Introduction et objectifs Le stenting endoveineux est un traitement permettant de prendre en charge les obstructions veineuses proximales chroniques et aigus avec pour objectif de limiter le syndrome post-thrombotique (SPT). Actuellement, l’absence de recommandations de protocoles standardisés pour le suivi échographique justifie une étude approfondie sur les indicateurs associés à la perméabilité des stents pour détecter précocement les complications. Méthodologie Une étude rétrospective a été menée incluant des patients avec un stenting veineux proximal post-thrombose veineuse profonde. Les paramètres Doppler mesurés incluaient la vélocité de pic, le débit veineux fémoral commun, le ratio de débit entre les veines stentées et non stentées controlatérales, ainsi que la modulation respiratoire des flux veineux. Les facteurs associés aux occlusions et/ou sténose≥50 % de stent et la présence d’un SPT ont été évalués. Résultats Cent soixante-deux patients ont été inclus dans l’étude, avec plus de 1100 écho-Dopplers évalués. Parmi eux, 30 patients ont présenté une sténose sévère du stent (≥50 %), avec une incidence significativement plus élevée chez les patients plus jeunes (p=0,002). La sténose sévère était corrélée à un risque accru d’embolie pulmonaire (p<0,001) et de complications majeures. En comparaison, 29 patients plus âgés ont développé un SPT, indiquant une progression du risque avec l’âge (p=0,02). La modulation respiratoire et le ratio de débit se sont révélé être des marqueurs associés à la perméabilité des stents, un ratio de débit réduit traduisant des asymétries de flux entre les veines stentées et non stentées, était associé à un risque important de sténoses. Discussion Ces résultats soulignent l’importance de la modulation respiratoire et du ratio de débit comme indicateurs spécifiques de surveillance pour les stents veineux. Un suivi rigoureux de ces paramètres s’avère particulièrement crucial pour les patients jeunes à risque de sténose sévère et pour les patients plus âgés, plus susceptibles de développer un SPT. Conclusion L’échographie Doppler, via l’analyse de la modulation respiratoire et du ratio de débit, pourrait optimiser la surveillance post-stenting, permettant une détection précoce des complications et améliorant les résultats cliniques des patients.
BACKGROUND:Giant cell arteritis (GCA) is the most prevalent systemic vasculitis in people older than 50 years. Any delay in diagnosis impairs patients' quality of life and can lead to permanent damage, particularly vision loss. OBJECTIVE:To evaluate a diagnostic strategy for GCA using color Doppler ultrasound of the temporal artery as a first-line diagnostic test, temporal artery biopsy (TAB) as a secondary test, and physician expertise as the reference method. DESIGN:Prospective multicenter study with a 2-year follow-up. (ClinicalTrials.gov: NCT02703922). SETTING:Patients were referred by their general practitioner or ophthalmologist to a physician with extensive experience in GCA diagnosis and management in one of the participating centers: 4 general and 2 university hospitals. PATIENTS:165 patients with high clinical suspicion of GCA, aged 79 years (IQR, 73 to 85 years). INTERVENTION:The diagnostic procedure was ultrasound, performed less than 7 days after initiation of corticosteroid therapy. Only ultrasound-negative patients underwent TAB. MEASUREMENTS:Bilateral temporal halo signs seen on ultrasound were considered positive. Ultrasound and TAB results were compared with physician-diagnosed GCA based on clinical findings and other imaging. RESULTS:Diagnosis of GCA was confirmed in 44%, 17%, and 21% of patients by ultrasound, TAB, and clinical expertise and/or other imaging tests, respectively. Their diagnosis remained unchanged at 1 month, and 2 years for those with available follow-up data. An alternative diagnosis was made in 18% of patients. The proportion of ultrasound-positive patients among patients with a clinical GCA diagnosis was 54% (95% CI, 45% to 62%). LIMITATION:Small sample size, no blinding of ultrasound and TAB results, lack of an objective gold-standard comparator, and single diagnostic strategy. CONCLUSION:By using ultrasound of the temporal arteries as a first-line diagnostic tool in patients with high clinical suspicion of GCA, further diagnostic tests for patients with positive ultrasound were avoided. Background: The frequency of upper extremities vein thrombosis (UEVT) is rising with the increasing use of endovenous devices. These thromboses are particularly common among hospitalized patients. The epidemiology and risk factors for UEVT are poorly understood in a hospitalized population.Objective: To assess the prevalence of UEVT in hospitalized patients and study thrombosis risk factors according to their location. Methods: Prospective evaluation of patients hospitalized in a university hospital with clinical and Doppler ultrasound (DUS) assessment of the upper extremities.Results: Of the 400 patients included, 91(22.8 %) had UEVT including 8 (8.8 %) proximal thrombosis, 32 (35.2 %) arm venous thrombosis and 51 (56.0 %) forearm venous thrombosis; 7 (7.7 %) patients with UEVT had a concomitant symptomatic pulmonary embolism. In this population of hospitalized patients, 40 (10 %) had proximal or arm thrombosis and 51 (12.8 %) forearm thrombosis. All patients with UEVT had a venous catheter and 63 (69.2 %) of UEVT patients had therapeutic or prophylactic anticoagulation at the time of DUS evaluation. In multivariate analysis, peripheral intravenous catheter (PIVC) OR 3.71 [1.90; 7.91] (p < 0.001); MID line OR 3.58 [1.46; 8.91] (p = 0.005) and infection disease OR 2.21 [1.26; 4386] (p = 0.005) were associated with UEVT. Central venous catheter OR 66.24 [12.64; 587.03] was associated with proximal UEVT and MID line OR 12.61 [4.64; 35.77] (p < 0.001) with arm UEVT. Forearm UEVT were associated with PIVCOR 10.71[3.14; 67.37] (p = 0.001); infectious disease OR 2.87 [1.48; 5.60] (p = 0.002), iron infusion OR 3.11 [1.23; 7.47] and hospitalization for postpartum OR 4.37 [0.97; 18.06] (p = 0.04).Conclusion: One in 5 hospitalized patients suffers from UEVT. Proximal venous thrombosis is uncommon. The presence of a catheter and hospitalization for infection are the factors most frequently associated with UEVT.
