Background Obstructive sleep apnea syndrome(OSAS)is a sleep-related disease. Evidence has shown that OSAS may increase the risk of developing arterial stiffness(AS),but the mechanism of action still needs to be further explored. Objective To explore the mediating effect of waist circumference(WC) and fasting plasma glucose(FPG)on the association between OSAS and AS. Methods A total of 1 053 health examinees were selected from Physical Examination Center,the First People’s Hospital of Fuquan City from March 23 to November 30,2022. General demographic data were collected. The risk of OSAS was assessed using the STOP-Bang Questionnaire(high or low risk of OSAS was diagnosed by STOPBang score ≥ 4 points or <4 points). AS was assessed,and 553 cases with AS and 500 without were assigned to AS and nonAS groups,respectively. Multivariate Logistic regression analysis was used to explore the factors associated with AS. FPG was converted to exponential form(-2.576 1)to obtain the exponential value of FPG(FPGa). Multiple linear model was used to analyze the relationship of OSAS with WC and FPGa. The mediation effect of WC and FPG between OSAS and AS was analyzed using Hayes Process models 4 and 6 in R. Results AS and non-AS groups had statistically significant differences in mean age,sex ratio,prevalence of smoking and hypertension,mean body mass index,WC,neck circumference,FPG,triglyceride,and high-density lipoprotein cholesterol as well as the level of OSAS risk(P<0.05). Multivariate Logistic regression analysis showed that compared with individuals with low-risk OSAS,the risk of AS increased in those with high-risk OSAS(P<0.05),and the risk of AS increased by 0.048 times for every 1 cm increase in WC and 0.512 times for every 1 mmol/L increase in FPG(P<0.05). Multiple linear regression analysis showed that OSAS was associated with WC and FPGa(P<0.05),and WC was an associated factor of FPGa(P<0.05). The chained multimediator model showed that OSAS directly affected the incidence of AS[β=0.661,95%CI(0.284,1.038)]. The indirect mediation effect value(β) of the "OSAS→WC→AS" path was 0.224[95%CI(0.073,0.398)],accounting for 20.86% of the total effect. The indirect mediation effect value(β)of the "OSAS→FPGa→AS" path was 0.115[95%CI(0.024,0.216)],accounting for 10.71% of the total. The indirect mediation effect value(β) of the "OSAS→WC→FPGa→AS" path was 0.074[95%CI(0.036,0.126)],accounting for 6.89% of the total. Conclusion WC and FPG may partially mediate the relationship between OSAS and AS. In addition,they are involved in the process of "OSAS→WC→FPGa→AS" as chained mediators. People with high risk of OSAS should actively control WC to reduce the possibility of developing central obesity,and regulate FPG to prevent the occurrence of AS.
目的 分析睡眠效率与蒙特利尔记忆指数(MIS)得分的关系及其性别差异,为预防中老年人认知功能下降提供参考.方法 于2022年3-8月对福泉市第一人民医院健康体检中心符合纳入标准的699名体检人员进行面对面调查.应用R 4.2.2软件对数据进行Mann-Whitney U检验和Kruskal-Wallis H秩和检验,利用有向无环图得出睡眠效率与MIS关联中所需调整的最小控制变量集,使用分位数回归模型分析睡眠效率与记忆指数的关联性.结果 睡眠效率>85%者MIS得分中位数为11分(P25~P75:9~13分),高于睡眠效率≤85%者(中位数为10分,P25~P75:8~13分),差异有统计学意义(Z=-2.897,P<0.01).分位数回归模型分析结果显示,在总人群中,相较于睡眠效率高组(>85%),睡眠效率低(≤85%)在MIS得分P20-40分位的回归系数小于零(P<0.05);在男性中,睡眠效率低(≤85%)在MIS得分P60分位点及以下的回归系数小于零(P<0.05),回归系数绝对值由MIS得分P10分位的1.87变化至P60分位的0.71;在女性中,睡眠效率在MIS得分各分位点的影响效应均无统计学意义(P>0.05).结论 睡眠效率低对低分位MIS得分的中老年人影响较大,在男性中老年人可能是受影响的主要人群.
