ABSTRACTPurposeTo measure the impact of national regulatory actions implemented in France in August 2018 and June 2019 to reduce the risk of meningioma associated with the use of cyproterone acetate (CPA).MethodsUsing the French National Healthcare database, we calculated the monthly number of CPA users among cisgender women, men and transgender women in 2010–2021, the monthly proportion of users with cerebral imaging screening, and the annual rate of meningioma surgery associated with CPA use. CPA discontinuations and switches were analysed.ResultsBetween 2018 and 2021, the number of individuals exposed to CPA fell by 85% (55 000 in August 2018 versus 7900 users of high‐dose CPA in December 2021), corresponding to two waves of decrease in both use and initiation. This drop was greater among cisgender women (88%) than men (69%) or transgender women (50%). Cerebral imaging screening increased from 11% in June 2018 to 70% in June 2021 for ciswomen (13%–51% for men, 9%–60% for transwomen). After CPA discontinuation, no massive shift to a single product was observed, but, instead, dispersion towards other hormonal therapies. The overall annual rate of meningioma surgery associated with CPA exposure spectacularly decreased between 2017 and 2021 (−93% for ciswomen and −86% for men).ConclusionIn France, high‐dose CPA use sharply decreased after the implementation of national regulatory measures without a massive switch to other hormonal therapies. The increase in cerebral imaging screening did not result in an increase in meningioma surgery associated with CPA, but rather a massive drop of over 90%.
OBJECTIVE:To characterize multinational trends and patterns of opioid analgesic prescribing by sex and age. DESIGN, SETTING, AND PARTICIPANTS:We studied opioid analgesic prescribing from 2001 to 2019 with common protocol using population-based databases from eighteen countries and one special administrative region. MAIN OUTCOME MEASURES:We measured opioid prescribing by geographical region, sex and age, estimating annual prevalent, incident, and nonincident opioid prescribing per 100 population with a 95% confidence interval (CI) and meta-analyzed the multinational and regional opioid prescribing with a random-effects model. Time trends were reported through average annual absolute changes, estimated using linear mixed models. We further explored the effect of sex and age on prevalent opioid prescribing in the multivariable analysis. RESULTS:Over 248 million individuals were included. Pooled multinational opioid prescribing prevalence was 9.0% amongst included countries/regions. Opioid prescribing prevalence in 2015 ranged from 2.7% in Japan to 19.7% in Iceland. Average annual absolute changes in opioid prescribing prevalence per year ranged from - 1.53% (95% CI - 2.06, - 1.00; United States Medicaid) to + 1.24% (95% CI 1.02, 1.46; South Korea). Pooled multinational incident opioid prescribing (4.9%; 95% CI 4.1, 5.9) was higher than pooled multinational nonincident opioid prescribing (3.7%; 95% CI 2.9, 4.8). The female sex and older age were associated with higher opioid prescribing. Main limitations of this study include the absence of data from study duration or individuals not covered by the data sources and the lack of information on medication adherence and indication. CONCLUSIONS:Opioid prescribing remains unbalanced across geographical regions; however, results suggest a tendency to convergence across countries/regions. Differences in opioid prescribing by sex and age were identified.
BACKGROUND AND PURPOSE:Chlormadinone acetate (CMA) is a synthetic progestin for which cases of intracranial meningioma have been reported following prolonged exposure. METHOD:An observational cohort study was conducted based on the French national health data system. Women aged 10-70 years and who started CMA between 2007 and 2017 were included. Participants were considered to be exposed if they had received a cumulative dose >360 mg of CMA during the first 6 months and very slightly exposed (control group) when they had received a cumulative dose ≤360 mg. The outcome was surgery or radiotherapy for one or more intracranial meningioma(s). Poisson models assessed the relative risk (RR) of meningioma. RESULTS:In total, 828,499 women were included: 469,976 in the exposed group (mean age 39.1 years, SD 10.1) and 358,523 in the control group (38.3 years, SD 11.0). Surgery or radiotherapy for intracranial meningioma between 2007 and 2017 was recorded for 164 and 104 women in the exposed and control groups, respectively. The incidence of meningioma was 18.5 and 6.8 per 100,000 person-years for the exposed and control groups respectively (crude RR = 2.7, 95% confidence interval [CI] 2.1-3.5; age-adjusted RR = 3.1, 95% CI 2.4-4.0). Meningioma incidence reached almost 47 cases/100,000 person-years in the most exposed group (>8.64 g), giving an age-adjusted RR of 6.9, 95% CI 5.1-9.2, relative to the control group. CONCLUSIONS:A strong dose-effect relationship was observed between prolonged use of CMA and risk of meningiomas. As with other progestogens, meningiomas associated with CMA are more likely to be found at the base of the skull.
