Purpose. - Focal F-18-fluoro-deoxy-glucose uptake in the myocardium can be a sign of resting myocardial ischemia. The purpose of our study was to assess the relevance of performing myocardial perfusion scintigraphy to screen for myocardial ischemia in patients with an incidental finding of focal myocardial F-18-fluoro-deoxy-glucose uptake on a routine F-18-fluoro-deoxy-glucose positron-emission-tomography-computed-tomography. Methods. - In our retrospective multicentric study, patients were included if they had had an incidental finding of myocardial focal F-18-fluoro-deoxy-glucose uptake on a routine F-18-fluoro-deoxy-glucose positron-emission-tomography-computed-tomography and had also undergone myocardial perfusion scintigraphy within 3 months before or after the F-18-fluoro-deoxy-glucose positron-emission-tomography-computed-tomography. Patients with a pattern of ischemia or scar on the myocardial perfusion scintigraphy in the same territory as the focal F-18-fluoro-deoxy-glucose uptake were considered positive. Results. - Seven of the 34 included patients were positive, with an abnormality on the MPS data in the same territory as the focal myocardial F-18-fluoro-deoxy-glucose uptake. 2 of the 6 patients with focal F-18-fluoro-deoxy-glucose uptake in the left anterior descending vascular supply territory and 2 of the 4 patients with focal F-18-fluoro-deoxy-glucose uptake in the standard right coronary artery territory had an abnormal myocardial perfusion scintigraphy. All 12 patients with focal F-18-fluoro-deoxyglucose uptake restricted to the basal anterolateral and basal inferolateral segments were negative. Conclusion. - Patients with an incidental finding of focal F-18-fluoro-deoxy-glucose uptake on a routine F-18-fluoro-deoxy-glucose positron-emission-tomography-computed-tomography may be considered as being at risk for coronary artery disease, when this uptake is multisegmentary in the same typical coronary territory and not restricted to the basal anterolateral and basal inferolateral segments. Le corticosurrénalome est une tumeur rare de l’enfant. Il sera suspecté devant une symptomatologie liée à une sécrétion hormonale d’origine tumorale (90 % des cas), devant un syndrome de masse tumorale ou lors de la découverte fortuite d’une lésion surrénalienne. L’échographie fœtale réalisée chez une femme enceinte à 37 SA révèle une masse surrénalienne droite tissulaire vascularisée de 38 mm de grand axe. En période post-natale, le bilan biologique révèle un hypercorticisme et une augmentation des dérivés androgéniques. Le diagnostic de corticosurrénalome est fortement suspecté devant la présentation clinico-biologique. Le bilan à distance sera évalué par TDM et par TEP/TDM au 18FDG. L’examen objective un hypermétabolisme intense de la masse surrénalienne droite (SUVmax = 14,7) sans foyer hyperfixant locorégional ou à distance. Cette fixation est concordante avec un corticosurrénalome stade T1N0M0 selon la classification OMS de 2004. Les résultats histologiques de l’exérèse de cette masse confirment un corticosurrénalome malin (score de Weiss modifié = 4) en marges R0. Pour la réalisation de la TEP/TDM chez un nourrisson de 26 jours, les recommandations européennes ont été suivies. Un jeun de 4 h a été respecté avant l’injection de 3 MBq/kg de 18FDG et le nourrisson a été nourri 30 min après l’injection du radiotraceur dans les bras de sa mère afin qu’il s’endorme sous la caméra, placé dans un matelas coquille, une fois le délai de 1 h respecté entre l’injection et l’acquisition. La dose efficace a été estimée à 6 mSv (18FDG : 5 mSV ; TDM : 1 mSv). Bien que l’utilisation de la TEP/TDM au 18FDG n’ait pas encore été évaluée dans cette indication chez l’enfant, elle semble pouvoir être utile dans le bilan d’extension de la maladie. Afin que sa réalisation se fasse dans les meilleures conditions, il est nécessaire de bien informer le personnel paramédical ainsi que les parents du déroulement de l’examen et d’adapter le moment de l’examen au rythme des tétées de l’enfant. Un jeûne de 4 h paraît suffisant et la sédation chez le nourrisson peut être favorisée par une tétée 30 minutes avant le début de l’acquisition TEP/TDM.
