OBJECTIVE:To test the hypothesis that women assigned to a natural ovulation regimen before frozen embryo transfer compared with a programmed regimen would have an increased chance of a healthy live birth and a reduced risk of pre-eclampsia or eclampsia. DESIGN:Multicentre, randomised, parallel group, assessor blinded clinical trial. SETTING:24 academic fertility centres in China. PARTICIPANTS:4376 ovulatory women (aged 20-40 years) planning to undergo a frozen single blastocyst transfer. INTERVENTIONS:Eligible participants were randomised (1:1) to receive a natural ovulation regimen or a programmed regimen of hormone replacement for endometrial preparation. Endometrial preparation and frozen embryo transfer timing were determined in the natural ovulation regimen group by monitoring natural follicle development and measuring serum levels of luteinising hormone, oestradiol, and progesterone. In the programmed regimen group, endometrial preparation was achieved by sequential administration of oestrogen and progesterone. MAIN OUTCOMES AND MEASURES:Primary outcomes were a healthy live birth and pre-eclampsia or eclampsia after a frozen embryo transfer. Secondary outcomes were cycle cancellation, biochemical pregnancy, clinical pregnancy, ongoing pregnancy, pregnancy loss, ectopic pregnancy, live birth, birth weight, and maternal, fetal, and neonatal complications. RESULTS:In the intention-to-treat analyses, 910 (41.6%) of 2185 patients in the natural ovulation regimen group and 890 (40.6%) of 2191 in the programmed regimen group achieved a healthy live birth (relative ratio 1.03 (95% confidence interval (CI) 0.96 to 1.10); P=0.49). The risk of pre-eclampsia was lower in the natural ovulation regimen group among patients who achieved clinical pregnancy than in the programmed regimen group (2.9% (38 of 1302) v 4.6% (61 of 1326); 0.63 (0.43 to 0.94); P=0.02). The incidences of early pregnancy loss (12.1% (158 of 1302) v 15.2% (201 of 1326); 0.80 (0.66 to 0.97)), placental accreta spectrum (1.8% (24 of 1302) v 3.6% (48 of 1326); 0.51 (0.31 to 0.83)), caesarean section (69.5% (776 of 1117) v 75.6% (831 of 1100); 0.92 (0.87 to 0.97)), and postpartum haemorrhage (2.0% (22 of 1117) v 6.1% (67 of 1100); 0.32 (0.20 to 0.52)) were lower in the natural ovulation regimen group. No differences between groups were observed for birth weight or neonatal complications. The rate of cycle cancellation was higher in the natural ovulation regimen (16.2% (354 of 2185) v 11.5% (251 of 2191), P<0.001). The prespecified per protocol and subgroup analyses yielded results consistent with the intention-to-treat analyses. CONCLUSIONS:In ovulatory women, a natural ovulation regimen for endometrial preparation was as effective as programmed regimen in terms of achieving a healthy live birth after frozen embryo transfer, but with a lower risk of maternal complications during pregnancy. ObjectiveWe investigated whether the addition of a luteal phase support drug benefits pregnancy and perinatal outcomes in modified natural-cycle frozen-thawed embryo transfer (mNC-FET) for women up to the age of 35 years.MethodsWe analyzed the clinical data of 3658 mNC-FET cycles of women up to the age of 35 years from the Reproductive Center of the Third Affiliated Hospital of Zhengzhou University from January 2018 to December 2020 in a retrospective cohort study. The cycles were divided into three groups based on the luteal phase support protocol used. The patients in group A received a combination of progesterone soft capsules and dydrogesterone (882 cycles), those in group B received dydrogesterone only (627 cycles), and those in group C received a combination of progesterone vaginal sustained-release gel and dydrogesterone (2149 cycles). Pregnancy and perinatal outcomes were compared among the three groups.ResultsLogistic regression analysis indicated that the three luteal phase support regimens were not associated with the live birth rate [OR(95% CI)B vs A=1.080, p=0.960; OR(95% CI)B vs C=0.252, p=0.291]. There were no significant differences in the newborn weight, premature delivery rate, pregnancy complications rate, and incidence of birth defects among the three groups.ConclusionsIn the mNC-FET cycle, patients under the age of 35 who chose dydrogesterone alone as a luteal phase support drug exhibited no difference in the live birth rate and perinatal outcome from patients who combined dydrogesterone with progesterone soft capsules or with progesterone vaginal sustained-release gel. However, the outcome still requires confirmation by large-sample prospective studies.
