BACKGROUND:Health problems associated with energy metabolism imbalance have received increasing attention. Our team was the first to propose the concept of gastrointestinal heat retention syndrome (GHRS). Triggered by high-calorie diets and excessive energy intake, GHRS is strongly associated with recurrent respiratory infections and other pediatric conditions in children. Existing research on GHRS has primarily focused on risk factor analyses, and no interpretable exploratory self-assessment model for estimating prevalence probability is currently available for routine household use. OBJECTIVE:This study aims to develop and validate an interpretable machine learning model to help caregivers conduct exploratory self-assessments of the probability that children meet the criteria for pediatric GHRS and to provide a practical assessment tool for household use. METHODS:This study conducted a questionnaire survey of kindergarten children in Longgang District, Shenzhen, China. Samples with missing information on GHRS-related symptoms and signs were excluded. Independent correlates were identified using univariate logistic analysis, collinearity testing, Least Absolute Shrinkage and Selection Operator regression, and multivariable logistic regression. After handling missing data and preprocessing, the dataset was randomly divided into training and test sets in an 8:2 ratio. SMOTETomek (Synthetic Minority Oversampling Technique Tomek Links) was included as a conservative resampling step in the training pipeline, and optimal features were selected using a combination of Pearson correlation analysis and recursive feature elimination. In addition, a Random Forest (RF) sensitivity analysis without SMOTETomek was performed, and the minimum required sample size was estimated according to the events per variable (EPV) rule. Seven machine learning models were developed and evaluated in terms of discrimination, calibration, and clinical utility. The SHAP (Shapley Additive Explanations) method was used to interpret the optimal model, which was subsequently deployed online using the Streamlit framework. RESULTS:A total of 120,198 questionnaires were collected, of which 108,447 were deemed valid and included in the analysis. The ratio of GHRS-positive to GHRS-negative cases was nearly balanced. The study identified 59 independent correlates of GHRS, including 10 protective factors and 49 risk factors. After data cleaning, a complete-case dataset of 49,798 participants was obtained, exceeding the minimum sample size requirement of 3747 cases based on the EPV rule. During internal validation, the RF model demonstrated acceptable discriminatory performance, stable calibration, and high net benefit in decision curve analysis, and was therefore selected as the primary analytical model. SHAP analysis identified 5 key predictive features. The resulting online tool collects information through 75 single-choice questions and automatically provides an estimated probability of GHRS together with lifestyle recommendations. CONCLUSIONS:Using cross-sectional data, this study developed and validated an interpretable model that enables caregivers to perform exploratory self-assessments of the probability that children meet the study-specific GHRS scale criteria. As a household self-screening tool, the model helps caregivers estimate this probability based on readily obtainable information and model-generated risk estimates.
AimObesity in children have become major public health issues worldwide. The study aimed to determine the prevalence of overweight and obesity among children in Longgang District, Shenzhen, China, in the context of COVID-19 and analyze associated risk factors.MethodsA cross-sectional study was conducted through online survey from May to July 2021. World Health Organization child growth standards and the health industry standards of the People’s Republic of China were used as the diagnostic criteria for overweight and obesity. The prevalences of overweight and obesity were calculated, and risk factors of obesity were analyzed using binary logistic regression.ResultsThe parents or guardians of 124,593 children were administered, and 108,581 subjects were included. The prevalences of overweight and obesity among kindergarten children in Longgang District, Shenzhen, were 8.3 and 7.9%, respectively. The risk factors for childhood obesity included congenital, family, and lifestyle factors.ConclusionIn 2021, the obesity prevalence of kindergarten children in Longgang District was close to some high-income countries and higher than Shenzhen before the outbreaks of COVID-19. This finding may be associated with social, family, and personal factors. Exploration of evidence-based, effective means of modifying children’s health habits, and social support is urgently needed.
