Adherence to proper indications for red blood cells (RBC) transfusion is essential because of its potential adverse effects and costs of therapy. Aim of these recommendations is to summarize typed of RBC concentrates and indications for RBC transfusions among different categories of the patients. Methods. The methodological approaches are based on the recommendations of the Russian expert council (leading specialists of the Russian Federation) and literature search for randomized clinical trials evaluating RBC storage duration, hemoglobin thresholds and clinical indications for RBC transfusion without language restrictions. Results. The draft clinical guidelines were reviewed on February 1, 2018 at First Russian Transfusiology Congress of the (Vladivostok). The main types of RBC concentrates, storage duration, transport conditions and indications for RBC transfusions are presented. The indications for RBC transfusions are analyzed for various clinical conditions (in obstetrics, neonatology, hematology, cardiology, neurosurgery, nephrology, in patients with sepsis and septic shock, patients with acute blood loss, in patients after hematopoietic stem cell and organ transplantation). Conclusion. The recommendations are intended for doctors of various specialties, health administrators, medical students.
Introduction . Acute leukemias are very rarely diagnosed during pregnancy, which makes large prospective or comparative studies of treatment of leukemias during pregnancy difficult. In 2009, the Russian Acute Lymphoblastic Leukemia Study Group decided to include women diagnosed with acute lymphoblastic leukemia (ALL) during various stages of pregnancy into the ALL-2009 prospective clinical study to determine the predictive role of pregnancy at diagnosis, and to estimate efficacy and tolerance of the ALL-2009 protocol in pregnant women. Materials and methods . During the period of 2009– 2017, 15 pregnant women with Ph-negative ALL aged 18–41 (median 28) years were enrolled in the multicenter clinical trial ALL-2009 (NCT number NCT01193933). Eleven women were treated at the National Research Center for Hematology in Moscow; the other four were treated in Russian regional hospitals. In cases when ALL was diagnosed during the first trimester of pregnancy (n = 3), the pregnancy was terminated. If ALL was diagnosed at week 34–40 (n = 3), the baby was delivered before the beginning of therapy. If ALL was diagnosed during the second or early third trimester, the treatment was performed during pregnancy. We compared toxicity and tolerance of chemotherapy according to the ALL-2009 protocol in pregnant patients and in young (30 years and younger) ALL patients. We also compared the results of treatment of pregnant patients with the control group (127 women of fertile age: 16–50 years old, with a median of 28 years old) enrolled in the study. For discussion of the results, we did a meta-analysis of available publications (retrieved via PubMed using the keywords “acute lymphoblastic leukemia” and “pregnancy”). Results . There was a significantly higher frequency of T-cell ALL than of B-cell ALL (53.3% vs 46,7% compared to 26% vs 69,3% in the control group, p = 0.025). Other than that, there were no significant differences in clinical or laboratory data between the groups. No significant differences were found in the duration of neutropenia, or in the duration and frequency of breaks in chemotherapy compared with other patients below 30 enrolled in the study. However, during the first remission induction phase, pregnant patients required blood transfusions, specifically platelets, at a higher rate (77.8% vs 46.6% of cases). Results of the treatment were similar regardless of pregnancy at the time of diagnosis (86.7% of complete remissions, vs 85.8% in the control group). Differences in the frequency of refractory leukemia (13.3% vs 4.7%) were not statistically significant. There were no cases of early mortality. Long-term results were also similar, both in terms of overall survival (58.6% vs 43.3% in the control group) and in terms of disease-free survival (46% vs 51%). Relapse probability was the same (49% vs 40.3%). Overall, the pregnant patients gave birth to 12 children (6 boys and 6 girls) at weeks 34–38 (median 35 weeks) of pregnancy. At the time of writing, all children are healthy and are developing in accordance with their age, which ranges from 2 years 1 month to 8 years 10 months (median 5 years and 2 months). Conclusion . The results of the study suggest that pregnancy at the time of diagnosis with ALL did not affect either the short-term or the long-term results of therapy according to the ALL-2009 protocol. The acceptable level of toxicity of the low-dose cytostatic therapy for both the mother and the child makes it possible to use the ALL-2009 protocol in treatment of pregnant patients.
Pathogen inactivation (PI) treatment of platelet concentrates (PCs) improves the transfusion safety. PI technology allows to store of PCs up to 7 days. The usage of platelet additive solution (PAS) reduces the risk of non-hemolytic transfusion reactions and enhances platelet quality during storage. The influence of the combinations of PI and PAS and PI and 100% plasma on the biochemical parameters of PCs remained unclear. The aim of the study was to analyze the impact PI technology on the biochemical parameters and functional activity of platelets suspended in 100% plasma or 70% PAS and 30% plasma. The pathogens reduction does not influence on the glucose consumption and citrate content, accelerates the accumulation of lactate and lowers the pH of the medium storage donor platelet concentrates. In the samples with additive solution SSP+ changes of biochemical parameters were expressed to a lesser degree.
