Certaines eaux conditionnées, eaux minérales naturelles (EMN, eaux de source principalement, peuvent avoir un contenu minéral à l’origine d’effets cliniques qui méritent d’être mentionnés pour l’information des consommateurs. L’effet diurétique est lié au volume d’eau et à la vitesse d’ingestion, mais les minéraux ne sont pas susceptibles de jouer un rôle concret ; il n’y a donc pas matière à mention. Les eaux bicarbonatées (à partir de 600mg/L d’hydrogénocarbonate) facilitent la digestion en agissant sur le transit gastroduodénal et les fonctions hépatobiliaires. Les eaux sulfatées (à partir de 200mg/L chez l’adulte et 140mg/L chez l’enfant) sont susceptibles d’accélérer le transit intestinal et d’avoir un effet laxatif qui est accru si les eaux sont riches en magnésium (à partir de 50mg/L). Ces effets doivent faire l’objet de mentions. La carence en fluor entraîne des caries dentaires que la fluoration des eaux de consommation humaine dans les limites fixées par les recommandations internationales permet de prévenir sans effet délétère, dentaire ou osseux. Un apport fluoré excessif conduit à une altération de la structure et de la qualité des dents et du squelette. Un apport supplémentaire de fluor qui serait bénéfique à la santé osseuse n’est pas déterminé à ce jour. Les apports quotidiens ne doivent pas dépasser 0,05mg/kg de poids corporel par jour. Les nourrissons et jeunes enfants ne doivent pas consommer une eau dont la concentration en fluor soit supérieur à 0,3mg/L s’ils font l’objet d’une supplémentation médicale en fluor. La composition physicochimique essentielle de toutes les eaux destinées à la consommation humaine doit être communiquée aux consommateurs de manière lisible. Les boissons, préparées à partir d’eaux conditionnées et addition de nutriments d’autre nature, doivent porter une information nutritionnelle adaptée à la consommation aux divers âges de la vie. Les eaux rendues potables par traitement peuvent exposer à des carences minérales encore mal évaluées. La consommation au long cours d’une eau conditionnée, EMN en particulier, devrait t être approuvée par le médecin traitant.
Occupational medicine is a human victory of our time. It is in danger: there is a significant shortage of doctors in the two professions of physician practitioners and occupational physicians that will not be compensated by replacing one with the other. To strengthen its attractiveness, it is necessary to stick to some basic principles: - occupational medicine is a specialty requiring in-depth training in toxicology, occupational pathology by organs and professions, occupational physiology and ergonomics, and finally hygiene and safety legislation; - the practice of occupational physician requires an appropriate training and knowledge. Some of the functions of the occupational physician cannot be performed at all by other actors without them necessarily having an appropriate education; - the physician practitioner may encounter situations that involve occupational medicine, in particular the detection of occupational diseases in his/her city office, which should lead to primary prevention and care actions if the disease is ongoing, but also to the knowledge of non-professional environmental factors which should be part of his/her compulsory training; - it is necessary to facilitate a close professional relationship between occupational physicians and physician practitioners for the benefit of the patient and for an appropriate practice of occupational medicine. The shared medical record cannot be fully available to all medical doctors, without several guarantees; - the rote of occupational nurses may be extended under the responsibility of the occupational physician, in particular with regard to vaccinations and nurse consultations with a protocol that could be national and made available to the occupational services to avoid too much difference from one city or region to another. The new law of 2 August 2021 increases the tasks and responsibilities of the occupational physician and thus increases the attractiveness to this profession as long as there is a consistent teaching of medicine and occupational health in all faculties of medicine, to avoid the lack of information on an exciting but too often unknown or untaught discipline. La médecine et santé au travail est une victoire humaniste de notre époque. Elle est en danger : il existe une pénurie importante de médecins pour les deux métiers de médecin généraliste et de médecin du travail que l’on ne compensera pas en remplaçant les uns par les autres. Pour renforcer l’attractivité envers la médecine et santé au travail, il convient de fixer quelques principes de base : – la médecine du travail est une spécialité nécessitant une formation approfondie en toxicologie, pathologie professionnelle par organes et par métiers, physiologie du travail et ergonomie et, enfin, législation d’hygiène et sécurité ; – de nombreuses fonctions du médecin du travail ne peuvent absolument pas être remplies par d’autres acteurs sans qu’ils aient obligatoirement un enseignement approprié ; – le médecin généraliste peut rencontrer des situations qui font appel à la médecine du travail, notamment le dépistage de maladies professionnelles à son cabinet de ville, qui devraient conduire à des actions de prévention primaire et de réparation si la maladie s’est installée, mais aussi à la connaissance de facteurs d’environnement non-professionnels qui devraient faire partie de sa formation obligatoire. Il faut faciliter la relation professionnelle étroite entre médecin du travail et généraliste dans l’intérêt du patient et pour un bon exercice de la médecine du travail. Le dossier médical partagé ne peut être totalement disponible pour les différentes sortes de médecins, sans certaines garanties ; – le rôle des infirmiers du travail peut être étendu sous la responsabilité du médecin du travail, notamment en matière de vaccinations et d’entretiens infirmiers, mais seulement dans le cadre de délégations précises avec un protocole qui pourrait être national pour éviter trop de diversités locales ou régionales. La nouvelle loi du 2 août 2021 augmente les tâches et les responsabilités du médecin du travail et augmente ainsi l’attractivité vers cette profession à condition que l’enseignement de la médecine et santé au travail soit présent et conséquent dans toutes les facultés de médecine, pour éviter le phénomène d’absence d’information sur une discipline passionnante, mais trop souvent méconnue ou non enseignée.
