目的:观察自拟疏肝补脾方治疗肝郁脾虚型慢性乙肝的临床疗效,同时对其作用机制进行探讨.方法:随机选取2011年8月～ 2014年9月我院收治的门诊及住院的肝郁脾虚型慢性乙肝患者94例,随机分到对照组和观察组中,对照组46例,观察组48例.对照组采用基础治疗加上西药常规治疗,给予患者口服水飞蓟宾胶囊,每次3粒,每日3次;观察组采用基础治疗加以本院自拟中药汤剂疏肝补脾方,2组均以1个月为1个疗程,连续治疗3个疗程.比较2组患者治疗前后血清中肝功能指标:谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL),乙肝病毒指标:乙型肝炎表面抗原(HBsAg)、e抗原(HBeAg)、乙肝病毒脱氧核糖核酸(HBV-DNA)的转阴率,以及淋巴细胞(CD4+、CD8+)的变化.结果:观察组的肝功能指标ALT、AST及TBIL的水平显著较对照组低(P＜0.05);乙肝病毒指标HBsAg、HBeAg及HBV-DNA转阴率均显著较对照组高(P＜0.05);观察组CD8+明显低于对照组(P＜0.05),而CD4+及CD4+/CD8+显著高于对照组(P＜0.05).结论:采用自拟疏肝补脾方治疗慢性乙型肝炎疗效确切,其机制可能与调节T淋巴细胞CD4+/CD8+有关.
Objective To discuss the clinical efficacy and safety of Reduning combined with con-ventional western medicine treatment of pediatric measles merger pneumonia. Methods 92 cases of patients with pediatric measles merger pneumonia in our hospital from January 2014 to January 2015 were randomly divided into the control group(n = 46)and the observation group(n = 46). Two groups of patients were given antibiotic and anti - inflammatory drugs and support treatment of conventional western medicine treatment,the control group was given ribavirin injection based on this,and the observation group was given Reduning injec-tion based on conventional western medicine treatment,two groups of patients were treated for a week,a total of two courses of treatment. Observe the clinical signs and symptoms of the two groups and record the antife-brile time,cough sputum ease time,White blood cells resume normal time,Blood biochemical index returned to normal time,the eruption completely scabby time,rales disappear time,X - ray shadow disappear time and hospital stays,and evaluate the adverse reactions. Results The antifebrile time,cough sputum ease time, White blood cells resume normal time,Blood biochemical index returned to normal time,the eruption com-pletely scabby time,rales disappear time,X - ray shadow disappear time and hospital stays of the observation group were significantly less than the control group,and all have statistically significant differences( P <0. 05). The effective rate of the observation group after treatment was 95. 7% ,which was higher than the rate of the control group(80. 4% ),the comparison has statistically significant differences(P < 0. 05). The com-parison of the adverse reaction between the two groups has no statistically significant differences(P > 0. 05).Conclusion Reduning combined with conventional western medicine treatment of c pediatric measles merger pneumonia can eliminate the patient′s symptoms and signs of recovery in a short period of time,and have good curative effect,high safety,which is worthy of clinical use.
