Un service de médecine nucléaire est constitué de plusieurs secteurs dont l’agencement, régi par le niveau d’exposition aux rayonnements (secteurs « froids » ou « chauds »), doit nécessairement tenir compte des contraintes de préparation et de distribution des médicaments radiopharmaceutiques, de stockage/gestion des déchets et effluents contaminés et du parcours patient (accueil, injection du MRP, acquisition des images). Le Guide no 32 de l’ASN précise spécifiquement les règles de conception et d’exploitation des services de médecine nucléaire. Sans oublier que l’ASN ne constitue pas la seule instance ayant des exigences concernant la structuration d’un service, il est parfois complexe de moduler les locaux de manière optimale avec toutes les difficultés et spécificités architecturales que cela peut engendrer. Enfin, le développement anticipé de la RIV, par son impact sur l’agencement du service, est un enjeu structurel à court/moyen terme.
En matière de règles de conception et d’exploitation, il existe, à ce jour, un nombre important de textes applicables et de prescriptions en rapport, compliquant la compréhension globale des exigences en la matière. On peut, néanmoins, grâce à ces derniers, identifier les exigences optimales en termes de radioprotection, tant sur le plan de la conception que de l’exploitation, pour élaborer et structurer un service de médecine nucléaire s’adaptant aux risques d’exposition aux rayonnements ionisants conformément au principe de radioprotection « ALARA ». Pour cela, de nombreuses instances interviennent et la chronologie précise des différents dépôts de dossiers et procédures, elle aussi, relativement peu compréhensible et pourtant primordiale à l’élaboration d’un service de MN. Plusieurs documents existent afin de guider les médecins nucléaires dans leur démarche servant parfois de « référentiel » et permettant de définir certaines notions réglementaires.
Cette première partie s’attachera à décrire tout d’abord l’historique et les différents enjeux autour de la médecine nucléaire, puis les multiples instances d’intérêts et la chronologie de dépôt de dossier, et enfin la participation d’acteurs tiers dans ce type de réalisation.
This first part will attempt to firstly describe the history and the various issues around nuclear medicine, then the multiple bodies of interest and the chronology of filing of files, and finally the participation of third parties in this type of achievement. In terms of design and operating rules, there are to date a large number of applicable texts and related prescriptions, complicating the overall understanding of the requirements in this area. Thanks to the latter, however, it is possible to identify the optimal requirements in terms of radiation protection, both in terms of design and operation, in order to develop and structure a nuclear medicine department that adapts to the risks of exposure to ionizing radiation in accordance with the ''ALARA'' radiation protection principle. For this, many instances are involved and the precise chronology of the various filings of files and procedures, also relatively difficult to understand and yet essential to the development of an NM service. Several documents exist to guide nuclear physicians in their approach, sometimes serving as a ''reference'' and allowing certain regulatory concepts to be defined. A nuclear medicine department is made up of several sectors whose layout, governed by the level of exposure to radiation ("cold" or "hot" sectors), must necessarily take into account the constraints of preparation and distribution of radiopharmaceuticals, storage/management of contaminated waste and effluent, and the patient pathway (reception, MRP injection, image acquisition. ASN Guide No. 32 specifically specifies the rules for the design and operation of nuclear medicine departments. Without forgetting that the ASN is not the only body with requirements concerning the structuring of a service, it is sometimes complex to modulate the premises in an optimal way with all the difficulties and architectural specificities that this can cause. anticipated RIV, through its impact on the layout of the service, is a structural issue in the short/medium term. ⠂C 2022 Published by Elsevier Masson SAS.
La scintigraphie aux leucocytes marqués (SLM) a démontré son apport dans le diagnostic de l’endocardite sur prothèse valvulaire (EPV) en complément de l’échocardiographie. Dans cette étude, 47 patients consécutifs avec une EPV et un SLM positif traités médicalement ont été identifiés rétrospectivement. En cas d’EPV, un traitement chirurgical est souvent recommandé, mais associé à un risque élevé. Il apparaît donc important de parvenir à identifier les patients avec une EPV susceptibles d’évoluer favorablement avec un traitement médical. Les objectifs de cette étude étaient de tester la valeur pronostique de l’intensité du signal sur la SLM chez des patients avec une EPV traités médicalement et de caractériser l’évolution du signal dans les prothèses valvulaires en SLM sous traitement par antibiotiques. Une recherche systématique a été réalisée dans notre base de données locale des patients avec une EPV et un SLM classifié comme positif. L’intensité du signal sur les acquisitions réalisées 24 heures après l’injection était classifiée comme modérée si le signal était inférieur au signal du foie et intense si le signal était supérieur ou égal au signal du foie. Les données cliniques, biologiques et d’imagerie ainsi que le suivi clinique ont été récupérés à partir des dossiers électroniques des patients. Parmi les 47 patients inclus (12 femmes ; âge moyen : 58 ± 13 ans), 10 patients avaient un signal intense et 37 patients un signal modéré sur SLM initiale. L’incidence du critère composite primaire (incidence de décès, chirurgie cardiaque tardive ou récidive d’endocardite) était significativement plus élevée dans le groupe de patients avec un signal intense vs modéré sur la SLM initiale (90 % vs. 11 %, p < 0,05). 25 patients ont été imagés par une seconde SLM au cours du suivi après un délai médian de 59 jours (intervalle interquartile : 41–97 jours) après la SLM initiale. La prévalence d’un signal positif sur la SLM diminuait progressivement de 89 % entre 3 et 6 semaines à 42 % entre 6 et 9 semaines, jusqu’à 8 % après 9 semaines de traitement antibiotique. La SLM apparaît donc comme un examen intéressant pour stratifier le risque des patients avec une EPV traitée médicalement ainsi que pour suivre localement l’efficacité du traitement antibiotique chez ces patients.
