Background: Patients with breast cancer (BC) show strong interest in complementary and alternative medicine (CAM), particularly for adverse effects of adjuvant endocrine treatment — e.g., with letrozole. Letrozole often induces myalgia/limb pain and arthralgia, with potential noncompliance and treatment termination. This analysis investigated whether CAM before aromatase inhibitor (AI) therapy is associated with pain development and the intensity of AI-induced musculoskeletal syndrome (AIMSS) during the first year of treatment. Patients and methods: The multicenter phase IV PreFace study evaluated letrozole therapy in postmenopausal, hormone receptor–positive patients with early BC. Patients were asked about CAM use before, 6 months after, and 12 months after treatment started. They recorded pain every month for 1 year in a diary including questions about pain and numeric pain rating scales. Data were analyzed for patients who provided pain information for all time points. Results: Of 1396 patients included, 901 (64.5%) had used CAM before AI treatment. Throughout the observation period, patients with CAM before AI treatment had higher pain values, for both myalgia/limb pain and arthralgia, than non-users. Pain increased significantly in both groups over time, with the largest increase during the first 6 months. No significant difference of pain increase was noted regarding CAM use. Conclusions: CAM use does not prevent or improve the development of AIMSS. Pain intensity was generally greater in the CAM group. Therefore, because of the risk of non-compliance and treatment discontinuation due to the development of higher pain levels, special attention must be paid to patient education and aftercare in these patients.

Der Receptor Activator of Nf-κB (RANK), sein Ligand (RANKL) sowie der lösliche Rezeptor Osteoprotegerin (OPG) sind mit Osteoporose postmenopausaler Frauen assoziiert sowie dem Auftreten von Knochenmetastasen des Mammakarzinoms. Ihre Expression unterliegt dem Einfluss der Sexualhormone Progesteron und Östrogen und scheint essentiell für Epithelzellproliferation zu sein. Ziel dieses Projektes war es den Zusammenhang zwischen der Serumkonzentration von RANKL/OPG und dem Brustwachstum während der Schwangerschaft zu untersuchen.
Background: Patients' compliance and persistence with endocrine treatment has a significant effect on the prognosis in early breast cancer (EBC). The purpose of this analysis was to identify possible reasons for non-persistence, defined as premature cessation of therapy, on the basis of patient and tumor characteristics in individuals receiving adjuvant treatment with letrozole. Patients and methods: The EvAluate-TM study is a prospective, multicenter, noninterventional study in which treatment with the aromatase inhibitor letrozole was evaluated in postmenopausal women with hormone receptor-positive EBC in the early therapy phase. Treatment persistence was evaluated at two pre-specified study visits after 6 and 12 months. As a measure of early therapy persistence the time from the start to the end of treatment (TTEOT) was analyzed. Cox regression analyses were carried out to identify patient characteristics and tumor characteristics predicting TTEOT. Results: Out of the total population of 3941 patients with EBC, 540 (13.7%) events involving treatment cessation unrelated to disease progression were observed. This was due to drug-related toxicity in the majority of cases (73.5%). Persistence rates were 92.2%, 86.9%, and 86.3% after 6, 12, and 15 months, respectively. The main factors influencing premature treatment discontinuation were older age [hazard ratio (HR) 1.02/year], comorbidities (HR 1.06 per comorbidity), low body mass index, and lower tumor grade (HR 0.85 per grade unit). Conclusion: These results support the view that older, multimorbid patients with low tumor grade and low body mass index are at the greatest risk for treatment discontinuation and might benefit from compliance and support programs.
Abstract Introduction The placement of intramammary marker clips has proven to be helpful for tumor localization in patients undergoing neoadjuvant chemotherapy and breast-conserving surgery. The purpose of our study was to investigate the feasibility of using a clip marker system for breast cancer localization and its influence on the imaging assessment of treatment responses after neoadjuvant chemotherapy. Patients and Methods Between March and June 2015, a total of 25 patients (n = 25), with a suspicion of invasive breast cancer with diameters of at least 2 cm (cT2), underwent preoperative sonographically guided core needle biopsy using a single-use breast biopsy system (HistoCore™) and intramammary clip marking using a directly adapted clip system based on the established O-Twist Marker™, before their scheduled preoperative neoadjuvant chemotherapy. Localization of the intramammary marker clip was controlled by sonography and digital breast tomosynthesis. Results Sonography detected no dislocation of intrammammary marker clips in 20 of 25 patients (80 %), while digital breast tomosynthesis showed accurate placement without dislocation in 24 patients (96 %) (p < 0.05). There was no evidence of significant clip migration during preoperative follow-up imaging after neoadjuvant chemotherapy. No complication related to the clip marking was noted and there was no difficulty in evaluating the treatment response to neoadjuvant chemotherapy. Among the breast-conserving surgeries performed, no cases were identified in which intraoperative loss of the marker clip had occurred. Conclusion Our study underscores the importance of intramammary marking clip systems before neoadjuvant chemotherapy. Placement of marker clips is advised to facilitate accurate tumor bed localization. With regard to digital breast tomosynthesis, its development continues to improve the quality of diagnostics and the therapy of breast cancer particularly for small breast cancer tumors or in neoadjuvant chemotherapy setting.

