Retrograde urethrography (RUG) is a dynamic radiological procedure in which the cumulative dose-area product (DAP comprises radiation doses derived from both fluoroscopy and radiography. In the present prospective study, digital retrograde urethrographs (Prestilix 1600X DRS. General Electric) are obtained in 40 consecutive male patients (mean age 60.6 years: range 23-88 years) presenting for evaluation of urinary pathology. The radiation dose was recorded as the dose-area product. From these data. a. computer program (Diasoft 2.0, PTW, Freiburg, Germany) then determined the respective proportions of the total radiation dose deriving from fluoroscopy and radiography, as well as the number of radiographic exposures. The average total radiation dose was 316 cGy cm(2). In five of the 40 patients examined (12.5%), the number of radiographic exposures registered by the program and, hence the calculated DAPs were erroneous. In the remaining 35 patients, the average proportion of the cumulative DAP resulting from fluoroscopy was 81% b. while that deriving from radiography was 19%. The computer program utilized in this study makes possible an intelligent evaluation of the DAP in dynamic procedures such as RUG. Since fluoroscopy is responsible for 81% of the cumulative DAP at RUG, a significant decrease in applied radiation dose may be achieved by reducing fluoroscopy time or by using pulsed fluoroscopy.
Aim: Femoroacetabular impingement has been shown to be an important cause for the development of osteoarthritis of the hip joint. Radial MRI of the hip has proven to be the diagnostic procedure of choice to differentiate which patient to operate. While examinations after instillation of contrast agents have been described, new papers have shown high accuracy without contrast medium. Since instillation of contrast medium is considered painful and poses the risk of infection, it is not performed for MRI evaluation of the hip at our institution. However, the question as to which sequence to use has not been settled yet. Therefore, four frequently used sequences using the same resolution were compared. Materials and Methods: A retrospective analysis of T(1)-weighted (T(1)), T(2)-weighted (T(2)) and proton density sequences (the latter with and without fat saturation [PDfs and PD]) of 50 hips in 48 patients with femoro-acetabular impingement and developmental dysplasia of the hip was performed by two raters blinded to the clinical information. The different sequences were rated separately. Evaluated criteria were subjective imaging quality, power to differentiate acetabular from femoral cartilage as well as the labrum from the joint capsule, and localisation and characterisation of pathological changes in joint cartilage, the labrum and at the femoral head-neck junction. Results: Subjective imaging quality was assessed to be similar for T(1), PDfs and PD, whereas it was signficantly worse for T(2). In discriminating specific intraarticular structures, however, only PDfs and PD were superior. The differentiation of acetabular from femoral cartilage was possible in only 44% and 40% of T(1) and T(2) images, but in 79% and 76% of PDfs and PD images. Similar results were found for the differentiation of the acetabular labrum from its vicinity, especially the joint capsule (36%, 35%, 77% and 74%, respectively). Intralabral pathology was seen in a comparable extent in all sequences. An intramedullary oedema at the head-neck junction, however, was signficantly more frequent in PDfs only (67% vs. 0%, 17% and 10% for PDfs vs. T(1), T(2) and PD). Conclusion: Despite their good subjective quality, T(1)- and T(2)-weighted sequences could not depict the acetabular labrumas efficiently as PD-weighted sequences. Concluding from our data, a PDfs sequence (possibly combined with a conventional PD sequence) is advantageous over T(1) and T(2) sequences. Intraarticular administration of contrast medium seems to be superfluous.