Les études sur la TEP/TDM aux analogues de la somatostatine radiomarqués au 68Ga chez les patients atteints de TNE sont pour la plupart rétrospectives ou hétérogènes en termes de recrutement. Cette étude multicentrique a analysé prospectivement l’intérêt de la TEP/TDM-68Ga-DOTANOC comparativement au bilan conventionnel optimisé (BCO: scintigraphie à l’Octréoscan® corps entier avec TEMP/TDM, scanner thoraco-abdomino-pelvien multiphase et lorsque nécessaire IRM hépatique), chez les patients présentant une TNE-GEP grade 1–2. Le gold standard incluait l’histologie, les données de l’ensemble des imageries et du suivi à 12 mois. Les données de 105 patients ont été analysées : 45 femmes, 60 hommes, d’âge médian: 62,8 ans, explorés pour bilan initial (n = 48), suspicion de rechute/progression (n = 31), suivi (n = 15) ou caractérisation d’une lésion suspecte de TNE en imagerie conventionnelle (n = 11). Les primitifs étaient majoritairement pancréatiques (42,9 %) puis gréliques (30,5 %), et les tumeurs de grade 2 dans 52,0 % des cas. La sensibilité de la TEP/TDM-68Ga-DOTANOC était significativement supérieure à celle du BCO dans l’analyse par patient (97,7 % vs 88,6 %, p = 0,043), par région (96,9 % vs 75,6 %, p < 0,0001), dans la détection des tumeurs primitives (95,8 % vs 81,2 %; p = 0,02), de l’envahissement ganglionnaire (94,9 % vs 66,7 %, p < 0,0001) et de la carcinose péritonéale (93,4 % vs 33,4 %, p = 0,007). La sensibilité de la TEP/TDM-68Ga-DOTANOC était également supérieure, mais non significativement, pour les autres localisations tumorales. Cette étude confirme la supériorité de la TEP/TDM-68Ga-DOTANOC par rapport au BCO pour le bilan lésionnel des TNE-GEP grade 1–2, notamment dans des localisations de petite taille ou d’analyse difficile comme la carcinose péritonéale.
Specific treatments were recently proposed for hereditary transthyretin (ATTRv) amyloidosis. It is essential to screen for disease onset among asymptomatic carriers to trigger treatment as early as possible. We describe their baseline clinical and paraclinical data, as an assessment to detect disease onset. We retrospectively collected data of asymptomatic ATTRv carriers with normal electroneuromyography (ENMG), between March 1st 2015 and January 31st 2019, including symptoms, physical exam, ENMG, cardiac evaluation (multimodal imaging, cardiac denervation), and skin biopsy (amyloid deposition, denervation). We included 130 patients, aged 43.6 years (± 13.5), selected in families of probands with an age of onset of 52.7 years (± 15.7), 41% male, carrying 18 different TTR variants including 65% Val30Met. Cardiac fixation on DPD scintigraphy (11/115) and/or amyloid deposits on skin biopsy (15/95), defining amyloid infiltration, were found in 22/130 patients (16.9%)-amounting to 25% when both were performed. Amyloid infiltration was associated with age (P = 0.024), age difference < 10 years from proband (P < 0.001), carpal tunnel syndrome (P = 0.022), interventricular septum thickness (IVST) > 12 mm (P < 0.001), late gadolinium enhancement in cMRI (Pp = 0.002), cardiac denervation (MIBG scintigraphy C/M < 1.85 (P = 0.044). Among patients with a positive DPD scintigraphy, 13% were < 50 years old and had an IVST < 12 mm. Multivariate analysis showed that age difference with proband and IVST were predictors for amyloid infiltration (OR = 1.11, IC [1,04–1,19] and OR = 1,76, IC [1,19–2,60]). Clinical and paraclinical assessment of asymptomatic carriers of ATTRv mutations shows that skin or cardiac amyloid infiltration precedes ENMG abnormalities in up to 25% of patients. This may trigger specific therapies in borderline cases. Prospective follow up of this cohort might help further define criteria for early onset of specific treatments.