The occurrence, transmission, and spread of infectious diseases are closely linked to environmental factors. As climate change intensifies, shifts in temperature, precipitation, and extreme weather events are modifying pathogen survival, vector ecology, and the geographical range of infectious diseases. At the same time, ecosystem degradation, land-use change, urbanization, poverty, and social unrest further shape population exposure, healthcare access, and disease vulnerability. This narrative review synthesizes the mechanisms by which physical, ecological, and social environmental factors influence infectious disease transmission. Unlike reviews that focus on single determinants, this review adopts an integrated framework to examine both individual mechanisms and cross-domain interactions. Guided by the One Health framework, we link these mechanisms to the human-animal-environment interface and highlight how environmental change can promote pathogen spillover, amplification, and spread. By clarifying these pathways, this review provides a theoretical basis for integrated infectious disease prevention, public health policy, and future research. Not applicable.
Background:Viral encephalitis (VE), a central nervous system disorder with high mortality and disability rates, poses a serious threat to childhood development. Xiyanping injection (XYPI), an andrographolide sulfonate preparation widely used in China, exhibits anti-inflammatory, antiviral, antitumor, antibacterial and neuroprotective properties. Methods:We conducted a retrospective study of 635 pediatric inpatients with VE who were hospitalized at seven medical centers in China between January 2015 and December 2021. Risk factors for poor prognosis were compared between inpatients treated with XYPI (n = 480) and those without XYPI treatment (n = 155). Propensity score matching was performed to reduce potential confounding. Clinical symptoms, hospitalization costs, complications and sequelae were evaluated simultaneously. Results:Multivariate Logistic regression identified XYPI treatment as an independent protective factor for poor prognosis (odds ratio [OR] = 0.251, 95% confidence interval [CI]: 0.113-0.559, p < 0.001). XYPI significantly shortened the duration of fever and headache, reduced hospitalization costs, and lower the incidences of respiratory infections, myocardial injury, and sequelae (all p < 0.05). Conclusion:Adjuvant XYPI therapy may improve clinical outcomes and reduce the economic burden in pediatric VE; however, randomized trials are warranted to validate these findings.
BACKGROUND:Sleep disturbances in preschool children are associated with dietary patterns and eating behaviors. Gastrointestinal symptoms, which frequently result from poor dietary factors, may further impair sleep. This study examines the mediating effect of gastrointestinal symptoms in the association between dietary factors and sleep disturbances. METHODS:A cross-sectional online investigation recruited 7425 participants in Beijing, with 7170 included in the final analysis. Validated scales, including the Sleep Disturbance Scale for Children, Chinese Preschoolers' Eating Behavior Questionnaire, Preschool Nutrition Quotient Questionnaire, and Gastrointestinal Symptom Rating Scale, were used to assess dietary composition, eating behaviors, gastrointestinal symptoms, and sleep disturbances. RESULTS:Gastrointestinal symptoms exacerbate the damage to sleep caused by unhealthy dietary factors. Proportions mediated of gastrointestinal symptoms ranged from 17.98 % to 31.35 % on the total score of the Sleep Disturbance Scale for Children. A dietary pattern high in beverages and low in bean and fish products adversely affected sleep through gastrointestinal symptoms. The association between unhealthy eating behaviors and sleep disturbances has been validated, with proportion mediated of 61.89 % between sleep breathing disorders and low satiety responsiveness. LIMITATIONS:As a cross-sectional study, these results cannot rule out reverse causality-where sleep disturbances may influence eating behavior. or gastrointestinal symptoms. CONCLUSIONS:These findings emphasize the importance of correcting unhealthy dietary factors as key drivers of sleep disturbances. Interventions aimed at improving gastrointestinal health can simultaneously mitigate the harmful effects of unhealthy dietary factors, offering dual benefits for sleep in preschool children.