Die Teilnahmerate an den jährlichen Früherkennungsmaßnahmen ist niedrig. Eine Möglichkeit zur Optimierung besteht durch den Einsatz von qualifizierten Personen mit reduzierter Sehkraft, welche einen besonderen Tastsinn besitzen. Die medizinische Tastuntersucherinnen (MTU) erhalten eine von „discovering hands“ entwickelte standardisierte Ausbildung. Prospektive klinische Studien fehlen allerdings.
Objective of this randomized-controlled multicenter-trial was to determine whether the amniotic fluid index (AFI) or the single deepest vertical pocket (SDP) technique for estimating amniotic fluid volume is superior for predicting adverse perinatal outcome. Pregnant women at term were randomized in AFI or SDP group for assessing amniotic fluid at four unversitary hospitals. These two groups were not different with regards to primary outcome parameter (rate of postpartum admission to NICU), but concerning secondary outcome parameters: there were significantly more cases with the diagnosis oligohydramnios (9,8 vs. 2.2 %), more induction of labour for oligohydrmanios (12.7 vs. 3.6 %) and more abnormal CTG (32.3 vs. 26.2 %) in the AFI group. None of the methods were superior to predict adverse perinatal outcome. However, the probability to diagnose oligohydramnios and to induce labour for oligohydramnios was higher when AFI technique was used.
Zielsetzung: Die komplette pathologische Remission (pCR) nach einer neoadjuvanten Chemotherapie ist ein deutlicher Prognosefaktor bei Brustkrebspatientinnen. Ziel dieser Untersuchung ist es, das Ansprechen auf eine neoadjuvante Chemotherapie in Abhängigkeit von der prozentualen Östrogen- und Progesteronrezeptorpositivität sowie Ki-67 darzustellen.

Pregnancies and breastfeeding are two important protective factors concerning breast cancer risk. Breast volume and breast volume changes might be a breast phenotype that could be monitored during pregnancy and breastfeeding without ionizing radiation or expensive equipment. The aim of the present study was to document changes in breast volume during pregnancy prospectively. In the prospective Clinical Gravidity Association Trial and Evaluation programme, pregnant women were followed up prospectively from gestational week 12 to birth. Three-dimensional breast surface imaging and subsequent volume assessments were performed. Factors influencing breast volume at the end of the pregnancy were assessed using linear regression models. Breast volumes averaged 420 ml at the start of pregnancy and 516 ml at the end of pregnancy. The first, second and third quartiles of the volume increase were 41, 95 and 135 ml, respectively. Breast size increased on average by 96 ml, regardless of the initial breast volume. Breast volume increases during pregnancy, but not all womens' breasts respond to pregnancy in the same way. Breast volume changes during pregnancy are an interesting phenotype that can be easily assessed in further studies to examine breast cancer risk.
Einleitung: Die adjuvante Antihormontherapie wird für Frauen mit hormonrezeptor-positivem Mammakarzinom für mindestens 5 Jahre empfohlen. Etwa die Hälfte der Patientinnen brechen die Therapie mit Aromataseinhibitoren innerhalb von 3 Jahren aufgrund von Nebenwirkungen oder Non-Compliance ab, was in mehreren Untersuchungen zu einer Prognoseverschlechterung führte. Die Compliance kann unter anderem anhand der Einnahmedauer, also der Therapiepersistenz, bestimmt werden, was die Identifikation von Risikofaktoren für Therapieabbrüche ermöglicht.
Einleitung: In der CGATE (Clinical Gravidity Association Trial and Evaluation) PRIMA Studie werden Schwangerschaften, deren Verläufe und das Auftreten von verschiedenen Erkrankungen bzw. der Einfluss von schwangerschaftsassoziierten Veränderungen auf das weitere Leben untersucht, wie z.B. Veränderungen des Brustkrebsrisikos, Hypertonie und Diabetes mellitus.
