L'intertrigo est une pathologie inflammatoire commune aux origines diverses. Il peut etre isole ou faire partie d'une maladie se manifestant ailleurs sur le corps. L'intertrigo peut donc n'etre qu'un signe d'une affection particuliere qui necessite un traitement specifique. Il peut aussi etre le reflet d'une agression physico-chimique de la peau soumise a l'occlusion des grands plis fermes, particulierement en cas d'embonpoint. Dans ce cas, un traitement similaire a celui de la dermite fessiere des nourrissons peut etre propose. Une pâte contenant ou non du nitrate de micronazole est indiquee pour prevenir ou traiter la plupart des intertrigos.
Ehlers-Danlos syndrome (EDS) is heterogenous with regard to genetic traits, clinical manifestation, the biomechanical and microscopic properties of connective tissues, and basic molecular defects. We report on nine relatives of four generations who suffered from large vessel dissections and cutaneous microscopic changes consistent with EDS. Measurements of the mechanical properties of skin were performed using a computerized suction device (Cutometer®). Morphological and biomechanical alterations suggestive of EDS were present in all examined subjects. A loose network of collagen bundles was admixed with clumsy elastic fibres. Factor XIIIa-positive dermal dendrocytes looked almost normal but were slim and rarefied in four subjects. The severity in ultrastructural alterations of the collagen network differed among the subjects. The group with the most prominent changes showed the most striking biomechanical alterations characterized by increased biologic elasticity without any excess in skin extensibility. A positive correlation was found between skin extensibility and elasticity. In conclusion, distinct alterations in the collagen scaffolding were found to be correlated to variable severity in biomechanical alterations of the skin. The predictive value of these changes for large vessel dissections in some families at risk remains to be settled.
Intertrigo is a common inflammatory disorder of various origins. It can represent a single skin manifestation or be part of a disorder possibly exhibiting specific manifestations on other parts of the body. Hence, intertrigo may represent a sign of a peculiar disease. It may also result from physicochemical aggressions of the skin following occlusion of the large skinfolds particularly in case of stoutness. In this intance, a treatment similar to that of diaper dermatitis can be offered. A paste enriched or not with miconazole nitrate is indicated to prevent or treat most presentations of intertrigo.

Only recently, an "epidemic" situation emerged, affecting hundreds of subjects who shared a status of administrative employee.OBSERVATION:The older etiopathogenic hypotheses give way to a newer one implying the electromagnetic fields generated by computers and their wirings. The resulting modifications in the intrinsic bioelectrical properties of the skin could influence the biology of macrophages exhibiting lipophagic activity in the hypodermis. The electroactivation of these cells could lead to TNF-alpha release.DISCUSSION:The semicircular lipoatrophy of the thighs is a problem that may affect an employee out of two. Rather than hoping an efficaceous drug therapy, prevention must be advocated by adapting the work conditions related to the use of computerized devices.
Un granuloma por cuerpo extraño es una lesión crónica constituida sobre todo por células de las líneas monocítica y macrofágica. La función de fagocitosis suele estar bien desarrollada. Esta reacción puede estar inducida por diversas macroestructuras de naturaleza xenobiótica, por micropartículas exógenas cristalinas o amorfas, así como por algunas estructuras de origen endógeno. En determinados contextos clínicos es difícil establecer una distinción clara entre una reacción por cuerpo extraño pura y un granuloma con componente inmunógeno.
The combination of calcipotriol and betamethasone dipropionate in a single topical formulation is a recent advance in treatment of plaque type psoriasis. Studies rooted on evidence-based medicine have been recently published. We report a synthesis and a critical analysis, critical analysis of these investigations, and translate their data with a view to apply them in daily practice.

The classical management of acne calls for prolonged oral and/or topical treatments; however, some patients request a rapid effect to make the papules disappear within a few hours or days.To test the effect of a single overnight application of a paste containing 0.25% miconazole nitrate on acne papules, and comparison with the effect of the same but unmedicated paste.Narrow-band reflectance spectroscopy was used to assess the changes in E index (erythema) after overnight application of the pastes. In the first study, a total of 117 acne papules were assessed in 15 adolescents. Measurements of the E index were performed at 24-h intervals for 4 days on acne papules and the surrounding normal-looking skin. The last two assessments were performed the mornings preceding and following, respectively, a single application of the medicated paste. The second part of the study consisted of a double-blind, split-face study on 25 adolescents with acne. They applied each of the two formulations on a randomized part of the forehead. A total of 161 acne papules received the medicated paste and 160 received the same but unmedicated paste. Measurements were performed the mornings before and after the overnight treatment.In the first part of the study, no significant daily difference was observed between the three series of pretreatment E indices, on both acne papules and normal skin. The treatment did not affect the E index of normal skin. By contrast, it decreased significantly the E index of the acne papules. The second part of the study also showed that the medicated paste significantly decreased the E index of acne papules. This effect was significantly (P < 0.05) superior to that of the unmedicated paste.A single overnight application of miconazole nitrate paste appears to reduce the erythematous aspect of acne papules.