ObjectiveThe objective was to analyze the impact of the uterine artery pulsatility index (PI) on pregnancy outcomes by measuring uterine artery blood flow on the day of endometrial transformation in patients undergoing frozen–thawed embryo transfer (FET).MethodsThis was a case-control study. In total, 2,036 patients who underwent FET at the Third Affiliated Hospital of Zhengzhou University from October 2019 to September 2020 were included. The patients were divided into a clinical pregnancy group and a nonclinical pregnancy group according to pregnancy outcome. A multivariate logistic regression model was used to analyze the factors affecting the clinical pregnancy rate. The receiver operating characteristic (ROC) curve was used to determine the optimal mean PI cutoff value of 1.75. After 1:1 propensity score matching (PSM), 562 patients were included. For statistical description and analysis, the patients were divided into two groups: a group with a mean PI > 1.75 and a group with a mean PI ≤ 1.75.ResultsThe clinical pregnancy group included 1,218 cycles, and the nonclinical pregnancy group included 818 cycles. There were significant differences in female age (P<0.01), infertility type (P=0.04), baseline follicle-stimulating hormone level (P=0.04), anti-Müllerian hormone (AMH) level (P<0.01), antral follicle count (P<0.01), number of transferred embryos (P=0.045) and type of transferred embryo (P<0.01). There was no significant difference in the mean bilateral PI (1.98 ± 0.34 vs. 1.95 ± 0.35, P=0.10). The multivariate analysis results showed that maternal age (AOR=0.95, 95% CI=0.93-0.98, P<0.01), AMH level (AOR=1.00, 95% CI=1.00-1.01, P=0.045), number of transferred embryos (AOR=1.98, 95% CI=1.47-2.70, P<0.01), and type of transferred embryo (AOR=3.10, 95% CI=2.27-4.23, P<0.01) were independent factors influencing the clinical pregnancy rate. The mean PI (AOR=0.85, 95% CI=0.70-1.05; P=0.13) was not an independent factor influencing the clinical pregnancy rate. Participants were divided into two groups according to the mean PI cutoff value of 1.75, and there was no significant difference between the two groups (P > 0.05).ConclusionIn this study, we found that the uterine artery PI on the day of endometrial transformation in patients undergoing FET is not a good predictor of pregnancy outcomes.
This study aimed to determine the influence of serum vitamin D levels on assisted reproductive and perinatal outcomes in young non-polycystic ovary syndrome (PCOS) patients. A total of 3397 non-PCOS women under 35 years who underwent their first IVF/ICSI cycle at the Reproductive Medicine Center of the Third Affiliated Hospital of Zhengzhou University, from 2018 to 2019, were included. The women were categorized into two groups based on their serum 25(OH)D concentrations: deficient group [25(OH)D < 50 nmol/L] and non-deficient group [25(OH)D ≥ 50 nmol/L]. Ovulation induction results, clinical pregnancy rate, cumulative live birth rate (CLBR), and perinatal outcomes of both groups were compared. A total of 1113 non-PCOS women had successful pregnancies in their first completed IVF cycle. Comparison of laboratory results between the two groups revealed a significantly higher number of oocytes retrieved in the vitamin D-non-deficient group (15.2 ± 6.8 vs. 14.5 ± 6.7, p = 0.015). After controlling for confounding factors, there was no significant difference in the CLBR between the vitamin D-deficient group and the non-deficient group (71.0
Research question: Can women with partial 17 a- hydroxylase de fi ciency (17-OHD) conceive naturally with adequate hormonal control and endometrial preparation? Design: This report presents two cases of women with partial 17-OHD who achieved successful pregnancies. The fi rst case involved a 27-year-old Chinese woman with recurrent cysts and infertility, and the second case involved a 32-year-old Chinese woman with a complex disorder requiring IVF. Both cases were treated with oral prednisone to control hormone concentrations and underwent endometrial preparation. Results: In the fi rst case, the patient resumed spontaneous ovulation, conceived naturally, and gave birth to a healthy baby. In the second case, after cryopreserving embryos due to a thin endometrium, the patient underwent frozen embryo transfer and achieved a singleton pregnancy. Conclusion: This study suggests that women with partial 17-OHD can conceive naturally with appropriate hormonal management and endometrial preparation. These fi ndings provide valuable insights into the reproductive potential of women with this disorder, and highlight the importance of further research in this area.