Background: Data on complications after upper extremity vein thrombosis (UEVT) are limited and heterogeneous. Methods: The aim of the present study was to evaluate the pooled proportions of venous thromboembolism (VTE) recurrence, bleeding, and post-thrombotic syndrome (PTS) in patients with UEVT. A systematic literature review was conducted of PubMed, Embase, and the Cochrane Library databases from January 2000 to April 2023 in accordance with the PRISMA (preferred reporting items for systematic reviews and meta-analyses) guidelines. All studies included patients with UEVT and were published in English. Meta-analyses of VTE recurrence, bleeding, and of PTS after UEVT were performed to compute pooled estimates and associated 95% confidence intervals (CIs). Subgroup analyses of cancer -associated UEVT and catheter-associated venous thrombosis were conducted. Patients with Paget-Schroetter syndrome or effort thrombosis were excluded. Results: A total of 55 studies with 15,694 patients were included. The pooled proportions for VTE recurrence, major bleeding, and PTS were 4.8% (95% CI, 3.8%-6.2%), 3.0% (95% CI, 2.2%-4.0%), and 23.8% (95% CI, 17.0%-32.3%), respectively. The pooled proportion of VTE recurrence was 2.7% (95% CI, 1.6%-4.6%) for patients treated with direct oral anticoagulants (DOACs), 1.7% (95% CI, 0.8%-3.7%) for patients treated with low-molecular-weight heparin (LMWH), and 4.4% (95% CI, 1.5%-11.8%) for vitamin K antagonists (VKAs; P = .36). The pooled proportion was 6.3% (95% CI, 4.3%-9.1%) for cancer patients compared with 3.1% (95% CI, 2.1%-4.6%) for patients without cancer (P = .01). The pooled proportion of major bleeding for patients treated with DOACs, LMWH, and VKAs, was 2.1% (95% CI, 0.9%-5.1%), 3.2% (95% CI, 1.4%-7.2%), and 3.4% (95% CI, 1.4%-8.4%), respectively (P = .72). The pooled proportion of PTS for patients treated with DOACs, LMWH, and VKAs was 11.8% (95% CI, 6.5%-20.6%), 27.9% (95% CI, 20.9%-36.2%), and 24.5% (95% CI, 17.6%-33.1%), respectively (P = .02). Conclusions: The results from this study suggest that UEVT is associated with significant rates of PTS and VTE recurrence. Treatment with DOACs might be associated with lower PTS rates than treatment with other anticoagulants. (J Vasc Surg Venous Lymphat Disord 2024;12:101688.)
