Tacrolimus is a broadly used immunosuppressive agent in organ transplanted patients. It is known that it may induce arrhythmias. We here report on a heart transplanted patient with supraventricular and ventricular arrhythmias with an elevated tacrolimus trough serum level due to severe diarrhea.
Objective Percutaneous treatment of coronary chronic total occlusions (CTO) remains one of the major challenges in interventional cardiology. The strategies of recanalisation in CTO have changed drastically due the development of new techniques such as the retrograde approach via collaterals. In this single-centre experience we sought to analyse the success rates with the use of different CTO techniques, the complication rates, and we evaluated predictors of failed CTO recanalisation attempts.Methods and Results In this single-centre observational study we analysed the prospectively entered data of 331 consecutive patients, undergoing percutaneous coronary intervention (PCI) for CTO in 338 lesions at the Heart Center Wuppertal between June 2007 and July 2010. Nineteen lesions were attempted twice and one lesion three times (=358 procedures). The lesion-related success rates were 81.1%. Single-wire usage was the predominant strategy used in 198 antegrade cases (65.6%) followed by parallel wire technique and see-saw technique in 94 cases (31.1%). In the retrograde procedures, the reverse CART technique was predominantly used (35.7%), followed by retrograde wire passage (17.9%), marker wire (17.9%) and CART (14.3%). The in-hospital complications were low and comparable with conventional PCI data. The presence of blunt stump, severe calcification, severe tortuosity and occlusion length > 30 mm were independent predictors of procedural failure.Conclusions A high degree of success with low in-hospital complications comparable with conventional PCI data can be expected in the hands of experienced CTO operators. A second try with a retrograde approach after antegrade failure should be considered.

Med. 28, 311–318 (2011)AbstractAims Left ventricular diastolic dysfunction is considered a precursor of diabetic cardiomyopathy, while diabetic cardiovascular autonomic neuropathy is associated with an increased risk of mortality. This study aimed to evaluate the association between left ventricular diastolic dysfunction and cardiovascular autonomic neuropathy, both diagnosed according to the current guidelines.Methods We evaluated 145 patients referred for an elective coronary angiography, 52 of whom had Type 2 diabetes and 48 had impaired glucose tolerance, while 45 subjects had normal glucose tolerance. Cardiovascular autonomic neuropathy was diagnosed using autonomic function tests, while left ventricular diastolic dysfunction was verified by tissue Doppler imaging echocardiography.Results Cardiovascular autonomic neuropathy was diagnosed in 15 (28.8%) patients with Type 2 diabetes and in six (12.5%) individuals with impaired glucose tolerance. The rates of left ventricular diastolic dysfunction were 81 and 33% in patients with and without cardiovascular autonomic neuropathy, respectively (P < 0.001). In the cardiovascular autonomic neuropathy group (n = 21), early diastolic relaxation velocity (Em) was significantly reduced (5.4 ± 0.9 vs. 7.3 ± 2.1 cm/s; P < 0.001) and the E/Em ratio was significantly higher (13.6 ± 4.6 vs. 10.3 ± 3.4 cm/s, P < 0.001) as compared with the group without cardiovascular autonomic neuropathy (n = 79). These findings remained significant after adjustment for age, sex, coronary artery disease, hypertension and HbA1c. A severe form of left ventricular diastolic dysfunction was observed in 33 and 15% of patients with and without cardiovascular autonomic neuropathy, respectively (P = 0.001).Conclusion Cardiovascular autonomic neuropathy is associated with a higher prevalence and a more severe form of left ventricular diastolic dysfunction in patients with diabetes or impaired glucose tolerance undergoing coronary angiography. Because both cardiovascular autonomic neuropathy and left ventricular diastolic dysfunction are associated with increased cardiovascular morbidity and mortality, screening for patients with left ventricular diastolic dysfunction and cardiovascular autonomic neuropathy with diabetes or impaired glucose tolerance may identify those at high risk.