Introduction. Proximal humeral fractures are common in the elderly population and are often associated with osteoporosis. Fixation of unstable proximal humeral fractures is problematic due to loss of fixation in osteoporotic bone. Fixed-angle devices are intended to provide superior mechanical stability due to the principle of an internal fixator. The NCB(R)-PH (non-contact-bridging proximal humerus) plate is a new fixed-angle device that locks the screws to the plate by secondary insertion of a locking cap. The aim of this study was to investigate if and to what extent NCB-PH plates applied in the locked mode provide higher mechanical stability in a proximal humerus fracture model. Methods. For this investigation 16 (8 pairs) fresh frozen cadaveric humeri were used. An AO/ASIF 11-B1 fracture of the proximal humerus was created in each bone and fixed with the NCB-PH plate. In a paired setting one bone was fixed with the plate in the locked mode and the other with the plate in the non-locked mode. The bones were then subjected to 100 cycles of axial loading and interfragmentary motion was measured. Bones surviving this test were subjected to load-to-failure testing and the force at which failure occurred was recorded. Results. Bones fixed with the plate in the locked mode showed a statistically significant lower (51%) interfragmentary rotation compared to bones fixed with the plate in the non-locked mode. There was also a tendency for the bones fixed with the plate in the locked mode to fail first under higher forces (16%) during load-to-failure testing. Conclusion. The NCB-PH plate provides superior interfragmentary stability when used in the locked mode in a human cadaveric proximal humerus fracture model. Therefore, we recommend that all screws should be inserted in the locked mode. The results suggest that the NCB-PH plate in the locked mode provides higher primary postoperative stability thus permitting early functional treatment of the patient.
Vascular pathologies are rare causes of an acute abdomen. If the cause is a vascular disease a rapid diagnosis is desired as vascular pathologies are associated with high mortality. A differentiation must be made between arterial and venous diseases. An occlusion of the superior mesenteric artery is the most common reason for acute mesenteric ischemia but intra-abdominal arterial bleeding is also of great importance. Venous pathologies include thrombotic occlusion of the portal vein, the mesenteric vein and the vena cava.Multi-detector computed tomography (MDCT) is predestined for the diagnostics of vascular diseases of the abdomen. Using multiphasic contrast protocols enables reliable imaging of the arterial and venous vessel tree and detection of disorders with high sensitivity and specificity. Although conventional angiography has been almost completely replaced by MDCT as a diagnostic tool, it is still of high importance for minimally invasive interventions, for example in the management of gastrointestinal bleeding.
Drug Prescribing for Patients with Chronic Kidney Disease in General Practice: a Cross-Sectional Study
Rectal cancer is one of the most common causes of death from cancer. Accurate staging is necessary for optimal treatment. The tumor node metastasis (TNM) system is used to describe numerically the anatomical extent of cancer. Various diagnostic methods provide accurate staging. Endorectal ultrasound (EUS) and magnetic resonance tomography (MR) are most adequate for determining tumor stage. Moreover, MR is highly accurate in predicting the circumferential resection margin. Accurate node staging remains however difficult with both EUS and MR. Modern multidetector row CT is predestined for detecting distant metastases as it is a widespread, fast, and reproducible method.
Das kolorektale Karzinom ist eines der häufigsten malignen Tumoren; in Deutschland ist es das zweithäufigste Malignom bei Männern und Frauen. Die präoperative Bildgebung ist beim Rektumkarzinom entscheidend, da die Patienten nur bei akkuratem Staging einem optimalen Behandlungskonzept zugeführt werden können. Die TNM-Klassifikation ist das gängige System, um die Tumorausdehnung zu beschreiben. Zahlreiche bildgebende Verfahren stehen zur Verfügung, um ein effizientes Tumorstaging durchzuführen. Für das T-Staging eignet sich der endorektale Ultraschall (EUS) und die Magnetresonanztomographie (MRT). Die MRT ist darüber hinaus eine wichtige Entscheidungshilfe bezüglich der Resektabilität und der Prognose, da mit ihr eine Infiltration der mesorektalen Faszie mit hoher Genauigkeit bestätigt oder ausgeschlossen werden kann. Ein zuverlässiges Lymphknotenstaging ist sowohl mit EUS als auch MRT nur eingeschränkt möglich. Für das Staging von Fernmetastasen ist in erster Linie die Multi-Detektor-Reihen-Computertomographie (MDCT) geeignet, die weit reichend verfügbar sowie schnell und reproduzierbar ist.