ABSTRACT Objective To assess the risk of intracranial meningioma associated with the use of selected progestogens. Design National case-control study. Setting French National Health Data System (ie, Système National des Données de Santé ). Participants Of 108 366 women overall, 18 061 women living in France who had intracranial surgery for meningioma between 1 January 2009 and 31 December 2018 (restricted inclusion periods for intrauterine systems) were deemed to be in the case group. Each case was matched to five controls for year of birth and area of residence (90 305 controls). Main outcome measures Selected progestogens were used: progesterone, hydroxyprogesterone, dydrogesterone, medrogestone, medroxyprogesterone acetate, promegestone, dienogest, and intrauterine levonorgestrel. For each progestogen, use was defined by at least one dispensation within the year before the index date (within three years for 13.5 mg levonorgestrel intrauterine systems and five years for 52 mg). Conditional logistic regression was used to calculate odds ratio for each progestogen meningioma association. Results Mean age was 57.6 years (standard deviation 12.8). Analyses showed excess risk of meningioma with use of medrogestone (42 exposed cases/18 061 cases (0.2%) v 79 exposed controls/90 305 controls (0.1%), odds ratio 3.49 (95% confidence interval 2.38 to 5.10)), medroxyprogesterone acetate (injectable, 9/18 061 (0.05%) v 11/90 305 (0.01%), 5.55 (2.27 to 13.56)), and promegestone (83/18 061 (0.5%) v 225/90 305 (0.2 %), 2.39 (1.85 to 3.09)). This excess risk was driven by prolonged use (≥one year). Results showed no excess risk of intracranial meningioma for progesterone, dydrogesterone, or levonorgestrel intrauterine systems. No conclusions could be drawn concerning dienogest or hydroxyprogesterone because of the small number of individuals who received these drugs. A highly increased risk of meningioma was observed for cyproterone acetate (891/18 061 (4.9%) v 256/90 305 (0.3%), odds ratio 19.21 (95% confidence interval 16.61 to 22.22)), nomegestrol acetate (925/18 061 (5.1%) v 1121/90 305 (1.2%), 4.93 (4.50 to 5.41)), and chlormadinone acetate (628/18 061 (3.5%) v 946/90 305 (1.0%), 3.87 (3.48 to 4.30)), which were used as positive controls for use. Conclusions Prolonged use of medrogestone, medroxyprogesterone acetate, and promegestone was found to increase the risk of intracranial meningioma. The increased risk associated with the use of injectable medroxyprogesterone acetate, a widely used contraceptive, and the safety of levonorgestrel intrauterine systems are important new findings.