Objective:To investigate the effect of chronic endometritis (CE) on the clinical outcomes of patients with failure of first embryo transfer.Methods:A total of 5 605 cycles of frozen-thawed single blastocyst transfer in the reproductive center of the Third Affiliated Hospital of Zhengzhou University from January 2017 to June 2021 were retrospectively collected. After the failure of first embryo transfer, all patients underwent hysteroscopy, and when necessary, endometrial pathology and immunohistochemistry were combined to diagnose CE. Patients were divided into two groups: non-CE group (5 033 cycles) and CE treatment group (572 cycles). The main outcome was live birth rate and the secondary outcomes included clinical pregnancy rate and early abortion rate. The quantitative data were represented by Median ( Q1, Q3). The rank sum test was used for comparison between groups. The factors related to live birth rate were analyzed by binary logistic regression model. Results:The incidence of CE was 10.21% (572 cycles) in patients with the failure of first embryo transfer. The maternal age in the non-CE group was 31.0 (29.0, 34.0) years old, and that in the CE treatment group was 31.0 (29.0, 34.0) years old ( P<0.001). There was a statistically significant difference in endometrial preparation between the two groups ( P=0.010). The endometrial thickness in the CE group was 9.0 (8.2, 10.3) mm on progesterone transformation day, which was higher than that of [9.5 (8.6, 11.0) mm] in the non-CE group ( P<0.001). There was no significant difference in clinical pregnancy rate (60.3% (3 035 cycles) vs 63.1% (361 cycles), P=0.193), early abortion rate (17.1% (520 cycles) vs 20.5% (74 cycles), P=0.112) and live birth rate (49.2% (2 477 cycles) vs 49.3% (282 cycles), P=0.969) between the non-CE group and the CE treatment group. The maternal age, endometrial thickness on progesterone transformation day and blastocyst grade were related factors of the live birth rate, and the OR(95% CI) were 0.94 (0.93-0.96), 1.10 (1.06-1.14) and 2.07 (1.84-2.32)), respectively (all P<0.001). Compared with the non-CE group, the CE treatment group did not affect the live birth rate after transplantation, the aOR (95% CI) was 0.99 (0.82-1.18), P=0.882. Conclusions:For patients who underwent the failure of first embryo transfer, hysteroscopy is recommended before single frozen blastocyst transfer, and if necessary, combined with immunohistochemical screening for CE. After standardized treatment, CE patients could obtain similar clinical pregnancy rate, early miscarriage rate and live birth rate as non-CE patients.
