To compare plasma levels of miR-126-3p between patients with essential hypertension (EH) and healthy individuals, and to investigate its correlation with renin–angiotensin–aldosterone system (RAAS) biomarkers. 29 healthy controls and 71 EH patients were enrolled. Total RNA, including miRNA, was extracted from plasma. Relative expression of miR-126-3p was determined by qRT-PCR. The discriminative ability of miR-126-3p and a combined panel was evaluated using ROC curves. The relative expression of miR-126-3p was significantly higher in the EH group (3.20 ± 2.15) than in the healthy group (1.54 ± 1.27, P < 0.001). Spearman correlation showed a significant negative correlation between miR-126-3p and serum ACE2 activity (r = − 0.205, p = 0.041). The AUC of a combined indicator (miR-126-3p, AngII, LDL-C, TC) was 0.859 (95% CI: 0.769–0.950, P < 0.001). Stepwise regression identified AngII, TC, LDL-C, and miR-126-3p as significant (P < 0.05), although collinearity was within an acceptable range (VIF < 5). miR-126-3p is significantly upregulated in the peripheral blood of EH patients and correlates negatively with ACE2. The combined panel shows moderate exploratory discriminative ability. However, due to the absence of continuous blood pressure data, no claim of added value over blood pressure measurement can be made. The potential role of miR-126-3p in modulating the RAAS counter‑regulatory axis requires further validation.
ABSTRACT This study aimed to investigate the role of let‐7a‐5p in the pathogenesis of essential hypertension (EH) and its correlation with the renin‐angiotensin‐aldosterone system (RAAS) biomarkers. Ninety‐eight EH patients and 24 healthy controls (HC) enrolled in the study were assayed for the relative expression of let‐7a‐5p in plasma by quantitative real‐time polymerase chain reaction (Q‐PCR), and biomarkers of the RAAS system, including angiotensin‐converting enzyme 2 (ACE2), Ang (1‐7), MAS1, angiotensin‐converting enzyme (ACE), angiotensin II (Ang II), and angiotensin II type 1 receptor (AT1R), were determined by enzyme‐linked immunosorbent assay (ELISA) The expression levels of the biomarkers of RAAS system were determined. The results showed that the expression levels of let‐7a‐5p in the plasma of EH patients were remarkably higher than those of HC. The prediction model of combined let‐7a‐5p showed high accuracy by constructing a subject operating characteristic (ROC) curve with an area under the curve (AUC) of 0.885, and the reliability of the model was further verified by the Hosmer–Lemeshow (H–L) goodness‐of‐fit test, the Model Calibration Curve, and the Decision Curve Analysis. Spearman correlation analysis revealed that the expression of let‐7a‐5p was positively correlated with ACE (r = 0.352, p < 0.001), and mediation analysis indicated that ACE partially mediated between let‐7a‐5p and the development of hypertension. The present study concludes with the potential of let‐7a‐5p as a companion diagnostic biomarker for EH. It suggests that there may be a complex regulatory mechanism between it and specific RAAS biomarkers, which provides a new perspective on the pathogenesis and diagnosis of EH.
As one of the most serious microvascular complications of diabetes mellitus (DM), diabetic retinopathy (DR) can cause visual impairment and even blindness. With the rapid increase in the prevalence of DM, the incidence of DR is also rising year by year. Preventing and effectively treating DR has become a major focus in the medical field. Traditional Chinese medicine (TCM) has a wealth of experience in treating DR and has achieved significant results with various herbs and TCM prescriptions. Traditional Chinese Medicine (TCM) provides a comprehensive therapeutic strategy for diabetic retinopathy (DR), encompassing anti-inflammatory and antioxidant actions, anti-neovascularization, neuroprotection, regulation of glucose metabolism, and inhibition of apoptosis. This review provides an overview of the current status of TCM treatment for DR in recent years, including experimental studies and clinical researches, to explore the clinical efficacy and the underlying modern mechanisms of herbs and TCM prescriptions. Besides, we also discussed the challenges TCM faces in treating DR, such as drug-drug interactions among TCM components and the lack of high-quality evidence-based medicine practice, which pose significant obstacles to TCM's application in DR.
