Hören bedeutet soziale Teilhabe. Die Prävalenz von Hörminderungen steigt mit dem Alter. Ziel dieses Projektes war es, die aktuelle Hörsituation in stationären Pflegeeinrichtungen einzuschätzen und Probleme in der Hörgeräteversorgung zu erkennen. In 4 verschiedenen stationären Pflegeeinrichtungen in der Region Hannover wurden an 74 Bewohnenden Ton- und Sprachaudiogramme gemessen und mit 97 Bewohnenden Fragebögen zur Versorgungssituation mit Hörhilfen durchgeführt. Von allen gemessenen Audiogrammen zeigten 75% eine versorgungspflichtige Schwerhörigkeit auf, die jedoch oft nur unzureichend versorgt war. Häufig gaben Bewohnende an, subjektiv gut zu hören, nannten aber gleichzeitig viele Situationen, in denen sie sich wünschen, besser zu hören. Vorhandene Hörgeräte wurden oftmals nicht oder nur wenig getragen, und Betroffene wünschten sich Hilfe im Umgang mit den Geräten. In einer alternden Gesellschaft ist eine wachsende Belastung der Pflegeeinrichtungen zu erwarten. Hörminderungen sind hier ein allgegenwärtiges Thema, welches die Pflege und Kommunikation zusätzlich erschwert und für welches mehr Bewusstsein geschaffen werden muss, um eine suffiziente Hörversorgung zu gewährleisten.
OBJECTIVES:This study investigates barriers to hearing aid acquisition and usage in nursing homes from the residents' perspective. DESIGN AND STUDY SAMPLE:85 residents (mean age 83.8 years) were recruited from four nursing homes. Residents underwent pure tone and speech audiograms, completed the Nursing Home Hearing Index (NHHI), and a questionnaire on hearing aids. RESULTS:25 residents had untreated severe hearing loss (29.4%), but many were reluctant to pursue rehabilitation (15/25). Those hesitant to adopt hearing aids had less severe hearing loss (55.2 dB vs. 66.0 dB for residents open towards hearing aids), were less aware of their hearing loss, felt less impact in their daily life (NHHI 29.3pts vs. 40.4pts), and held more negative preconceptions towards hearing aids. Among residents that already used hearing aids, problems with practical handling were the predominant barrier to regular use, followed by lack of perceived benefit, and barriers in healthcare access. CONCLUSIONS:Raising early awareness for hearing loss is important to improve chances for rehabilitation. Educating medical staff in hearing health is crucial to raise awareness for hearing loss, support residents in daily life, and improve adherence to hearing aids. On-site services are essential for nursing homes to facilitate access to healthcare providers.
Hearing means social participation. The prevalence of hearing loss increases with age. The aim of this project was to assess the current hearing situation in nursing homes and to identify problems in the provision of care.Pure tone and speech audiograms were measured in 74 residents in four different nursing homes in the greater Hanover area and questionnaires on the hearing aid care situation were carried out with 97 residents.Of all audiograms measured, 75% showed a hearing loss requiring hearing aids, which was often inadequately cared for. Residents often stated that they could hear well subjectively, but at the same time mentioned many situations in which they would have liked to hear better. Existing hearing aids were often worn not regularly or even not at all. Those who are affected by hearing loss wished for help in using their devices.In an ageing society, a growing burden on care facilities is to be expected. Hearing loss is an omnipresent issue here, for which more awareness must be created in order to ensure adequate hearing care. Furthermore, hearing loss means challenged communication and therefore can deteriorate the care of patients.
Sinugene orbitale Komplikationen bei Kindern sind relativ seltene, aber dennoch kritische Krankheitsbilder, die richtig erkannt und zeitnah therapiert werden müssen, um schwerwiegende Folgeschäden zu vermeiden. Ziel der vorliegenden Arbeit ist die Vertiefung der Kenntnisse über die klinischen, diagnostischen und therapeutischen Besonderheiten der sinugenen orbitalen Komplikationen im Kindesalter. Es flossen Erkenntnisse aus einer Literaturrecherche und deren Auswertung (PubMed, Google Scholar), eigener Erfahrungen sowie der Datenerhebung im eigenen pädiatrischen Patientenkollektiv ein. Sinugene orbitale Komplikationen bei Kindern entstehen häufig auf dem Boden einer akuten, bakteriellen Sinusitis. Das richtige Erkennen einer prä- vs. postseptalen Entzündung ist für den klinischen Alltag von höchster Bedeutung, um zeitnah die richtige Diagnostik und Therapie einzuleiten und somit bleibende Folgeschäden zu vermeiden. Rein präseptale Entzündungen werden i. d. R. konservativ behandelt, bei Hinweisen auf eine postseptale Komplikation ist eine zeitnahe Schnittbildgebung erforderlich. Je nach Ausmaß der postseptalen Komplikation ist ggf. eine operative Versorgung notwendig. In der aktuellen Literatur kristallisieren sich 3 „Hauptdiskussionspunkte“ heraus: 1) Modellierung von Symptomkomplexen, anhand deren Ausprägung sicher zwischen einer prä- und einer postseptalen Entzündung unterschieden werden kann, 2) Auswahl der richtigen Bildgebung (Magnetresonanztomographie, MRT, vs. Computertomographie, CT), 3) Indikationen zur operativen Versorgung. Alle Punkte werden im vorliegenden Artikel näher beleuchtet.