L’artérite à cellules géantes (ACG) est la vascularite primitive la plus fréquente. Certaines études ont démontré que l’échographie avec prise de contraste (CEUS) améliore la visualisation vasculaire. Le but de cette étude était d’analyser la concordance inter- et intra-observateur pour l’échographie avec prise de contraste pour le suivi de l’activité de la maladie dans l’ACG dans la paroi des artères axillaires et des artères carotides communes. Les patients atteints d’ACG ont eu une CEUS des artères axillaires et de la carotide commune. Ces images ont été évaluées par 7 médecins de médecine vasculaire de 4 hôpitaux qui étaient expérimentés dans l’échographie Doppler des patients atteints d’ACG. Deux semaines plus tard, les observateurs ont à nouveau évalué les mêmes images. Les patients atteints d’ACG ont été recrutés entre décembre 2019 et février 2021. Une analyse du contraste des images échographiques avec une gradation en trois classes (grade 0, 1 et 2) a été réalisée. Une nouvelle analyse en 2 classes, prise de contraste de paroi positive ou négative, a ensuite été réalisée. Soixante segments artériels ont été évalués chez 30 patients. Pour l’échelle à 3 classes, la concordance intra-juge était substantielle : κ 0,70 ; la concordance inter-juges était passable : κ de 0,22 à 0,27 ; 34 vidéos avaient une épaisseur de paroi inférieure à 2 mm et 26 vidéos avaient une épaisseur de paroi supérieure à 2 mm. Pour les parois d’une épaisseur inférieure à 2 mm : la concordance intra-juge était substantielle : κ 0,69 ; la concordance inter-juge était passable : κ 0,35. Pour les parois d’une épaisseur de 2 mm ou plus : la concordance intra-juge était modérée : κ 0,53 ; la concordance inter-juge était passable : κ 0,25. Pour l’analyse de la prise de contraste pariétale en 2 classes : la concordance inter-juges était de passable à modérée : κ de 0,35 à 0,41 ; et pour les parois d’une épaisseur de 2 mm ou plus : la concordance inter-juges était de passable à substantielle : κ de 0,22 à 0,63. L’analyse visuelle de la prise de contraste dans la paroi des artères axillaires et carotides communes a montré une bonne concordance intra-juge chez les patients atteints d’ACG. La concordance inter-juges était faible, en particulier lorsque le contraste était analysé en 3 classes. La concordance inter-juges pour l’analyse en 2 classes était également faible. La concordance inter-juges était plus élevée dans l’analyse en 2 classes pour les parois de 2 mm d’épaisseur ou plus.
Background:Endovenous interventional procedures can be used in addition to therapeutic anticoagulation to treat deep vein thrombosis in selected patients with proximal vein involvement (vena cava, iliac and/or common femoral). The aim of this study was to compare venous patency and the post-thrombotic syndrome (PTS) in patients treated with pharmaco-mechanical catheter-directed thrombolysis (PMT) versus recanalization-stenting for PTS after a proximal lower limb deep vein thrombosis.Methods:Between January 2014 and December 2020, this retrospective and monocentric study included patients with very symptomatic acute iliofemoral deep vein thrombosis treated with PMT within 21 days after diagnosis (PMT group) and patients with PTS caused by chronic venous obstruction treated with recanalization and stenting (CRS group).Results:A total of 116 patients were included (26 PMT, 90 CRS). The rate of primary patency was 81.8% (18/22 patients) in the PMT group and 78.4% (69/88) in the CRS group (P>0.99). The rate of venous patency at the last follow-up was 76.9% (20/26) in the PMT group and 82.2% (74/90) in the CRS group (P=0.57). The median number of stents was 2 (range, 0-5) in the PMT group and 3 (range, 0-7) in the CRS group (P<0.001). The median stent length was 150 mm (range, 60-390 mm) and 280 mm (range, 120-820 mm), respectively (P<0.001). The median last Villalta score was 2 (range, 0-10) in the PMT group and 2 (range, 0-21) in the CRS group (P=0.55). The rate of venous claudication at the last follow-up was 19.0% (4/21) in the PMT group and 12.0% (10/83) in the CRS group (P=0.47).Conclusions:In this study, there was no difference in venous patency and in the rate and severity of PTS between the PMT and CRS groups. The number of stent and their length were significantly lower in the PMT group compared with the CRS group.
L’Artérite de Takayasu (AT) est une vascularite des gros vaisseaux touchant principalement les femmes de 25 à 40 ans et est plus fréquente en Asie. Ses facteurs pronostiques sont, à ce jour, mal connus. L’atteinte aortique est une des manifestations emblématiques de la maladie et touche entre 65 et 95 % des patients de la littérature. Cependant, l’aortite comme entité propre n’a jamais été étudiée pour sa valeur pronostique. Les patients atteints d’AT répondant aux critères ACR ou de Sharma suivis au CHU de Nantes ont été inclus pour analyser leurs caractéristiques initiales et leur évolution en fonction de la présence ou non d’une aortite au diagnostic. L’aortite était définie, en tomodensitométrie ou en imagerie par résonance magnétique, par un épaississement pariétal supérieur ou égal à 2,2 mm. Les patients ont été comparés selon leurs caractéristiques cliniques