Myospherulosis is a rare disorder that may develop in various organs including the skin. It is characterized by a typical histological presentation resulting from the interaction between erythrocytes and lipids. We present a case of cutaneous myospherulosis associated with membranous lipodystrophy. The lesions were supervening on severe steroid-induced skin atrophy upon which ointments had been applied.
L'association du calcipotriol et du dipropionate de betamethasone en une seule formulation topique est une avancee recente dans le domaine du traitement du psoriasis en plaques. Des etudes conduites dans l'esprit de la medecine basee sur l'evidence ont ete publiees recemment. Nous en presentons une synthese et une analyse critique en integrant et traduisant les resultats afin de les appliquer en pratique quotidienne.
A number of relationships exist between diabetes mellitus and a series of cuteanous disorders. Skin lesions may reveal diabetes in a more or less specific way. They may also represent complications supervening in an already treated diabetic patient. Some of these dermatoses (acanthosis nigricans, purpuric and pigmented capillaritis) are markers of macrovascular complications. The same disorders and some others (xerosis, Dupuytren's disease) are more frequently associated with microangiopathy in Type II diabetes. Other skin diseases (alopecia areata, vitiligo) are markers of auto-immunity in Type I diabetes.
Un granulome à corps étranger est une lésion chronique constituée de manière prépondérante par des cellules de la lignée des monocytes et des macrophages. La fonction de phagocytose y est habituellement bien développée. Cette réaction peut être induite par diverses macrostructures de nature xénobiotique, des microparticules exogènes cristallines ou amorphes, ainsi que par quelques structures d'origine endogène. La distinction entre une réaction purement xénique et un granulome à composante immunogène n'est pas aisée à définir dans certains contextes cliniques.
Ethnic (racial) differences exist for a series of physiological skin characteristics. Colour is responsible for typology and resistance to ultraviolet irradiations. Other differences have also been reported for the transepidermal water loss, the size and cohesion of corneocytes, the vascular reactiviy, the biomechanical properties, the pH, the lipidic content, the superficial microflora and the structure of mast cell granules.
Despite improvements in antifungal strategies, the outcome of treating onychomycoses often remains uncertain. Several factors account for treatment failure, of which the pharmacokinetics and pharmacodynamics of the antifungal are of importance. The taxonomic nature and ungual location of the fungus cannot be neglected, besides the type of nail and its growth rate. In addition, the biological cycle of the fungus and the metabolic activity of the pathogen likely play a marked influence in drug response. The presence of natural antimicrobial peptides in the nail is also probably a key feature controlling the cure rates. There are many outstanding publications that cover the full spectrum of the field. The purpose of this review is to put in perspective some facets of activity of the topical treatment using amorolfine nail laquer. The antifungal activity of the drug is likely less pronounced in onychomycosis than that expected from conventional in vitro studies. However, the nail laquer formulation should reduce the propensity to form antifungal-resistant spores and limit the risk of reinfection.
Il existe des differences ethniques (raciales) pour un ensemble de caracteristiques physiologiques de la peau. La couleur est responsable de la typologie et de la resistance aux rayonnements ultraviolets. D'autres differences ont ete egalement suggerees au niveau de la perte insensible d'eau, de la taille et de la cohesion corneocytaire, de la reactivite vasculaire, des proprietes biomecaniques, du pH, du contenu lipidique, de la microflore de surface et de la structure des mastocytes.
The topical treatment of psoriasis benefits from the alternate use of dermocorticosteroids and vitamin D3 analogues. A new galenic formulation allows to combine them in a single application. Dovobet (LEO Pharma) ointment is the association of calcipotriol 50 microg/g with betamethasone dipoprionate 0.5 mg/g. This formulation boosts the therapeutic activity of calcipotriol. It also decreases the irritative inflammatory reaction due to calcipotriol without increasing the atrophogenic risk of the dermocorticoid.