PURPOSE: The purpose of this multicenter study was to compare a dissection display and an endoluminal display for CT colonography (CTC) by means of detection rates and evaluation time in a screening collective. MATERIALS AND METHODS: 4 blinded readers evaluated CTC datasets from 42 patients with 55 endoscopically confirmed polyps. The datasets were read in a randomized order using two different 3D visualization methods (endoluminal view vs. dissection display; EBW 2.0.1, Philips Medical Systems, Best/NL). Patients underwent cathartic cleansing as well as stool and fluid tagging. All readers except one were experienced in performing CTC. The per-lesion/per-patient sensitivity, per-patient specificity, and evaluation time were calculated. RESULTS: The overall per-lesion sensitivity using the dissection display (and endoluminal view) was 60 % (53 %) for reader 1, 58 % (60 %) for reader 2, 67 % (71 %) for reader 3 and 55 % (58 %) for reader 4. The per-patient sensitivity using the dissection display (and endoluminal view) was 85 % (85 %) for reader 1, 80 % (85 %) for reader 2, 95 % (90 %) for reader 3 and 80 % (80 %) for reader 4. The per-patient specificity was 68 % with dissection view (77 % endoluminal view) for reader 1, 82 % (82 %) for reader 2, 59 % (59 %) for reader 3 and 82 % (73 %) for reader 4. The experienced readers were significantly faster using the perspective-filet view. CONCLUSION: Using a dissection display of CTC datasets does not result in superior detection rates for polyps if datasets are stool and fluid-tagged. 3 out of 4 readers evaluated the datasets significantly faster with the dissection display.
Contrast-enhanced multi-detector row helical CT angiography is establishing itself as an accurate, rapid, and non-invasive diagnostic modality in patients with acute gastrointestinal bleeding. On arterial phase MDCT images ongoing hemorrhage can be revealed as an area of active extravasation of contrast material within the bowel lumen. This pictorial essay gives a short overview of current diagnostic modalities in assessing acute GI tract bleeding, typical MDCT findings, and depicts potential pitfalls in the detection of acute GI bleeding with MDCT.
Purpose: Neoadjuvant therapy may reduce local rectal cancer recurrence after total mesorectum extirpation. This study was performed to assess whether multi-detector row CT (MDCT) is capable of reliably differentiating UICC I (surgery) from UICC II-IV (neoadjuvant therapy).Materials and Methods: 29 patients underwent preoperative MDCT of the abdomen in a portal venous phase. Two blinded readers independently evaluated the datasets on a dedicated workstation using axial and coronal reformations. Local tumor extension (T), nodal status (N) and distant metastases (M) were evaluated and the UICC stage was determined. Findings were correlated with postoperative histology.Results: Histologically, 9 patients were UICC 1; 20 UICC>I (II: 7; III: 11; IV: 2). Reader 1 correctly identified 3/9 as UICC 1, overstaged 6/9, and correctly staged 20/20 as UICC > I. Reader 2 correctly identified 4/9 as UICC 1, overstaged 5/9, under-staged 4/20 and correctly staged 16/20 as UICC > I (PPV UICC I 100% [50%] reader 1 [reader 2], NPV 77% [76%], accuracy 79% [69%]). Reasons for overstaging by reader 1 (reader 2) included false-positive lymph nodes (LN) in 5 (5), overgrading T1 tumors as T3 in 1(0), and T overgrading in 4/5 (2 / 5) patients with false-positive LN.Conclusion: MDCT failed to reliably identify UICC I in rectal cancer patients. Therefore, a strategy based solely on MDCT to identify patients who would benefit from neoadjuvant therapy does not seem appropriate.