L’hypermétabolisme myocardique physiologique, en l’absence de régime sans hydrates de carbone, sur la tomographie à émission de positons au FDG couplée au scanner (TEP-TDM au FDG) rend difficile l’interprétation des anomalies à ce niveau, avec de multiples diagnostics différentiels. Nous rapportons le cas d’une patiente âgée de 40 ans, sans antécédents particuliers, ayant présenté des céphalées temporales gauches intenses. Une imagerie par résonance magnétique (IRM) cérébrale a mis en évidence une pachyméningite temporale dont la biopsie a conclu à une origine tuberculeuse. Une TEP-TDM au FDG a été réalisée dans le cadre du bilan d’extension de la tuberculose. La TEP a mis en évidence un hypermétabolisme cérébral temporo-polaire en rapport avec la pachyméningite connue, un hypermétabolisme médullaire focal centré sur le foramen de conjugaison gauche à l’étage C6–C7 suspect d’épidurite, ainsi que des adénopathies hypermétaboliques sus et sous-diaphragmatiques. Par ailleurs, il est mis en évidence une plage hypermétabolique cardiaque intense (SUVmax : 17,5) du sillon interventriculaire antérieur, associée à un hypermétabolisme linéaire du toit de l’oreillette droite. Devant ces anomalies, une IRM cardiaque a été réalisée, elle s’est révélée sans particularités. L’hypothèse d’une activation de la graisse brune n’a pas été retenue devant la topographie non évocatrice et l’absence de signal graisseux en IRM. Une surveillance TEP a donc été proposée. L’évolution sous traitement antituberculeux a été marquée par une progression générale de la maladie avec persistance des anomalies myocardiques décrites initialement et apparition d’un hypermétabolisme au niveau du bord libre du ventricule droit. On note aussi l’apparition en IRM d’un aspect suggérant un tuberculome au niveau du segment septal concordant avec les anomalies TEP initiales. Le cas rapporté illustre l’importance devant être accordée à l’analyse de l’aire cardiaque en TEP-TDM au FDG même en l’absence d’un régime sans hydrates de carbone et rappelle le caractère précoce des anomalies métaboliques en TEP évoquant le diagnostic de tuberculose myocardique.
Abstract Introduction The risk ok type A aortic dissection (AAD) depends on the degree of aortic wall's alteration, which can result in dilatation or tortuosity. The estimate of this risk relies solely on the evaluation of the diameter of the ascending aorta. Purpose The purpose of this study is to evaluate the presence and importance of aortic tortuosity in patients with type A aortic dissection. Method Postoperative CT scans of patients with type A aortic dissection were compared with CT scans from controls matched for gender and age. After 3D reconstruction, total length (actual distance along aortic center line = Ltot) and geometric length (length of a straight line between start and end of the aortic segment = Lgeo) were measured to calculate the tortuosity index (TI = Ltot / Lgeo). Results Ltot, Lgeo and TI from different aortic segments of the AAD group were higher than in the control group. Ltot and TI of the whole aorta (from aortic valve to bifurcation) were greater in patients with type A aortic dissection (527.7±46.1 mm vs. 475.8±39.7, p<0.0001; and 2.05±0.24 vs. 1.98±0.21, p=0.002 respectively). Total length and TI were greater after exclusion of the ascending part, and a value of this TI >1.3 identifies AAD patients with an accuracy of 74.8% (AUC = 0.792, p<0.0001). TI is altered by risk factors for aortic dissection: it increases with hypertension and age but not by tobacco use, and TI decreases in diabetes. Conclusions Type A aortic dissection is associated with longer aorta and increased aortic tortuosity. This index may help recognize patients at risk for type A aortic dissection. During the last decade, disease modifying therapies have been proposed for hereditary amyloidogenic transthyretin (ATTRv) amyloidosis. It is essential to screen for disease onset among asymptomatic mutation carriers in order to trigger effective therapy as early as possible. We describe clinical and paraclinical data from ATTRv asymptomatic carriers, as a baseline assessment to detect disease onset and progression. We retrospectively collected data of asymptomatic ATTRv carriers with normal electroneuromyography (ENMG), between March 1st 2015 and January 31st 2019, including: demographics, symptoms, physical exam, ENMG (initial and follow-up obtained in 73 patients, 56.2%), neurovegetative tests, skin biopsy (amyloid deposition, denervation), and cardiac evaluation (multimodal imaging, cardiac denervation and arrhythmias). We included 130 patients, aged 43.6 years (± 13.5), selected in family of a proband with an age of onset of 52.7 years (±15.7), 