Évaluer la nécessité d’une correction du diffusé dans l’évaluation des zones gâchettes (ZG) post-infarctus par TEMP myocardique double isotope 201Tl/123I-MIBG sur caméra CZT. Évaluer la nécessité de l’acquisition tardive 123I-MIBG pour l’exploration de ces ZG. Nous avons inclus 69 patients après revascularisation pour SCA ST+ pour recherche de ZG (myocarde viable mais dénervé). Trois tomoscintigraphies myocardiques de repos étaient réalisées : 201Tl simple isotope, 201Tl/123I-MIBG précoce et tardive. Les projections 201Tl de la troisième étaient reconstruites sans et avec correction du diffusé (méthode du triple fenêtrage). La viabilité et la dénervation étaient appréciées par des scores sommés de perfusion (SS Tl) et d’innervation (SS MIBG) selon un modèle à 17 segments. Un score sommé gâchette (SSG) était calculé selon le même modèle. Nous avons comparé les performances diagnostiques des différentes acquisitions 201Tl avec et sans correction du diffusé dans l’identification des nécroses considérant l’IRM ou l’ETT comme gold standard. Tous les patients présentaient des zones gâchettes (37 % ± 18 de la surface du VG). Sans correction du diffusé, le diffusé de l’123I induit des différences significatives entre les SS Tl simple et double isotope (p < 0,0001). Les SS Tl double isotope tardif et les SSG étaient significativement plus élevés sur les acquisitions corrigées du diffusé (p < 0,0001). Les acquisitions 201Tl corrigées du diffusé avaient les meilleures performances diagnostiques dans l’identification des nécroses. Les SSG tardifs étaient significativement plus élevés que les précoces (p < 0,0001). Malgré une résolution en énergie accrue des CZT, une correction du diffusé est nécessaire pour les acquisitions 201Tl/123I-MIBG dans l’exploration des zones gâchettes post-infarctus.
L’infiltration du système nerveux périphérique par des cellules lymphomateuses (ou neurolymphomatose) est une manifestation extra ganglionnaire rare rencontrée essentiellement dans les lymphomes non hodgkiniens de type B. L’atteinte peut être unique ou multiple et peut intéresser un nerf crânien, un nerf périphérique ou une racine nerveuse. Sept mois après la fin de traitement d’un lymphome B diffus à grandes cellules de localisation naso-sinusienne droite en rémission complète, le patient se plaint de céphalées frontales isolées. L’IRM cérébrale est en faveur d’une pachyméningite et la ponction lombaire objective un infiltrat de nombreuses cellules lymphoïdes atypiques d’allure blastique. Une TEP/TDM au 18FDG est réalisée dans le bilan d’extension de cette récidive tumorale. La TEP/TDM met en évidence plusieurs foyers hyperfixants étagés à l’émergence des racines nerveuses évoquant une polyradiculopathie d’origine lymphomateuse. Il n’existe pas d’argument en faveur d’une récidive locale naso-sinusienne ou d’une atteinte ganglionnaire. Le patient a bénéficié de 6 cycles de chimiothérapie générale et d’injections intrathécales. L’examen TEP/TDM réalisé en fin de traitement montre une réponse complète avec disparition de l’ensemble des foyers hyperfixants radiculaires. L’envahissement lymphomateux du système nerveux périphérique est une atteinte rare de diagnostic difficile. L’analyse du liquide céphalorachidien est souvent négative et la biopsie nerveuse à l’aveugle est régulièrement non contributive. Plusieurs auteurs considèrent que les résultats concordants de la TEP/TDM et de l’IRM permettent de suggérer le diagnostic de neurolymphomatose. Peu de cas ont été décrits en TEP/TDM, mais celle-ci semble jouer un rôle important pour évoquer le diagnostic, réaliser un bilan d’extension, orienter un geste biopsique et évaluer l’efficacité thérapeutique.