et biologiques au diagnostic ainsi que le recours aux traitements. Les rechutes et les évènements vasculaires au cours du suivi ont fait l’objet d’une analyse de survie censurée à 60 mois, selon un modèle de Cox (test du log-rank). Sur les 43 patients inclus, 23 étaient atteints d’aortite (groupe A+, 53 %) contre 20 sans aortite (groupe A-). Les patients étaient caucasiens en majorité86 %). Les AT de type I (47 %) et de type V (37 %) de Tokyo étaient les plus fréquentes. L’atteinte aortique thoracique (47 %) était légèrement plus fréquente que l’atteinte abdominale (33 %) Le groupe A+ présentait une CRP plus élevée (84,0 vs 34,2 mg/L, p = 0,01), plus de signes ostéoarticulaires (30 % vs 5 %, p = 0,03), de péricardites (26 % vs 0 %, p = 0,01), de douleurs thoraciques (22 % vs 0 %, p = 0,03) ou abdomino-lombaires (43 % vs 5 %, p = 0,004) au diagnostic. Les patients du groupe A- avaient plus souvent été diagnostiqués avant 2008 (p = 0,008). L’atteinte vasculaire des troncs supra aortiques était comparable dans les 2 groupes (p = 0,09). Les patients avec aortite avaient plus d’atteintes iliaques (39 % vs 10 %, p = 0,03) et artérielles viscérales (64 % vs 25 %, p = 0,02). L’atteinte du tronc artériel brachiocéphalique concernait uniquement les patients du groupe aortite (61 % vs 0 %, p = 0,0001). La typologie des lésions était quant à elle similaire dans les 2 groupes (épaississements : 100 % dans les 2 groupes, p > 0,99 ; sténoses : 78 % vs 85 %, p = 0,57 ; occlusions : 26 % vs 40 %, p = 0,33 ; dilatations : 26 % vs 5 %, p = 0,06). La maladie thromboembolique veineuse concernait 26 % des patients du groupe A+ contre 5 % dans le groupe A-, différence non significative (p = 0,06). L’aortite était associée à un recours plus fréquent à la corticothérapie (p = 0,0001) et aux immunosuppresseurs (p = 0,0001). Le reste des traitements était comparable. Les patients du groupe A+ présentaient significativement plus de rechutes au cours du suivi (p < 0,0001). Le Hazard Ratio pour la survenue d’une rechute dans le groupe A+ était de 20,2 [7,4–54,4]. Il n’y avait pas de différence sur la survenue d’évènements vasculaires (p = 0,8). Dans l’AT, peu de facteurs pronostiques sont connus actuellement. Le sexe masculin, l’élévation de la CRP et la présence de carotidodynies avaient associés à la survenue d’une rechute dans une cohorte française de 318 patients. L’atteinte de l’aorte thoracique, la présence d’un rétinopathie et l’évolution progressive au diagnostic étaient associés aux complications vasculaires. Notre étude est, quant à elle, la première identifiant un caractère pronostique péjoratif de l’aortite au diagnostic en termes de rechutes dans l’AT. Si ces résultats sont confirmés sur d’autres cohortes, l’introduction d’un immunosupresseur dès le diagnostic pourrait se discuter chez ces patients. Dans cette étude, l’aortite au diagnostic de l’AT était associée à la survenue de rechutes, au recours à la corticothérapie et aux traitements par immunosuppresseurs, mais pas à la survenue d’évènements vasculaires au cours du suivi.
Objective The aim of this study was to analyze inter- and intra-observer agreement for contrast-enhanced ultrasonography (CEUS) for monitoring disease activity in Giant Cell Arteritis (GCA) in the wall of axillary arteries, and common carotid arteries. Methods Giant cell arteritis patients have CEUS of axillary arteries and common carotid. These images were rated by seven vascular medicine physicians from four hospitals who were experienced in duplex ultrasonography of GCA patients. Two weeks later, observers again rated the same images. GCA patients were recruited in from December 2019 to February 2021. An analysis of the contrast of the ultrasound images with a gradation in three classes (grade 0, 1, and 2) was performed. Grade 0 corresponds to no contrast, grade 1 to moderate wall contrast and grade 2 to intense contrast. A new analysis in 2 classes: positive or negative wall contrast; was then performed on new series of images. Results Sixty arterial segments were evaluated in 30 patients. For the three-class scale, intra-rater agreement was substantial: κ 0.70; inter-rater agreement was fair: κ from 0.22 to 0.27. Thirty-four videos had a wall thickness of less than 2 mm and 26 videos had a wall thickness greater than 2 mm. For walls with a thickness lower than 2 mm: intra-rater agreement was substantial: κ 0.69; inter-rater agreement was fair: κ 0.35. For walls with a thickness of 2 mm or more: intra-rater agreement was substantial: κ 0.53; inter-rater agreement was fair: κ 0.25. For analysis of parietal contrast uptake in two classes: inter-rater agreement was fair to moderate: κ from 0.35 to 0.41; and for walls with a thickness of 2 mm or more: inter-rater agreement was fair to substantial κ from 0.22 to 0.63. Conclusion The visual analysis of contrast uptake in the wall of the axillary and common carotid arteries showed good intra-rater agreement in GCA patients. The inter-rater agreement was low, especially when contrast was analyzed in three classes. The inter-rater agreement for the analysis in two classes was also low. The inter-rater agreement was higher in two-class analysis for walls of 2 mm thickness or more.
