PURPOSE:Also called acute febrile neutrophilic dermatosis, Sweet's syndrome is an inflammatory disorder with a prominent cutaneous expression. It is characterized by a variety of manifestations, clinical and histological findings. The objective of this study was to describe their clinical, pathological and therapeutic characteristics. METHODS:We report on a series of 47 patients who presented a Sweet's syndrome, collected in our institution in Tunis between 1997 and 2011. RESULTS:The patient population consisted of 11 men and 36 women. The mean age was 47 years with extreme ranging from 28 to 74 years. An associated disorder was observed in ten patients: inflammatory disease (three cases), inflammatory bowel disease (two cases), tuberculosis (three cases) and diabetes (three cases). One case of pregnancy was observed. Cutaneous lesions consisted of erythematous plaques or nodules. Lesions were located mainly on the upper or lower extremities. All biopsy specimens demonstrated a dermal infiltrate composed predominantly of neutrophils. Fibrinoid necrosis and intramural inflammation were observed in eight cases. CONCLUSION:The skin disorder can precede, follow, or appear concurrent with the diagnosis of an associated disease which requires careful surveillance.
Le syndrome métabolique (SM) est un facteur de risque cardiovasculaire et de survenu de diabète. En plus de la morbidité cardiovasculaire, la survenue d'une maladie lithiasique semble également être probable. Il est donc important de voir l'association entre l'uricémie (AU) et le syndrome métabolique et ses composantes. L'étude a porté sur 2 712 sujets (1 228 hommes et 1 434 femmes) âgés de 35 à 70 ans, résidant dans la région du Grand Tunis. SM a été défini selon les critères du « National Cholesterol Education Program Adult Panel III » NCEP ATPIII. Le SM est présent chez 31,2 % de la population, cette prévalence est de 23,9 % chez les hommes et de 37,3 % chez les femmes. La concentration de AU est significativement plus élevée chez les patients ayant le SM aussi bien chez les hommes que chez les femmes (p < 0,001). L'uricémie est significativement plus élevée chez les patients ayant une hypertriglycéridémie, un HDL-cholesterol bas, un taux élevé de la tension artérielle et une obésité viscérale aussi bien chez les hommes que chez les femmes (p < 0,001). Le taux de AU est significativement plus bas chez les patients ayant une hyperglycémie indépendamment du sexe (p < 0,001). Le taux de AU est positivement corrélé avec les triglycérides, La pression artérielle systolique et diastolique et l'obésité viscérale dans les deux sexes (p < 0,001). Une corrélation négative statistiquement significative est retrouvée avec le taux des HDL-c aussi bien chez les hommes (r = −0,081, p < 0,001) que chez les femmes (r = −0,133, p < 0,001) et avec la glycémie uniquement chez les hommes (r = −0,16, p < 0,001). Notre étude montre une association entre l'uricémie et le syndrome métabolique et ses différentes composantes. Il est donc important pour les praticiens de reconnaître le SM en consultation afin d'identifier ces patients qui doivent bénéficier d'une suivie médical régulier.
La sclérodermie est une maladie auto-immune dont la physiopathologie reste mal élucidée et de mécanisme probablement plurifactoriel. L’observation de cas de sclérodermies chez des sujets professionnellement exposés aux solvants organiques fait suggérer le rôle probable des facteurs environnementaux dans la genèse de cette maladie. L’induction de la sclérodermie par l’exposition aux solvants serait-elle en faveur d’un effet immunotoxique de ces derniers ? On se propose alors de rapporter l’observation d’un cas de sclérodermie chez une ouvrière dans l’industrie d’habillage des volants et exposée à de nombreux solvants organiques.
Peret et al. [1] showed a phase shift in liver glycogen circadian rhythm induced by timed food intake in the rat. Furthermore, our previous results in rats showed a significant decrease in pancreas fresh weight and its insulin content as well as in basal insulin secretion after two months of nocturnal fasting [2]. These observations indicate that the timing of food intake can influence endogenous functions. A review of specific characteristics of Ramadan fasting indicated that it could serve as an excellent research model to investigate the role of daily meal timing in metabolic endogenous bioperiodicity.
