Aim To study the effect of arterial hypertension (AH) in combination with frequent alcohol consumption on the formation of risk for cardiovascular death and all-cause death according to results of a 27-year prospective cohort study. Material and methods This 27‑year prospective cohort study of an unorganized population of the Tomsk city (1546 people aged 20–59 years, including 630 men and 916 women) investigated AH prevalence and alcohol consumption (1988–1991) and analyzed the predictive significance of the effect of AH in combination with frequent alcohol consumption on the formation of risk for all-cause and cardiovascular death. AH was diagnosed at blood pressure ≥140 / 90 mm Hg. Frequent alcohol users were defined as those who consumed alcohol more than once a week. Results The combination of AH and frequent alcohol consumption increased the risk of all-cause death 4.1 times compared to that for persons without these risk factors (p<0.001). This was true for all age groups of the total cohort (higher relative risk, RR, was observed for persons aged 20–39 years) and for men (except for the group aged 40–59 years). RR of cardiovascular death was 5.3 (p<0.001) for frequent alcohol users with AH. It was established that frequent alcohol consumption additionally increased RR of all-cause death for persons with AH (RR 1.89; p<0.05) primarily at the expense of persons aged 20-39 years. Prediction of 27‑year survival for frequent alcohol users with AH was 35.3 %. Conclusion A combination of AH with frequent alcohol consumption considerably increases the risk of all-cause and cardiovascular death. Frequent alcohol consumption significantly impairs the prediction of 27-year survival for persons with AH by additionally (1.9 times) increasing the risk of all-cause death. Binary AH combinations with frequent alcohol consumption exert a more pronounced adverse effect on young men and women.

In this study, genotype and allele distributions of the C242T (rs4673) polymorphism in the CYBA gene, which encodes the α subunit of NADPH oxidase (p22phox), were examined in the sample of long livers and in the population sample of the city of Tomsk. Statistically significantly higher frequency of T allele among female long livers (34.625%), compared to the females from Russian population (26.32%) was demonstrated (χ2 = 5.226; p = 0.022; OR = 1.48). Thus, the T allele is associated with a high life expectancy in females from the Russian population. No such association was observed for males from the same population.

In this study we genotyped polymorphism Thr12Ser (rs1937) in TFAM gene in two groups: patients with coronary artery disease and control group (people from Tomsk population). For genotyping was used restriction fragment length polymorphism. We have found significant higher genotype CC frequency in men with coronary artery disease — 4.52% compared with control men — 0.45% (χ 2 = 6.315; р = 0.043; OR = 10.45). Thus genotype CC is associated with coronary artery disease in men fromTomsk population.
In 17-year cohort p1rospective study, the dynamics of arterial hypertension (AH) and arterial pressure (AP) levels has been examined as functions of the boy mass. The cohort included 630 men and 916 women aged from 20 to 59. The overweight was diagnosed at the Body Mass Index index no lower than 25 kg/m 2 . In the primary research, the AH prevalence rate was 35,7% among overweighted people and 12,5% ( р < 0,001) among people with normal weight. In the 17-year observation, the increase of the body mass up to overweight was accompanied by an increase in the AH rate and AP levels, while the decrease of the body mass down to the normal one was accompanied by a decrease of these indices (except only for men aged from 40 to 59). Among overweighted people revealed at the I and II stages of the study, the systolic ((155,71 ± 28,59) mmHg) and diastolic ((93,68 ± 13,77) mm Hg) AH was higher than that in people, who became overweighted the observation: (137,08 ± 23,02) and (86,73 ± 12,19) mm Hg, respectively ( р < 0,001).
Results of the investigation of the genetic-demographic structure of Tomsk city are presented. It was shown that the Russians constitute about 90% population Tomsk. This population is characterized by intensive migration processes: only 30% of the investigated residents were born in Tomsk; maximal migration flow was observed from Western Siberia, Eastern Siberia, Kazakhstan and Ukraine. The population of Tomsk reproduces only 10% of its gene pool, while 40% of it is formed by the migrants from remote regions of the country. Dynamics of the portion of offsprings of heterolocal marriages in age groups was observed. Comparative analysis of genetical demographic structure of Tomsk's population and other previously studied urbanized populations of CIS was made.