Background and aims. The analysis of the left ventricular contractile function plays a major role in the diagnosis and management of patients with cardiopathies. The aim of our study was to compare gated blood pool SPECT and myocardial perfusion scintigraphy for the assessment of the left ventricular wall contractility at the global and the segmental scales.Material and methods. The data of 23 99mTc-Tetrofosmin perfusion scintigraphies, and 50 201Thallium perfusion scintigraphies were compared to those of gated blood pool SPECT performed at close interval.Results. The Correlations were good (r = 0.81 to 0.94) concerning the global parameters (left ventricular ejection fraction, end-diastolic and endsystolic volumes) in the two groups. Quite good correlations were also found at the segmental scale (r = 0.49 to 0.62), between the segmental ejection fraction calculated in gated blood pool SPECT and the wall thickening or the wall motion estimated in perfusion scintigraphy. These correlations were significantly lower in the "(201)Thallium perfusion scintigraphy" group than in the "Tc-99m-Tetrofosmin perfusion scintigraphy" group, especially for hypokinetic segments.Conclusion. Although they use very different approaches, GBPS and MPS give data about global and segmental left ventricular wall contraction that are well correlated, but not strictly interchangeable. Purpose. We investigated the prognostic significance of F-18 fluorodeoxyglucose (FDG) uptake measured as maximum Standardized Uptake Value (SUVmax) in primary tumor by positron emission tomography/computed tomography (PET/CT) in cervical cancer. The secondary objective was to determine the accuracy of the PET/CT for detecting pelvic lymph node (PLN) and para-aortic lymph node (PALN) metastases.Methods. This retrospective study included 49 consecutive patients with stage IBI to IVB cervical cancer. Univariate analysis was performed to determine the relationships between SUVmax value and pathological prognostics factors. Survival was estimated by Kaplan-Meier method. The gold standard of LN metastases was histologic.Results. A significant difference in SUVmax was observed between stage I and stage II, stage 1 and stage IV and tumor size < 4 cm and > 4 cm (P = 0.0001). There was a significant correlation between the SUVmax and tumor maximal size (r= 0.597) (P < 0.0001). PLN metastasis was found to be predictive of progression-free survival (P = 0.0007). The negative predictive value (NPV) of the PET/CT for PALN was 100% for locally advanced cervical carcinoma in 24 patients. The specificity and NPV of the PET/CT for PLN in eight early-stage cervical cancer were 100% and 87.5% (7/8) respectively. The PET/CT false-negative PLN measured less than 2 mm.Conclusion. Our results demonstrate a correlation between SUVmax and tumor maximal size, which represents an indicator of tumor aggressiveness. PET/CT is effective to predict the absence of PALN in locally advanced cervical carcinoma. PET/CT is not sufficient to predict PLN in early-stage cancer without lymphadenectomy. Myocardial perfusion SPECT is a routine for the assessment of patients with coronary artery disease (CAD). However, attenuation artifacts may decrease the specificity of the test. These artifacts can be corrected with an attenuation correction. We prospectively included 70 patients who underwent myocardial perfusion SPECT with (IRAC) and without (IRNC) attenuation correction using transmission CT imaging integrated in the acquisition system in patients with low prevalence of CAD. Automatic quantitative analysis with summed stress score (SSS) and rest (SRS) and summed difference score (SDS) was used as interpretation criteria. The results showed a specificity of 80% for IRAC and 56% for IRNC, a positive predictive value of 40% for IRAC and 23% for IRNC, without any significant change in sensitivity. An unpaired t-test showed no significant difference between the overall population and one where an artifact was corrected for heart rate and breath rate during the acquisition effort, the body mass index, chest and abdomen circumferences, and the ratio of these two parameters. Attenuation correction significantly improves the specificity of myocardial scintigraphy with no significant difference in sensitivity. The majority of corrections were for artifacts from the inferior wall in men. There is no correlation between the anthropomorphic and physiological parameters and the occurrence of an artifact of attenuation corrected with CT data. (C) 2012 Elsevier Masson SAS. Certains cancers sont particulièrement ostéophiles comme les cancers de la prostate, du poumon, du rein, du sein et de la thyroïde. Le profil métastatique (métastases osseuses prédominantes sur les métastases viscérales) et le pronostic relatif conféré par les métastases osseuses fait préférer une imagerie spécifique osseuse ou corps entier. De par leur fréquence, les cancers du sein, de la prostate ou du poumon constituent 80 % des indications oncologiques de la scintigraphie osseuse. Le développement de la tomographie par émission de positons (TEP) peut faire discuter l'indication préférentielle de l'une ou l'autre de ces modalités. Les modalités actuelles de l'imagerie métabolique TEP des métastases osseuses comprennent une imagerie à cible mixte (tumorale et inflammatoire) comme la TEP-18F-fluoro-déoxy-glucose (FDG), une imagerie osseuse réactionnelle en TEP-fluorure de sodium (FNa), et une imagerie tumorale directe spécifique au cancer primitif causal comme la TEP-18F-fluoro-3,4-dihydroxyphénylalanine (FDOPA) ou la TEP à la fluoro-choline. La sensibilité du FDG est tributaire du phénotype des métastases osseuses et de la nature du primitif. Ainsi, la TEP-FDG est abandonnée dans les cancers de la prostate au profit de la TEP à la fluoro-choline. Lorsque le profil métastatique est mixte (osseux et viscéral), des cocktails de radiopharmaceutiques TEP pourraient se discuter dans un futur proche. Les performances de chaque examen sont discutées dans un contexte cancer-spécifique.Cancers prone to spread to bone include prostate, lung, kidney, breast and thyroid cancers. While bone scanner has been widely used in the past decades, PET-based imaging modalities are increasingly used. Current modalities of PET imaging of bone metastases include tumor and inflammatory targeting with FDG-PET, bone imaging with NaF-PET, and direct cancer-specific markers such as FDOPA-PET or PET using choline. The cancer-specific metastatic patterns and the relative prognosis conferred by osseous metastases (versus visceral metastases) may determine the need for bone scan, FDG-PET for the detection of bone metastases. Because some cancers have a mixed skeletal and visceral, cocktails PET radiopharmaceuticals may also be discussed in the future. The cancer-specific context and performances of bone scan and PET imaging are discussed.