Einleitung: Die Standardtherapie eines Bartholin'schen Empyems ist in Deutschland die sogenannte „Marsupialisation“, welche in Vollnarkose durchgeführt wird. Eine Alternative hierzu stellt der Word Katheter dar. Hierbei handelt es sich um ein altbewährtes Verfahren, das seit 40 Jahren in den USA im ambulanten Setting zum Einsatz kommt. Die Datenlage zur Anwendung des Word Katheters ist insbesondere hinsichtlich der Rezidivraten gut. Die Vorteile dieses Verfahrens (kurzer ambulanter Eingriff, minimal-invasiv, keine Narkose) veranlassen uns zu überprüfen, wie dieses zugelassene Verfahren von den Patientinnen als auch von den durchzuführenden Ärzten angenommen wird. Zudem soll die Durchführbarkeit dieses Verfahrens in einem ambulanten Setting evaluiert werden.
Die Compliance einer adjuvanten, endokrinen Therapie mit einem Aromatasehemmer bei Patientinnen mit einem Mammakarzinom wird von vielen Faktoren beeinflusst. Schmerzen, insbesondere Arthralgien, spielen hierbei eine große Rolle. Ziel der vorliegenden Arbeit war es den Schmerzverlauf unter einer Letrozoltherapie bei Patientinnen mit und ohne vorbestehender Schmerzsymptomatik zu vergleichen. Insgesamt wurden 3530 Patientinnen mit einem Mammakarzinom im Rahmen der PreFace-Trafo001-Studie, einer prospektiven, multizentrischen Phase IV Studie zur adjuvanten, endokrinen Therapie des Mammakarzinoms, mit dem Aromatasehemmer Letrozol behandelt. Unter anderem wurden Angaben zur Schmerzverteilung und Intensität zu den Behandlungsmonaten 1, 3, 6 und 12 anhand eines Patientinnentagebuches schriftlich und grafisch erhoben. 1240 Patientinnen wurden anhand von Vollständigkeit der Tagebücher und abhängig von Schmerzen (keine, mittlere Schmerzintensität, keine Schmerzen) vor der Diagnose des Mammakarzinoms ausgewählt. Von diesen wurden die gezeichneten Schmerzareale digitalisiert. Mithilfe einer eigens programmierten Software wurden Schmerzlandkarten zu verschiedenen Zeitpunkten berechnet. Zur Vergleichbarkeit wurden in den Schmerzlandkarten die maximale Schmerzausdehnung und die maximale Häufigkeit zu den Zeitpunkten errechnet. Anhand der Information in den Tagebüchern wurde die Schmerzstärke bestimmt. Patientinnen mit Schmerzen vor der Mammakarzinomdiagnose präsentierten zu jedem Zeitpunkt signifikant mehr Schmerzen als Patientinnen ohne Schmerzen vor Diagnose. Sie erreichten ihr Schmerzmaximum bereits nach drei Monaten, wohingegen Patientinnen ohne vorherige Schmerzen ihr Maximum erst nach sechs Monaten erreichen.
Einleitung: Regelmäßige Ultraschalluntersuchungen haben zum Ziel, die Sicherheit zur Entdeckung fetaler und mütterlicher Krankheitszustände in der Schwangerschaft zu erhöhen. Für die Prädiktion von Schwangerschaftsdauer und Geburtsgewicht gibt es jedoch nur wenige Prädiktoren aus dem Schwangerschaftsverlauf. Ziel dieser Pilot-Untersuchung war die Untersuchung und Identifikation von geeigneten Ultraschall Texturmerkmalen (engl.:„Texture features“) der Plazenta die die Schwangerschaftsdauer vorhersagen könnten.
Studienziel: Eine relevante Anzahl von Brustkrebspatientinnen wendet zusätzlich zur konventionellen Krebstherapie integrative Heilmethoden an. Insbesondere moderne Krebstherapien wie Aromatasehemmer sind mit deutlichen Nebenwirkungen wie Knochen- und Gelenkschmerzen verbunden, was die Anwendung von integrativen Heilmethoden zur Beschwerdelinderung triggern könnte. Ziel der hier vorgestellten Untersuchung ist die Erfassung des Gebrauchs von integrativen Heilmethoden in der adjuvanten Therapiesituation im Verlauf.