Diastolic dysfunction is associated with a high rate of morbidity and mortality and has a high prevalence in patients with diabetes. Aim of the study was to investigate the prevalence of diastolic dysfunction in patients with newly detected glucose metabolism disorder (GMD) submitted for coronary angiography. Oral glucose tolerance test, echocardiography, and tissue Doppler imaging were performed in patients referred to coronary angiography. Prevalence of diastolic dysfunction was 97, 88, and 74% in the known diabetes, newly detected diabetes, and new diagnosed impaired glucose toleranc group, respectively. This is higher than previously reported. Severity of diastolic dysfunction was associated with higher 2-h plasma glucose levels and with new diagnosed diabetes. Screening patients with newly detected GMD for diastolic dysfunction may identify patients with double risk for cardiovascular morbidity and mortality and this group might be a target population to avoid development heart failure.
Hintergrund: Aufgrund der hohen Prävalenz des unentdeckten Diabetes mellitus (DM) bei Patienten mit koronarer Herzerkrankung (KHK) soll nach den Leitlinien der European Society of Cardiology und der European Association for the Study of Diabetes ein oraler Glukosetoleranztests (OGTT) bei allen Patienten mit KHK ohne vorbekannten DM durchgeführt werden. Ein lückenloses Diabetes-Screening mittels OGTT ist sehr zeit-, personal- und kostenaufwendig und daher in der kardiologischen Routine nur schwer umzusetzen. Um ein Diabetes-Screening praktikabler zu machen, wurde untersucht, welche klinischen Faktoren herangezogen werden könnten, um zuverlässig Personen mit bisher unbekanntem DM für eine Diabetesdiagnose mittels OGTT vorzuselektieren und damit die Anzahl der benötigten OGTTs zu reduzieren.
Differentiation between typical and atypical atrial flutter solely based upon surface ECG pattern may be limited. However, successful ablation of atrial flutter depends on the exact identification of the responsible re-entrant circuit and its critical isthmus.
Aufgrund ihres variablen Ursprungs ist eine erfolgreicheKatheterablation von ektopen atrialen Tachykardien (EAT) mittelskonventioneller Strategie oft schwierig. Keine Studiendatenexistieren in diesem Zusammenhang über den kombinierten Einsatzeines 3-dimensionalen elektroanatomischen Mappingsystems(CARTO™) mit der gekühlten Ablation, die im experimentellenVergleich zu Standardkathetern tiefere Läsionen erzielen kann.Bei 20 Patienten (15 weiblich; Durchschnittsalter 52,5 ± 15,4Jahre) wurden jeweils ein für die klinische EAT verantwortlicherFokus gemappt. Zwölf waren rechts-atrial gelegen, 8 linksseitig,3 davon innerhalb einer Pulmonalvene (PV). Aufgrund jüngsterBerichte über die mögliche Entstehung von PV-Stenosen bzw.-Verschlüssen in der ablativen Therapie von fokalemVorhofflimmern wurde bei letzteren auf eine direkte Ablation desFokus innerhalb der PV verzichtet und stattdessen einePV-Diskonnektion (1× unter Anwendung eines Lasso™-Katheters, 2×mittels zirkumferentieller Ablation) durchgeführt. In einemanderen Fall musste das Mapping aufgrund persisitierendermechanischer Terminierung der Tachykardie ohne Möglichkeit derReinduktion abgebrochen werden. Hier erfolgte im Sinusrhythmusdie Abgabe von 3 Impulsen am Ort der vor Terminierung frühestengemappten Aktivierung. Drei Wochen später kam es zu einemerneuten Auftreten der EAT. Alle verbliebenen 17 Patientenwurden erfolgreich abladiert und hatten im Follow-up keinerneutes Auftreten anhaltender Tachykardien. Diedurchschnittliche Untersuchungszeit betrug 192 ± 53,3 Minuten(rechts-atrial 161 ± 37,9 Min.; links-atrial 229,6 ± 46,2 Min.),die durchschnittliche Durchleuchtungszeit war 22,8 ± 9,7 Minuten(rechts-atrial 17,1 ± 6,2 Min.; links-atrial 29,8 ± 8,9 Min.).Es kam zu keinen Komplikationen. Zusammenfassend erwies sich derkombinierte Einsatz eines elektroanatomischen Mappings mit dergekühlten Ablation als eine sichere und durchführbare Strategiein der Behandlung komplexer EAT’s. Der Vorteil dieser Technikscheint in der Möglichkeit zur genauen Identifikation des Fokusverbunden mit der Applikation effektiver Radiofrequenz-Impulsezu liegen. Aufgrund der manchmal zeitaufwendigen „Punkt fürPunkt“-Datenaufnahme zur Erstellung eines präzisenelektroanatomischen Maps kann das Darstellen der frühestenAktivierung an der exakten anatomischen Lokalisation durchtransiente oder persistierende Terminierung der Tachykardieeingeschränkt sein.