The aim of this study was to identify clinical factors associated with exercise-induced vasculitis (EIV). This study included EIV cases and controls matched for age. Cases included were all members of a hiking club and participated in extended hiking trips. Exercise-induced vasculitis was diagnosed based on clinical signs occurring only after prolonged walks. Chronic venous disease was defined using the Clinical Etiological Anatomical Pathophysiologic classification. This study included 162 hikers: 32 EIV cases and 130 matched controls. Mean age at EIV diagnosis was 47.1 years and 24 (75.0%) of EIV cases were women. Chronic venous disease was present in 19 (57.6%) of EIV cases vs 39 (30.0%) in controls (P = .001); those with EIV had significantly more saphenous vein insufficiency and C3 venous insufficiency than controls, 85.0 vs 52.6% and 8 (25.0%) vs 13 (10.0%) (P = .02), respectively. For EIV cases, mean walking distance per hike was significantly higher than for controls (P = .002). Exercise-induced vasculitis symptoms were typical with rash and/or purpura on the leg in warm conditions. Lesions spontaneously disappear in <10 days. In this study, EIV cases had more chronic venous disease and longer mean walking distances than controls.
Objective: Post-thrombotic syndrome (PTS) is one of the main complications that occurs after venous thrombosis. There are few data on the proportion of patients that will develop upper extremity PTS (UE-PTS) after upper extremity venous thrombosis (UEVT). The main objective of the study was to assess the prevalence of PTS in a UEVT cohort and to identify predictive factors of UE-PTS. Methods: This study included patients with a history of proximal or arm UEVT, diagnosed on duplex ultrasound examination, between January 1, 2015, and December 31, 2017, in a university hospital. After UEVT, each patient was evaluated by a prospective standardized recording of clinical manifestations and duplex ultrasound examination in case of upper limb symptoms. UE-PTS was defined as a modified Villalta score of 4 or higher. Results: Ninety-two patients were included; 68 (73.9%) had deep vein thrombosis (DVT) and 24 (19.2%) arm superficial vein thrombosis. Thirteen patients had PTS (14.1%), 12 (17.6%) in the DVT group and 1 (4.2%) in the superficial vein thrombosis group. There was a history of DVT in 92.3% of the cases of PTS. PTS was more frequent in patients with strokes with limb movement reduction (P = .01). On multivariate Cox analysis, a history of stroke (hazard ratio, 5.4; 95% confidence interval, 1.46-20.22; P= .01) was predictive of UE-PTS. Conclusions: UE-PTS occurred in 14.1% of cases after UEVT. Stroke with a decrease in limb movement was a predictor of developing PTS. Diagnostic criteria should be established for UE-PTS and prospective studies are needed to improve the description and management of UE-PTS.
Background Upper extremity venous thrombosis (UEVT) represents about 10% of venous thrombo-embolic disease. This is mainly explained by the increasing use of central venous line, for oncologic or nutritional care. The factors associated with venous recanalization are not known. Objective The aim of this study was to investigate prognosis factor associated with venous recanalization after UEVT. Methods This study included patients with UEVT diagnosed with duplex ultra-sonography (DUS) from January 2015 to December 2017 with DUS evaluations during follow-up. A multivariate Cox proportional-hazards-model analysis was performed to identify predictive factors of UEVT complete recanalization. Results This study included 494 UEVT, 304 proximal UEVT and 190 distal UEVT. The median age was 58 years, 39.5% were women. Clinical context was: hematological malignancy (40.7%), solid cancer (14.2%), infectious or inflammatory context (49.9%) and presence of venous catheters or pacemaker leads in 86.4%. The rate of recanalization without sequelae of UEVT was 38%. For all UEVT, in multivariate analysis, factors associated with complete vein recanalization were: thrombosis associated with central venous catheter (CVC) (HR:2.40, [1.45;3.95], p<0.001), UEVT limited to a venous segment (HR:1.94, [1.26;3.00], p = 0.003), occlusive thrombosis (HR:0.48 [0.34;0.67], p<0.0001), the presence of a PICC Line (HR:2.29, [1.48;3.52], p<0.001), a thrombosis of deep and distal topography (HR:1.70, [1.10;2.63], p = 0.02) or superficial thrombosis of the forearm (HR:2.79, [1.52;5.12], p<0.001). For deep and proximal UEVT, non-occlusive UEVT (HR:2.23, [1.49;3.33], p<0.0001), thrombosis associated with CVC (HR:1.58, [1.01;2.47], p = 0.04) and infectious or inflammatory context (HR:1.63, [1.10;2.41], p = 0.01) were factors associated with complete vein recanalization. Conclusion In this study, factors associated with UEVT recanalization were UEVT limited to a venous segment, thrombosis associated with CVC, a thrombosis of deep and distal thrombosis topography and superficial thrombosis of the forearm. Occlusive thrombosis was associated with the absence of UEVT recanalization.