Proton pump inhibitors (PPIs) are largely used in older adults and data are needed in off-label indications, such as the prevention of upper gastrointestinal bleeding (UGIB) in patients receiving oral anticoagulants (OACs). This study aimed to assess whether PPIs reduce the risk of UGIB in patients initiating oral anticoagulation. We conducted a longitudinal study based on the French national health database. The study population included 109,693 patients aged 75–110 years with a diagnosis of atrial fibrillation who initiated OACs [vitamin K antagonist (VKA) or direct OAC (DOAC)] between 2012 and 2016. We used multivariable Cox models weighted by inverse of probability of treatment to estimate the adjusted hazard ratio (aHR) of UGIB between PPI users and nonusers over a 6- and 12-month follow-up. En août 2018, un risque augmenté, dose dépendant, de méningiomes intracrâniens lié à l'utilisation prolongée du progestatif acétate de cyprotérone (ACP) à fortes doses (>=25mg/jour) a été rendu public en France, avec un impact médiatique considérable. Des mesures de réduction du risque ont été prises par l'ANSM et la CNAM: information ciblée des professionnels et usagers avec rappel des indications (hirsutisme féminin sévère, traitement palliatif du cancer prostatique...), recommandation de dépistage régulier par IRM cérébrale et d'approche neurochirurgicale conservatrice en cas de diagnostic de méningiome, délivrance conditionnée par l'attestation annuelle d'information (co-signée prescripteur/usager). L'objectif de l’étude était d’évaluer l'impact des mesures à partir du Système national des données de santé (SNDS). Pour chaque mois entre janvier 2010 et décembre 2021 et chacun des groupes femmes, hommes et femmes transgenres, ont été calculés le nombre d'utilisateurs d'ACP et le pourcentage des patients avec >=1 IRM cérébrale durant les cinq années précédentes. Entre août 2018 et décembre 2021, le nombre de personnes traitées par l'ACP a diminué de 85 % (femmes 88 %, hommes 69 %, femmes transgenres 50 %) pour atteindre 7900 (55 000 en août 2018, 85 000 en janvier 2010) ; 92 % des personnes traitées ont arrêté, les instaurations ont reculé de 94 %. Nous n'avons pas constaté de report massif sur des produits spécifiques mais une dispersion vers d'autres hormones usuelles et la spironolactone, diurétique avec action antiandrogène. Le dépistage par IRM cérébrale est devenu fréquent (70 % des femmes utilisatrices d'ACP et 50 % des hommes en décembre 2021 contre environ 12 % en juin 2018). Le nombre annuel de femmes opérées de méningiome intracrânien attribuable à l'usage d'ACP a diminué de 93 %. Parmi les limites de l’étude figurent la méconnaissance des indications des prescriptions résiduelles ainsi que des résultats des IRM de dépistage qui devront faire l'objet d’études ad hoc. Suite aux actions réalisées, la consommation d'ACP et le nombre de méningiomes intracrâniens opérés attribuables à ce médicament se sont fortement réduits, et ce malgré un taux de dépistage élevé. Concernant l'activité ambulatoire du champ PMSI psychiatrie, les établissements publics et ESPIC ont fourni en 2020 pour la première fois les éléments nécessaires pour le calcul de l'identifiant patient qui permet le chaînage avec DCIR. Pour 2021, cette information a été intégrée dans le portail du Sniiram-SNDS. L'objectif de ce travail était d'explorer la qualité effective de ce chaînage. Les établissements produisent le fichier ANO-AMBU qui contient pour chaque IPP (identifiant permanent du patient) le ou les identifiant(s) chaînable(s) avec DCIR (abstraction faite des différents cryptages). L'IPP, un identifiant défini au niveau de l’établissement, se trouve également dans le fichier des résumés par acte ambulatoire anonymisé (R3A). Pour chaîner les R3A à DCIR, on procède alors d'abord à une jointure entre R3A et ANO-AMBU. Pour 2021, parmi les 2 538 009 lignes dans ANO-AMBU, 1 380 129 (54,4 %) sont exemptes d'anomalie concernant l'identifiant chaînable selon les différents codes retour. D'autre part, les 22 607 536 R3A