Im vergangenen Jahr konnten wir zeigen, dass der Hörstatus und die Hörgeräteversorgung in stationären Pflegeinrichtungen einen Optimierungsbedarf aufweisen, und dass mehr Bewusstsein geschaffen werden muss, um eine adäquate Hörrehabilitation zu gewährleisten.
Introduction Hearing is an elementary component of a socially active life. In an increasingly ageing population, hearing disorders are an omnipresent issue. During the past year, we were able to show that improvement of hearing ability and hearing aid provision in nursing homes are is required, and that more awareness needs to be raised to ensure adequate hearing rehabilitation.
Background: Sinogenic orbital complications in children are relatively rare but critical conditions that require accurate diagnosis and timely appropriate treatment to prevent severe sequelae. Objective: This article concentrates on clinical, diagnostic, and therapeutic specifics of sinogenic orbital complications in children. Materials and methods: The work is based on a literature review (PubMed, Google Scholar) and own experience as well as data from the authors' own pediatric population. Results: In children, sinogenic orbital complications are usually secondary due to acute bacterial sinusitis. Correct distinction between pre- and postseptal cellulitis is of utmost importance for correct diagnostic and therapeutic management in order to prevent long-term sequelae. Preseptal cellulitis can be treated conservatively. If there are signs of postseptal involvement, prompt cross sectional imaging will be required. Depending on the severity of the postseptal involvement, surgical treatment may be necessary. Conclusion: Recent literature debates three main issues: 1) developing models for different combinations of symptoms that reliably distinguish between pre- and postseptal cellulitis; 2) selection of appropriate cross-sectional imaging (MRI vs. CT); and 3) indications for surgery. All three issues will be discussed in detail in this article.
Background Dementia affects millions of people worldwide. The focus of this study is the identification of dementia developments and social withdrawal tendencies in inpatient care facilities in Lower Saxony in patients with and without hearing aids in a 1-year-follow up.
Hintergrund Aufgrund der Alters- und Sektorstrukturen haben Bewohnende stationärer Pflegeeinrichtungen oft Nachteile beim Zugang zur Hörgeräteversorgung. Background Nasal obstruction is a frequent symptom in both adults and children and it is a common reason to see an otorhinolaryngologist. Endoscopy of the nasal cavity and the epipharyngeal space along with anterior rhinomanometry is regarded the gold standard since many years to estimate the severity of nasal obstruction in the particular patient. Endoscopy shows anatomical reasons for an obstruction, whereas the nasal flow volume and nasal resistance can be determined using anterior rhinomanometry. Currently, there are only few data available for rhinomanometry results in children. The purpose of the present study was to evaluate the application of this technique in the pediatric population for objective evaluation of nasal flow. Whether it achieves reproducible results and which clinical parameters have some influence on the results were studied. Patients and methods 427 children (average age of 8.5 years, range 7 months through 17 years) who were admitted to evaluate nasal patency or for allergy testing were examined. After clinical examination and endoscopy of the nasal cavity and epipharyngeal space, anterior rhinomanometry was performed before and after application of decongestant nose drops separately for each nose side in 334 children. The nasal flow with a pressure of 150 Pasc was measured and served for statistical evaluation. Flow values were correlated to clinical and endoscopic parameters along with results of allergy tests (prick tests). Results Reproducible rhinomanometric measurements were possible in children age 3 years and older. However, the standard deviation and variation of measurements were significant in this cohort of patients. Statistically highest significant correlations were found between flow measurements and body height along with the age of the children ( p < 0.01) and status following adenoidectomy ( p < 0.05). No statistically significant correlations were found between rhinomanometry and results of prick tests. Conclusions The study demonstrates that rhinomanometry can be applied in the pediatric population for objective evaluation of nasal obstruction and for determining the effects of decongestant nose drops. The highest correlation was found between nasal flow and children’s body height, children’s age and status following adenoidectomy. The correlation between nasal flow and clinically/endoscopically determined degree of nasal obstruction was lower. However, definition of normal flow values for particular age groups is challenging since the results showed high variation and standard deviation. Yet with regard to individual patient, the technique achieves reliable results in nasal provocation tests, which are widely used for allergy testing in children. When performed in children it should always be considered that there are age-specific requirements for the examination and interpretation of results in this patient cohort.