Primary hypertension is a significant risk factor for cardiovascular diseases. However, the pathogenesis of primary hypertension involves multiple biological processes, including the nervous system, circulatory system, endocrine system, and more. Despite extensive research, there is no clear understanding of the regulatory mechanism underlying its pathogenesis. In recent years, miRNAs have gained attention as a regulatory factor capable of modulating the expression of related molecules through gene silencing. Therefore, exploring differentially expressed miRNAs in patients with essential hypertension (EH) may offer a novel approach for future diagnosis and treatment of EH. This study included a total of twenty Han Chinese population samples from Hefei, China. The samples consisted of 10 healthy individuals and 10 patients with EH. Statistical analysis was conducted to analyze the general information of the two-sample groups. High-throughput sequencing and base identification were performed to obtain the original sequencing sequences. These sequences were then annotated using various databases including Rfam, cDNA sequences, species repetitive sequences library, and miRBase database. The number of miRNA species contained in the samples was measured. Next, TPM values were calculated to determine the expression level of each miRNA. The bioinformatics of the differentiated miRNAs were analyzed using the OECloud tool, and RPM values were calculated. Furthermore, the reliability of the expression was analyzed by calculating the area under the Roc curve using the OECloud tools. Statistical analysis revealed no significant differences between the two samples in terms of age distribution, gender composition, smoking history, and alcohol consumption history (P > 0.05). However, there was a notable presence of family genetic history and high BMI in the EH population (P < 0.05). The sequencing results identified a total of 245 miRNAs, out of which 16 miRNAs exhibited differential expression. Among the highly expressed miRNAs were let-7d-5p, miR-101-3p, miR-122-5p, miR-122b-3p, miR-192-5p, and miR-6722-3p. On the other hand, the lowly expressed miRNAs included miR-103a-3p, miR-16-5p, miR-181a-2-3p, miR-200a-3p, miR-200b-3p, miR-200c-3p, miR-221-3p, miR-30d-5p, miR-342-5p, and miR-543. This study initially identified 16 miRNAs that are aberrantly expressed and function in various processes associated with the onset and progression of essential hypertension. These miRNAs have the potential to be targeted for future diagnosis and treatment of EH. However, further samples are required to provide additional support for this study.
Objective To investigate the effect of Ziyin Qianyang Prescription on myocardial fibrosis in spontaneously hypertensive rats (SHRs) and its mechanism. Methods A total of 40 male SHRs were randomly divided into model group, enalapril group, and high- and low-dose Ziyin Qianyang Prescription groups, with 10 rats in each group, and 10 male Wistar rats were selected as normal group. Each administration group was given the corresponding drug by gavage twice a day for 6 consecutive weeks. Blood pressure was recorded for the rats in each group; Masson staining was used to observe the pathological changes of myocardial tissue; Western blot was used to measure the protein expression levels of connective tissue growth factor (GTGF), collagen type I (Col-1), and alpha-smooth muscle actin (α-SMA) in myocardial tissue, and RT-PCR was used to measure the mRNA expression levels of GTGF, Col-1, and α-SMA in myocardial tissue. Results Compared with the model group, the high- and low-dose Ziyin Qianyang Prescription groups and the enalapril group had a significant reduction in blood pressure (P <0.05), and the high-dose Ziyin Qianyang Prescription group and the enalapril group had a significant reduction in blue myocardial collagen fibers, as well as significant reductions in the protein and mRNA expression levels of Col-1, CTGF, and α-SMA (P <0.05). Conclusion Ziyin Qianyang Prescription can reduce blood pressure and inhibit myocardial fibrosis in SHRs, possibly by inhibiting the proliferation and transdifferentiation of myocardial fibroblasts.
凡遇疑难病例,常以其诊断之疑治疗之难,极其考验医者思辨之巧.现选用高尔鑫应用"朱涛如新制龙牡汤"治疗围绝经期重度心律失常、难治性重度失眠和小儿多动症疑难病例三则分析其临证经验.围绝经期重度心律失常案,重在调和营卫,宁心安神,疏肝解郁;顽固性失眠案,重在滋肝肾之阴,并济水火,和营卫使阴阳交泰而安眠;小儿眼睑瞤动多动症案,重在滋阴养络,安神定志,涤痰息风.