correspondent à 2 117 166 IPP différents (en moyenne 10,7 R3A par IPP). Après jointure, 13 167 425 (58,2%) des 22 607 536 R3A ont un identifiant chaînable sans anomalie signalée par les codes retour. Ce taux montre une grande variabilité en fonction de l’établissement (de 0,0 % à 99,8 %) et aussi régionale (de 29,2 % pour la Corse à 79,4 % pour la Guadeloupe). Parmi les 13 167 425 R3A, 12 875 608 (97,8 %) ont effectivement leur identifiant chaînable dans le référentiel patient DCIR, avec au total 978 979 identifiants chaînables différents. Rapporté à l'ensemble des R3A, cela correspond à un taux de chainage de 57,0 %. Pour près d'un million de personnes, les informations disponibles dans le SNDS et chaînables ont été enrichies des éléments sur les actes ambulatoires en psychiatrie en 2021 (diagnostics médicaux…). Toutefois, l'utilisation ou non de ces nouvelles informations doit être bien examinée en fonction de l’étude, car le taux de chaînage reste avec 57,0 % bien inférieur à 100 % ; ce chaînage n'existe pas pour le secteur privé. Le nombre de femmes et hommes ayant débuté un traitement ACP baissait de 2154 en 06/2018 à 183 en 06/2021 (-91,5 %) et de 676 à 187 (-72,3 %), respectivement. Parmi les femmes, le pourcentage des utilisatrices d'ACP avec IRM cérébrale augmentait de 10,5 % en 06/2018 à 69,2 % en 06/2021 (9,8 % et 43,7 %, respectivement, à l'instauration). Parmi les hommes, les pourcentages étaient de 13,3 % et 48,1 % (11,4 % et 32,1 %), respectivement. BACKGROUND:Continuation of biologics for inflammatory disorders during pregnancy is still a difficult decision. Many women with inflammatory bowel diseases (IBDs) stop anti-tumor necrosis factor (anti-TNF) treatment after 24 weeks.OBJECTIVE:To evaluate the benefits and risks of anti-TNF continuation after 24 weeks of pregnancy for mothers with IBD and their offspring.DESIGN:Target trial emulation between 2010 and 2020.SETTING:Nationwide population-based study using the Système National des Données de Santé.PATIENTS:All pregnancies with birth exposed to anti-TNF between conception and 24 weeks of pregnancy in women with IBD.INTERVENTION:Continuation of anti-TNF after 24 weeks of pregnancy.MEASUREMENTS:Occurrence of maternal IBD relapse up to 6 months after pregnancy, adverse pregnancy outcomes, and serious infections in the offspring during the first 5 years of life was compared according to anti-TNF continuation after 24 weeks of pregnancy using inverse probability-weighted marginal models.RESULTS:A total of 5293 pregnancies were included; among them, anti-TNF treatment was discontinued before 24 weeks for 2890 and continued beyond 24 weeks for 2403. Continuation of anti-TNF was associated with decreased frequencies of maternal IBD relapse (35.8% vs. 39.0%; adjusted risk ratio [aRR], 0.93 [95% CI, 0.86 to 0.99]) and prematurity (7.6% vs. 8.9%; aRR, 0.82 [CI, 0.68 to 0.99]). No difference according to anti-TNF continuation was found regarding stillbirths (0.4% vs. 0.2%; aRR, 2.16 [CI, 0.64 to 7.81]), small weight for gestational age births (13.1% vs. 12.9%; aRR, 1.01 [CI, 0.88 to 1.17]), and serious infections in the offspring (54.2 vs. 50.2 per 1000 person-years; adjusted hazard ratio, 1.08 [CI, 0.94 to 1.25]).LIMITATION:Algorithms rather than clinical data were used to identify patients with IBD, pregnancies, and serious infections.CONCLUSION:Continuation of anti-TNF after 24 weeks of pregnancy appears beneficial regarding IBD activity and prematurity, while not affecting neonatal outcomes and serious infections in the offspring.PRIMARY FUNDING SOURCE:None.

It has recently been reported that vitamin K antagonists (VKAs) impair the bone marrow microenvironment and may predispose to myelodysplastic syndrome (MDS). In a study of the French national health care database, Neumann et al examined data on this issue for more than 120 000 individuals on oral anticoagulation for atrial fibrillation, confirming that there is no increase in MDS in patients treated with VKAs in comparison with those treated with direct oral anticoagulants.
