Management of COVID-19 patients with drug-induced liver lesions requires a comprehensive approach to treatment with correction of metabolic disorders and prevention of adverse events. We describe our experience with using Remaxol, a drug with hepatoprotective, antihypoxic, antioxidant, and cytoprotective effects. It is applied for hepatotropic support in COVID-19 patients receiving antiviral therapy. Intravenous infusions with Remaxol (400 mL) in comprehensive treatment of COVID-19 patients promoted normalization of liver function tests, which made it possible to continue antiviral therapy without dose correction and interruptions. Remaxol was well tolerated and caused no side effects; none of the patients required its cessation. Key words: hepatotoxicity, hepatoprotectors, Remaxol, COVID-19
Remaxol was evaluated as a means of hepatotropic support for patients with COVID-19 receiving antiviral therapy. Patients were observed in two groups of 30 people each and received intravenous infusions of Remaxol (400.0 ml) or “active placebo” (400.0 ml. 0.9% sodium chloride) for 10 days. In these groups of patients, an analysis was made of the dynamics of clinical parameters and laboratory tests that characterize the functionality of the liver. The results of the analysis made it possible to state that the use of the corrector of cellular and energy metabolism metabolism Remaxol as a hepatoprotector in complex therapy contributes to favorable clinical dynamics, stabilization of biochemical parameters of liver function and allows completing the course of etiotropic therapy in all patients.Good tolerability of the drug “Remaxol” and the absence of pronounced side effects were noted. The course of Remaxol made it possible to carry out therapy with antiviral agents without dose adjustment and duration of treatment.
The article justifies the use of interferon inducers in a clinical practice of pediatrician. The authors isolated a group of derivatives of acridone acetic acid, which effectiveness is associated with the features of pharmacokinetics and pharmacological activity. They demonstrated preventive and therapeutic efficacy of cycloferon in the complex treatment of children with influenza and other acute respiratory viral infections, herpes virus, arbovirus, rotavirus infections, viral hepatitis and tick-borne encephalitis due to its immunomodulatory potency. The article analyzed the use of cycloferon in frequently sick children, patients with allergic diseases, chronic tonsillitis, etc.; also it showed a decrease in the frequency of relapses of both infectious and allergic diseases, the possibility of combined use with other etiotropic and symptomatic drugs.
The aim of the research: the study of features of clinical course of acute intestinal infections (AII) on the background of coronary heart disease (CHD), diabetes (D), mental and behavioral disorders on the background of alcohol consumption (AC); development of pathogenetic therapy for these comorbid conditions by means of using succinate-containing drugs. Materials and Methods: 197 patients with AII (without associated diseases, and with CHD, D, AC) were examined in dynamics. The patients received conventional infusion therapy or 1,5 % solution of Reamberin at 10 mg per 1 kg of body weight. Pharmacoeconomic efficiency of using Reamberin was calculated. Results: Associated pathology determines more severe course of AII and possibility of complications, including decompensation of chronic disease. Introduction of Reamberin in the infusion therapy for patients with AII enabled proved decrease in the number of existing symptoms in all groups by the second day of treatment, as well as decrease in the persistence of the main symptoms of AII. The absence of cases of decompensation of chronic disease in patients receiving Reamberin was noted, in cases of their presence on the background of traditional therapy. Specific economy as a result of using Reamberin in the complex therapy of AII was 23383,5 rubles mainly due to the reduction of the duration of the disease and more rapid vocational rehabilitation. Conclusion: The presence of comorbid pathology affects the typical manifestations of AII, which requires optimization of therapy. The findings show the ability of Reamberin to influence the clinical course of AII, including in cases with associated diseases, by reducing the persistence of the main symptoms of the disease as well as the number of complications associated with decompensation of chronic conditions. High cost-effectiveness of using Reamberin in the treatment of AII, along with a proven positive clinical effect, is an important argument in favor of the widespread use of the succinate-containing drugs in clinical practice.

