How an infection propagates inside the lung is not well understood. Capturing its dynamics might help to understand how pathologies such as COVID19 can lead to rapid airways inflammation and respiratory failure. We hypothesized that respiratory failure might result from the interaction between the propagation of the infection from airway to airway (inner contagiousness) and the pathogen virulence. We develop a mathematical model of the infection and inflammation of the proximal lung (511 susceptible airways) by a generic pathogen that propagates from neighbor to neighbor between the airways. The degree of respiratory failure is evaluated by computing the mean number of infected airways (NI) and the mean drop in oxygen transfer to blood (DOx), assuming no compensation from patient ventilation. We simulated 840 idealized patients, covering 3 different degrees of virulence (Cured (C), Aseptic (A) and Septic (S) outcomes) and 14 degrees of contagiousness (1≤c≤14, arbitrary units). When virulence increases, the pathogens remain longer in the airways, increasing the propagation probability: NI(C)=51, DOx(C)=8.9%; NI(A)=410, DOx(A)=47.2%; NI(S)=511, DOx(S)=55.5%. For low contagiousness, c=1, NI(C)=1.6, DOx(C)=2.2%; NI(A)=132, DOx(A)=25.8%. However, NI(S)=511 and DOx(S)=52.2%. High contagiousness, c=14, leads to a large propagation whatever the virulence (NI(S/A)=511 and DOx(S/A)=57.5%; NI(C)=428 and DOx(C)=38.6%). Medium virulence and contagiousness also lead to a large propagation: for c=7, NI(A)=508, DOx=52.5%. Residence time of pathogens and inner contagiousness are interacting factors that might bring high NI and DOx. This interaction might be a core determinant of potential respiratory failure.
Introduction: Previous models of fractional exhaled nitric oxide (FeNO) have shown their usefulness in asthma and, more recently, in case of airway caliber change (ACG) (Karamaoun et al., 2016). Most bronchial tree models, except few (Buess et al., 2020), present inherent limitations, as they are based on symmetric branching geometry (SG). This prevents from depicting the NO dynamics accurately, especially for ACG. Objectives: To develop a new computational framework of NO dynamics in an asymmetric branching geometry (AG) i.e., branching bronchi of non-equal radii. Then, to propose new scenarios for AG based on FeNO signal. Methods: An AG model is developed, wherein the equations of Karamaoun et al. are solved numerically. Calculations are performed in AG, in healthy and bronchoconstriction (BC). Results: For AG→SG, computations reproduce normal FeNO behavior, as flow-dependency, and previous results in healthy and BC. For AG, the model confirms the importance of asymmetry in FeNO signal. Even for a mild asymmetry ratio of 0.8 on the radii, FeNO50 deviates from 15 ppb in SG to 9.75 ppb in AG (-35%) in healthy and from 18 ppb in SG to 11 ppb in AG (-40%) in strong BC. Discussion: The compact framework gives insights in AG in healthy and diseased lung, at low-computational price. We observe that the bronchial NO production is probably underestimated in SG. Model improvments are ongoing, as results deviate for high asymmetry ratio. Conclusion: Our results confirm the importance of asymmetry in FeNO signals, especially in case of ACG. Furthermore, they open the way to new diagnosis approaches. FeNO scenarios could be used as templates of patients’ data and analyzed further, namely by deep learning methods.
To analyse current practice patterns and to evaluate (long-term) effectiveness and adverse events of sacral neuromodulation with InterStim™ Therapy based on data collected in a national register and to discuss the strengths and weaknesses of the register.This is a French multicenter prospective observational trial including patients with a permanent implant (2003-2009). Voiding diary variables and patient satisfaction were analysed based on last follow-up visit since implantation.One thousand four hundred and eighteen patients (median age: 63 years, 1206 females) were included in the database (median follow-up: 12 months). One thousand and eighty-nine patients had non-neurological disease. The principal diagnosis was overactive bladder syndrome ([OAB], n=1170) and retention (n=151). Implantation occurred in 1358 patients; 1172 patients had greater than or equal to one registered follow-up. Clinical improvement of greater than or equal to 50% was seen in 447/527 patients with OAB at 12 months follow-up (median number of voids per 24 hours decreased from 15 at baseline to 8 at 12 months) and in 42/54 of patients with retention (median number of catheterization per 24 hours dropped from 5 at baseline to 0 at 12 months). Clinical improvement remained relatively stable up to 60 months. Median patient satisfaction with treatment was between 60 and 80%. Five hundred and twenty-four patients had at least one adverse event; loss of efficacy (n=244) occurred most frequently.In this large database sacral neuromodulation with InterStim™ Therapy seems to be an effective treatment through routine practice in the long-term (up to 60 months) for patients refractory to medical treatment.