Background: Thromboangiitis obliterans (TAO) is a distal non atherosclerotic thrombotic vasculitis affecting tobacco smokers. The role of cannabis co-exposure remains controversial. The study aims to assess how cannabis consumption influences clinical presentation and outcome of TAO in tobacco smokers. Patients and methods: TAO patients, according to Papa's criteria, were included in a retrospective bicentric study between the 1st January 2003 and the 1st march 2020. Clinical characteristics, arterial involvement at TAO diagnosis, vascular event and amputations during follow-up were analyzed according to cannabis consumption. Results: Seventy-three patients with TAO patients were included. Forty-five patients were in Tobacco group (T) and 28 in Tobacco and cannabis group (T&C). Tobacco exposure was less important in T&C group than in T group (19.4±11.3 vs 31.6±16.6 pack-years) (p=0.005) and patients in T&C group were younger at TAO diagnosis than in T group (p=0.008). Patients in T&C group presented more claudication (33.3% vs 8.9%, p=0.01) and less upper limbs resting ischemia (25.9% vs 51.1%, p=0.04) than patients in the T group. No differences were found between groups with regard to arterial distribution. Amputation rate for patients who had at least one major or minor amputation did not differ between T and T&C group (25% vs 14.8%, p=0.38). Conclusions: Cannabis consumption was associated with a younger age of TAO onset. However, it does not affect amputation-free survival, Tobacco exposure is less important in T&C patients; data of this bicentric study suggest that cannabis could be a cofactor of tobacco which accelerates TAO onset.
Abstract Upper extremity digital ischaemia (UEDI) is a rare heterogeneous condition whose frequency is 40 times less than that of toe ischaemia. Using a large cohort, the aim of this study was to evaluate aetiologies, prognosis and midterm clinical outcomes of UEDI. All patients with UEDI with or without cutaneous necrosis in a university hospital setting between January 2000 to December 2016 were included. Aetiologies, recurrence of UEDI, digital amputation and survival were analyzed retrospectively. Three hundred twenty three patients were included. UEDI due to cardio-embolic disease (DICE) was the highest occurring aetiology with 59 patients (18.3%), followed by DI due to Systemic Sclerosis (SSc) (16.1%), idiopathic causes (11.7%), Thromboangiitis obliterans (TAO) (9.3%), iatrogenic causes (9.3%), and cancer (6.2%). DICE patients tended to be older and featured more cases with arterial hypertension whereas TAO patients smoked more tobacco and cannabis. During follow-up, recurrences were significantly more frequent in SSc than in all other tested groups (P < .0001 vs idiopathic and DICE, P = .003 vs TAO) and among TAO patients when compared to DICE patients (P = .005). The cumulated rate of digital amputation was higher in the SSc group (n = 18) (P = .02) and the TAO group (n = 7) (P = .03) than in DICE (n = 2). This retrospective study suggests that main aetiologies of UEDI are DICE, SSc and idiopathic. This study highlights higher frequency of iatrogenic UEDI than previous studies. UEDI associated with SSc has a poor local prognosis (amputations and recurrences) and DICE a poor survival. UEDI with SSc and TAO are frequently recurrent.
L'artérite à cellules géantes (ACG) est la vascularite systémique la plus fréquente. Ces dernières années, l'échographie Doppler couleur des artères temporales (CDU) s'est révélée être un outil de diagnostic non invasif, mais sa place dans l'algorithme de diagnostic reste à définir. Une limitation de la CDU est la reproductibilité inter-opérateurs. L'analyse d'images sous un angle différent est désormais possible avec le développement d'algorithmes d'intelligence artificielle. Nous proposons d'explorer cette technologie pour la détection d'un signe de halo sur les images CDU. Nous avons réalisé une étude rétrospective multicentrique auprès de 137 patients avec une suspicion clinique d'ACG entre janvier 2017 et avril 2019. Les images CDU (n = 1311) ont été analysées avec le logiciel VIA. Trois ensembles d'images (images de formation, de validation et de test) ont été créés et analysés avec une technique de segmentation sémantique à l'aide d'un réseau neuronal convolutionnel U-Net. Sur les 137 patients inclus, 71 patients (51,8 %) avaient un diagnostic d'ACG finalement retenu, 6 patients ACG avaient un thrombus d'artère temporale, 43 présentaient un signe du halo soit 60,6 % des patients à ACG. Un total de 1311 images échographiques d'artères ont été analysées. Un groupe d'images standardisées d'artères temporales (groupe 1, 502 images) a été constitué avec une même profondeur, une même sonde echographique, les mêmes paramètres dopplers ; et un groupe d'images non standardisées d'artères temporales, occipitales, faciales (groupe 2, 809 images) ont été analysées au sein de 3 jeux d'images (Entraînement, Validation et Test). L'identification de signe échographique d'ACG au moyen réseau neuronal convolutionnel était très performante. L'aire sous la courbe (AUC) pour le diagnostic positif était respectivement de 0,974 pour le groupe 1 et de 0,901 pour le groupe 2 sur l'ensemble de validation et de test. Un seuil de positivité de l'image a été déterminé en définissant une valeur dépendante du nombre de pixel discriminant dans l'image permettant d'identifier les images positives. Avec ce seuil, une spécificité de 95 % et une sensibilité de 60 % ont été obtenues pour l'ensemble de test. L'étude des fausses interprétations a montré que les modalités d'acquisition et la présence de thrombus étaient des facteurs de confusion pour l'algorithme. Cette étude est la première à proposer une méthode d'analyse automatisée des images d'échodoppler au cours de l'ACG. Cet outil d'analyse d'images automatisé pour le diagnostic d'ACG doit faire l'objet d'une validation externe, et pourrait être utilisé comme aide au diagnostic ou pour la formation des médecins pour le diagnostic échographique de l'ACG.