Objective: To evaluate urinary interleukin-8 (IL-8), an inflammatory cytokine, as a screening method for detecting asymptomatic bacteriuria in pregnancy.Methods: Clean-catch urine samples from 200 pregnant women undergoing screening for asymptomatic bacteriuria were evaluated by urine culture, urine dipstick analysis, and measurement of IL-8. Interleukin-8 levels were measured by a chemiluminescent immunoassay (Immulite IL-8, Diagnostic Products Corp., Los Angeles, CA), and a receiver operating characteristic curve was used to determine the optimal cutoff point. Asymptomatic bacteriuria was defined as at least 100,000 colony-forming units of a single organism per mL. Dipstick testing included nitrite assessment as positive or negative and leukocyte esterase as negative, trace, 1+, 2+, or 3+. Dipstick testing was considered positive if nitrite was positive or leukocyte esterase was trace or greater. Sensitivities, specificities, positive and negative predictive values were determined for urinary leukocyte esterase and nitrite and compared with those of IL-8. χ2 and Mann-Whitney U tests were used for statistical analyses.Results: Twenty women were identified with asymptomatic bacteriuria by urine culture. The median urinary IL-8 levels for women with and without asymptomatic bacteriuria were 356 pg/mL and 125 pg/mL, respectively (P < .01, Mann-Whitney U test). Using an optimal cutoff point of 264 pg/mL, IL-8 had a sensitivity, specificity, positive and negative predictive value of 70%, 67%, 19%, and 95% for predicting asymptomatic bacteriuria. Urine dipstick analysis with either a positive leukocyte esterase or nitrite had a sensitivity, specificity, positive and negative predictive value of 45%, 62%, 12%, and 91%, respectively, for detecting asymptomatic bacteriuria. The differences between these testing methods were not statistically significant.Conclusion: Urinary interleukin-8 is not an acceptable screening method for asymptomatic bacteriuria in pregnancy because it fails to detect 30% of women with this condition.
L’os est un site métastatique d’importance dans les cancers broncho-pulmonaires. L’incidence élevée des métastases osseuses et leurs complications sont responsables d’un handicap fonctionnel important et d’une altération de la qualité de vie des patients. Leur prise en charge est multimodale et personnalisée avec de nombreuses spécialités qui interviennent au décours de discussions collégiales idéalement menées en RCP dédiées (RCP d’Oncologie Osseuse Secondaires − RCP-OOS). À côté du traitement systémique médicamenteux spécifique antalgique et anti-résorptif (acide zolédronique, antirank ligand), de nombreuses techniques, de plus en plus performantes (radiothérapie, techniques radiologiques interventionnelles, chirurgie mini-invasive) peuvent s’associer pour assurer une antalgie et une consolidation adéquates. Le pronostic des cancers broncho-pulmonaires ne cesse de s’améliorer (thérapie ciblée, immunothérapie), inscrivant la prise en charge de ces patients dans la durée, et rendant d’autant plus nécessaire le traitement dédié de ce site métastatique pour assurer aux patients la meilleure qualité de vie possible.© 2021 SPLF. The high incidence of bone metastases and their complications are responsible for significant functional impairment and impact patients’ quality of life. Their management is multimodal and personalized with many specialties involved. Ideally, the strategy will be decided during a bone multidisciplinary discussion in harmony with the oncological strategy. In addition to the specific analgesic and anti-resorptive systemic treatments (zoledronic acid, denosumab), many techniques, more and more efficient (radiotherapy, interventional radiological techniques, mini-invasive surgery) can be combined to ensure analgesia and adequate consolidation. Since prognosis of lung cancers continues to improve thanks to targeted therapy and immunotherapy, inscribing the management of these patients over time has become essential to ensure patients the best quality of life.© 2021 SPLF.