The molecular nature of calcium (Ca(2+))-dependent mechanisms and the ion channels having a major role in the apoptosis of cancer cells remain a subject of debate. Here, we show that the recently identified Orai1 protein represents the major molecular component of endogenous store-operated Ca(2+) entry (SOCE) in human prostate cancer (PCa) cells, and constitutes the principal source of Ca(2+) influx used by the cell to trigger apoptosis. The downregulation of Orai1, and consequently SOCE, protects the cells from diverse apoptosis-inducing pathways, such as those induced by thapsigargin (Tg), tumor necrosis factor α, and cisplatin/oxaliplatin. The transfection of functional Orai1 mutants, such as R91W, a selectivity mutant, and L273S, a coiled-coil mutant, into the cells significantly decreased both SOCE and the rate of Tg-induced apoptosis. This suggests that the functional coupling of STIM1 to Orai1, as well as Orai1 Ca(2+)-selectivity as a channel, is required for its pro-apoptotic effects. We have also shown that the apoptosis resistance of androgen-independent PCa cells is associated with the downregulation of Orai1 expression as well as SOCE. Orai1 rescue, following Orai1 transfection of steroid-deprived cells, re-established the store-operated channel current and restored the normal rate of apoptosis. Thus, Orai1 has a pivotal role in the triggering of apoptosis, irrespective of apoptosis-inducing stimuli, and in the establishment of an apoptosis-resistant phenotype in PCa cells.
Background The PERINICE network is dedicated to multi-field care of pelvic static disorders and adult urinary and/or anal incontinence in the Nord/Pas-de-Calais region. It is the typical network borne from the need to offer homogenous treatment and care through a network with all the characteristics of a high-quality health network: multi-expertise with over 500 health professionals specialised in one of the eight specialities of perineology geographical proximity with a group of centres allowing a direct access across the region within a range of less than 50 km professional training through the development of best practice recommendations, the training of students within 'mobility courses/internships' across the relevant expertises and on-the-job training assessment of professional practices ('EPP'), quality of care, improvement of the patient quality of life shared standardised IT medical file ('DMP') unique to all the network and IT database which enables evaluation, information sharing, virtual meetings and helps creating an epidemiologic register ('Register'). It is essential to improve medical as well as economical practices Programme: Overview, Development and Follow-up Expanded bladder is a common cause of urinary incontinence due to over-hydration, as well publicised by the media. The treatment received through the network and its specialists allowed to highlight this situation. Its discovery and associated best practice recommendations have been rapidly communicated to all the 500 professionals of the network. This information is disseminated in the network through the various means of communication: amendment to the DMP with the addition of the item expanded bladder' IT learning tool accessible through the PERINICE Web site ongoing 'on-the-job' training sessions discussions and exchanges during the multi-field meetings training and teaching of future professionals and physiotherapists through 'mobility internships' The general population also benefits from these developments given the information shared by informed professionals and the public access of the Web site: http://www.perinice.fr This allows to prevent vesical expansion through hydration and insufficient number of mictions. Clinical Results The data from the PERINICE Register suggest that based on 811 patients, 288 (ie, 30.7%) had an expanded bladder. These results confirm the clinical feel and reinforce the new recommendations of clinical best practice. Since then, the initial treatment we offer is to correct bad habits through self-reeducation including reduction of liquid intake and space between mictions. Self-reeducation itself has cured 35% of these patients while avoiding surgery. Discussion (Potential for Development, Restrictions) and Conclusion (Lessons Learnt and Messages for Others) As a result of data centralisation, the network allows to develop specific protocols that would not be identified by one isolated