Due to its variable origin success for ablation of ectopic atrial tachycardia (EAT) has been difficult to achieve using conventional mapping and ablation strategies. In contrast, no information in the literature is available about the use of a nonfluoroscopic, 3-dimensional electroanatomic mapping system (CARTO(TM)) combined with the cooled ablation technology creating deeper lesions in experimental studies compared to standard catheters. In 20 consecutive patients (15 female; age 52.5 +/- 15.4 years), a single focus responsible for clinical EAT has been mapped. Twelve EATs were located in the right atrium, whereas 8 foci were left sided including 3 origins within a pulmonary vein (PV). Due to the reported development of PV stenosis in the ablative treatment of focal atrial fibrillation, direct ablation applied inside the PV was avoided. instead, PV-disconnection achieved by the use of a Lasso(TM) catheter in 1 case and by circumferential ablation around the PV in 2 other patients was preferred. In 2 patients, ablation was not attempted because of an origin located directly in the area of the atrioventricular node. In another case, CARTO(TM) mapping was stopped due to persistent mechanical termination of the tachycardia with no possibility of induction. In the latter, ablation was performed in sinus rhythm at the earliest mapped site before terminating. Three weeks later another episode of EAT was noted in this patient. In the remaining 17 cases, ablation was associated with acute success and no recurrences of sustained tachycardia in all patients. Mean duration time was 192 +/- 53.3 min (right atrium 161 +/- 37.9 min; left atrium 229.6 +/- 46.2 min), and average fluoroscopic time was 22.8 +/- 9.7 min (right atrium 17.1 +/- 6.2 min; left atrium 29.8 +/- 8.9 min). There was no incidence of serious complications associated with this procedure. In conclusions, electroanatomical mapping including cooled ablation was a safe and feasible strategy in treating EATs. The benefit of this technique may imply the combination of both precise localization of the focus and effective applications of radiofrequency pulses, thereby minimizing acute failures or reablation. Due to the time consuming point by point data acquisition, the ability to generate precise maps demonstrating the earliest activation at their exact anatomical location can be limited by transient or persistent termination of the tachycardia.
Wir berichten über eine 25-jährige Patientin mit einer langen Anamnese von symptomatischen, paroxysmalen supraventrikulären Tachykardien. Mittels elektroanatomischem Aktivierungsmap konnte eine ektope atriale Tachykardie mit Fokus innerhalb der rechten oberen Pulmonalvene, ca. 3cm distal des Ostiums, identifiziert werden. Aufgrund jüngster Erfahrungen in der ablativen Therapie von fokalem Vorhofflimmern wurde auf eine direkte Ablation des Fokus innerhalb der Pulmonalvene verzichtet und stattdessen unter Anwendung eines 10 polaren Spiralkatheters eine elektrische Pulmonalvenendiskonnektion am atriovenösen Übergang durchgeführt. Die Patientin war während des gesamten Follow-up komplett beschwerdefrei.
Wir berichten über Verlauf und mögliche Therapieoptionen bei einem 46-jährigen Patienten mit Zustand nach überlebtem plötzlichen Herztod auf dem Boden rezidivierender ventrikulärer Tachyarrhythmien bei Prinzmetal-Angina. Nach Diagnosestellung erhielt der Patient einen Implantierbaren Cardioverter-Defibrillator, der im Verlauf trotz additiver medikamentöser Therapien weiterhin gehäuft Episoden detektierte und diese jeweils mittels Schockabgaben terminierte. Auch eine perkutane transluminale Koronarangioplastie mit Stentimplantation im vasospastischen Bereich der rechten Koronararterie blieb ohne anhaltenden Effekt. Schließlich wurde der Patient einer operativen Myokardrevaskularisation in Kombination mit einer Sympathektomie zugeführt. In einem Nachbeobachtungszeitraum von 6 Monaten war der Patient komplett beschwerdefrei, ventrikuläre Tachyarrhythmien wurden nicht mehr dokumentiert.