ObjectiveTo explore whether the duration of estrogen treatment before progesterone application affects neonatal and perinatal outcomes in artificial frozen embryo transfer (FET) cycles.MethodsThis was a retrospective cohort study. Patients who underwent FET via artificial cycles and delivered a singleton live birth between January 2015 and August 2019 were included in the analysis. According to the duration of estrogen treatment before progesterone application, we divided the cycles into four groups: ①≤12 days, ②13-15 days, ③16-19 days, and ④≥20 days. The ‘≤12 days group’ was considered the reference group. The main outcome measures were preterm birth (PTB), small-for-gestational age (SGA), low birth weight (LBW), macrosomia, large-for-gestational age (LGA), gestational diabetes mellitus (GDM), gestational hypertension, premature rupture and placenta previa.ResultsOverall, 2010 FET cycles with singleton live births were included for analysis. Cycles were allocated to four groups according to the duration of estrogen treatment before progesterone application: ①≤12 days (n=372), ②13-15 days (n=745), ③16-19 days (n=654), ④≥20 days (n=239). The neonatal outcomes, including PTB, SGA, LBW, macrosomia and LGA, were comparable among the groups (P=0.328, P=0.390, P=0.551, P=0.565, P=0.358). The rates of gestational hypertension, premature rupture and placenta previa (P=0.676, P=0.662, P=0.211) were similar among the groups. The rates of GDM among the four groups were 4.0% (15/372), 6.7% (50/745), 6.4% (42/654), and 11.3% (27/239), with statistical significance (P=0.006). After multiple logistic regression analysis, the duration of estrogen treatment did not affect the rate of GDM or other outcomes.ConclusionThe estrogen treatment duration before progesterone application does not affect neonatal and perinatal outcomes in single frozen blastocyst transfer cycles.
目的:为改善超重和肥胖患者在体外受精中的卵子和胚胎质量、提高临床妊娠率等提供数据基础.方法:回顾性分析2017年1月至2019年12月于郑州大学第三附属医院行IVF/ICSI-ET的超重和肥胖患者139例,每位患者分别行超促排卵2周期,前后2周期均采用长方案行IVF/ICSI,第2周期对体质量进行合理的干预,体质量减少>5%,按减重前后的BMI分为2组.减重前BMI>24 kg/m2.减重后较减重前体质量减少5%以上.使用配对设计的回顾性分析,比较患者体质量干预前后两个周期的体外受精临床结局指标(如获卵数、2PN数及D3可利用胚胎数、临床妊娠率等).结果:减重前后两组BMI水平的差异具有统计学意义(P<0.001);减重后组行IVF/ICSI的获卵数、2PN数、HCG日E2、D3可利用胚胎数、D3优质胚胎数、囊胚形成率及移植妊娠率均显著优于减重前组(P<0.05);减重后组的周期取消率显著低于减重前组(P=0.028);两组患者Gn总天数和Gn总量的差异无统计学意义(P>0.05).结论:超重和肥胖患者在进入体外受精周期前应给予合理的体质量干预;合理的体质量干预可以有效地增加卵子数量、提高胚胎质量及临床妊娠率,最终改善临床结局.
Abstract Background:In the freeze-thaw embryo transfer, the use of best luteal phase support protocols is continuously being discussed, especially in the natural cycles. Previous comparisons of luteal phase support protocols in natural cycles focused on pregnancy outcome, with less attention to perinatal outcome. Thus, convincing conclusion about which protocol is more appropriate could not be drawn. At the same time, with increasing luteal support drugs, the benefit of using these drugs also needs evidence. Methods: The clinical data of 3658 mNC-FET cycles in the Reproductive Center of the Third Affiliated Hospital of Zhengzhou University from January 2018 to December 2020 with the participants’ age of less than or equal to 35 years old,were retrospectively analyzed. The cycles were divided into three groups based on luteal phase support protocols used. The patients received a combination of progesterone soft capsule and dydrogesterone in the group A (882 cycles), the patients received dydrogesterone in the group B (627cycles), and the patients received a combination of progesterone vaginal sustained-release gel and dydrogesterone in the group C (2149 cycles). Pregnancy and perinatal outcomes in the group A, group B, and group C were compared. Results: HCG positive rate, clinical pregnancy rate, and implantation rate were higher in the group B than in the groups A and C (67.30% vs. 57.37% and 61.05%; 64.43% vs. 55.67 % and 57.88 %; 54.83 % vs 42.70 % and 45.92 %). The 12-week pregnancy rate and live birth rate in the group B were higher than those in the group A (56.30% vs. 49.55%;53.26% vs. 46.83%). There were no significant differences in newborn weight, premature delivery rate, pregnancy complications rate, and incidence of birth defects among the three groups. Logistic regression analyses showed that three different luteal phase support regimens were not associated with live birth rate (0R 95% CI B VS. A=1.23,P=0.056;0R 95%CI B VS. A=1.07,P=0.374). Conclusions: In the mNC-FET cycle, patients younger than or equal to 35 years of age, who chose dydrogesterone alone as luteal support drug, had no difference in live birth rate and perinatal outcome between progesterone soft capsules or progesterone vaginal sustained-release gel combined with dydrogesterone, but the outcome still needs to be confirmed by large sample prospective studies.