professional. The example of expanded bladder illustrates this well through preventing 35% of surgeries. The network brings together professionals who, through joint discussion, are at the heart of improving quality, cure and economical and health impact. In addition, the register, based on the network database and through epidemiologic and statistical data, allows to confirm, negate or scientifically demonstrate clinical feels without awaiting results of backdated studies with low meaning. Contexte Le réseau PERINICE, a pour objet la prise en charge transversale des troubles de la statique pelvienne et de lincontinence urinaire et/ou anale de l9adulte, du Nord Pas de Calais. C9est l9exemple type du réseau de soins né de la nécessité de proposer une prise en charge de qualité homogène en possédant toutes les caractéristiques d9un réseau de soins de qualité. Multidisciplinarité avec plus de 500 professionnels de santé issus des 8 spécialités impliquées dans la périnéologie. Proximité géographique avec un ensemble de centres permettant un accès au réseau sur toute la région dans un rayon de moins de 50km. Formation des professionnels, par l'élaboration de recommandations sur les bonnes pratiques, la formation des internes au sein de « stages de mobilité » et par la FMC. Evaluation des pratiques professionnelles (EPP), de la qualité des soins, de l9amélioration de la qualité de vie du patient. Informatisation du dossier médical partagé (DMP) commun à tous les centres du réseau, de la base de données, qui permet l'évaluation, le transfert de l9information, les réunions de concertation à distance, et participe à la création d9un registre épidémiologique. Elle est nécessaire à l9amélioration des pratiques tant au niveau médicale qu'économique. Programme: description, mise en œuvre, éléments de suivi La vessie distendue est une cause très fréquente d9incontinence urinaire secondaire à de mauvaises habitudes d9hydratation excessive, relayées par les médias. La prise en charge par le réseau et ses différents spécialistes a permis de mettre en évidence l9existence de cette entité clinique. Sa découverte, jusqu9alors ignorée, et les recommandations de bonnes pratiques cliniques qui en découlent ont pu être rapidement transmises à l9ensemble des 500 professionnels de santé du réseau. La diffusion de ces informations par le réseau s9opère par ses différents outils de communication: la modification du DMP par l9ajout de l9item « vessie distendues », l9E-learning accessible sur le portail internet de PERINICE, les sessions de Formation Continue, les discussions lors des réunions multidisciplinaires, la formation des futurs professionnels au sein des « stages de mobilités » et formation des MK. La population bénéficie à son tour de ces avancées par les informations véhiculées par les professionnels déjà informés et par l9accès « grand public » au site http://www.perinice.fr. Cela permet de prévenir la distension vésicale par hyperhydratation et insuffisance des mictions. Résultats en termes d9impact clinique La consultation du Registre de PERINICE nous rapporte que dans notre série de 811 patients, 288 soit 30,70% avaient une vessie distendue. Ces résultats confirment l9impression clinique et appuient les nouvelles recommandations de bonnes pratiques cliniques. Depuis nous proposons comme traitement initial de corriger les mauvaises habitudes par une auto-rééducation avec diminution des apports liquidiens et de l'écart inter-mictionnel. L9auto-rééducation seule, a permis de guérir 35% de ces patients leur évitant une intervention chirurgicale. Discussion (perspectives de développement, limites), conclusion (leçon apprises et messages pour les autres) Le réseau par la centralisation des données permet de mettre en évidence des caractéristiques diagnostiques et thérapeutiques qui resterait invisible au regard du praticien isolé. L9exemple des vessies distendues en évitant 35% d9interventions chirurgicales montre l9intérêt du réseau qui orchestrant la concertation entre les professionnels fait progresser les pratiques professionnelles vers une amélioration de la qualité, de la morbidité et de l9impact médico-économique. De plus le registre élaboré à partir de la base de données du réseau, permet, par des données épidémiologiques et statistiques, de confirmer, d9infirmer ou de prouver scientifiquement les impressions cliniques, sans attendre les résultats peu fiables d'études cliniques rétrospectives.