Insufficient real-world data on acute liver injury (ALI) risk associated with oral anticoagulants (OACs) exist in patients with nonvalvular atrial fibrillation (NVAF). Using the French national healthcare databases, a propensity-weighted nationwide cohort study was performed in NVAF patients initiating OACs from 2011 to 2016, considering separately those (1) with no prior liver disease (PLD) as main population, (2) with PLD, (3) with a history of chronic alcoholism. A Cox proportional hazards model was used to estimate the hazard ratio with 95% confidence interval (HR [95% CI]) of serious ALI (hospitalised ALI or liver transplantation) during the first year of treatment, for each non-vitamin K antagonist (VKA) oral anticoagulant (NOAC: dabigatran, rivaroxaban, apixaban) versus VKA. In patients with no PLD (N = 434,015), only rivaroxaban new users were at increased risk of serious ALI compared to VKA initiation (adjusted HR: 1.41 [1.05–1.91]). In patients with chronic alcoholism history (N = 13,173), only those initiating dabigatran were at increased risk of serious ALI compared to VKA (2.88 [1.74–4.76]) but an ancillary outcome suggested that differential clinical follow-up between groups might partly explain this association. In conclusion, this study does not suggest an increase of the 1-year risk of ALI in NOAC versus VKA patients with AF.
Les modèles structuraux marginaux (MSM) ont été développés pour estimer l'effet d'une variable d'exposition dépendant du temps tout en ajustant sur des facteurs de confusion dépendant également du temps. Pour une application réussie dans une étude observationnelle, des informations concernant les principaux facteurs de confusion sont indispensables, point particulièrement délicat dans un contexte de traitement préventif et d'âge avancé, où notamment les comorbidités impactent fortement l'observance et les évènements étudiés comme le décès. Notre objectif était de tester les MSM sur l'arrêt des statines chez les sujets âgés en prévention primaire. À partir des données du Système national des données de santé (SNDS), 120 173 patients ayant eu 75 ans en 2012–14, sans antécédent de maladie cardiovasculaire et avec un taux de possession de statines ≥ 80 % dans les deux années précédentes ont été suivis durant 2,4 ans (moyenne). L'arrêt de statines a été défini par trois mois consécutifs sans exposition. Si les statines étaient reprises, le suivi a été arrêté. Les covariables dépendant du temps comprenaient l'utilisation des médicaments cardiovasculaires et des nombreuses comorbidités et indicateurs de fragilité. Les évènements étudiés étaient l'hospitalisation pour évènement cardiovasculaire et le composite avec le décès. Au total, 8639 patients (7,2 %) ont eu l'évènement composite, dont 37,5 % avec décès comme premier évènement ; 17 204 patients (14,3 %) ont arrêté les statines. Contrairement à l'évènement cardiovasculaire (hazard ratios comparant l'arrêt des statines versus poursuite de 1,26 [IC95 % : 1,14–1,40] et 1,33 [1,18–1,50] en brut et ajusté, respectivement), une différence importante par rapport aux résultats obtenus par des essais randomisés a été observé pour l'évènement composite : hazard ratios de 2,25 (2,10–2,40) et 1,59 (1,46–1,73) en brut et ajusté, respectivement. Les résultats suggèrent que l'utilisation des MSM a permis de corriger une part importante de la confusion dépendant du temps aussi dans le contexte de patients âgés traités en prévention primaire. Cette correction restait toutefois insuffisante après l'addition du décès dans l'évènement étudié, dû à davantage de facteurs de confusion, en partie non observés.
Objectives To describe (i) the trend in oral anticoagulant (OAC) use following the introduction of non-vitamin K antagonist oral anticoagulant (NOAC) therapy for stroke prevention in atrial fibrillation (AF) patients and (ii) the current patterns of use of NOAC therapy in new users with AF in France. Design (i) Repeated cross-sectional study and (ii) population-based cohort study. Setting French national healthcare databases (50 million beneficiaries). Participants (i) Patients with identified AF in 2011, 2013 and 2016 and (ii) patients with AF initiating OAC therapy in 2015-2016. Primary and secondary outcome measures (i) Trend in OAC therapy use in patients with AF and (ii) patterns of use of NOAC therapy in new users with AF. Results Between 2011 and 2016, use of OAC therapy moderately increased (+ 16%), while use of antiplatelet therapy decreased (-22%) among all patients with identified AF. In 2016, among the 1.1 million AF patients, 66% used OAC therapy and were more likely to be treated by vitamin K antagonist (VKA) than NOAC therapy, including patients at higher risk of stroke (63.5%), while 33% used antiplatelet therapy. Among 192 851 new users of OAC therapy in 2015-2016 with identified AF, NOAC therapy (66.3%) was initiated more frequently than VKA therapy, including in patients at higher risk of stroke (57.8%). Reduced doses were prescribed in 40% of NOAC new users. Several situations of inappropriate use at NOAC initiation were identified, including concomitant use of drugs increasing the risk of bleeding (one in three new users) and potential NOAC underdosing. Conclusions OAC therapy use in patients with AF remains suboptimal 4 years after the introduction of NOACs for stroke prevention in France and improvement in appropriate prescribing regarding NOAC initiation is needed. However, NOAC therapy is now the preferred drug class for initiation of OAC therapy in patients with AF, including in patients at higher risk of stroke.