Packaged waters, mainly natural mineral waters (NMW) and spring waters, may have a mineral content causing clinical effects which deserve to be mentioned for the information of consumers. The diuretic effect is related to the volume of water and the speed of ingestion, but minerals are not likely to play a concrete role; so there is nothing to mention. Bicarbonate waters (from 600 mg/ L of hydrogen carbonate) facilitate digestion by acting on gastroduodenal transit and hepatobiliary functions. Sulfated waters (from 200 mg/L in adults and 140 mg/L in children) are likely to accelerate intestinal transit and have a laxative effect which is increased if the waters are rich in magnesium (from 50 mg/L). These effects must be mentioned. Fluoride deficiency leads to dental caries that fluoridation of water for human consumption within the limits set by international recommendations can prevent without dental or bone deleterious effects. An excessive intake of fluoride leads to an alteration in the structure and quality of teeth and the skeleton. An additional intake of fluoride that would benefit bone health is not determined at this time. Infants and young children should not consume water with a fluoride level greater than 0,3 mg/L if they are receiving medical fluoride supplementation. The essential physicochemical composition of all water intended for human consumption must be communicated to consumers in a legible manner. Beverages, prepared from waters (NMW, spring waters) and the addition of other nutrients, must carry nutritional information suitable for consumption at different stages of life. Water made drinkable by treatment can expose to mineral deficiencies that are still poorly assessed. Long-term consumption of packaged water, natural mineral water particularly, would t be approved by the attending physician. (C) 2022 l'Academie nationale de medecine. Les tatouages cutanés sont pratiqués depuis fort longtemps mais leur toxicité reste peu connue du grand public qui a de plus en plus recours à ces techniques sans être informé des risques encourus, infectieux, allergiques, dermatologiques, toxiques systémiques et même ophtalmologiques. Cette situation est aggravée par la mode des tatouages « corps entier ». Cette séance dédiée est une mise au point des risques encourus liés aux tatouages.
Skin tattoos have been used for a long time but their toxicity remains little known to the general public who increasingly resort to these techniques without being informed of the risks involved, infectious, allergic, dermatological, systemic toxic and even ophthalmological. This situation is compounded by the fashion for "whole body" tattoos. This dedicated session is an update on the risks associated with tattoos. Suite à plusieurs cas de dermites de contacts au métabisulfite de sodium diagnostiqués à la Consultation de Pathologie Professionnelle et Environnementale du CHU de Nantes, nous nous sommes fixés deux objectifs : – établir une revue détaillée des cas recensés au sein du RNV3P ; – réaliser une revue de la littérature dans la base de donnée PUBMED des cas de dermite de contact au métabisulfite de sodium impactant la pharmacovigilance. (a) Chaque consultation à la CRPPE donne lieu à la saisine du diagnostic dans la base nationale du RNV3P. Nous avons interrogé la base sur les cas recensés entre 2001 et 2017 avec les mots clés : suivant « dermite de contact allergique, sodium métabisulfite, métier ». (b) Nous avons conduit avec les mêmes mots clés : une revue de la littérature dans Pubmed. (a) 45 cas de dermite de contact au métabisulfite de sodium ont été retrouvés avec augmentation de la prévalence à partir de 2011. 35 % des cas présentent une polysensibilisation : les tests positifs associés sont les antiseptiques/conservateurs (méthylisothiazolinone, thiomersal, benzylhémiformal, formol, phenoxyéthanol, Kathon, BHT..) et les métaux (Ni, Cr, Co). Les activités/métiers les plus représentées sont : – secteur des soins avec une prévalence de 34 % (IDE/IBODE/chirurgien/anesthésiste/étudiant/AS/ASH) ; – la métallurgie : 33 % (ajusteur/monteur, tourneur/fraiseur, mécanicien/maintenance des machines, monteur/assembleur, agent logistique) ; – le secteur agro-alimentaire : 12 % (boulanger, charcutier, cuisinier). (b) La législation européenne a attribué au métabisulfite de sodium le code E224 en tant que conservateur contenu dans les aliments. On le retrouve aussi dans les cosmétiques, les teintures capillaires, les produits de frisage/lissage des cheveux, les autobronzants, des additifs du collagène en esthétique. La revue de la littérature montre clairement l’existence de cas de réactions anaphylactiques et dermites de contact après anesthésie chez ces patients imputables au sodium métabisulfite. Nous avons montré l’importance de signaler l’allergie au métabisulfite de sodium aux médecins/chirurgiens vu qu’il entre dans la composition d’antalgiques/anesthésiques ; sur le plan professionnel, le secteur de soins et le secteur industriel doivent faire l’objet d’une attention particulière ; une base de données nationale des produits industriels contenant le métabisulfite de sodium, notamment les huiles de coupe, les lubrifiants est en cours de constitution avec l’appui de la CARSAT des Pays de Loire.