The feasibility and safety of use of active immunization in the manifested forms of hepatitis A based on the study of the characteristics of the course of hepatitis on the background of post-exposure vaccination has been carried out. Clinical observation of 245 patients with hepatitis A has been conducted, who were divided into two groups: the main group included 87 people, vaccinated with «Havrix» in the hotbed of infection with following hepatitis A. The comparison group consisted of 158 patients with hepatitis with a natural course of infectious process. The safеty of use of emergency immunization with «Havrix» confirmed by the absence of negative impact of vaccination on the main clinical and laboratory characteristics of the manifested forms of the disease in people with post-exposure vaccination.

Phospholipids, sphingomyelins, cholesterol, etc. are characteristic to cellular membranes. The composition of lipids of the both sides of the membranes differs. This fact determines asymmetry of the structure of bili-pid layer. The reason for many pathologies is the changes in the properties of cellular membranes with the modification of their components. The study of structure and functioning of cellular biomembranes is essential for many researchers. The condition of membranes, their quality, their quantitative composition and modification under the influence of different factors as well as their interaction with carbohydrate and protein component are of great importance for the functioning of both membranes, cells and the body in general. Analysis and structuring of lipids and their functions in biological membranes are studied.

The aim of the study: Modification in the treatment of patients with herpes associated with psoriasis. Materials and methods: 30 patients were included in the study divided into two groups. In the 1st group the patients received standard treatment for herpes and two daily applications of cikloferon liniment for seven days. In the 2nd group the patients received only standard treatment for herpes. The patients were followed-up for six months. The study was open-label randomized. Results: The results obtained show that general infectious symptoms of herpes subsided considerably faster in the 1st group of patients treated with cikloferon liniment. The applications with the topical immunomodulator enhanced more dynamic disappearance of erosions. During the six months follow-up exacerbations of herpes were noted in 10% of patients in the 1st group and in 30% of patients in the second group. Cikloferon liniment was well tolerated and no side effects were noted. Conclusion: Cikloferon liniment used for the treatment of patients with herpes exacerbation associated with psoriasis contributes to more dynamic disappearance of infectious syndrome, enhances epithelization of herpetic erosions, and lowers the duration of erosions and local inflammation as well as the number of exacerbations in catamnesis.
Проведен анализ клинико-патогенетической эффективности линимента циклоферона в комплексном лечении герпетического стоматита и пародонтита у 80 больных. В качестве группы контроля проведено исследование лабораторных параметров у 15 практически здоровых лиц, не предъявлявших жалоб на состояние здоровья и не имевших признаков заболеваний полости рта, сопоставимых по полу и возрасту с обследуемыми больными. Эффективность лечебных мероприятий оценивалась с учетом стоматологических индексов: индекс гигиены полости рта, индекс кровоточивости, папиллярно-маргинально-альвеолярный индекс. Показано, что использование линимента циклоферона в комплексе терапевтических мероприятий у больных со стоматитом и пародонтитом позволяет редуцировать инфекционную нагрузку в пародонтальных карманах, местное воспаление, нормализовать параметры иммунитета, снизить эндотоксикоз, что подтверждалось достоверными отличиями индексных показателей на 10–12-й день болезни. На этом фоне отмечается более динамичное выздоровление и уменьшение частоты рецидивов стоматита и пародонтита.
There were examined 28 patients with toxic diphtheria stomatopharynx of the 3-rd degree to evaluate of clinico-laboratory efficiency of plasmapheresis usage in treatment of patients with severe forms of diphtheria. There was established that the use of plasmapheresis in treatment of patients with diphtheria allows to speed up the disappearance of intoxication signs, specific inflammation in a stomatopharynx, to reduce the frequency of complications of disease and to decrease their severity. Plasmapheresis usage dynamically un-proves the readings of glycoproteins exchange and activity of isoferments of aminotransferases.
The clinico-biochemical examination of 125 patients with different forms of diphtheria was carried out to improve the quality of diagnostic of diphtherial infection. It included the complex investigation of readings of glycoproteins and isofermentative spectrum of aminotransferases. There was established that in the development of pathological process in diphtherial infection the disturbance of metabolism in connective tissue play an important role and isofermentative spectrum of aminotransferases is characterized by the marked disbalance mainly with the increase of mitochondrial isoferments. The degree of the revealed changes is clearly correlated with the severity of diseases and pathological shifts in toxic form of diseases are preserving after the end of the acute phase of diseases as diphtherial complications.
