The use of homeopathy is increasing, and skin diseases represent one of the most important indications in the employment of plant extracts as medicinal remedies.1-5 A 45-year-old woman was admitted to our department for widespread dermatitis. Fifteen days before its appearance, having noted the presence of vesicles on the right thigh and on the vulva and presuming them to be herpes lesions, she had applied the mother tincture of Rhus toxicodendron (alcohol solution). The patient had been treating the recurrent herpes with this homeopathic medicine for more than 10 years. After 10 days, she had also taken 7CH micropellets of the same substance (3-5 micropellets more times a day for 4 days). The patient reported that an erythema, limited to the sites of tincture application at first, had extended to the other thigh, the forearms, and the neck and face. Upon examination, a bright erythema with distinct boundaries involved the pubis, the inguinal area, and the inner surface of the thighs. Laboratory investigations revealed leukocytosis and elevated anti-herpesvirus IgG antibodies. Histopathologic examination demonstrated eczematous findings. After remission, the patient underwent patch tests that revealed positive reactions to a balsam of Peru 25%, fragrances mix 8%, and Dermatophagoides mix 30%. An open test, performed on the forearm with the R toxicodendron tincture, showed an erythemato-oedematous response at 48 hours and an erythemato-edemato-vesicular reaction at 96 hours that was still present after 7 days. Patch test with 65° ethyl alcohol gave negative results. The open test performed as control in 8 healthy informed subjects revealed negative responses.
The present work reports the results of a multicentre study of toothpaste allergic contact cheilitis (TACC) conducted by GIRDCA (Gruppo Italiano Ricerca Dermatiti da Contatto e Ambientali). The study examined 54 patients with eczematous lesions on the lips, the possible cause of which was suspected to be the use of toothpastes. Patch tests were conducted with a standard series, a specially‐targeted series (toothpaste cheilitis series, TCS), and with suspected toothpaste(s). A stop‐restart test (SRT) was carried out with these, together with a use test to identify possible alternative products. The TCS produced 17 positive reactions in 13 patients, the most frequent being to spearmint oil. Of the 54 patients, 5 displayed positive reactions only to the TCS. The patch tests with toothpaste produced positive reactions in 11/32 patients, the SRT a positive response in 10/12 cases. The diagnosis of TACC was confirmed in 15/54 patients. Alternative products were identified for 5 patients. In conclusion, the allergens most frequently responsible for TACC were the flavourings, and the additional series proved to be useful in many cases (together with patch tests with toothpastes and the SRT) for correct diagnosis and to initiate effective prevention.

Background/aims: The aim is to evaluate, using evaporimetry, the possibility of getting further information supporting clinical reading of allergic, irritant reactions and doubtful patch test reactions.Methods: The investigation was carried out on 204 patients (182 female and 22 male, mean age 31.6 years), patch tested routinely as suspects of allergic contact dermatitis. We evaluated 326 reactions (203 allergic, 123 irritant or doubtful).Results: Mean values pf TEWL were: for the positive allergic reactions, 7.21 (SD, 2.26) at 48 h, 15.77 (SD, 5.50) at 72 h; and for the irritant or doubtful reactions, 7.55 (SD, 1.72) at 48 h, and 5.77 (SD, 1.41) at 72 h. TEWL in the 2 reactions groups at 72 h was significantly different (p<0.01).Conclusions: The study shows (i) concordance between the evaporimeter values and the visual score; (ii) at 72 h, the evaporimeter values are increased in the allergic reactions but not in irritant or doubtful reactions; (iii) evaporimetry in the differential diagnoses of patch test reactions was deemed useful.

The survey was carried out between January 1988 and June 1990. It consisted in a clinical and allergological study of 1489 housewives, 880 of which had contact dermatitis at their hands. (group of "cases"). The other 609 represented the control group. The anamnesis was gathered with a guided questionnaire: the questions concerned familiar and personnel pathology, use of detergents and cosmetics, use of means of prevention and therapies. Patch tests with haptens of GIRDCA series and with the most common detergent components were performed. In 98% of cases hands were the lace of beginning of symptoms; in the group of "cases" 46% were positive to at least one patch test. Nickel sulphate gave positivities in the 28% of cases. Results of the search of this metal (and of chrome and cobalt) in detergents are described. Other possible occasions of contact are described. Other causes of sensitization are represented by rubber additives, by perfumes and by preservatives. Cutipositivities for detergents are very really limited. Means of protection, in particular barrier creams, are described as scarcely used.

Dismantling end-of-life ships in an environmentally sound and safe manner is of great concern as well as being a major challenge nowadays. When dismantling the vessel, on-board hazardous materials such as asbestos, polychlorinated biphenyls (PCBs), glass fibre, solid foam and waste oil can incur severe negative implications on the environment and human health. The characteristics and harm of on-board hazardous materials are profiled in this paper. Current removal and disposal methods of hazardous materials are analysed. Further practical measures and suggestions to deal with the hazardous materials in the ship breaking yards are proposed. Two case studies about green disposal of the main hazardous materials during shipbreaking are presented. Both of the two companies are following Hong Kong Convention and verified by DNV-GL classification, which makes the recycling procedures of hazardous materials are quite similar to each other. In essence, more attention should be paid to the disposal of hazardous materials as integral to safe and environmentally sound practices when breaking the ships.
A multicenter study was performed in 9 Italian centers by members of the GIRDCA, to evaluate the frequency and source of contact sensitization in a group of 302 hairdressers with dermatitis. Occupational habits and use of preventive measures were specifically investigated both in these 302 hairdressers and in a further group of 240 hairdressers who answered a questionnaire. The results showed the presence of an occupationally relevant sensitization in 60.9% of the 302 hairdressers. This proportion included 52 hairdressers who had negative patch tests to the hairdressers' series but showed positive reactions to other allergens, such as nickel, rubber additives, preservatives and fragrances, which were judged relevant to their occupation. Among hair dyes, PPD caused 73 reactions (24.2%), PAP 32 reactions (10.6%), ONPPD 24 reactions (7.9%), and PTD 40 reactions (13.2%). A low incidence of sensitization was detected in our hairdressers to resorcinol and pyrogallol (1.3% for each substance). Among permanent wave allergens, positive reactions to GMTG were found in 11.3% of patients, while ATG gave a lower rate of positive reactions (5.0%). Allergic contact dermatitis due to APS was also relatively common (11.3%). 4 hairdressers in this study gave a positive reaction 30 min after a provocative test with latex gloves, patch testing to the rubber series being negative. Enquiry regarding preventive measures revealed that the majority of hairdressers use gloves when doing hair dyeing, but rarely use them for washing dyed hair or for doing permanent waving. The infrequent use of preventive measures by Italian hairdressers was confirmed by the results of the questionnaire, and possibly explains the high frequency of skin problems (12.5%) in the hairdressing population that was specifically interviewed.