Ziele: Ziel dieser Studie war, die Verwendung von mangan-haltigem Kontrastmittel (MnDPDP) als azinuszell-spezifisches Kontrastmittel in der MRT des Pankreas bei Patienten mit AIP zu evaluieren. Methode: Es wurden die MRT-Untersuchungen von 11 Patienten (7männlich, 4 weiblich; Ø 46y) mit einer Autoimmunpankreatitis analysiert. Alle Untersuchungen wurden an einem 1,5 T Gerät (Siemens Symphony) durchgeführt. Für die Routinebildgebung wurde gadoliniumhaltiges Kontrastmittel (Magnevist) verwendet, anschließend folgte die Applikation eines mangan-haltigen Kontrastmitels (Teslascan) mit Akquisition von T1-gewichteten Sequenzen nach 60 Minuten. Die erhobenen Bilddaten wurden analysiert hinsichtlich allgemeiner Pankreatitiszeichen, sowie Hinweise für eine Autoimmunpankreatitis. Das Enhancement des Pankreasparenchyms nach Manganapplikation wurde evaluiert. Verlaufsuntersuchungen waren bei 6 Patienten vorhanden. Ergebnis: Neben allgemeinen Zeichen der Pankreatitis zeigten alle Patienten die für eine AIP typische Auftreibung des Pankreasparenchyms. In den Sequenzen nach Applikation von Mangan konnten in allen Fällen Areale mit einer verminderten Manganaufnahme gezeigt werden (10 Pat.: umschriebenes Areas mit reduzierter Manganaufnahme; 1 Pat.: diffuse Verminderung der Manganaufnahme). Drei Patienten erhielten eine Steroidtherapie mit klinischer Verbesserung, aber auch wieder zunehmender Manganaufnahme der betroffenene Areale in der MRT. Drei Patienten, die keine Therapie erhalten hatten, zeigten im Verlauf eine zunehmende Verminderung der Mangan-Aufnahme, eine zunehmende Schwellung des Pankreas oder eine sich ausbildende Atrophie. Schlussfolgerung: Bei einer Autoimmunpankreatitis können in der MRT Areale mit einer verminderten Mangan-Aufnahme nachgewiesen werden. Nach erfolgreicher Therapie ist dieser Befund reversibel. Die Mangan-MRT kann somit bei der Diagnosestellung der Autoimmunpankreatitis nützlich sein.
Ziele: Ziel dieser Studie war es zu untersuchen ob in der CT Colonographie (CTC) mithilfe einer elektronischen Darmreinigung (electronic colon cleansing, ECC) signifikant höhere Sensitivitäten und Spezifitäten in der Detektion kolorektaler Läsionen erzielt werden können als ohne ECC. Methode: Zwei verblindete Radiologen evaluierten stuhl- und flüssigkeitsmarkierte CTC Datensätze von 79 Patienten mit 153 endoskopisch verifizierten, kolorektalen Polypen mit und ohne Einsatz von ECC. Die primäre Auswertung erfolgte mit einer 3D Software (Philips EBW 3.1, Philips Medizinsysteme, Best, NL). Die per-Polyp Sensitivität sowie die per-Polyp und per-Patient in der Detektion der Polypen wurde errechnet. Ergebnis: Die per-Polyp Sensitivität für klinisch relevante Polypen (>6mm) ohne elekrotonische Darmreinigung war 60,4% (Reader 1: 59,7%, Reader 2: 61,1%), für Polypen >10mm 58,3% (Reader 1: 66,7%, Reader 2: 50%). Unter Verwendung der automatischen Darmreinigung stieg die Sensitivität für Polypen >6mm auf 73.6% (Reader 1: 76.4%; Reader 2: 70.8%), für Polypen >10mm auf 83.3% (Reader 1: 83.3%; Reader 2: 83.3%). Die Spezifität/Patient lag bei 68% (Reader 1) und 82% (Reader 2) ohne und 86% (Reader 1) bzw. 77% (Reader 2) mit ECC. Schlussfolgerung: Wird ein primärer 3D Ansatz zur Interpretation von CTC Datensätzen gewählt, kann unter Verwendung einer elektronischen Darmreinigung die Detektionsrate für klinisch relevante Polypen deutlich gesteigert werden.