Objective To observe the characteristics of sublingual collaterals in patients with chronic atrophic gastritis based on image data; To conduct correlation analysis on TCM syndrome types and characteristics of sublingual collaterals. Methods Totally 256 patients with chronic atrophic gastritis who were treated in Hangzhou Yuhang District Hospital of Traditional Chinese Medicine were selected and divided into 5 TCM syndrome types,namely, spleen-stomach damp-heat syndrome(n=63), liver-stomach disharmony syndrome(n=56), spleen-stomach deficiency syndrome(n=51), stomach-yin deficiency syndrome(n=44), and stomach collateral obstruction(n=42).Sublingual collateral images of the patients were collected with a smartphone, and the RGB and HSV of the sublingual collaterals were obtained by combining computer vision and manual interpretation, as well as the sublingual collaterals morphological indicators such as the trunk color, length, width, varicose degree, filling degree,and other performance. The correlation between TCM syndrome types and characteristics of sublingual collaterals was analyzed. Results There were differences in the trunk color, length, width, varicose degree, filling degree, and concurrent manifestations of sublingual collaterals in different syndrome types. According to correlation analysis,there was a negative correlation between the color of sublingual collaterals and their parameters R, G, B, and V(P<0.05). But the integral value was higher than other syndrome types. TCM syndrome types were negatively correlated with R, G, B and V values(P<0.05). The degree of sublingual collateral stasis in the stomach collateral obstruction syndrome was higher than that in the other syndrome types, and the degree of sublingual collateral stasis in the spleenstomach deficiency syndrome was lower than that in the other syndrome types. Conclusion The objective index of sublingual collaterals is sensitive to chronic atrophic gastritis, which are basically consistent with the theory and clinical conclusions of TCM, and can be used as evidence for diagnosing the disease.
OBJECTIVE:To explore the correlation between tongue and oral microbiota, we studied the microbial community structure of different tongue coating types in patients with chronic gastritis.METHODS:16S rDNA gene sequencing and bioinformatics analysis were used to study the dynamic changes and correlation of microbial flora in patients with chronic gastritis, healthy people, and patients with different tongue fur. In addition, it was also discussed between the severity of gastritis and the microflora of tongue fur.RESULTS:The microbial diversity of tongue fur in patients with chronic gastritis was significantly different from healthy controls. There were significant changes in bacterial communities' diversity and relative abundance between extra tongue fur in patients but not in healthy people. Oral bacteria with relative abundance > 1% and < 0.05 among different tongue fur flora were dominant bacteria, including 12 phyla such as and , and 256 genera such as and .CONCLUSIONS:The changes in oral flora in patients with chronic gastritis were related to tongue fur. Therefore, the significant microbiota might enlighten further study on the correlation between tongue inspection and oral microbiota in patients with chronic gastritis.
为提高学生中医脉诊技能,从当前教学实际出发,探索中医脉诊技能实训的教学方法改革,课程组提出基于"理论-实训-实验"的线上线下多元融合教学模式,采用慕课、自适应学习系统丰富线上教学资源,采取集中示教、分组训练、体验式教学、中医脉象分析系统实验等方式开展多元线下教学.多元化的教学模式有助于调动学生的学习兴趣、突出自适应学习、培养学生的自主学习能力、提高学习效果,有助于强化学生的临床实践技能.
作为一种基因表达调节剂,miRNA可能与房颤诱导的离子通道重塑和纤维化有关,可能是房颤的新型诊断生物标志物以及潜在治疗靶点.此外,宏观的中医证候与微观的miRNA之间具有潜在相关性:一方面,中医证候的动态变化特征与miRNA的时序特异性、组织特异性等生物学特征有高度相似性,以miRNA作为心房颤动中医证候研究的切入点具有重要价值;另一方面,miRNA在中医药防治心房颤动中具有巨大优势.相关动物实验及临床试验已验证其临床疗效,但目前还没有明确的miRNA被确定为心房颤动有效的生物标志物或治疗靶点,今后的研究可从扩大样本量、控制变量影响程度着手,提高实验数据的准确性和可用性.同时,对miRNA与房颤中医证候相关性的研究不足,房颤中医证候要素形成的具体生物学标志物尚不明确,需进一步寻找能够指导心房颤动中医辨证分型的生物标志物,并将其定量化、标准化.同时,研究与心房颤动中医药相关的miRNA表达谱和靶基因,以miRNA调控网络为切入点研究中医药治疗心房颤动的作用机制.
