡率明显上升且呈年轻化趋势[1]。结直肠癌的治疗方式主要包括新辅助放化疗、手术等综合性治疗方案[2],因此结直肠癌准确的定位与分期对指导临床医生选取正确的治疗方案及评估术后具有至关重要性[3]。计算机 X线断层扫描(CT)及磁共振成像(MRI)明确结直肠癌的诊断、术前分期有着明显的优势及应用价值。
Objective:To explore the value of diffusion-weighted imaging (DWI) combined with conventional magnetic resonance imaging (MRI) in diagnostic staging and the decision of treatment method in patients with cervical cancer.Methods:The data of DWl MRI from 42 patients pathologically confirmed with cervical cancer were retrospectively analyzed.The apparent diffusion coeffcient (ADC) and exponential ADC (eADC) values were measured.The ADC and eADC values were compared according to different pathologic types and stages of cervical cancer,and between the cervical cancer and normal cervix.The diagnostic accuracy of DWI combined with conventional MRI for pathologic staging was examined.The ADC and eADC values were compared between radiochemotherapy group and the surgery group.Results:The cervical cancer demonstrated high signal intensity on DWl,whereas the normal cervix showed no obviously abnormal signal intensity.The mean ADC value of 42 patients with cervical cancer was lower than that of normal cervix (P =0.000);the mean ADC value of cervical squamous-cell cancer was lower than that of adenocarcinoma (P =0.036);the mean ADC value of radiochemotherapy group before treatment was lower than that of surgery group (P =0.000),whereas the eADC value was higher than that of surgery group (P =0.000).There were no significant differences in mean ADC and eADC values among different pathologic stages of cervival cancer (all P values < 0.05).The diagnostic accuracy of conventional MRI and DWI combined with conventional MRI for pathologic staging were 92.9% (39/42) and 95.2% (40/42),respectively (Kappa =0.903 5 and 0.935 8,respectively;P < 0.01).Conclusion:DWl combined with MRI exerts a high value in diagnostic staging of cervical cancer.The ADC value can be used as reference in selection of therapeutic methods for stage Ⅱ B cervical cancer.
Objective To evaluate the diffusion weighted imaging (DWI) and conventional magnetic reso-nance imaging (MRI) in preoperative staging and diagnosis of endometrial carcinoma. Methods The MRI and DWI data of 38 cases with pathologically confirmed endometrial carcinoma cancer were retrospectively analyzed. Tumor of signal characteristics and invasion range were observed, and different staging of ADC value were measured. Invasive depth and extent were evaluated and compared with pathological findings. Results Endometrial carcinoma was usu-ally isointensity on T1WI, hyperintensity on T2WI and DWI. The average ADC value of endometrial carcinoma was (0.832 ± 0.081) × 10-3 mm2/s, which decreased gradually with the increasing. The differences in ADC value of different stages were statistically different (P<0.01). The accuracy of conventional MRI and DWI combined conventional MRI was 84.21%and 89.47%in assessing pathological staging, with no statistically significant difference (P>0.05). The ac-curacy of conventional MRI and DWI combined conventional MRI was 84.21%and 94.74%in assessing deep myo-metrial invasion, with no statistically significant difference (P>0.05). Conclusion DWI combined MRI has a higher value in assessing preoperative staging and diagnosis of endometrial carcinoma, providing an important basis for the selection of treatment method and prognosis of endometrial carcinoma.