Ziel der vorliegenden Arbeit war die retrospektive Auswertung von Frakturen des Schädels und der Schädelbasis bei Kindern zur Orientierung bei der Versorgung. Bisher liegen nur wenig Daten über diese Verletzungen bei dieser Patientengruppe vor. Ausgewertet wurden stationäre Fälle mit der Diagnose einer Schädel‑, Schädelbasis- oder Mittelgesichtsfraktur im Kinder- und Jugendkrankenhaus Auf der Bult von 2015–2020 . Alter, Geschlecht, Unfallmechanismus, Diagnostik, Therapie und etwaige Komplikationen wurden erfasst. Daten von insgesamt 224 Patienten wurden in Felsenbein- (Laterobasis-; n = 9), Frontobasis- (n = 4), Orbitaboden- (n = 2), Nasenbein- (n = 107) und Schädelkalottenfrakturen (n = 104) gruppiert. Bei Nasenbeinfrakturen betrug das Durchschnittsalter 10,9 Jahre (64 % männlich), bei Fronto‑/Otobasisfrakturen 6,0 Jahre und (85 % m.), bei Schädelkalottenfrakturen 1,0 Jahre (64 % m.). Häufigste Genese waren Sturzereignisse (63 %), Zusammenstöße (25 %) und tätliche Angriffe (10 %). Sonographiert wurde bei Frakturen der Schädelkalotte in 94 % der Fälle; zu 87 % wurde eine Fraktur nachgewiesen. Bei der Nasenbeinfraktur erfolgte in 92 % der Fälle eine Röntgenaufnahme, in 8 % eine alleinige Sonographie. Bei 95 % dieser Patienten wurde das Nasenbein aufgerichtet. Klassische Frakturzeichen (z. B. Hämatotympanon, okuläre Symptome) oder Zeichen zentralnervöser Störungen (z. B. Amnesie, Nausea) wiesen 12 von 13 Kindern mit Fronto‑/Otobasisfrakturen auf, in diesen Fällen erfolgte stets eine Computertomographie (CT, bei keinem trat eine Liquorrhö auf). Die Bildgebung ist anhand der klinischen Verdachtsdiagnose und des Verlaufs zu wählen. Die meisten Patienten können suffizient ohne bleibende Beeinträchtigung behandelt werden, eine Ausnahme bildet die Nasenbeinfraktur.
The purpose of this study was a retrospective evaluation of fractures of the head region and skull base to provide a guideline for ENT doctors for clinical management of this patient group.
A 4-year-old girl was admitted to hospital with disturbance of balance. After being questioned, parents remembered an otitis with effusion 3 months earlier. CT-scans revealed destruction of both temporal bones. Initial biopsy showed granulomatous, necrotic inflammation, which led to comprehensive differential diagnoses. A second tissue sample confirmed Langerhans cell histiocytosis.
Background: The term "orbital complication" does not designate an independent nosological entity, but is rather a collective designation for diseases or disease effects that involve the orbit and its internal structures by extension from outside. In general, their most prominent manifestation is swelling of the orbital soft tissues, usually unilaterally. The incidence of sinogenic orbital complications is approximately 1.6 per 100 000 children and 0.1 per 100 000 adults per year. Methods: This review is based on publications retrieved by a selective search of the literature on the epidemiology, diagnosis, and treatment of sinogenic orbital complications. Results: Acute sinusitis is the most common cause of orbital complications. These are diseases of the orbit with potentially serious consequences for the eye and the risk of intracranial complications such as cavernous sinus thrombosis, meningitis, or brain abscess. Aside from acute sinusitis, many other infectious and non-infectious diseases can extend to and involve the orbit. Because of the complexity and severity of the condition, its diagnosis and treatment are always an interdisciplinary matter. The treatment is primarily conservative, under observation in a hospital, and generally consists of the treatment of acute sinusitis with measures to combat edema along with the administration of broad-spectrum antibiotics. Surgical intervention is needed in severe cases or if there is an abscess. An endonasal approach is usually used for drainage. Conclusion: In 95-98% of cases in stages I-IV, healing is complete and without further sequelae. Even if vision is affected preoperatively, it usually recovers fully when therapy is appropriate. Approximately 15% of the patients who undergo surgery need more than one operative procedure.