The purpose of this study was to explore the accuracy of multi-detector row helical CT (MDCT), using a biphasic mesenteric angiography protocol for evaluation of acute mesenteric ischemia (AMI). In total, 79 consecutive patients with clinical signs of AMI underwent contrast enhanced 16- or 40-channel MDCT. MDCT findings were correlated with surgery, endoscopy and clinical outcome. Sensitivity, specificity, and positive and negative predictive values were calculated using the patients in which AMI had been excluded as a control group. In 28 patients the final diagnosis was AMI. In 27 patients (96.4%) MDCT correctly diagnosed AMI (specificity of 97.9%). A sensitivity of 93%, specificity of 100%, and positive and negative predictive values of 100% and 94%, respectively were achieved for the CT findings of visceral artery occlusion, intestinal pneumatosis, portomesenteric venous gas or bowel wall thickening in combination with either portomesenteric thrombosis or solid organ infarction. Our findings suggest that mesenteric MDCT angiography is an accurate tool for fast diagnostic work-up of patients with suspected AMI.
Obwohl Frakturen der lumbalen und thorakalen Wirbelsäule eine der häufigsten Verletzungen innerhalb der klinischen Tätigkeit als Radiologe darstellen, werden sie häufig unterschätzt und die zugrunde liegende Verletzungsmechanismen nicht verlässlich erkannt. Ziel der Präsentation ist es neben den anatomischen Strukturen sowie den radiologischen Beurteilungskriterien, Schritt für Schritt die verschiedenen Traumamechanismen und die daraus resultierenden Fraktur- und Verletzungsmuster aufzuarbeiten, die den Schlüssel zur richtigen Befundung darstellen. Besonderes Augenmerk gilt der Rolle und Indikation der verschiedenen Bildgebungsverfahren wie konventionelles Röntgen, CT und MRT. Weiterhin werden die nicht immer offensichtlichen Instabilitätszeichen demonstriert und ihre Bedeutung für das therapeutische Management aufgezeigt. Abschließend werden Fallstricke und Komplikationen mit ihren Konsequenzen erläutert.
Solitary rib stress fracture is a specific sports-related injury in elite competitive rowers. The standard in terms of diagnostic imaging used to be the Technetium-99m MDP (methylene diphosphonate) bone scintigraphy.Methods: A male elite rower with point-shaped, dexter thoracic pain underwent a multidetector spiral CT (40-slice) with subsequent reconstruction of the ribs in volume rendering technique.Results: The diagnostic imaging showed a non-dislocated fracture of the fourth rib on the right side which is also the origin of the pars inferior of the m. serratus anterior.Discussion: Over the last couple of years, major developments in the field of computed tomography have opened a variety of new options in terms of diagnostic imaging. This case report describes the first-time diagnosis of a stress-induced rib fracture using a multidetector spiral CT and discusses the many advantages of this method compared to bone scintigraphy. These are the widespread availability of multidetector spiral CT, the shortener time needed for the scan itself and receipt of the scan result. In addition, a peripheral venous injection of iodinated contrast is not necessary.