Objective. Giant cell arteritis (GCA) is the most common systemic vasculitis in adults. in recent years, colour Doppler ultrasound of the temporal arteries (CDU) has proven to he a powerful non-invasive diagnostic tool, but its place in the diagnosis of GCA remains to be defined. A limitation of the CDU is the inter-operator reproducibility. Image analysis from a different perspective is now possible with the development of artificial intelligence algorithms. We propose to assess this technology for the detection of the halo sign on CDU images. Methods. Three public hospitals retrospectively collected data from 137 patients suspected of having GCA between January 2017 and April 2019. CDU images (n=1,311) were labelled with the VIA software. Three sets (training, validation and test) were created and analysed with a semantic segmentation technique using a U-Net convolutional neural network. Results. The area under the curve (AUC) was 0.931 and 0.835 on the validation and test set, respectively. An image positivity threshold was determined by focusing on the specificity. With this threshold, a specificity of 95% and a sensitivity of 60% were obtained for the test set. The analysis of the false interpretation showed that the acquisition modalities and the presence of thrombus caused confusion for the algorithm. Conclusion. We propose an automated image analysis tool for GCA diagnosis. The 2018 EULAR guidelines for image acquisition must be respected before generalising this algorithm. After external validation, this tool could be used as an aid for diagnosis, staff training and student education.
Upper extremity vein thrombosis (UE-VT) are more and more frequent pathologies and yet little studied. The aim is to describe the clinical and ultrasound features, UE-VT-related diseases, and the prevalence of pulmonary embolism (PE) and associated deaths. All UE-VT patients diagnosed by Doppler-ultrasound in Nantes University Hospital, from January 2015 to December 2017, were included retrospectively. UE-VT suspicion patterns, clinical features, UE-VT topography, and prevalence of PE and death were analyzed. Seven hundred and fifty-five UE-VT were analyzed, including 427 deep thrombosis (UE-DVT) and 328 superficial thrombosis (UE-SVT). In 86.2% (n = 651) UE-VT were related to endovascular devices. Among these thrombosis, one third is in connection with a PICC LINE and one quarter with a peripheral venous line. Forty nine percent (n = 370) of the patients had solid neoplasia or hematological malignancies. An inflammatory or systemic infectious context was found in 40.8% (n = 308) of the cases. The most frequently observed clinical sign at the UE-VT diagnosis was edema (28.6%). Among the UE-SVT it was the presence of an indurated cord (33.2%) and among the UE-DVT the indication of the Doppler-ultrasound was mainly a suspicion of infection on endovascular device (35.1%). In 10.6% (n = 80) of the cases the UE-VT were asymptomatic. The most frequently thrombosed veins were brachial basilic veins (16.7% of all thrombosed segments) followed by jugular (13%) and subclavian (12.3%) veins; 61.3% (n = 463) of UE-VT were in the right upper extremity; 63.3% (n = 478) UE-VT were occlusive. The occurrence of PE is 4% and the death rate is 10.2%, mainly related to the severe comorbidities of patients with UE-VT. UE-VT occurs in particular clinical contexts (hematological malignancies, solid cancers, systemic infections) and in the majority of endovascular devices (86.2%). The occurrence of PE is low.
Décrire les caractéristiques cliniques, les étiologies, les prises en charge et l’évolution des thromboses aiguës d’aortes natives non anévrismales dans une large cohorte. Cette étude rétrospective a inclus des thromboses d’aortes natives non anévrismales prises en charge au CHU de Nantes entre 2003 et 2018. Les données étudiées étaient l’âge de survenue, le sexe, les facteurs de risques cardiovasculaires, la clinique, la localisation du thrombus aortique, les atteintes d’autres territoires artériels au diagnostic, les étiologies, les thérapeutiques, la survenue d’évènements vasculaires à distance du diagnostic et la survie. Quarante-huit patients ont été inclus, dont 25 hommes (52,1 %). L’âge médian était de 55 ans. La majorité des patients (87,5 %, n = 42) présentait au moins un facteur de risque cardiovasculaire. Les facteurs de risque cardiovasculaires les plus représentés étaient le tabac (79 %, n = 38) et les antécédents personnels vasculaires (48 %, n = 23). Les douleurs abdominales et les claudications ou ischémies critiques de membre inférieur étaient les présentations cliniques les plus fréquentes (31 %, n = 15), venaient ensuite les ischémies aiguës de membres inférieurs (27 %, n = 13). Les thromboses de l’aorte abdominale sous-rénale étaient prédominantes (73 %, n = 35) ; 79 % des cas (n = 38) présentaient une atteinte dans un autre territoire artériel au diagnostic. L’athérome était l’étiologie la plus fréquente (54 %, n = 26), suivi des thromboses sans cause retrouvée (21 %, n = 10), puis des néoplasies et des causes cardio-emboliques (10,4 %, n = 5). La prise en charge thérapeutique reposait surtout sur la chirurgie (62,5 %, n = 30) associée initialement à une héparinothérapie (75 %, n = 36) et/ou une antiagrégation plaquettaire (77 %, n = 37). Au cours du suivi, 10 patients (21 %) ont présenté un nouvel événement vasculaire et 8 patients sont décédés (17 %). La présentation clinique des thromboses aortiques est peu spécifique, l’étiologie la plus fréquente est l’athérome avec des thrombus sur plaque. La prise en charge est le plus souvent chirurgicale associée à un traitement anticoagulant et/ou antiagrégant. Un nouvel événement vasculaire survient dans 21 % des cas.
