Surgical site infection after head and neck surgery may be life threatening and induces increasing in healthcare cost. The objective of this present study was to identify predictive factors associated to surgical site infection in head and neck cancer surgery. Numerous predictive factors were checked for univariate case-control and multivariate analysis. This retrospective study included 71 patients that have been treated for cancer of the upper aerodigestive tract (UADT) during a one year period (2010). Control group were selected in this same perdio with a 1:2 ratio according to epidemiological profile and predictive factor for infection. Surgical site infection was defined according to the CDC classification. NNIS index was calculated. The incidence of surgical site infection was 15,5 %. Only the T3-T4 stages were identified as an independent predictive factor (P = 0,04) on multivariate analysis. The NNIS index, used by the Center for disease control and prevention (CDC) as the main predictive composite criteria for prediction of Surgical site infection was not correlated to a higher risk of infection. This study suggests that the current NNIS index may be not suitable for prediction of surgical site infection in UADT cancer. A specific predictive index should include the initial tumor stage for HNC surgery and could lead to a better adaptation of specific prophylactic antibiotic treatment.
De nombreux traitements complémentaires à la chirurgie ont été proposés pour le traitement des cancers différenciés à faible risque de récidive : 100mCi, 30mCi, et absence d'iode radioactif. La tendance actuelle est à diminuer la dose d'iode. Le but de cette étude est d'évaluer l'efficacité de l'administration d'iode chez ces patients. Les dossiers des patients qui ont présenté un CDT sur une période de 10 ans ont été étudiés de manière rétrospective. Seuls les patients à faible risque de récidive selon le définition du consensus européen ont été inclus (T1b, T1m ou T2, N0Nx). Le traitement chirurgical, l'histologie, la biologie, les modalité de l'administration d'iode radioactif ont été étudiés. La rémission a été définie par une thyroglobulinémie basse avec une imagerie négative lors du bilan à 6 mois après le traitement chirurgical. Une analyse statistique uni et multivariée a été réalisée en fonction des traitements reçus et des facteurs de risque de récidive actuellement reconnus. Cent quinze patients ont été inclus avec un suivi moyen de 67 mois. Onze patients (9,5 %) n'ont pas eu d'irathérapie. La rémission a pas été obtenue chez 94 patients (81,7 %). L'analyse multivariée a révélé que la non-rémission était significativement corrélée à la plurifocalité (OR = 3,28 IC95 % 1 ; 10 762) et à l'effraction de la capsule tumorale (OR = 5991 IC95 % 1294 ; 27 732). Cette étude suggère que dans une population principalement traitée par administration de 100mCi d'iode 131, le taux de non-rémission reste élevé pour des cancers à faible risque. En particulier pour les tumeurs plurifocales et/ou en rupture capsulaire, des doses plus élevées peuvent être nécessaires pour obtenir la rémission.
Les cancers des sinus sont des tumeurs rares, agressives et de diagnostic tardif. Elles diffèrent des autres tumeurs des VADS par le facteur de risque (poussières de bois), les lésions prénéoplasiques (papillomes inversés). Le diagnostic est évoqué devant une symptomatologie rhinosinusienne unilatérale ou continue ou d’une lyse osseuse ou d’un comblement unilatéral et hétérogène à l’imagerie. Le diagnostic de certitude est histologique. Le bilan doit comporter une TDM injectée du massif facial, encéphalique et cervicothoracique et une IRM du massif facial et encéphalique. Dans deux tiers des cas, les tumeurs sont classées T3T4 et l’envahissement ganglionnaire ne concerne que 10 % des carcinomes épidermoïdes et 4 % des adénocarcinomes. Ces tumeurs doivent être prises en charge par des centres de référence rompus à toutes les modalités thérapeutiques. La décision de traitement est prise en RCP et basée sur l’expertise locale, les thesaurus régionaux et surtout les recommandations nationales du REFCOR. Le traitement de choix est une résection en marges saines suivie d’une irradiation conformationnelle 3D et avec modulation d’intensité (IMRT) si possible. Malgré des améliorations dans les dernières décennies, le pronostic reste mauvais : 38 % de récidives locales et 63 % de survie à cinq ans.

Reconstruction of segmental substance loss is warranted by the repercussions on the vital prognosis when it is anterior and on the quality of life when it is posterior. The ideal means of reconstruction is the free fibular flap, which is limited by cost, morbidity of the donor site, and selection of the patient's surgical team. For these reasons, it may be necessary to turn to pedicled osteo-myocutaneous flaps, abandoned because of their reputedly very high failure rate, but few have been reported in the literature.Poor functional results of mandibular reconstruction plates make this a last-resort solution. Tissue engineering is currently the most promising line of research. It runs counter to the principles of oncology itself because postoperative radiotherapy reduces the osteoinduction potential of the biomaterials proposed.