The late Ming and early Qing dynasties were among the periods with a high incidence of infectious diseases in Chinese history. In this study, we aimed to analyze historical data to investigate the geographical factors and disease spectrum of infectious diseases during that time. Our objective was to provide insights for the modern prevention and control of infectious diseases. We collected and organized historical materials from this period. Descriptive statistical methods were used to explore the correlation between epidemics and natural factors. Furthermore, we collected traditional Chinese medicine prescriptions for preventing and treating infectious diseases during this period. By predicting potential diseases treatable through core prescriptions, we aimed to clarify the disease spectrum of epidemics. Cluster analysis results showed that among geographical factors such as famine, drought, and locusts, as well as social phenomena such as wars and inflation, drought had the highest clustering value and the strongest correlation with other factors. Disease spectrum analysis revealed three common types of drug combinations used to treat epidemics: Rheum officinale-ginger-aurantium fruit-magnolia bark-mirabilite, licorice-anemarrhena root-ginseng-poria-atractylodes, and white peony root-licorice-ginger-Chinese angelica-bupleurum. Diseases with a high degree of enrichment for these drugs included Kaposi’s sarcoma-associated herpesvirus infection, human cytomegalovirus infection, and hepatitis B. During the late Ming and early Qing dynasties in China, epidemics occurred frequently and were closely related to factors such as drought, locusts, war, and inflation. The epidemics likely involved a combination of various infectious diseases, including Kaposi’s sarcoma-associated herpesvirus infection, human cytomegalovirus infection, and hepatitis B.
Objective: To uncover the underlying mechanisms of action of the Yinlai decoction on high-calorie diet-induced pneumonia through proteomics analysis. Methods: Based on the Gene Expression Omnibus (GEO) database, lung tissue samples from normal and high-fat diet (HFD) fed mice in the GSE16377 dataset were selected as test cohorts to identify differentially expressed genes and conduct bioinformatics analyses. In the animal experiments, mice were randomly divided into the control (N), high-calorie diet pneumonia (M), and Yinlai decoction treatment (Y) groups. Mice in the M group received high-calorie feed and a 0.5 mg/mL lipopolysaccharide solution spray for 30 min for 3 d. The mice in the Y group were intragastrically administered 2 mL/10 g Yinlai decoction twice daily for 3 d. Pathological evaluation of the lung tissue was performed. Differentially expressed proteins (DEPs) in the lung tissue were identified using quantitative proteomics and bioinformatics analyses. The drug-target relationships between Yinlai decoction and core DEPs in the lung tissue were verified using AutoDock Vina and Molecular Graphics Laboratory (MGL) Tools. DEPs were verified by western blot. Results: GEO data mining showed that an HFD altered oxidative phosphorylation in mouse lung tissue. The Yinlai decoction alleviated pathological damage to lung tissue and pneumonia in mice that were fed a high-calorie diet. A total of 47 DEPs were identified between the Y and M groups. Enrichment analysis revealed their association with energy metabolism pathways such as the tricarboxylic acid cycle (TCA) and oxidative phosphorylation. The protein-protein interaction network revealed that Atp5a1, Pdha1, and Sdha were the target proteins mediating the therapeutic effects of Yinlai decoction. Molecular docking results suggested that the mechanism of the therapeutic effect of Yinlai decoction involves the binding of brassinolide, praeruptorin B, chrysoeriol, and other components in Yinlai decoction to Atp5a1. Conclusion: The Yinlai decoction alleviated lung tissue damage and pneumonia in mice that were fed a high-calorie diet by regulating the TCA and oxidative phosphorylation. Our study highlights the importance of a healthy diet for patients with pneumonia and provides a scientific basis for the prevention and treatment of pneumonia through dietary adjustments.