目的:分析青春期骨性错畸形患者髁突位置与性别及颅面骨骼形态的关系.方法:使用正畸模拟治疗软件(Dolphin imaging 11.8)对391例15~16岁不同骨性错畸形患者的锥形束CT(CBCT)图像进行三维重建,在髁突矢状面上对颞下颌关节前、上、后间隙进行测量并计算髁突相对位置.采用SPSS 23.0软件包分析不同性别、矢状骨面型和垂直骨面型髁突位置的差异,并通过多因素logistic回归模型探讨其关联性大小.结果:单因素分析显示,髁突位置在不同矢状骨面型和性别间具有显著差异(P<0.05).多因素分析显示,女性发生髁突前移位的可能性是男性的1.942倍(95%CI:1.166~3.236,P=0.011).2个骨性指标中,右侧关节结节后斜面斜度越大,髁突越易后移,其OR值为1.032(95%CI:1.000~1.066,P=0.048).结论:关节结节后斜面斜度可能与髁突后移位有关,女性髁突比男性更易前移.
背景 中老年人的晚睡行为可能影响其血脂代谢,但二者的关联性仍需深入分析.目的 探讨不同性别、中心性肥胖、睡前加餐和吸烟人群睡眠时型与血脂异常的关联性.方法 对 2022 年 3-8 月在福泉市第一人民医院体检中心体检的 40~65 岁人群开展问卷调查(n=697),收集研究对象的一般资料和睡眠情况,采用"清晨型-夜晚型量表"的单项问题来评价睡眠时型.采用非条件二分类Logistic回归模型评估睡眠时型与血脂异常患病风险的关联,在总人群中以性别、中心性肥胖、睡前加餐进行分层分析,在男性人群中以是否吸烟进行分层分析.采用敏感性分析排除夜班工作的影响.结果 纳入体检者中,睡眠时型为清晨型偏好393人(56.4%)、夜晚型偏好304人(43.6%),检出血脂异常 334 人(47.9%).剔除夜班人群后的非条件二分类Logistic回归分析结果显示,睡眠时型是血脂异常的影响因素[OR(95%CI)=1.54(1.10,2.16)](P<0.05),夜晚型人群患高TG血症、低HDL-C血症的风险高于清晨型人群[OR(95%CI)分别为 1.48(1.04,2.12)、1.79(1.18,2.72)](P<0.05).在总人群中进行分层分析结果显示:在男性人群和中心性肥胖人群中,夜晚型人群患低HDL-C血症的风险分别是清晨型的1.80倍[95%CI为(1.12,2.91)](P<0.05)和 1.73 倍[95%CI为(1.02,2.81)](P<0.05);在睡前加餐人群中,夜晚型人群患高TG血症的风险是清晨型的 3.43 倍[95%CI为(1.30,8.99)](P<0.05),在女性人群、非中心性肥胖人群中睡眠时型对血脂异常及其各指标均无明显影响(P>0.05).在男性人群中以吸烟分层,结果显示:在吸烟的男性人群中,夜晚型人群患低HDL-C血症的风险是清晨型的1.83倍[95%CI为(1.03,3.26)](P<0.05),在吸烟和不吸烟的男性人群中,睡眠时型对高TC血症、高TG血症、高LDL-C血症和非HDL-C异常均无明显影响(P>0.05).结论 在 40~65 岁人群中,夜晚型睡眠偏好可能是血脂异常的危险因素.不同性别、是否为中心性肥胖、睡前是否加餐、是否吸烟人群的睡眠时型与血脂异常之间的关联性可能存在一定差异.
调查少数民族护生职业成熟度及生命意义感的现状,并分析二者的关系.结果提示,民族地区护理教育者应根据少数民族护生的身心发展特点,结合少数民族地区特殊的民族因素影响开展生命教育和职业规划教育,以促进民族地区护理事业发展.
目的 通过结构方程模型探本科男护生职业认同影响因素的作用路径及大小.方法 于2020年5月-8月,利用便利抽样抽取贵州省某医学院校所有本科男护生进行一般情况问卷调查、护生职业认同量表(PIQNS)和职业相关社会支持量表的电子问卷调查,通过因子分析构建影响男护生职业认同的潜变量因子,用AMOS 22.0软件建立结构方程模型,分析各影响因子间的作用方式.结果 本科男护生职业认同得分中位数为55分,职业相关社会支持得分中位数为70分;相关性分析显示,职业认同与职业相关社会支持得分呈正相关(r=0.456);结构方程路径分析显示,对本科男护生职业认同影响效应最大的是专业意愿因子,总效应为0.516,直接效应为0.470,而专业意愿因子和突发公共卫生事件相关因子均可通过影响职业相关社会支持对职业认同产生间接效应,其效应值分别为0.192、0.279,职业相关社会支持仅对职业认同产生直接影响,效应值为0.241.结论 职业相关社会支持和突发公共卫生事件会对本科男护生职业认同产生影响,但专业意愿因素依然是影响男护生职业认同的主要因素.
