Introduction. The pandemic of coronavirus infection, declared by WHO on March 11, 2020, in the first months practically did not affect the inhabitants of the Russian Federation, which gave the healthcare system an advantage in the form of a temporary lag to prepare for the mass admission of patients to infectious diseases hospitals.Aim. To disclose the issues of organization and operation of a children’s covid and provisional hospital on the basis of a separate medical institution as a single whole entity, which functioned for 495 days from 04/08/2020 to 08/15/2021 on the basis of the City Children’s Hospital №1 of Kazan.Materials and methods. Descriptive statistics of 1554 cases of coronavirus infection COVID-19 in childhood are given, topical aspects of epidemiology, the clinical course of the disease and the significance of comorbid pathology are discussed.Results. In the process of work, we noted three rises in the incidence: in June 2020, December 2020 and July 2021. In 61.2% (n = 951) of cases, intrafamily infection was noted, in 11.9% (n = 185) – infection in children’s groups, in 26.9% (n = 418) of cases the source of infection was not identified. The course of the disease was moderate in 74.3% of patients (1154 children), severe in 4.1% (64 children) and mild in 21.6% (336 children), whose hospitalization was justified by social reasons or serious comorbid pathology. The incidence of pneumonia was 21.4% (333 children). COVID-19 was diagnosed in 47 newborns, which proceeded in a severe (2.1% of cases, n = 1), moderate (66% of cases, n = 31) and mild (31.9% of cases, n = 15) form.Conclusions. City Children’s Hospital № 1 of Kazan, transformed into a temporary children’s covid and provisional hospital, provided medical care to 7831 children, including 1554 patients with COVID-19 in the period from 04/08/2020 to 08/15/2021. This organizational decision made it possible to provide those in need with the necessary medical care, organize the routing of patients, and create a reserve of beds.
Chronic obstructive pulmonary disease is one of the most urgent problems of the modern medical community. Despite the introduction of constantly updated schemes of early diagnosis and treatment into practical medicine, there is still an upward trend in the number of cases, under-examined persons, and prognostically unfavorable outcomes. The review highlights the findings of epidemiological studies confirming the widespread prevalence of the disease, accompanied by rapid disability progression, high mortality, and significant economic damage. The review lists the main exogenous and endogenous risk factors for the development of chronic obstructive pulmonary disease, including occupational etiology. Attention is drawn to the possibility of disease prevention in a professional environment with the proper motivation of the patient and the application of economic efforts. The article discusses the main causes of underdiagnosis and late diagnosis of the disease. Lifestyle modification makes an undeniable contribution to the prevention of chronic obstructive pulmonary disease and improving prognosis in the developed disease. Certain psychological characteristics that reduce adherence to treatment of such patients should be considered in organizing the management of this category of persons and creating special schools. It is important to create a classification of endotypes of chronic obstructive pulmonary disease, as well as sufficient public awareness about this disease with the aim of the earliest possible diagnosis.

The article presents the analysis of the health status of children born prematurely during the first year of their life. There is the comparison of morbidity rates for five years of the operation of the catamnesis center of Kazan. Profoundly premature children demonstrated the highest level of morbidity with more frequent pathologies of the central nervous system, respiratory organs, eyes and anemia of premature children. In 2017 there was a decrease in the frequency and severity of respiratory diseases (bronchopulmonary dysplasia with chronic respiratory insufficiency 1.6 times), the nervous system (severe ischemic and hypoxic-hemorrhagic lesions of CNS 2.7 times), eyes and its adnexa (a complicated retinopathy of premature children 1.7 times) in extremely premature infants.

The hypertrophic osteoarthropathy (HOA) may occur as isolated finding or is often part of the syndrome which is characterized by periostosis of the long bones and occasional painful joint enlargement. The majority of cases of HOA are due to malignant thoracic tumors. The authors present a case of hypertrophic osteoarthropathy as a paraneoplastic syndrome in a patient with peripheral lung cancer.

Aim. To assess the effect and safety of palivizumab to prevent the bronchopulmonary dysplasia complications in preterm children according to follow-up. Methods. The study included 48 very preterm infants (birth weight ranged from 732-1640 g) with bronchopulmonary dysplasia, who were divided into two groups. The main group included children who received passive immunization by palivizumab (31 children), the comparison group included un-immunized children (17 patients). The diagnosis of bronchopulmonary dysplasia in children at 28 days of postnatal life has been set according to clinical and radiological data. After being discharged from the department of pathology of newborn, children were followed-up for 12 months in the center of preterm children follow-up. Average age of patients with bronchopulmonary dysplasia was 4.2±1.3 months at passive immunization initiation. The drug was administered by intramuscular injection at a dose of 15 mg/kg once monthly. Results. Lower respiratory tract infections were observed in 6 (19.3%) patients from the first group, including 4 cases of obstructive bronchitis and 2 cases of pneumonia. These patients received incomplete course of immunization. In children who did not receive specific prophylaxis, respiratory infections were registered in 7 (41.2%) patients, with 2 cases accompanied by severe respiratory failure requiring mechanical ventilation. No adverse effects were identified while palivizumab treatment. Conclusion. Palivizumab is currently the only effective drug to prevent the severe respiratory syncytial virus infection; good safety of the drug makes it possible to reconsider the contraindication of its use in high-risk children with severe bronchopulmonary dysplasia.
In 2010, the Russian Federation (RF) registered palivizumab--innovative drug, based on monoclonal antibodies for passive immunization of seasonal respiratory syncytial virus (RSV) infection in children of disease severe progress risk group, which include primarily premature infants, children with bronchopulmonary dysplasia and hemodynamically significant congenital heart disease. Currently, palivizumab is included in the list of recommended medicines and medical care standards of different countries, including Russia. In the review the results of Russian research on the progress of RSV infection, its epidemiology and immunization experience gained over the 2010-2014 period are summarized in relation to the foreign data. During the four epidemic seasons palivizumab immunization covered more than 3,200 children of severe RSV infection risk group with a progressive annual increase in the number of patients who received the drug. Geography of palivizumab immunization is also greatly expanded in our country during this time. If during the first two seasons measures of immunization were taken mainly in Moscow and St. Petersburg, at the present time, thirty one territorial entities of the Russian Federation have the experience in the drug application. Analysis of the results of RSV infection immunization (made in several regions) confirms the high clinical efficacy and palivizumab safety already demonstrated in international studies. In addition, the analysis presents the potential to improve the efficiency of the integrated RSV infection immunization programs, realizing in the establishment of high-risk child group register, adequate counseling for parents, as well as the development of the routing of patients and coordination of interaction between different health institutions during the immunization.