Ziele: Kinder mit leukopoetischen Erkrankungen erhalten oft eine Schädel-MRT zur Beurteilung einer intrakraniellen Beteiligung. In den diffusionsgewichteten Sequenzen fielen dabei Signalanhebungen im Knochenmark der Schädelkalotte auf. Lokalisierte Manifestationen von Erkrankungen des leukopoetischen Systems weisen oft eine erhöhte Zelldichte auf, so ist für zerebrale Lymphome eine Diffusionsrestriktion typisch. Ziel der Studie war es, die Häufigkeit dieser Diffusionsveränderungen bei leukopoetischen Erkrankungen zu bestimmen. Methode: Retrospektiv wurden MRT-Untersuchungen von 43 Kindern (1–24 Jahre, durchschnittlich 8,4 Jahre) mit einer Erkrankung des leukopoetischen Systems (8 Pat. mit AML, 1 Pat. mit CML, 27 Pat. mit ALL, 7 Pat. mit Lymphom) evaluiert, welche in der Zeit vom 01.06.2005 bis 01.06.2007 eine MRT des Schädels inklusive diffusionsgewichteter Sequenz vor Therapie erhalten hatten. 11 der Kinder erhielten eine Kontroll-Untersuchung nach Therapie. Als Kontroll-Gruppe dienten 37 Kinder (1–20 Jahre, durchschnittlich 11,4 Jahre), welche aus anderen Indikationen eine Schädel-MRT erhalten hatten (keine Kinder mit Malignomen/nach Chemotherapie). Die Untersuchungen wurden an einem 1,5 T Gerät durchgeführt (Siemens Symphony, Erlangen). Die Signalanhebung im Knochenmark der Schädelkalotte in der diffusionsgewichteten Sequenz wurde beurteilt (deutliche/mäßige/geringe/keine Signalanhebung). Ergebnis: Von 43 untersuchten Kindern mit einer leukopoetischen Erkrankung konnte bei 18 (42%) eine vermehrte Signalanhebung des Knochenmarks in der diffusionsgewichteten Sequenz nachgewiesen werden (8/43 deutliche, 6/43mäßige, 4/43 diskrete, 25/43 keine Signalanhebung). Nach Therapie konnte bei keinem (0%) Kind eine vermehrte Signalanhebung gezeigt werden, bei 5 Kindern war vorhergehend eine Signalanhebung nachweisbar gewesen. In der gesunden Kontrollgruppe hatten 5 (13,5%) Kinder eine vermehrte Signalanhebung des Knochenmarks. Schlussfolgerung: Bei Kindern mit leukopoetischen Erkrankungen kann in der diffusionsgewichteten MRT in 42% eine Signalanhebung im Knochenmark der Schädelkalotte nachgewiesen werden. Der Befund ist in einer Kontroll-Gruppe mit Kindern ohne leukopoetische Erkrankung, als auch bei erkrankten Kindern nach Therapie signifikant seltener.
Nach einem kurzen Überblick über die Epidemiologie und Prognose der akuten gastrointestinalen (GI-)Blutung werden die verschiedenen verfügbaren Diagnostik-Methoden (wie beispielsweise Gastro-/Koloskopie, Kapselendoskopie, digitale Subtraktionsangiographie, Szintigraphie und MSCT) vorgestellt. Zusätzlich wird kurz auf die jeweiligen Limitationen der einzelnen Diagnostiken eingegangen. Einen besonderen Stellenwert bei diesem Poster nimmt die MSCT-Technik zur Detektion aktiver GI-Blutungen ein. In Fallbeispielen werden die CT-Zeichen akuter GI-Blutungen veranschaulicht. Es folgen Beispiele möglicher diagnostischer Fallstricke/Pitfalls, welche falsch positive bzw. falsch negative MSCT-Ergebnisse liefern können (z.B. fehlende Abgrenzbarkeit der KM-Extravasation im Darmlumen aufgrund einer Verdünnung durch orale Wasserapplikation, Aufhärtungsartefakte, Fremdkörper wie Nahtmaterial oder oral verabreichte Medikamente, Kontrastmittelreste in Divertikeln).
The purpose of this study was to evaluate the accuracy of multi-detector row helical CT (MDCT) for detection and localization of acute upper and lower gastrointestinal (GI) hemorrhage or intraperitoneal bleeding. Thirty-six consecutive patients with clinical signs of acute bleeding underwent biphasic (16- or 40-channel) MDCT. MDCT findings were correlated with endoscopy, angiography or surgery. Among the 36 patients evaluated, 26 were examined for GI bleeding and 10 for intraperitoneal hemorrhage. Confirmed sites of GI bleeding were the stomach (n = 5), duodenum (n = 5), small bowel (n = 6), large bowel (n = 8) and rectum (n = 2). The correct site of bleeding was identifiable on MDCT in 24/26 patients with GI bleeding. In 20 of these 24 patients, active CM extravasation was apparent during the exam. Among the ten patients with intraperitoneal hemorrhage, MDCT correctly identified the bleeding source in nine patients. Our findings suggest that fast and accurate localization of acute gastrointestinal and intraperitoneal bleeding is achievable on MDCT.