The building and public works sectors cover many activities with multiple skin hazards: this includes exposure to cement, acids, solvents and paints, to various adhesives and glues including epoxy and other very allergenic resins, to wood dusts (for joiners, carpenters and cabinet makers), carcinogenic hydrocarbons which affect those who work with asphalt in road construction, and crystalline silica dust which is responsible for contact dermatitis and scleroderma in stone masons, miners and potters. The study analyzes health care workers’ (HCWs) occupational risk perception and compares exposure to occupational risk factors in Moroccan and French hospitals.
International studies on occupational risks in public hospitals are infrequent and only few researchers have focused on psychosocial stress in Moroccan Health Care Workers (HCWs). The aim of this study was to present and analyze Moroccan HCWs occupational risk perception. Across nine public hospitals from three Moroccan regions (northern, central and southern), a 49 item French questionnaire with 4 occupational risks subscales, was distributed to 4746 HCWs. This questionnaire was based on the Job Content Questionnaire. Psychosocial job demand, job decision latitude and social support scores analysis were used to isolate high strain jobs. Occupational risks and high strain perception correlation were analyzed by univariate and multivariate logistic regression.
Many factors influence the return to work of workers with chronic low back pain (CLBP). They have been said to vary according to socio-professional group. This study first aimed to compare prognostic factors influencing the return to work of CLBP healthcare workers (HCWs) and other workers (non-HCWs) after rehabilitation coupled with an occupational intervention. The second objective was to improve the evolution of indicators such as clinical examination, psychosocial impact and pain impact.
La main est fréquemment le siège d’accidents de travail ou de maladies d’origine professionnelle.Les troubles musculo-squelettiques des membres supérieurs sont la première maladie professionnelle reconnue et le syndrome du canal carpien est le plus fréquent d’entres eux.La main est la localisation la plus fréquente des dermatoses professionnelles. Leurs causes sont très souvent multifactorielles, associant des irritants chimiques, physiques, allergènes et des facteurs endogènes (principalement la dermatite atopique).L’exposition professionnelle à des microtraumatismes itératifs et l’utilisation d’outils vibrants peuvent aussi être la cause de syndrome du marteau hypothénar et d’acrosyndromes.La chronicisation fréquente et la gêne fonctionnelle qu’induisent ces atteintes peuvent entraîner un handicap durable, source d’une inaptitude au poste de travail.Le médecin du travail est un interlocuteur privilégié pour l’aide au maintien au poste et pour la prévention de la désinsertion socioprofessionnelle.De nombreuses pathologies de la main liées à l’activité professionnelle peuvent bénéficier d’une déclaration en maladie professionnelle, permettant au patient d’obtenir une indemnisation et la protection de l’emploi.La prévention des lésions des mains d’origine professionnelle doit être mise en œuvre par tous les acteurs de santé, notamment pour les professions et les secteurs d’activité à risque.Hand is frequently the site of work accidents or occupational diseases. The musculoskeletal upper limb is the first recognized occupational disease and carpal tunnel syndrome is the most common of them.The most common location of occupational dermatoses is the hand. Their causes are often multifactorial, involving chemical irritants, physical, allergens and endogenous factors (mainly atopic dermatitis).Occupational exposure to microtrauma and iterative use of vibrating tools may also be the cause of hypothenar hammer syndrome and acrosyndromes. The frequent chronicity and functional impairment induced by these attacks can cause lasting disabilities, an inability to source workstation.Occupational physician is a focal point for helping to maintain the position and the prevention of socioprofessional disinsertion.Many pathologies of the hand related to professional activity may benefit from a statement in occupational disease and thus allow the patient to obtain compensation and employment protection.Prevention of occupational hand diseases should be made by all health actors, especially in occupations and industries at risk.