To investigate the reproductive outcomes by comparing two kinds of antibiotic schemes for chronic endometritis (CE) in infertile women's fresh embryo transfer (FET) cycles and identify subgroups of patients with CE who need long‐term antibiotics treatment.
目的 研究英文文献汇报在生殖医学研究生中应用效果.方法 选取2019年12月至2020年6月生殖医学科一、二、三年级研究生参与英文文献汇报,于完成7个月的文献汇报后,向全程参与的25名研究生发放问卷,调查英文文献汇报对其产生的影响.结果 所有评价均认为,英文文献阅读和汇报对自己有一定的帮助.没有研究生进行负面评价,99.4%的学生给予了比较有帮助的积极评价.结论 英文文献汇报对于提高生殖医学研究生英语水平和科研能力有一定作用.
Objective:To investigate the embryo transfer strategies of early follicular phase prolonged protocol, to reduce the risk of multiple birth rate while achieving a higher live birth rate.Methods:It was a retrospective cohort study. Patients who underwent gonadotropin-releasing hormone (GnRH) agonist protocols in the Reproductive Center of the Third Affiliated Hospital of Zhengzhou University from January 2017 to February 2019 were included. According to the type of embryos transferred, we divided all cycles into four groups, fresh cycle cleavage stage embryo transfer group (group A), fresh cycle blastocyst transfer group (group B), freeze-thaw cleavage stage embryo transfer group (group C), freeze-thaw blastocyst transfer group (group D). The main outcome measures were live birth rate and multiple birth rate. Binary logistic regression was used to correct confounding factors.Results:The risk of moderate to severe ovarian hyperstimulation syndrome (OHSS) in group A [4.8% (110/2283)] was higher than that in group C [1.0% (6/614), P<0.001] and group D [2.1% (16/762), P<0.001]. The risk of moderate to severe OHSS in group B [3.8% (42/1116)] was higher than that in group C ( P<0.001). The multiple birth rate of the two embryos transfer group [group A 23.4% (535/2283), group C 20.8% (128/614) ] was significantly higher than that of the one blastocyst transfer group [group B 1.4% (16/1116), group D 0.9% (7/762)], and the difference was statistically significant ( P<0.001). The clinical pregnancy rate [68.7% (767/1116)] and the live birth rate [59.6% (665/1116)] of group B were higher than those of group A [63.5% (1449/2283), P=0.003; 54.8% (1250/2283), P=0.008], group C [60.1% (369/614), P<0.001; 51.6% (317/614), P=0.001] and group D [62.7% (478/762), P=0.007; 52.8% (402/762), P=0.003], but there was no statistical difference among group A, group C and group D. Taking group B as a reference, the live birth rate of group A (a OR=0.86, 95% CI=0.74-0.99, P=0.044), group C (a OR=76, 95% CI=0.62-0.93, P=0.008) and group D (a OR=0.79, 95% CI=0.65-0.95, P=0.013) was lower than that of group B. Conclusion:For the early follicular phase prolonged protocol, based on the control of OHSS, fresh cycle single blastocyst transplantation is preferred. While obtaining a higher live birth rate, the multiple birth rate is significantly reduced.