目的 本研究观测胃肠积热合并肺炎大鼠脂肪组织变化情况,及脂肪因子和炎症因子的水平.方法 本研究选用28只SD大鼠,随机分为正常组、胃肠积热组、肺炎组、胃肠积热合并肺炎组进行造模.观察各组大鼠脂肪组织(附睾周脂肪、肩胛骨间脂肪、肾周脂肪)和肺组织病理改变,采用Western Blot检测脂肪组织、肺组织的脂肪因子(瘦素、脂联素)和炎症因子(肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、转化生长因子β1(transforming growth factor,TGF-β1))水平.结果 与正常组相比,胃肠积热组大鼠肩胛骨间脂肪的脂肪分数升高(P<0.05);与肺炎组相比,胃肠积热合并肺炎组大鼠附睾周和肩胛骨间脂肪的脂肪分数升高(P<0.05).与正常组相比,胃肠积热组大鼠脂肪组织病理可见明显的脂肪细胞肥大、增生,细胞大小不均,排列不整齐;与肺炎组相比,胃肠积热合并肺炎组大鼠的脂肪细胞也出现类似的变化.与正常组相比,胃肠积热组大鼠附睾周脂肪TNF-α 升高(P<0.05),肩胛骨间脂肪、肾周脂肪和肺中瘦素、TNF-α、TGF-β1升高(P<0.05);与肺炎组相比,胃肠积热合并肺炎组大鼠附睾周脂肪脂联素降低(P<0.05),肩胛骨间脂肪瘦素、TGF-β1升高(P<0.05)、脂联素降低(P<0.05),肾周脂肪瘦素、TNF-α升高(P<0.05),肺中瘦素、TNF-α升高(P<0.05)、脂联素降低(P<0.05).结论 胃肠积热证态存在代谢与免疫的双重失衡,短期高热量饮食喂养的胃肠积热模型大鼠脂肪组织中可观测到营养物质异常积累,促炎/抗炎脂肪因子分泌水平改变,可能与全身的代谢性炎症、肺炎的免疫损伤加重相关.
《伤寒论》中对给药剂量和给药频次精准控制的详细记载体现了滴定给药模式理念,尤其多应用在治疗急危重症的方剂中,同时通过明确其治疗目标而设计出即保证疗效又避免其毒副作用的给药途径.本文通过对《伤寒论》原文中相关方剂的服用方法分析,归纳出其药物滴定方式主要通过控制剂量与频次(时间)两个变量的组合为给药方法,以汗、吐、下等法为途径实现其祛邪目标,结合用药后临床反馈评估是否达到疗效.这种滴定给药的精准控制模式的临床意义:(1)使不同病患对于中药处方的疗效与不良反应(毒性)达到兼顾平衡;(2)将药效从量变到质变这一过程平稳过渡,达到祛邪而不伤正的疗效,为中医药治疗急危重症提供参考.
ObjectiveGastrointestinal heat retention syndrome (GHRS) is associated with lung-heat syndrome and is related to recurrent respiratory infection. Upper respiratory tract infection (URTI) lung heat syndrome is common in children. The study will explore the effect of GHRS on the structure and function of gut microbiota in children with URTI lung-heat syndrome.MethodsParticipants were divided into both groups using the self-developed URTI scale and the “GHRS Diagnostic Scale · Pediatric Part”: GHRS-positive children (LS group) and GHRS-negative children (L group). General information, clinical symptoms, and stool were collected. We used 16S rRNA amplicon sequencing technology to determine the gene sequence of the V3–V4 region in feces and measure the gut microbiota of the both groups at the genus level.ResultsA total of 23 children were included in the both groups. There were 12 cases in the LS group and 11 cases in the L group. There was no statistical difference between the both groups in age, gender, height, weight, and body mass index. The effective sequences shared by the both groups accounted for 85.66% of the total. In the gut microbiota, there was no difference in the α diversity and the β diversity between the both groups. Compared with the L group, the LS group had a significant increase in the relative abundance of the Ruminococcus gnavus group, Prevotella-9, Staphylococcus, and Actinomyces (P < .05). The functions of the both groups of microbiota primarily concentrate on metabolism, genetic information processing, and environmental information processing. The relative abundance of signaling molecules and interactions in the LS group were higher than that in the L group (P < .05). The redundancy analysis (RDA) showed that the URTI score had the greatest impact on the distribution of microbiota.ConclusionGHRS may affect the development of URTI lung-heat syndrome by changing the relative abundances of gut microbiota.