40.8% male, carrying 20 different variants of the TTR gene, including 63.8% Val30Met. Amyloid deposits on skin biopsy and/or cardiac fixation on bone scintigraphy, characteristic of amyloid infiltration, were found in 22/130 patients (16%). Skin biopsy was positive in 11 patients, and cardiac fixation on bone scintigraphy was positive in 15 patients. Amyloid infiltration was statistically associated with age (p=0.024), age difference from index case (p<0.001), electrophysiological carpal tunnel syndrome (p=0.022), interventricular septum thickness >12 mm (p<0.001), contrast enhancement in cardiac MRI (p=0.002), cardiac denervation using MIBG scintigraphy C/M ratio <1.85 (p=0.044). Multivariate analysis showed that age difference with index case and interventricular septum thickness were predictors for amyloid infiltration (OR=1.11, IC [1,04–1,19] and OR=1,76, IC [1,19–2,60]). Subgroup analysis showed electroneuromyography alteration during follow up in 11 patients (delay 26.9 months, range 10–49) with statistic association with amyloid deposits (55.6 versus 7.1% when ENMG was unchanged, p<0.001) and interventricular septum thickness (p<0.010). Clinical and paraclinical assessment of asymptomatic carriers of ATTRv mutations shows that skin or cardiac amyloid infiltration may precede ENMG abnormalities in 16% of patients. This may trigger specific therapies in borderline cases. Age close to age of onset among index case and septum interventricular thickness appear as risk factors for developing electroneuromyogram alteration. Type of funding source: None
Les hypermétabolismes spléniques et médullaires ont été décrits dans la littérature comme des signes indirects d’une pathologie inflammatoire et/ou infectieuse en TEP au 18F-FDG. Cette étude a pour but d’évaluer la valeur diagnostique de ces signes chez des patients avec endocardite infectieuse (IE) connue ou suspectée. Il s’agit d’une étude ancillaire de l’étude princeps multicentrique TEPvENDO, PHRC national dans lequel une TEP au 18F-FDG a été réalisée dans une population présentant une probabilité élevée d’EI. Les patients ont été inclus prospectivement pour l’étude TEPvENDO dans 8 centres français. La valeur moyenne de Standardized Uptake Value (SUV) a été déterminée pour la rate et la moelle osseuse. Les hypermétabolismes splénique et médullaire étaient définis par des valeurs de SUV supérieures à celles du foie. Un total de 129 patients a été inclus dans cette étude, 47 % étaient porteurs d’une valve prothétique. Selon les critères modifiés de Duke-Li, l’EI était certaine pour 88 patients, possible pour 25 patients et exclue pour 16 patients. Les anomalies de fixation cardiaque évoquant une endocardite infectieuse étaient retrouvées chez 41 % (36/88) de la population EI certaine, 8 % (2/25) pour celle EI possible et 6 % (1/16) pour celle EI exclue (p < 0,001). Les taux correspondants pour la présence d’un hypermétabolisme splénique ou médullaire étaient de 82 % (71/87), 60 % (15/25) et 47 % (7/15) (p = 0,005). Ces hypermétabolismes étaient un facteur prédictif indépendant d’une EI certaine (p = 0,014 ; odd ratio (OR) : 3,2 [intervalle de confiance à 95 % : 1,3–8,0]), mais ils étaient avant tout liés à la présence d’une hémoculture positive, constituant un critère majeur de Duke-Li (p = 0,001 ; OR : 4,7 [1,9–11,6]. Une EI certaine a été rapportée chez 97 % (29/30) des patients présentant à la fois une anomalie de fixation cardiaque et un hypermétabolisme splénique ou médullaire, chez 78 % (7/9) de ceux présentant uniquement une anomalie de fixation cardiaque, 67 % (42/63) de ceux avec seulement un hypermétabolisme splénique ou médullaire et 37 % (10/27) de ceux sans hypermétabolisme splénique ou médullaire et sans anomalie de la fixation cardiaque. Un hypermétabolisme splénique ou médullaire constitue un signe significatif bien qu’indirect de l’endocardite infectieuse et, par conséquent, il pourrait être considéré en complément des critères diagnostiques conventionnels.
The coronary flow reserve is a quantitative parameter defined by the ratio maximal myocardial blood flow to rest myocardial blood flow, which allows to give functional information on the whole coronary arterial tree, integrating both epicardial arteries and microcirculatory. The coronary flow reserve is a powerful tool to guide therapy and to assess prognosis. Exploratory tools, initially limited to experimental invasive techniques, have evolved over the last 10 years, allowing to envisage its use in daily clinical practice. This article reviews the pathophysiology of the coronary flow reserve and the various invasive and non-invasive exploration tools available to practitioners, integrating them into clinical practice.