Objective:Essential hypertension (EH) is a common cardiovascular disease that endangers human health. Its pathogenesis is complex and has not been fully elucidated. We explore the association between EH and interactions among polymorphisms of the angiotensin converting enzyme (ACE) gene in the Hefei region, Anhui, China.Methods:A total of 500 participants (400 hypertensive and 100 normotensive) were included in this study. The polymorphisms were detected via improved multiple ligase detection reaction (iMLDR). To improve the accuracy of prediction, multifactor dimensionality reduction (MDR) was used to analyze the overall effect of interactions among seven loci on the incidence of EH.Results:The frequencies of polymorphisms in the ACE genes rs12709426, rs4291, rs4309, rs4331, rs4343, rs4459609, and rs4461142 in the EH group were not statistically significantly different from those in the control group. We also found that the single nucleotide polymorphism (SNP) rs12709426 only had a homozygous AA genotype and no polymorphisms. There were no differences in the frequency of genetic polymorphisms between the EH and control groups. The best model explaining the EH group was the combined effect of ACE genes rs4291, rs4309, and rs4461142.Conclusion:There is an interaction effect among ACE gene loci in EH patients in Hefei region, Anhui, China. Also, the ACE gene SNP rs12709426 only has a homozygous AA genotype and does not show an association with EH.
目的 基于网络药理学方法探讨怀牛膝治疗高血压的作用机制.方法 通过中药系统药理学分析平台(TCMSP)筛选怀牛膝的有效成分及相应的靶蛋白,通过 Genecards、OMIM 数据库获取高血压的靶点,利用Venn在线软件获取怀牛膝-高血压交集靶点,运用Cytoscape 3.7.2 软件构建"化合物-靶点-疾病"网络图.使用String数据库绘制蛋白互作(PPI)网络,利用David数据库和R语言 3.6.3 中clusterProfiler包对关键靶点进行GO功能富集及KEGG通路分析,通过分子对接技术将怀牛膝主要活性成分与关键靶点进行对接验证.结果 共筛选出怀牛膝潜在活性成分16 个,有效作用靶点183 个.通过Venn图获得 76 个药物疾病共同靶点,PPI网络发现IL6、AKT1、VEGFA、CCL2、MMP9、NOS3、PTGS2 等可能是怀牛膝治疗高血压的关键靶点.GO分析共包含396 条富集结果,其中生物过程307 条,分子功能54 条,细胞组成35 条.KEGG通路分析发现60 个条目.分子对接筛选得到与靶点有结合潜力的化合物.结论 怀牛膝通过"多成分-多靶点-多途径"发挥降压作用,为深入研究其物质基础及作用机制奠定了基础,也为后期实验验证和临床应用提供了相关依据.
Background Tongue images (the colour, size and shape of the tongue and the colour, thickness and moisture content of the tongue coating), reflecting the health state of the whole body according to the theory of traditional Chinese medicine (TCM), have been widely used in China for thousands of years. Herein, we investigated the value of tongue images and the tongue coating microbiome in the diagnosis of gastric cancer (GC). Methods From May 2020 to January 2021, we simultaneously collected tongue images and tongue coating samples from 328 patients with GC (all newly diagnosed with GC) and 304 non-gastric cancer (NGC) participants in China, and 16 S rDNA was used to characterize the microbiome of the tongue coating samples. Then, artificial intelligence (AI) deep learning models were established to evaluate the value of tongue images and the tongue coating microbiome in the diagnosis of GC. Considering that tongue imaging is