名老中医传承是进一步推动中医药创新发展的关键所在,但目前名老中医研究存在重"术"轻"道"的问题,缺乏全面挖掘和传承的方法学体系及研究范式,缺乏对名老中医防治重大、难治疾病的群体研究.认为名老中医传承工作的重点应聚焦在名老中医的"道""术"研究上.通过应用扎根理论、病例系列研究、队列研究,以及数据挖掘等定性、定量混合研究方法,创新性构建了名老中医道术传承范式.提出了不同名老中医诊治重大、难治疾病的道术异同对比研究策略,为未来基于名老中医群体开展重大、难治疾病诊疗方案提供参考意见.并利用表述性状态传递(REST)互联网架构提供网络化开放式的稳定服务,搭建医疗、科研、传承、推广一体化的服务平台作为推广范式,进一步推广名老中医道术研究成果,惠及基层医生、科研人员和广大群众.
Background The impact of COVID-19 has most likely increased the prevalence of stunting. The study aimed to determine the prevalence of stunting among kindergarten children in the context of coronavirus disease 2019 (COVID-19) in Longgang District, Shenzhen, China, and its risk factors. Methods A cross-sectional study was conducted to identify children from 11 sub districts of 481 kindergartens in the Longgang District of Shenzhen City from May to July 2021. In the context of COVID-19, an online survey was conducted to gather demographic information, height, birth information, and lifestyle. The prevalence of stunting was calculated, and the risk factors were analyzed using binary logistic regression with three stepwise models. Results A total of 118,404 subjects were included from May to July 2021, with a response and questionnaire effective rates of 85.75% and 95.03%, respectively. The prevalence of stunting and severe stunting were 3.3% and 0.8%, respectively. Model 3 showed that risk factors for stunting were male sex [odds ratio (OR) = 1.07], low birth weight (OR = 2.02), insufficient sleep time (OR = 1.08), less food intake than their peers (OR = 1.66), slower eating than their peers (OR = 1.16), accompanied by grandparents alone or non-lineal relatives (reference: parents accompanying) (OR = 1.23, 1.51), and children induced to eat (OR = 1.17). Protective factors included only-child status (OR = 0.66), reported high activity (OR = 0.37, 0.26, 0.23), parents with high education levels (father: OR = 0.87, 0.69; mother: OR = 0.69, 0.58), high monthly income per capita of the family (OR = 0.88, 0.74, 0.68), and allowing children to make food choices (OR = 0.82). Conclusion The stunting rate of children in kindergartens in Longgang District is 3.3%, close to the level of developed countries but higher than the average level of developed cities in China. The relatively high stunting rate in children under 3 years old in 2021 may be associated with the influence of COVID-19. Appropriate policies should be formulated for individuals and families with children to help children establish good living habits and reduce stunting.

Background Increasing attention has been paid to the potential relationship between gut and lung. The bacterial dysbiosis in respiratory tract and intestinal tract is related to inflammatory response and the progress of lung diseases, and the pulmonary diseases could be improved by regulating the intestinal microbiome. This study aims to generate the knowledge map to identify major the research hotspots and frontier areas in the field of gut-lung axis. Materials and methods Publications related to the gut-lung axis from 2011 to 2021 were identified from the Web of Science Core Collection. CiteSpace 5.7.R2 software was used to analyze the publication years, journals, countries, institutions, and authors. Reference co-citation network has been plotted, and the keywords were used to analyze the research hotspots and trends. Results A total of 3315 publications were retrieved and the number of publications per year increased over time. Our results showed that Plos One (91 articles) was the most active journal and The United States (1035 articles) published the most articles. We also observed the leading institution was the University of Michigan (48 articles) and Huffnagle Gary B, Dickson Robert P and Hansbro Philip M, who have made outstanding contributions in this field. Conclusion The Inflammation, Infection and Disease were the hotspots, and the regulation of intestinal flora to improve the efficacy of immunotherapy in lung cancer was the research frontier. The research has implications for researchers engaged in gut-lung axis and its associated fields.
中医药防治疫病历史悠久,积累了丰富的经验,形成了独特的辨证论治体系.疫病常用的辨证体系包括卫气营血辨证、三焦辨证、六经辨证、表里辨证,各体系能够从不同角度对于疫病病机进行辨证.在疫病的诊疗中,要注重辨证与辨病相结合,在辨证思维的指导下,辨别疫病的病性、病所、病期、病势、病理,则可确定相应的治则治法,从而确定方剂、配伍选药.文章对疫病的辨证与辨病进行了梳理和总结,有利于提高疫病的临床辨治水平.