目的 分析来曲唑联合羟乙基淀粉注射液在卵巢过度刺激综合征(OHSS)高危患者中的应用效果.方法 于2016年1月至2018年12月,根据用药方案将于该院行取卵术和全胚冷冻的OHSS高危患者206例分为观察组(103例)与对照组(103例),对照组患者术后予以羟乙基淀粉注射液治疗,观察组患者术后予以来曲唑联合羟乙基淀粉注射液治疗,比较患者治疗前后血清雌二醇(E2)水平,以及轻度、中度、重度OHSS发生率.结果 治疗前(人绒毛膜促性腺激素注射日),E2水平组间比较差异无统计学意义(P>0.05).治疗后,观察组E2水平明显下降并低于对照组(P<0.05).与对照组相比,观察组轻度OHSS发生率较高,中度OHSS发生率较低(P<0.05),但重度OHSS发生率组间比较差异无统计学意义(P>0.05).结论 OHSS高危患者取卵术后行来曲唑联合羟乙基淀粉注射液治疗,可降低雌激素水平,促使中度OHSS轻度化,降低中度OHSS发生率,具有较好的OHSS防治效果.
Objectives: The objective of this study is to evaluate the efficacy of autocross-linked hyaluronic acid (HA) compared with intrauterine device (IUD) for preventing intrauterine adhesions (IUAs) in infertile patients after hysteroscopic adhesiolysis. Materials and Methods: A randomized clinical trial (ChiCTR-IOR-16007746). Upon completion of adhesiolysis, 3 ml of HA gel was placed into the uterine cavity in Group A; 3 ml of HA gel and an IUD were placed in Group B; and only an IUD was placed in Group C. A second hysteroscopic examination was performed in all patients at approximately 1 month postoperatively for the evaluation of IUA. The primary outcome measure was the effective rate of IUA prevention based on the American Fertility Society (AFS) scoring system. Results: Eighty-nine women were randomly distributed into two groups for intention to treat with 30 patients in Group A, 24 patients in Group B, and 35 patients in Group C. Patients were scored and stratified into three degrees and were enrolled using the simple random sampling method. The three groups were well balanced. There were no significant differences in age, endometrial thickness, the previous number of pregnancy, and the distribution of adhesion categories across mild, moderate, and severe between the three groups. The effective rate of IUA prevention, the AFS score after therapy, and the percentage improvements of Chinese score and AFS score before and after surgery were statistically significant difference between Groups A and C. The clinical pregnancy rate in Group A was higher than those in Groups B and C, but the difference was not statistically significant. Conclusion: HA gel has an advantage over an IUD in reducing IUA recurrence and decreasing adhesions.
Objective: The objective of the study was to evaluate the effects of recurrent hydrosalpinx after proximal tubal ligation and distal salpingostomy on the outcomes of in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) treatment.Materials and Methods: Seven hundred and twenty-six patients with hydrosalpinx undergoing laparoscopic surgery before IVF were enrolled in the study. Five hundred and sixty-two patients treated with proximal tubal ligation and distal salpingostomy were included in Group A. One hundred and sixty-four cases managed with salpingectomy were grouped into Group B. Group A were further divided into two subgroups. One hundred and forty-six patients in Group A1 had a recurrence of hydrosalpinx. Four hundred and sixteen patients in Group A2 had no repetition of hydrosalpinx. We compared the pregnancy outcomes of their subsequent fresh embryo transfer cycles among the three groups.Results: There were no significant differences among the three groups in terms of age, body mass index (23.56 ± 3.27 vs. 23.13 ± 3.42 vs. 23.63 ± 3.73, P = 0.195), basal hormone level (7.03 ± 1.75 vs. 7.08 ± 2.26 vs. 7.44 ± 2.93, P = 0.195), antral follicle count (12.25 ± 5.92 vs. 12.63 ± 5.71 vs. 11.70 ± 4.98, P = 0.188), duration of gonadotropin (Gn) (11.19 ± 2.1 vs. 10.93 ± 1.84 vs. 10.79 ± 2.03, P = 0.182), consumption of Gn (2136.73 ± 855.65 vs. 1997.15 ± 724.72 vs. 2069.05±765.12 , P = 0.14), endometrial thickness (1.1 ± 0.27 vs. 1.1 ± 0.24 vs. 1.1 ± 0.17, P = 0.352), base follicle-stimulating hormone (6.21 ± 3.43 vs. 6.52 ± 3.20 vs. 5.89 ± 3.10, P = 0.1), number of embryos transferred (1.87 ± 0.36 vs. 1.83 ± 0.42 vs. 1.88 ± 0.37, P = 0.224), and number of high-grade embryos (3.77 ± 2.42 vs. 4.01 ± 2.72 vs. 4.17 ± 2.74, P = 0.41). No differences were detected in clinical pregnancy rate (50% vs. 54.8% vs. 50%, P = 0.439), the live birth rate (86.3% vs. 82.0% vs. 87.8%, P = 0.398), fertilization rate (64.1% vs. 64.4% vs. 64.7%, P = 0.928), and biochemical pregnancy rate (4% vs. 4.5% vs. 7%, P = 0.332) among the three groups.Conclusion: The recurrence of hydrosalpinx after tubal ligation does not affect the outcomes of IVF/ICSI. It is not necessary to worry about the effect of recurrent hydrosalpinx on pregnancy outcomes of IVF/ICSI that may due to the spread of inflammation through lymphatic circulation or blood circulation.