more convenient and economical as a diagnostic tool, we further conducted a prospective multicentre clinical study from May 2020 to March 2022 in China and recruited 937 patients with GC and 1911 participants with NGC from 10 centres across China to further evaluate the role of tongue images in the diagnosis of GC. Moreover, we verified this approach in another independent external validation cohort that included 294 patients with GC and 521 participants with NGC from 7 centres. This study is registered at ClinicalTrials.gov, NCT01090362. Findings For the first time, we found that both tongue images and the tongue coating microbiome can be used as tools for the diagnosis of GC, and the area under the curve (AUC) value of the tongue image-based diagnostic model was 0.89. The AUC values of the tongue coating microbiome-based model reached 0.94 using genus data and 0.95 using species data. The results of the prospective multicentre clinical study showed that the AUC values of the three tongue image-based models for GCs reached 0.88-0.92 in the internal verification and 0.83-0.88 in the independent external verification, which were significantly superior to the combination of eight blood biomarkers. Interpretation Our results suggest that tongue images can be used as a stable method for GC diagnosis and are significantly superior to conventional blood biomarkers. The three kinds of tongue image-based AI deep learning diagnostic models that we developed can be used to adequately distinguish patients with GC from participants with NGC, even early GC and precancerous lesions, such as atrophic gastritis (AG). 目的 基于CiteSpace分析三焦辨证研究热点及前沿.方法 检索中国知网从建库至2021年6月5日的所有关于三焦辨证领域的文献.采用Excel 2013对年发文量趋势分析,用CiteSpace 5.6.R2软件对作者合作、关键词聚类以及研究热点、前沿等进行可视化分析.结果 共检索到1230篇相关文献.年发文量一直处于上升趋势;发文量排前3位的作者是原晓风(8篇)、杨进(7篇)、赵进喜(5篇);北京中医药大学(115篇)是发文量最多的研究机构.关键词聚类分析结果为:#0湿热病、#1三焦、#2新型冠状病毒肺炎、#3三焦辨证、#4中医学、#5《伤寒论》、#6三仁汤、#7病机、#8学术思想、#9甘温除热、#10中医辨证.可以看出三焦辨证涵盖了中医药、机制、临床以及现瘟疫的研究.近20年的研究热点可以分为学术研究类(关键词:温病条辨、文献研究、中医、学术思想、病因病机、综述),临床类(关键词:名医经验、辨证论治、中医药治疗、辨证),疾病类(关键词:艾滋病、温病、眩晕、湿热、Covid-19).结论 三焦辨证领域有基础理论和疾病等多方位研究,通过图谱初步直观展现了三焦辨证领域发展脉络、研究热点及前沿趋势,为中医诊断、温病学研究提供研究方向.
目的 探讨原发性高血压中医证型的分布规律及其与危险因素的相关性.方法 收集290例原发性高血压患者的一般资料,采用无序多分类Logistic回归分析原发性高血压中医证型与性别、年龄、体质量指数、饮食偏嗜、吸烟史、饮酒史、高血压病家族史的相关性.结果 原发性高血压中医证型以痰湿壅盛证为主.单因素分析结果显示,原发性高血压中医证型与年龄、饮酒史、高血压家族史及饮食偏嗜具有相关性(P<0.05).无序多分类Logistic回归分析显示,以阴阳两虚型为对照组,年龄≤65岁(P<0.05,OR=29.328)和高血压病家族史(P<0.05,OR=9.499)与肝火亢盛型相关;年龄≤65岁(P<0.05,OR=9.241)和饮食偏咸(P<0.05,OR=4.840)与痰湿壅盛型相关;年龄≤65岁(P<0.05,OR=10.304)和高血压病家族史(P<0.05,OR=8.312)与阴虚阳亢型相关.结论 痰湿壅盛型是原发性高血压的主要证型,饮食偏咸是痰湿壅盛型的危险因素;家族史是肝火亢盛型、阴虚阳亢型的危险因素.
高血压作为心脑血管系统的高发病,传统西医治疗治疗过程中对药物依赖性强,同时存在一定的副作用,而中医药治疗高血压具有整体调节、协同增效、减毒减副等优势.名老中医的学术思想是他们数十年的学术研究、临床实践与中医药理论以及前人经验相结合的智慧结晶,传承研究继而发展名老中医的学术思想、临床经验,可以提高中医药服务诊疗水平,推动中医学术进展.通过查阅检索国医大师及全国名老中医等当代名中医对高血压的辨证论治经验,发现名老中医认为高血压发病以肝为本,与心、肾、脾等脏密切相关,病理因素多归于"风""火""痰""虚""瘀"等,大致将高血压分为肝火亢盛型、肝阳上亢型、肝气郁结型、痰湿壅盛型、痰瘀互结型、肝肾亏虚型等类型,治疗常用清肝降火、平肝潜阳、疏肝解郁、健脾化痰、逐瘀祛痰、滋补肝肾等法,疗效显著,颇具特色.