Les ischémies digitales (ID) sont une entité rare et hétérogène auxquelles est cependant, confronté le médecin vasculaire ; l’ischémie du membre supérieur est quarante fois moins fréquente que celle des membres inférieurs. L’objectif de cette étude était d’évaluer les étiologies, les traitements, les pronostics et l’évolution à long terme des ID dans la plus grande cohorte jamais rapportée. Nous avons inclus les patients présentant des ischémies digitales, avec ou sans nécrose, limitées ou non aux doigts sur la période de janvier 2000 à décembre 2016 au sein du CHU de Nantes. Nous avons recueilli les données rétrospectives portant sur l’étiologie, la récidive d’ID, les amputations digitales et la survie globale. Les ischémies digitales d’origine cardio-emboliques (IDCE) étaient l’étiologie principale avec 59 patients (18,3 %), suivie des patients atteints de sclérodermie systémique (SSc) avec 52 patients (16 %), les ID idiopathiques représentaient 38 patients (11,7 %), les maladies de Buerger avec 30 patients (9,3 %), les causes iatrogéniques avec 30 patients (9,3 %) et les néoplasies avec 20 patients (6,2 %). Les patients avec une IDCE étaient significativement plus vieux et présentaient plus d’hypertension artérielle alors que les maladies de Buerger consommaient plus de tabac et de cannabis. Sur le plan clinique, les patients avec une IDCE avaient plus de pâleur des doigts et de douleur à la fois nociceptive et neuropathique alors que les SSc présentaient plus de nécrose digitale. Le suivi à long terme a montré des récidives significativement plus fréquentes dans le groupe SSc par rapport à tous les autres groupes (p < 0,0001 versus idiopathique et IDCE, p = 0,003 versus maladie de Buerger) et plus de récidives dans le groupe maladie de Buerger par rapport au groupe IDCE (p = 0,005). La mortalité était significativement plus importante dans le groupe IDCE par rapport au groupe SSc (p < 0,0001), idiopathique (p = 0,0093) et maladie de Buerger (p = 0,0007). Dans cette étude, les étiologies principales d’ID étaient les cardiopathies emboligènes, la SSc et les ID idiopathiques. Cette étude montre une augmentation de l’étiologie iatrogénique en comparaison aux séries d’ID précédentes. Les ID de la SSc sont associées à un mauvais pronostic local alors que les IDCE sont associées à un mauvais pronostic général. Les ID dans la SSc et la maladie de Buerger sont associées à plus de récidive.
Abstract Aim: Upper Extremity Digital ischemia (UEDI) is a rare heterogeneous condition with digital upper limb ischemia occurring forty times less frequently than toe ischemia. Using the largest cohort ever published, the aim of this study was to evaluate etiologies, treatments, prognosis and long-term clinical outcomes of DI. Method: Patients included had UEDI with or without cutaneous necrosis. They were followed in a university hospital setting between January 2000 to December 2016. Etiologies, recurrence of UEDI, digital amputation and overall survival were analyzed retrospectively. Results: DI due to cardio-embolic disease (DICE) was the highest occurring etiology with 59 patients (18.3%), followed by UEDI due to Systemic Sclerosis (SSc) (16.1%), idiopathic causes (11.7%), Thromoboangitis obliterans (TAO) (9.3%), iatrogenic causes (9.3%) and cancer (6.2%). DICE patients tended to fall into an older age bracket and featured a greater numbers of cases with hypertension whereas TAO patients smoked more tobacco and cannabis. DICE presented a greater number of cases with paleness and neuropathic and nociceptive pain whereas SSc patients presented more cases with necrotic pathophysiology. Long-term follow-up of this study showed recurrences were significantly more frequent in SSc than in all others tested groups (p <0.0001 vs idiopathic and DICE, p=0.003 vs TAO) and that TAO patients had significantly more recurrences than DICE (p = 0.005). Global survival was significantly lesser in DICE than in SSc (p<0.0001), idiopathic (p=0.0093) and TAO (p =0.0007). Conclusion: The main etiologies of UEDI were CE, SSc and idiopathic. This study highlighted an increase of iatrogenic UEDI cases compared to previous studies. UEDI associated with SSc had a poor local prognosis and DICE presented poor general prognosis. UEDI with SSc and TAO presented with greater recurrence.