Objective: To evaluate the effectiveness of highly purified human menopausal gonadotropins (hp-HMG) plus recombinant follicle stimulating hormone (r-FSH) vs r-FSH vs r-FSH plus recombinant luteinizing hormone (r-LH) in vitro fertilization (IVF) / intracytoplasmic sperm injection (ICSI) patients。 Design : Retrospective cohort study Methods: This was a retrospective study. Among a total number of 3568 patients who had undergone IVF/ICSI applications, 409 eligible patients were included.Total units of follitropin alpha preparations used in ovulation induction, total number of meiois-2 phase oocytes, total number of used oocytes in ICSI cycle, fertilization rate and clinical pregnancy rates of both groups were analyzed. In this retrospective cohort study, women undergoing IVF/ICSI Gonadotropin releasing hormone (GnRH) antagonist cycles downregulation. 409 patients were included in the study. Among them One group followed the current standard protocol of no LH or hp-HMG supplementation given(n=64). The other had LH supplementation in the form of r -LH (Luveris; Merck Serono, Switzerland) (n=221), Another group had hp-HMG supplementation in the form of hp-HMG (Menopur , Ferring,Germany)(n=121).In the Subgroup analysis were decided by AFC ,7 < AFC〈20 or AFC>20 of the three group. Result: Mean duration of stimulation and was longer in the group of patients treated with hp-HMG plus rFSH compared to the group of patients treated with r-FSH and the group of r-LH plus r-FSH (13.24 days and 12.72days and12.21days, respectively; P<0.05). The amount of GN does for patients treated withhp-HMG plus rFSH compared to the group of patients treated with r-FSH plus r-LH and the group of r-FSH alone respectively (P<0.05). Clinical pregnancy rates were 76.6% and 60.9% and 62.9% (P<0.05)in the groups of patients treated with hp-hMG plus rFSH, r-FSH plus r-LH,r-FSHalonerespectively. What’s morea greater live birth rate was noted in the hp-hMG plus r-FSH group, there was statistically significant difference between the three groups (P>0.05).in the subgroup analysis when AFC>20 hp-HMG plus rFSH group have more lower ovarian hyperstimulation syndrome (OHSS) than r-FSH plus r-LH and rFSH alone,respectively. Clonclusion: The higher oocyte yield with r-FSH does not result in higher quality embryos.hp-HMG or r-LH supplementation is an option for improving IVF outcome in patients ovulation induction with r-FSH during GnRH agonist down-regulation. Particularly, hp-HMG is recommended as it may have a beneficial action on implantation in selected group especially AFC more than 20patients.