The occupational skin diseases affecting mechanics appear to represent approximately 10% of the cases of occupational dermatitis. Irritation or allergic skin diseases are observed. Irritants factors are multiple and the major allergens are the preservatives in oils and greases but also in soaps. Metals (chromium and cobalt) and rubber are often encountered allergens. In the metal machining workers irritation dermatitis are observed in relation with caustic effect due to the presence of amines and preservatives in too high concentration. BACKGROUND:Nantes University Hospital comprises 20 activity sectors. AIMS:To investigate the role of the work environment at the individual level, as well as the workplace level, in explaining the variability in employees' perception of stress. METHODS:A self-administered enhanced Karasek Job Content Questionnaire was sent to employees. The main variables were the psychological job demand (PJD) score and the job decision latitude (JDL) score. Univariate and multivariate logistic regression analyses were conducted to estimate crude odds ratio (OR) and adjusted OR. RESULTS:One thousand eight hundred and sixty-eight workers were included. Nursing managers (25.9 ± 3.4), non-specialized nurses (25.6 ± 3.5) and physicians (25.3 ± 3.4) had the highest PJD. Cleaning staff (61.4 ± 11.4) and nurse aides (63.6 ± 8.8) had the lowest JDL. Items correlated with high PJD are: unacceptable work schedule, adjusted OR 2.16 (95% CI = 1.3-3.5); unsatisfactory workstation accessibility, OR 1.92 (95% CI = 1.1-3.2); getting from A to B, OR 1.67 (95% CI = 1.2-2.4); and heavy manual handling, OR 1.62 (95% CI = 1.1-2.3). Sleeping tablet use was linked to high PJD (P < 0.01), extra workload (P < 0.05) and tiredness (P < 0.05). Use of painkillers was correlated with musculoskeletal disorders (P < 0.05). CONCLUSIONS:Our study highlighted women >40 years old, nurse managers, physicians, permanent and/or full-time workers having a high PJD. Nursing aides, medical secretary and nurses presented with high strain. Better control measures should be implemented for those socioprofessional categories to improve prevention measures. This study should be repeated in the future with a multi-centre approach to determine the generalizability of the findings.
OBJECTIVES/HYPOTHESIS:Wood dust is a well-established risk factor for intestinal type sinonasal adenocarcinoma. The 5-year overall survival has varied from 20% to 80% according T1-T4 stages; 5-year survival according to histologic subtype has varied from 20% to 50%. To date, no study has evaluated whether environmental, occupational, and personal risk factors have any impact on both overall and cancer-specific survival. We aimed to determine whether exposure to carcinogenic risk factors besides wood exposure can influence the survival of patients with sinonasal ethmoid carcinoma.STUDY DESIGN:Retrospective cohort study of the association of survival data and occupational and personal carcinogenic risk factors.METHODS:All patients hospitalized for ethmoid adenocarcinoma at the Nantes University Hospital between 1988 and 2004 were included . Data concerning TNM classification, histology, type and quality of tumor resection at the macro- and microscopic level, and occupational and personal exposure to carcinogens were collected. Statistical analysis was conducted using univariate and multivariate linear regression.RESULTS:A total of 98 patients were included with a response rate of 98%. Data showed 86% of patients had been exposed to wood dust. The 5-year survival was 62%. We first identified four factors that independently influenced overall survival: diplopia (P = .0159), spread to the orbit (P = .0113), bilateral involvement (P = .0134), TNM stage (P < .001). When the analysis included all occupational environmental factors (wood dust, solvent, and metals exposure) as well as personal risk factors, the length of exposure to metals (P = .0307) and tobacco exposure (P = .0031) also were found to influence 5-year overall survival. We identified high prevalence of colon cancer (4%) and double cancer (18%).CONCLUSIONS:We showed exposure to both environmental (tobacco) and occupational (metal dust) factors could influence survival in the diagnosis of a cancer. Our study suggests that screening for colon cancer should be offered to wood dust workers. A prospective multicentric study should be necessary to confirm our results.
Contact dermatitis to cutting fluids is a very frequent cause of occupational skin disease, with different mechanisms for those caused by irritation or allergies. The different allergens in question are many and complex. Different batteries of patch tests are available but the interpretation of the relevance of positive tests is not simple. The aim of this work is to make clearer the investigative methods and the interpretation of the relevance of the results of patch tests, for more effective prevention.
