患者男,57岁,2020年10月19日入海军军医大学第一附属医院,主诉:高热、双下肢乏力伴肿痛3 d.2020年10月16日患者因进食醉虾后出现发热,最高达39.1℃,2020年10月17日开始出现双下肢乏力,站立不稳,摔倒在地,被送往当地医院.
Objective:To understand the status quo, work attitude, and turnover intention of burns and trauma women doctors, analyze the influence of basic situation, work direction, work intensity, work-life balance and physical health on the work attitude and turnover intention, and reveal the importance of the burns and trauma women doctors′ occupational factors to work attitude and turnover intention, which will provide a reference for the improvement of the work system of burns and trauma women doctors in china.Methods:A national questionnaire survey of burns and trauma women doctors was organized by the Burns Professional Committee of the Chinese Medical Women′s Association. From October 12, 2019 to November 28, 2019, the basic information, work direction, work intensity, work-life balance, physical health, work attitude and turnover intention of 187 burns and trauma women doctors were collected. A descriptive analysis of the occupational status of burns and trauma women doctors in China was carried out. At the same time, univariate and multivariate regression analysis of factors affecting work attitude and turnover intention were carried out to find the risk factors.Results:The survey shows that most of the 187 Chinese burns and trauma women doctors work in tertiary hospitals (82.89%) and are distributed in 27 provinces, municipalities, autonomous regions. Eastern area of China accounts for the largest proportion (29.41%), with master′s degree (48.66%) as the main degree. The working years are mostly within 15 years (63.60%), most people (66.31%) work more than 50h a week, work 5-6 times a month was the common frequency of night shifts (31.02%), and their income ranges from 3, 000 to 12, 000 yuan. The proportion of abnormal physical examination (37.43%) and sleep disorders (49.73%) is large. There are not many burns and trauma women doctors (42.25%) with a enthusiastic work attitude, and 34.22% of them have the idea of resign. The main reasons for them to leave are over-high work intensity, low income, poor health, and poor development of career. Univariate logistics regression analysis found that weekly working hours, monthly night shifts, monthly income, sleep disorder, family members support have impact on the work attitude, and age, degree, title, working years, weekly working hours, monthly night shifts, monthly income, exercise, and family members support have impact on the turnover intention. In the multivariate logistics regression analysis, the factor that still affect work attitudes is sleep disorders. After eliminating the interference of other factors, the only factor that still affects the turnover intention is the degree.Conclusion:Burns and trauma women doctors are mainly employed in tertiary hospitals. They are distributed in various regions of the country. The working life of them is mostly within 15 years. Most of them have high work intensity, poor physical condition and sleep disorders. Among burns and trauma women doctors, the proportion of negative work attitudes accounted for 57.75%. The factor related to work attitude was sleep disorders. 34.22% of people have the idea of resign, and the related factor is their degree.
The case of long-term complicated infectious mass on the inner thigh root after transobturator urethral sling is rare. This paper reported a case, who underwent a surgery of the "trans-obturator mid-urethral slings" for stress urinary incontinence 9 years ago. A mass on the root of the right thigh was found 3 months ago, accompanied with low fever. About 1cm tape was exposed on the front wall of the right side of the vagina. The patient underwent resection of the mass on the root of the right thigh and partial removal of the tape under spinal anesthesia. After one year’ follow-up, there was no significant change in urinary control ability compared with that before the operation.
Background: Hypertrophic scars are one of the main complications that affect the quality of life of patients after burns. Many methods have been shown to be effective in the treatment of hypertrophic scars, such as ablative fractional CO2 laser (AFCL) and platelet-rich plasma (PRP). However, there are few studies on the effect of the combined application of these measures. The purpose of this study was to explore the therapeutic effect of AFCL combined with PRP on hypertrophic burn scars. Methods: A retrospective clinical observation study was conducted on 50 patients with hypertrophic burn scars. The AFCL+PRP group included 31 patients who received AFCL combined with PRP treatment; the AFCL group included 19 patients who received AFCL treatment only. The University of North Carolina 4P Scar Scale (UNC4P) and the Vancouver Scar Scale (VSS) scores that were collected before each treatment were used as indicators of the effectiveness of the previous treatment. The scores recorded at the second, fourth and seventh months were analysed. Results: The demographic data of the 2 groups were not significantly different. Before treatment, there was no difference in the UNC4P and VSS scores between the 2 groups. There was a significant decline in the UNC4P and VSS total scores over 6 months in both groups (p < 0.05) and scores in the 2 groups were comparable after 3 and 6 months (p < 0.05). UNC4P scores in the AFCL+PRP group decreased from a mean of 8.26 to 2.61 (p < 0.05) with a concomitant drop in VSS scores from a mean of 11.74 to 6.06 (p < 0.01). In the AFCL group UNC4P and VSS scores decreased from 7.68 to 4.63 (p < 0.05) and from 10.89 to 8.16 (p < 0.05), respectively. The sub-items of these 2 assessments were analysed and the results suggest that AFCL combined with PRP can comprehensively improve scarring. Conclusions: This study shows that PRP is an effective adjunct for AFCL in the treatment of hypertrophic burn scars and that the combination of PRP and AFCL proved to be more useful than AFCL alone. This combination may be a new and effective clinical practice for the treatment of scars. However, larger and higher-level clinical studies are still needed to determine its efficacy and possible mechanisms.