舌色是舌诊观察中必不可少的一部分,常作为中医区别正气盛衰、病邪性质、预测病情进退的指标.传统舌色识别以肉眼观察为主,不可避免受主客观因素影响,难以定量化,制约了舌诊客观化发展.课题组以前期图像识别和大数据分析的基础,研发人工智能舌色辨识系统,实现手机移动端便捷采集舌象;通过机器学习、深度学习的神经网络算法,为各类人群的舌色特征配置最优算法模型;再根据专家标注结果修正,优化模型,提高舌色的颜色属性综合判定准确性,实现舌色精准识别和多维度定量化分析.通过AI舌色辨识与移动设备相结合,为舌诊客观化提供新的技术方法.
目的:探讨初诊胃癌患者的舌象特点,分析其舌象变化的规律.方法:采用中医人工智能健康状态辨识系统提取301例初诊胃癌患者的舌象特征,探究胃癌患者不同肿瘤特征的舌象特点.结果:胃癌患者不同TNM分期舌色、苔质分布差异有统计学意义(P<0.01),Ⅰ期多见淡红舌,Ⅱ、Ⅲ期多见红绛舌,Ⅳ期多见青紫舌,并随着病情进展,淡红舌逐渐减少,青紫舌逐渐增多;Ⅰ期多见薄苔,Ⅱ、Ⅲ、Ⅳ期多见厚腻苔.肿瘤直径≥5 cm时厚腻苔显著增多(P<0.05),<5 cm时薄苔显著增多(P<0.05);肿瘤标志物CA50>25 ng/mL、CA199>37 ng/mL多见厚腻苔,CA199≤25 ng/mL多见薄腻苔,CA125>35 ng/mL时薄苔显著减少,CEA>5 ng/mL时厚苔显著增多(P<0.05).结论:不同临床分期的胃癌患者舌色、苔质有所不同;不同肿瘤大小的胃癌患者苔质不同;肿瘤标志物异常升高以厚腻苔为主.
目的 探讨百会实按灸结合通督调神针法治疗脑卒中后眩晕的临床疗效.方法 选取脑卒中后眩晕患者80例,根据随机数字表法分为对照组和观察组各40例,两组均在脑卒中对症治疗及康复治疗的基础上进行,对照组采用口服盐酸倍他司汀治疗,观察组采用百会实按灸结合通督调神针法治疗,两组均连续治疗2w.比较两组总体疗效及安全性、眩晕评估评分量表(DARS)、眩晕障碍评定量表(DHI)、眩晕病中医证候评价量表评分、经颅多普勒诊断/监护系统检测(TCD)检测指标.随访3个月,评估患者复发率.结果 观察组治疗有效率显著高于对照组(P<0.05);治疗前两组椎-基底动脉平均血流速度、DARS、DHI、中医证候评分差异无统计学意义(P>0.05),治疗后两组椎-基底动脉平均血流速度显著增加,且观察组显著快于对照组(P<0.05);治疗后两组DARS、DHI、中医证候评分显著降低,且观察组显著低于对照组(P<0.05);观察组复发率显著低于对照组(P<0.05);两组治疗后未见实验室检查结果 异常,无严重不良反应事件.结论 百会实按灸结合通督调神针法治疗脑卒中后眩晕能扩张椎-基底动脉,增加脑血流量,改善血流动力学指标,可较快改善患者眩晕症状,减轻眩晕程度,对中医证候及总体疗效也有较好治疗效果,并具有不易复发的优点,安全性高.
Objective:To study the association between essential hypertension(EH) and angiotensin converting enzyme(ACE)gene polymorphism in Hefei,Anhui Province,and to analyze the influence of age,sex,smoking,drinking,diet,family history and other factors on EH. Methods:The patients(n=400)with EH and 100 healthy cases were included as the control group. The polymorphisms of ACE gene rs12709426 were detected by applying Improved Multiple Ligase Detection Reaction. In order to detect the clinical and biochemical indexes,the questionnaire was used to collect the general situation of patients and various influencing factors,and the automatic biochemical analyzer was used to detect them. Results:The ACE gene rs12709426 only had homozygous AA genotype and no gene polymorphism. There was no significant difference in genotype polymorphism frequency between EH group and control group(χ~2=0,P > 0. 999).