We report a 25-year-old female patient with a long history of symptomatic paroxysmal supraventricular tachycardia. Electroanatomic activation mapping demonstrated a focal tachycardia originating in the right upper pulmonary vein, 3 cm distal to the ostium. Due to the recent experiences in the management of focal atrial fibrillation with catheter ablation, direct ablation applied inside the pulmonary vein was avoided. Instead, an electrical disconnection of the pulmonary musculature from the left atrium guided by a circumferential 10-electrode mapping catheter was performed. The patient has since been asymptomatic during follow-up.
Hintergrund Die primäre PTCA entwickelt sich zum Goldstandard der Myokardinfarkttherapie. Ein Problem ist die Rezidivstenose, welche durch die Implantation von Stents bereits seit einigen Jahren zu umgehen versucht wird. Langzeiterfahrungen liegen jedoch bisher nur in wenigen Studien bis zu 12 Monaten vor. Methodik In einer prospektiven Studie wurden 717 Patienten von 1989–1999 mit akutem Myokardinfarkt konsekutiv in einer Datenbank erfasst, welche mittels primärer PTCA mit (n=130) oder ohne (n=587) Stent-Implantation behandelt wurden. Rezidivstenose und Re-Interventionsrate wurden nach 4 Monaten im Rahmen einer koronarangiographischen Kontrolle untersucht. Nach im Mittel 30 Monaten erfolgte eine telefonische bzw. schriftliche Nachbefragung. Ergebnisse Stichprobe: Alter 59,6 (SD 11,5) Jahre, 74,9% Männer/25,1% Frauen, 49,8% koronare Mehrgefäßerkrankung, 14,6% Diabetiker, 10,9% kardiogener Schock, 83% Ischämiezeit <6h. Die Residualstenose direkt nach Intervention maß in der PTCA-Gruppe 28,51%, in der Stent-Gruppe 10,75% (p<0,001). Die Rate an Frühverschlüssen war mit 5,1% versus 0% signifikant unterschiedlich. Gestorben nach 30 Tagen waren in der PTCA-Gruppe 5,1%, in der Stent-Gruppe 14,6% (p<0,001). Dies war durch den mit 2,7 vs. 15,4% signifikant höheren Anteil von Schock-Patienten in der Stent-Gruppe erklärbar. Nach Subgruppenanalyse nach kardiogenem Schock finden sich zwischen Nur-PTCA und Stent keine signifikanten Mortalitätsunterschiede mehr. In der koronarangiographischen Kontrolle sahen wir bei den Stent-Patienten 40% weniger Rezidivstenosen (38,7 vs. 23,6%; p=0,005) mit entsprechend deutlich seltenerer Notwendigkeit der Re-Intervention (p<0,001). Nach im Mittel 30,7 Monaten zeigte sich in der Stent-Gruppe bezüglich Angina pectoris und Luftnot ebenfalls ein Profit für das Stent-Kollektiv. Erneute Dilatationen waren in der PTCA-Gruppe mit 27,4 vs. 16,5% deutlich häufiger (p=0,03). Das ursprüngliche Infarktgefäß war hierbei mit 27,2 vs. 12,4% signifikant öfter betroffen. Bezüglich der Langzeitmortalität sahen wir differenziert nach Schock-Patienten zwischen Nur-PTCA und Stent-Patienten keinen signifikanten Unterschied (mit CS: 47,8 vs. 58,6%, p=ns; ohne CS: 9,5 vs. 6,4%, p=ns). Der sekundäre Endpunkt aus Langzeitmortalität, Re-Infarkt, Re-PTCA und ACB-OP wurde bei Patienten ohne kardiogenen Schock jedoch von deutlich weniger Stent-Patienten erreicht (42,1 vs. 26,6%, p=0,008). Bei Patienten mit CS zeigte sich kein signifikanter Unterschied (56,5 vs. 72,4%, p=ns). Schlussfolgerung Die koronare Stent-Implantation im Rahmen der primären PTCA des akuten Infarktes führt auch im Langzeitverlauf zu einer signifikant geringeren Rate an interventionspflichtigen Rezidivstenosen, vermag allerdings keinen Überlebensvorteil zu erbringen.