肺系疫病是指以肺系为主要传染途径而导致的疫病,其发生发展是包括了病原微生物、气候、地理、环境等在内的多种生物、物理因素与机体脏腑、气、津、血等相互作用所产生综合反应的结果.不同人群因个人体质不同、接触病原微生物的风险大小不同,存在着不同的患病机率,四季气候的变化、不同的地理、自然环境同样可影响病原微生物的生长繁殖和传播,从而对传染病的发生发展产生不同的影响.文章基于中医学"三因制宜"原则,综合考虑不同人群、不同季节、不同地域的特点,因人、因地、因时制宜制定相应的中药处方策略,为肺系疫病的中医药预防提供参考.
ObjectiveTo explore the structural and functional characteristics of microbiota in oropharynx of sub-healthy children with gastrointestinal heat retention syndrome (GHRS) differentiated by traditional Chinese medicine, and screen the biometric operational taxonomic units (OTUs) to assist the clinical diagnosis.MethodsWe recruited children according to the "GHRS diagnostic scale", collected their oropharyngeal swabs, and sequenced the 16S rDNA V4 region. We described the bacterial structure with alpha-indexes, beta-distances, and relative abundances; moreover, we screened the differential genera/OTUs with Wilcoxon rank-sum test, MetagenomeSeq analysis, and linear discriminant analysis effect size (LEfSe) analysis, in which biometric OTUs were selected to construct the receiver operating characteristic curve to verify the diagnostic value. The bacterial function was predicted with Kyoto Encyclopedia of Genes and Genomes pathways according to 16S rDNA gene by using Phylogenetic Investigation of Communities by Reconstruction of Unobserved States.ResultsThe study population was composed of 10 children with GHRS and 10 healthy control children. GHRS children were more likely to overeat (gluttony, P = .033). Alpha-indexes, such as Sobs, abundance-based coverage estimator, Bootstrap, and Qstat, were significantly higher in the GHRS group, while beta-distances did not exhibit any significant intergroup differences. There were 9 differently distributed non-predominant genera between the groups in Wilcoxon rank-sum test, as well as 13 non-predominant genera in MetagenomeSeq analysis and 3 non-predominant OTUs in LEfSe analysis. OTU44 and OTU196 were used to construct the receiver operating characteristic curve, and the area under curve was 0.92. Predicted functions showed that pathways related to oxidative phosphorylation and carbon metabolism were enriched in healthy control samples, while the pathway related to renin secretion was remarkably enriched in GHRS samples.ConclusionUnique oropharyngeal microbial structure and function were identified in GHRS children. OTU44 and OTU196 were specific OTUs, which could be used as biomarkers of GHRS to assist clinical diagnosis.

目的:探究补肾益肺消癥方干预特发性肺间质纤维化(IPF)进展的可能机制,为"肺络微型癥疱"理论在中医药防治IPF奠定客观基础.方法:通过TGF-β1诱导MRC-5细胞转分化肺成纤维细胞,制备补肾益肺消癥方冻干粉,激光共聚焦显微镜观察MRC-5细胞线粒体自噬体数量及状况,通过Western Blot法检测NOX4、JNK、p62、Parkin、PINK1蛋白的表达.结果:模型组在TGF-β1刺激后较正常细胞组线粒体自噬现象明显增多,而西药对照组及中药低、中、高浓度组线粒体自噬现象较模型组减少.与正常组比较,模型组TGF-β1可显著激活JNK/p62/Parkin信号通路,各给药组NOX4、JNK、p62表达显著降低(P<0.05),Parkin、PINK1表达显著升高(P<0.05),但中药中浓度组对于降低JNK的表达明显优于其他给药组(P<0.05).结论:补肾益肺消癥方治疗IPF的机制之一可能是通过抑制细胞线粒体自噬功能与调控JNK/p62/Parkin通路减缓肺组织的纤维化进程.