Objective: To investigate when is the appropriate time interval between hysteroscopic polypectomy and the start of FET cyclesDesign: Retrospective cohort study. Setting: Academic center. Patient(s): All patients diagnosed with endometrial polyps undergoing hysteroscopic polypectomy before FET.Intervention(s): Hysteroscopic polypectomy.MainOutcomeMeasure(s): Patients were divided into four groups based on the time interval between hysteroscopic polypectomy and the start of FET Demographics, baseline FET characteristics, pregnancy outcomes after FET were compared among the groups. A total of 1703 patients met inclusion criteria: 547 patients in group 1 who underwent FET after hysteroscopic polypectomy 1-3menses cycles, 389 patients in group 2 who underwent FET after hysteroscopic polypectomy 4-6 menses cycles, 421 patients in group 3 who underwent FET after hysteroscopic polypectomy 7-12menses cycles, and 373 patients in group 4 who underwent FET after hysteroscopic polypectomy more than 12 menses cycles, whats more the group 1 were divided into 3 groups depend on the time interval between hysteroscopic polypectomy and the start of FET. The FET outcomes were compared.Result(s): There were no differences in the mean number of embryos transferred. The overall pregnancy outcomes were similar for groups 1, 2, 3,and 4: implantation rate (38%, 38.2%, 39.5%and 36.8, respectively), clinical pregnancy rate (51.9%, 48%, 53.2and 50%), spontaneous miscarriage rate (8.4%,8.4%, 12.2and 8.9%), and live birth rate (42.4%. 40.4%, 42.8% and 40.9%). Conclusion(s): IVF outcomes seem to be unrelated to the time interval between the hysteroscopic polyp resection and the initiation of the FET; The abortion rates may be lower if the treatment is started in the first few months post operatively.
Objective To evaluate and compare the clinical impact of different operation methods of hydrosalpinges when performed before in vitro fertilization and embryo transfer (IVF-ET) in patients with hydrosalpinges. Methods The data of 939 of infertile patients through tubal factors undergoing IVF-ET from 2012 to 2014 were analyzed retrospectively. All patients were divided into two groups according to the management of hydrosalpinx per person, 533 patients with hydrosalpinx in group A who were treated with operation while 406 patients with hydrosalpinx in group B were untreated. Group A were divided into three subgroups according to surgical methods, 311 patients with hydrosalpinx in group A1 who were treated with proximal tubal ligation, 113 patients with hydrosalpinx in group A2 who were treated with salpingostomy, 109 patients with hydrosalpinx in group A3 who were treated with salpingectomy. The clinical outcome of IVF-ET was evaluated. Results The clinical pregnancy rate (50.7%) and the live birth rate (91.1%) in group A were significantly higher than those in group B (40.4%, 77.4%, respectively) (P<0.05). The ectopic-pregnancy rate (3.7%) in group A was significantly lower than that in group B (12.8%) (P<0.05). There were no significant differences among group A1, group A2 and group A3. Conclusion Surgical of the fallopian tube, when performed in women with unilateral or bilateral hydrosalpinges before their IVF treatment, represents potential benefits—significantly increasing the clinical pregnancy rate and significantly decreasing the ectopic pregnancy rate. There is no significant difference of the ovarian reserve and IVF-ET outcome after salpingostomy, proximal tubal ligation or salpingectomy of the fallopian tube.
microRNA (miR)-141-3p has context-dependent effects on tumor progression. In this study, we attempted to explore the expression and function of miR-141-3p in cervical cancer. We found that miR-141-3p expression was significantly increased in cervical cancer specimens relative to normal cervical tissues. Moreover, miR-141-3p levels were associated with tumor size and lymph node metastasis status. Ectopic expression of miR-141-3p significantly increased cervical cancer cell proliferation, colony formation, invasion, and epithelial to mesenchymal transition, whereas depletion of miR-141-3p suppressed cervical cancer cell proliferation and invasion. FOXA2 was identified to be a target of miR-141-3p. Overexpression of miR-141-3p led to a marked inhibition of endogenous FOXA2 in cervical cancer cells. FOXA2 silencing phenocopied the effects of miR-141-3p overexpression on cervical cancer cell proliferation and invasion. Enforced expression of FOXA2 blocked the effects of miR-141-3p on cervical cancer cell proliferation and invasion. miR-141-3p overexpression significantly accelerated the growth of xenograft tumors, which was accompanied by a striking reduction in FOXA2 expression. miR-141-3p acts as an oncogene in cervical cancer largely through repression of FOXA2. Targeting miR-141-3p may represent a potential therapeutic strategy for cervical cancer.