BACKGROUND:The study compared treatment failure when using three therapeutic strategies in bipolar disorders: (i) mood stabilizers (MSs: lithium, valpromide, divalproate, carbamazepine, lamotrigine) without second-generation antipsychotic (SGAP); (ii) SGAPs (aripiprazole, olanzapine, risperidone, quetiapine) without MS; (iii) combination of MSs and SGAPs. METHODS:A historical cohort study was conducted using the French national healthcare databases in 20,086 outpatients aged 21+, newly treated with one of the three treatment strategies in 2011-2012, and diagnosed with a bipolar disorder. A composite outcome was based on indicators of treatment failure identified over 12 months: treatment discontinuation, switch or addition, psychiatric hospitalisation, suicide attempt, and death. For each strategy, the cumulative incidence of treatment failure was calculated while adjusting for covariates by propensity score weighting. RESULTS:A total of 8,225 patients (40.9%) were newly dispensed MSs, 9,342 (46.5%) SGAPs, and 2,519 (12.5%) both MSs and SGAPs. The one-year adjusted cumulative incidence of treatment failure was 75.7% (95%CI 74.9;76.3) in patients using MSs, 75.3% (74.6;76.0) in patients using SGAPs, and 60.5% (58.3;62.6) in patients with the combination. The adjusted difference in incidence for SGAPs compared with MSs was -0.40% (-1.4;0.6 p = 0.4) in the whole population, -2.2% (-3.3; -1.2 p < 0.002) in patients under 65 years and +6.7% (4.1;9.1 p < 0.002) in patients 65 years and over. LIMITATIONS:Combinations of MSs and SGAPs could not be directly compared with MS or SGAP monotherapies. CONCLUSIONS:One-year treatment failure was high. Overall, no difference in treatment failure was observed between MS or SGAP strategy but differences might exist depending on age.
Aims The role of statin therapy in primary prevention of cardiovascular disease in persons older than 75 years remains a subject of debate with little evidence to support or exclude the benefit of this treatment. We assessed the effect of statin discontinuation on cardiovascular outcomes in previously adherent 75-year-olds treated for primary prevention. Methods and results A population-based cohort study using French national healthcare databases was performed, studying all subjects who turned 75 in 2012-14, with no history of cardiovascular disease and with a statin medication possession ratio >= 80% in each of the previous 2 years. Statin discontinuation was defined as three consecutive months without exposure. The outcome was hospital admission for cardiovascular event. The hazard ratio comparing statin discontinuation with continuation was estimated using a marginal structural model adjusting for both baseline and time-varying covariates (cardiovascular drug use, comorbidities, and frailty indicators). A total of 120 173 subjects were followed for an average of 2.4years, of whom 17 204 (14.3%) discontinued statins and 5396 (4.5%) were admitted for a cardiovascular event. The adjusted hazard ratios for statin discontinuation were 1.33 [95% confidence interval (CI) 1.18-1.50] (any cardiovascular event), 1.46 (95% CI 1.21-1.75) (coronary event), 1.26 (95% CI 1.05-1.51) (cerebrovascular event), and 1.02 (95% CI 0.74-1.40) (other vascular event). Conclusion Statin discontinuation was associated with a 33% increased risk of admission for cardiovascular event in 75-year-old primary prevention patients. Future studies, including randomized studies, are needed to confirm these findings and support updating and clarification of guidelines on the use of statins for primary prevention in the elderly.