The study was organized to discover diagnostically valuable clinical material for detection of etiologic agent of pneumonia in oncological hematological patients, rate of association of nosocomial pneumonia with herpes viruses and evaluation of viral load in patients with depressed immunity. In oncological hematological patients, half of nosocomial pneumonia cases is associated with herpes virus. In every third patient DNA of Epstein-Barr virus and DNA of type I and II are detected. The most informative material in this case is broncho-alveolar lavage fluid and the most convenient diagnostic technique is polymerase chain reaction in real-time. The low viral load in broncho-alveolar lavage fluid is specfic for Epstein-Barr virus, cytomegalovirus and human herpes virus type VI. The concentration of DNA ofsimple herpes virus type I and type II is located in both high and low values. The paradox phenomena is established concerning more benevolent course of nosocomial pneumonia associated with simple herpes virus type I and II in patients with higher viral load in broncho-alveolar lavage fluid. The further research in this direction is needed.
PURPOSE OF THE STUDY:To analyze an efficiency of hypofibrinogenemia treatment. In the Scientific Center for Hematology (Moscow) significant hypofibrinogenemia occurs in 3% of patients with hemoblastosis. 1000 doses of cryoprecipitate are used for a hypofibrinogenemia treatment every year (21-23 doses for each patient). Containing of fibrinogen in a one cryoprecipitate dose is from 108 mug to 711 mug (M = 276 mug). Volume of one dose is from 8 to 90 ml (M = 24 ml). Hypofibrinogenemia occurred in all patients required a cryoprecipitate transfusion (M = 1 g L(-1), min 0.5 g L(-1), max 2 g L(-1)). We fixed an increasing of fibrinogen level in plasma by 0.7 +/- 0.2 g L(-1) after the cryoprecipitate transfusion. We analyzed a world experience of the use of fibrinogen containing blood components.CONCLUSIONS:Fresh frozen plasma transfusion cannot be a choice method of treatment for hypofibrinogenemia. Fibrinogen's concentrate has the same effectiveness as a cryoprecipitate both for congenital and acquired deficit of fibrinogen. The frequency of complications due to fibrinogen's concentrate is low. Currently clinical studies of recombinant fibrinogen are conducted. Ways of implementation of fibrinogen preparations in Russia are discussed.
The experience of treatment of 366 patients with haemophilia who were urgently hospitalized in hеmatological Scientific Center over the last 10 years is presented in the article. There were 114 (31.1%) patients with acute diseases of abdominal cavity organs, 150 (41%) patients with bleeding from upper gastrointestinal tract, 102 (27.9%) patients with acute hematomas of retroperitoneal space. Urgent operations were performed in 48 (22.2%) patients who were hospitalized with clinical symptoms of acute abdomen syndrome. It was developed the criteria of diagnosis and choice of treatment tactic on the basis of the received results. Application of presented algorithms led to improve the quality of urgent surgical care to patients with haemophilia.
Purpose of the study: To analyze complications of the pulmonary artery catheterization in patients with blood diseases. Materials and methods: 93 cases of pulmonary artery catheterization in patients with blood diseases were included in the retrospective study. Results: Indications for pulmonary artery catheterization were septic shock (78.5%) and acute respiratory failure (21.5%). In 31 cases (33.3%) pulmonary artery catheterization was performed in conditions of agranulocytosis and in 81 cases (87%) in conditions of thrombocytopenia platelets median 43*109per liter, minimal 7*109, maximal 150*109 per liter). Patients received transfusions of platelets in case of thrombocytopenia less than 30*109per liter. Early complications of pulmonary artery catheterization occurred in 5patients with thrombocytopenia (5.4%), the complications was connected with bleeding (hematoma, bleeding from place of puncture, lung bleeding) and mechanical (arterial puncture, pneumothorax, hemothorax). Number of attempts of the central vein catheterization was risk factor the complications. The frequency of catheter-associated sepsis was 5.89 cases each 1000 catheter-days and the frequency of infections of the soft tissues was 9.78 cases each 1000 catheter-days. Catheter-associated infection complications occurred in cases of catheter use over 5 days. Catheter-associated sepsis occurred in 2 of 3 patients with agranulocytosis. Other complications included intermittent arrhythmias during catheter moving in the heart chambers (58), catheter balloon rupture (4), and thrombosis of catheter lumen (3). Conclusions: Pulmonary artery catheterization can be used in patients with blood diseases and first of all in cases of septic shock and acute respiratory failure. Alternative less invasive methods of monitoring should be used in patients with agranulocytosis.