Ziele: Evaluation der diagnostischen Genauigkeit der Multidetektor-Spiral-Computertomographie (MSCT) bei Patienten mit Verdacht auf eine akute mesenteriale Ischämie (AMI) unter Verwendung eines biphasischen mesenterialen CT-Angiographie-Scan-Protokolls. Methode: 77 Patienten (30 weiblich, 47männlich; Alter 18–93 Jahre) mit klinischen Zeichen einer AMI wie beispielsweise einem erhöhten Laktat-Spiegel erhielten eine mesenteriale CT-Angiographie mittels 16- oder 40-Zeilen-MSCT. Nach intravenöser Gabe eines jodhaltigen Kontrastmittels (KM) wurden mittels Bolus timing eine arterielle und venöse mesenteriale CT-Angiographie durchgeführt. Das KM wurde mittels einer KM-Pumpe und einer Flussrate von 4ml/s in einer Dosis von 1,2ml/kg Körpergewicht verabreicht. Axiale Schichten (Schichtdicke 1–2mm) wurden akquiriert und die CT-Angiographien wurden mittels multiplanarer Rekonstruktionen (MPR) und maximum intensity projection-Rekonstruktion (MIP) nachbearbeitet. Die MSCT-Bilder wurden nach CT-Zeichen für eine AMI untersucht. Diese CT-Zeichen wurden ausgewertet und retrospektiv mit den Ergebnissen der Chirurgie (n=47), Endoskopie (n=4) oder dem klinischen Verlauf (n=26) korreliert. Die Patienten ohne AMI (48/77) dienten als Kontrollgruppe zur Bestimmung der Sensitivität, Spezifität sowie des positiven und negativen Vorhersagewertes der Gruppe mit bestätigter AMI (n=29). Ergebnis: Bei 29 Patienten wurde eine akute mesenteriale Ischämie bestätigt. In 28 Patienten (96,6%) diagnostizierte die MSCT diese korrekt. In der Kontrollgruppe stimmte die CT-Diagnose in 47/48 Fällen (97,9%) mit der endgültigen Diagnose überein. In einem der 48 Patienten (2,1%) suspizierte der befundende Radiologe einen akuten ischämischen Verschluss der A. colica media. Intraoperativ zeigte sich jedoch eine akute Cholezystitis, der Patient wurde cholezystektomiert (falsch positives MSCT). Verglichen mit der Kontrollgruppe wurde für die MSCT eine Sensitiviät von 93,3% sowie eine Spezifität von 97,9% in der Diagnostik der AMI erreicht, wenn folgende CT-Zeichen berücksichtigt wurden: Verschluss einer Viszeralarterie, Pneumatosis intestinalis, Gasansammlung in der V. portae u/o der V. mesenterica bzw. eine Kombination aus Darmwandverdickung und portomesenterialer Thrombosierung bzw. Darmwandverdickung und Organinfarkt. Schlussfolgerung: Die mesenteriale MSCT-Angiographie gibt uns die Möglichkeit, eine akute mesenteriale Ischämie schnell und akkurat zu diagnostizieren.
Im Rahmen der stetig wachsenden Zahl an Multidetektor-Spiral-CT(MSCT)- und MRT-Untersuchungen kommt es zu einem Anstieg der zufällig entdeckten zystischen Pankreasläsionen. Dies ist unter anderem auf die höhere Ortsauflösung und schnellere Scanzeiten der neuen MSCT-/MRT-Geräte zurückzuführen.
