Fulani in Africa are known to be less susceptible to Plasmodium falciparum (Pf) malaria. This study explored a potential involvement of antibody-mediated merozoite phagocytosis mechanism in this natural protection against malaria.Before the start of the malaria transmission season (MTS) in Benin, the functionality of antibodies against Pf merozoites was determined by the opsonic phagocytosis (OP) assay in plasma samples from Fulani, Bariba, Otamari and Gando groups. These individuals were actively followed-up for malaria detection from the beginning to the end of MTS. Anti-GLURP Immunoglobulin G antibody quantification, malaria Rapid Diagnostic Test (RDT) and spleen palpation were performed before and after MTS.In Bariba, Otamari and Gando, but not in Fulani, plasma from adults promoted higher levels of OP than the children (P = 0.003; P = 0.012; P = 0.031 and P = 0.122). A high proportion of Fulani children had higher OP and anti-GLURP (P < 0.0001) antibody levels as compared to non-Fulani children; whereas this was not observed for Fulani adults (P = 0.223). High OP levels before MTS were significantly related to negative RDT after MTS (P = 0.011).Our results highlight the ability of opsonizing antibodies to potentially enhance natural protection of young Fulani individuals against Pf malaria in Benin.
Mite parasites can be transmitted from animals to humans and cause prurigo.We describe a case of mite transmission in a 75-year-old woman referred for pruritus and erythematous maculopapular rash. On clinical examination mites were seen on the patient's skin. The mites were collected and characterized using microscopy. The species was identified as Dermanyssusgallinae, also known as the poultry red mite, an ectoparasite that commonly infests bird nests. The source of the patient's contamination was her henhouse, where mites were found in the wooden beams. Molecular analysis by mitochondrial DNA sequencing was performed on a mite collected from the patient and on a mite collected from the henhouse. This analysis confirmed that both belonged to the D. gallinaes.str species, and that the source of contamination was poultry farming.This case describes transmission to a human of the mite D. gallinaes.str via hens, resulting in prurigo, as confirmed by morphological and molecular analysis.

Concordance of severity scores assessed on photographs in Hidradenitis suppurativa
L'hidradénite suppurée (HS) est une maladie inflammatoire chronique de la peau dont la prise en charge est complexe. Une évaluation photographique du patient permet de conserver la mémoire d'un état clinique. Les images peuvent ainsi être interprétées par plusieurs évaluateurs notamment par un groupe d'experts, et à distance comme dans le cadre de la télémédecine. Les photographies peuvent également être utilisées pour l'évaluation d'un critère de jugement principal en aveugle, dans les essais thérapeutiques. L'objectif principal de cette étude était d'évaluer la concordance entre experts de scores d'activité clinique réalisés sur photographies de patients ayant une hidradénite suppurée. Vingt-six patients vus consécutivement lors d'une consultation spécialisée de patients ayant une HS ont été inclus. Chaque patient a été examiné individuellement et indépendamment par deux dermatologues séniors (C.D. et M.D.) qui ont réalisé les scores "cliniques" suivants : Classification de Hurley, HS-PGA (Hidradenitis Suppurativa- Physician Global Assessment), Score de Sartorius, et Sartorius modifié. Une série de photographies standardisées a été prise lors de cette consultation médicale (sept par patient). Chaque set de photographies a été ensuite évalué en aveugle par neuf dermatologues experts, membre du groupe thématique HS-France de la Société Française de Dermatologie, et cinq internes en dermatologie, afin de réaliser les scores "photographiques" suivants : Classification de Hurley, HS-PGA, IHS4, score de Sartorius, score de Sartorius modifié et HSS, pour chaque patient. Dix-huit femmes et huit hommes d'âge médian (IQR) de 35 ans (28–42 ans) ont été inclus dans l'étude. Au cours de la consultation, 5 (19,2 %) patients avaient un stade "clinique" I de Hurley, 19 (73,1 %) un stade "clinique" II de Hurley et 2 (7,6 %) un stade "clinique" III de Hurley. Le score "clinique" de Sartorius médian était de 30,5 (IQR 20–42). Onze patients avaient un set de photographies complet. La concordance entre experts sur les photographies était de 0,67 [cd1] pour la classification de Hurley, de 0,60 pour le HS-PGA et de 0,64 pour le score IHS4. Pour les patients avec un jeu complet de photographies, le coefficient de corrélation de concordance entre experts (IC95 %) était de 0,48 (0,18–0,70) pour le score de Sartorius, de 0,54 (0,24–0,74) pour le score de Sartorius modifié, et de 0,52 (0,15–0,76) pour le score HSS. Il s'agit d'une étude pilote sur l'évaluation photographique de la gravité de l'hidradénite suppurée à partir de plusieurs scores cliniques existants. Selon les scores, la concordance inter-évaluateurs était modérée à bonne entre les experts. À l'heure du développement des évaluations patients numériques aussi bien en clinique qu'en recherche, il y a nécessité de choisir des scores reproductibles sur photos.

Nous rapportons l’observation d’un pyoderma gangrenosum (PG) récidivant du post-partum, associé à une cardiomyopathie du post-partum (CMPP).Une femme de 23 ans signalait avoir eu un abcès du sein gauche lors de sa quatrième grossesse. Cette lésion avait évolué, après drainage chirurgical, vers une ulcération inflammatoire traitée par des soins locaux atraumatiques. Lors de sa cinquième grossesse, la patiente développait une ulcération large et douloureuse en regard d’une cicatrice de césarienne, malgré l’introduction d’une antibiothérapie à large spectre. Les prélèvements bactériologiques étaient négatifs. L’examen histologique était en faveur d’un PG. Une semaine après l’introduction d’une corticothérapie générale, elle présentait brutalement des signes d’insuffisance cardiaque. L’échocardiographie trans-thoracique permettait de retenir le diagnostic de CMPP, dont l’évolution était fatale.Cette observation soulève la question de la relation entre PG et grossesse, et décrit l’association d’un PG et d’une CMPP. La survenue du PG lors de la grossesse est rare ; elle pourrait être induite par l’élévation des taux de G-CSF constatée chez les femmes enceintes. L’association à une CMPP chez notre patiente pourrait être expliquée par le développement d’un climat anti-angiogénique en fin de grossesse, auquel s’ajouterait une agression myocardique inflammatoire neutrophilique en lien avec le PG.We report a case of recurrent post-partum pyoderma gangrenosum (PG) complicated by post-partum cardiomyopathy (PPCM).A 23-year-old woman presented with a previous medical history of aseptic abscess of the left breast in her fourth pregnancy, which developed after surgical drainage of an inflammatory ulceration treated by atraumatic topical care. During her fifth pregnancy, the patient presented a large and painful ulceration in relation to the scar of the Caesarean section, despite the introduction of broad-spectrum antibiotic therapy. Bacteriological samples were negative. Histological examination militated in favor of PG. One week after initiation of corticosteroid therapy, the patient suddenly showed signs of heart failure. Based on trans-thoracic echocardiography PPCM was diagnosed, and the outcome was fatal.This observation raises the question of the relationship between PG and pregnancy and describes the association of PG and PPCM. PG occurs rarely during pregnancy and it may be induced by the rise in G-CSF levels found in pregnant women. The association with PPCM seen in our patient could have been due to the development of an anti-angiogenic climate at the end of pregnancy, together with inflammatory myocardial aggression linked to the PG.