The effect of Ramadan intermittent fasting (RIF) was studied on a battery of blood lipid markers in 15 elite judo athletes during a period when they were maintaining their training load without competing. Nine-to-twelve hours postprandial serum lipid and lipoproteins were measured on five occasions: before, three times during Ramadan, and three weeks post-Ramadan. Dietary data were collected using a 24-hour recall method for three days before, during and after the Ramadan month. Mean energy intake (12.9 MJ/d) remained similar throughout the study as did the macronutrient constituents of the diet. Mean body mass was slightly reduced (2 %; p < 0.01) by the end of Ramadan due mainly to a 0.65 +/- 0.68 kg decrease in body fat (p < 0.05). The RIF produced significant changes in some of the blood lipid levels: both HDL-C and LDL-C increased by 0.12 (p < 0.01) and 0.20 mmol.L-1 (p<0.05), respectively. During Ramadan, mean non-esterified fatty acid (NEFA) levels decreased from 0.73 to 0.28mmol.l(-1) (p<0.01) during the first week, then increased (p<0.05) to 1.22mmol.l(-1) over the middle of Ramadan and recovered to pre-Ramadan concentrations for the end and the post-Ramadan periods. Apolipoprotein A1 (Apo-A1) levels were significantly elevated at the end (p < 0.01) and the post-Ramadan periods (p < 0.05). Three weeks after Ramadan, blood levels of glucose, NEFA, Apo-A1, and Apo-13 did not return to the values observed before Ramadan. In conclusion, the present results show that the combination of the change in diet pattern during Ramadan, along with intense exercise training, induced a significant decrease in body mass associated with a reduction in body fat and changes in some of the serum lipids and lipoproteins. Nevertheless, all the measured serum parameters remained within normal levels for young and active individuals. The volunteers, in this study, were able to maintain a constant training load during RIF.
La tyrosinemie de type I est un desordre metabolique grave, ce desordre est le resultat d'un deficit en fumaryl aceto-acetone, etape terminale du metabolisme de la tyrosine. Il en resulte une maladie progressive du foie et une deficience tubulaire renale accompagnee d'un rachitisme hypophosphatemique. La forme aigue de la maladie est revelee des le premier trimestre de la vie par un tableau d'insuffisance hepato-cellulaire grave. Jusqu'au milieu des annees 90, l'evolution de cette forme etait presque constamment mortelle, a defaut d'une transplantation hepatique en urgence. La decouverte du (2,2 nitro; 4 trifluoromethyl benzoyl; 1,3 cyclohexane dione) ou NTBC a transforme le pronostic de cette affection avec possibilite de survie prolongee avec un mode de vie normal. Les deux observations que nous rapportons illustrent bien l'importance d'une prescription precoce du NTBC dans la prise en charge de la forme aigue de la tyrosinemie hereditaire de type I.
Le mois du Ramadan est caracterise par un jeune diurne avec des prises alimentaires strictement nocturnes. Or, plusieurs auteurs ont montre que les fonctions endogenes sont non constantes au cours des 24 heures et possedent chacune un rythme circadien particulier. Nos travaux anterieurs ont montre que le jeune du Ramadan induit des changements dans les rythmes de certains parametres. Le but de la presente etude est de mettre en evidence les eventuels changements qui touchent le rythme circadien de l'excretion renale du sodium (Na+) et du potassium (K+). Ces changements sont evalues en fonction des apports quotidiens, avant et au cours du Ramadan. Nos resultats montrent que les evolutions de l'excretion urinaire du Na+ et K+ ont des allures sinusoidales, faiblement influencees par le jeune du Ramadan. Cependant, nous avons constate que la moyenne des six valeurs du Na+, mesurees au cours des 24h, aussi bien que l'amplitude de leur pic, sont tres diminuees au cours du Ramadan ainsi que les apports quotidiens du Na+. De ce fait, le rapport, apport/elimination du Na+ n'a change que tres legerement au cours du jeune. Nos resultats ont montre, en outre, que le pic de l'excretion du K+ est toujours diurne, avec une amplitude nettement augmentee au cours du Ramadan. Cependant, la moyenne quotidienne de cette excretion est tres stable, puisque aucune difference n'a ete notee au cours du jeune. Quant a l'apport alimentaire du K+, il subit un accroissement au cours du Ramadan, avec un rapport apport/elimination augmente.