Connaître les indications et les bases d’interprétation en IRM dynamique du pelvis. Comprendre la prévalence de l’interprétation dynamique et fonctionnelle sur l’interprétation biométrique. Savoir quel trouble statique dépister devant quel symptôme clinique, quel antécédent opératoire et quel résultat d’exploration para-clinique préalable. Connaître les limites de l'IRM en sachant notamment déterminer le caractère pathologique ou non d’un trouble statique en IRM. Connaître le point de vue et les attentes du chirurgien face à l’IRM. Comprendre l’apport primordial d’une synthèse multidisciplinaire. Les indications de l’IRM dépendent des moyens et compétences diagnostiques en place. L’élytrocèle se manifeste essentiellement par une colpocèle postérieure mais parfois aussi par des douleurs ou une incontinence à la différence de l’hédrocèle dont la présentation clinique est proche du prolapsus rectal. Il est utile de connaître les différents types de prolapsus rectal (intus-susception, prolapsus interne, prolapsus intra-anal et prolapsus extériorisé), le prolapsus interne se manifestant essentiellement par une ulcération rectale. Le prolapsus extériorisé est par contre peu radio-logique. La rectocèle est presque exclusivement découverte à l’occasion d’une constipation terminale ou d’une colpocèle postérieure. Les anomalies fonctionnelles non symptomatiques sont nombreuses telles que le prolapsus rectal interne qui est le plus souvent asympto-matique mais aussi ou la petite rectocèle accompagnant un trouble statique pluri-compartimental. L’interprétation dynamique et fonctionnelle prime sur les repères, mesures et classifications notamment en matière de rectocèle ou le caractère « rétentionniste » parait primordial mais aussi en matière de prolapsus rectal interne ou l’exclusion de la poche de rectocèle souvent associée semble bien corrélée au caractère pathologique. Le prolapsus du cul-de sac de Douglas n’est pas toujours évident à mettre en évidence : la seule péritonéocèle sans contenu intestinal doit a priori être retenue comme pathologique source d’engagement intermittent ou secondaire du grêle et/ou du sigmoïde. La rectocèle n’est pas toujours primitive, elle peut notamment se développer en post-opératoire, par exemple en amont d’une myorra-phie ou d’un sphincter. L’IRM dynamique du pelvis ne doit pas être réalisée devant tout trouble de la statique pelvienne : elle trouve sa pertinence lorsque son indication est bien posée par le spécialiste d’organe et que son interprétation est réalisée en réunion multidisciplinaire, intégrée au contexte, aux antécédents, à l’examen clinique et aux explorations paracliniques complémentaires. Ceci permet une interprétation dynamique avec corrélation radio-clinique permettant de déterminer le caractère pathologique ou non des nombreuses anomalies dynamiques détectées par l’IRM, notamment au sein de ce compartiment postérieur ou la fréquence des troubles statiques asymptomatiques est particulièrement élevée.
Savoir l’anatomie échographique et la technique de l’échographie périnéale. Discuter des indications de l’échographie périnéale. Connaître la sémiologie de l’échographie périnéale dans les troubles de la statique pelvienne et en particulier dans l’étude des prothèses pelvi-périnéales. Intérêts de l’échographie périnéale dans les troubles de la statique pelvienne. Etude de la mobilité cervico-urétrale. Dépistage des cystocèles, des hystéroptoses, des péritonéocèles et des rectocèles. Prothèses : vérification de leur bon positionnement, recherche de complications : exposition, rétraction, abcès… Sphincter de l’anus : atrophie, rupture. L’échographie périnéale est un examen utile dans l’exploration des troubles de la statique pelvienne. Comme l’IRM, elle peut détecter des prolapsus. Son apport principal est dans l’étude des prothèses pelvi-périnéales (TVT, TVTO, TVM antérieur ou postérieur) pour vérifier leur bon positionnement et l’absence de complications de celles-ci : exposition, rétraction, complications infectieuses. Pour l’étude des prothèses, l’échographie périnéale est nettement supérieure à l’IRM de statique.L’échographie périnéale peut également permettre de vérifier l’intrégrité du sphincter de l’anus ou au contraire de mettre en évidence des atrophies sphinctériennes ou des ruptures.
Accumulating data point to K+ channels as relevant players in controlling cell cycle progression and proliferation of human cancer cells, including prostate cancer (PCa) cells. However, the mechanism(s) by which K+ channels control PCa cell proliferation remain illusive. In this study, using the techniques of molecular biology, biochemistry, electrophysiology and calcium imaging, we studied the expression and functionality of intermediate-conductance calcium-activated potassium channels (IKCa1) in human PCa as well as their involvement in cell proliferation. We showed that IKCa1 mRNA and protein were preferentially expressed in human PCa tissues, and inhibition of the IKCa1 potassium channel suppressed PCa cell proliferation. The activation of IKCa1 hyperpolarizes membrane potential and, by promoting the driving force for calcium, induces calcium entry through TRPV6, a cation channel of the TRP (Transient Receptor Potential) family. Thus, the overexpression of the IKCa1 channel is likely to promote carcinogenesis in human prostate tissue.
