Geburtstraumatische Läsionen des Plexus cervicobrachialis sind selten, ihre Häufigkeit schwankt in der Literatur zwischen 0,5–2,5 % aller Geburten. 80–95 % aller Läsionen bilden sich spontan zurück. Die Therapie bei persistierenden Plexusläsionen ist vom Verletzungsmuster abhängig. Obwohl Schulterdystokie, Makrosomie, Beckenendlage und Instrumenten assistierte Geburten als Risikofaktoren identifiziert werden konnten, fehlen diese bei Dreiviertel aller Fälle. Die Folgen einer solchen Läsion sind, neben Lähmung und Sensibilitätsstörung, ein verringertes Knochenwachstum der betroffenen Seite. Diagnostisch ist vor allem das klinische Bild des Neugeborenen relevant, da der Arm in einer für die Art der Verletzung typischen Position zu liegen kommt. Nachdem eine geburtstraumatische Armplexusläsion festgestellt wurde, ist der Verlauf der neurologischen Symptome ausschlaggebend für die Therapie. Wenn nach den ersten 6 Monaten (in Sonderfällen nach 4 Monaten) keine Besserung festgestellt werden kann, ist eine Operation indiziert. Der operative Zugang erfolgt supraklavikulär transversal durch Durchtrennung des Platysmas. Der Truncus superior und die einzelnen Plexuswurzeln werden dargestellt und mittels Reizstrom identifiziert. Nachdem die Rekonstruktionsstrategie intraoperativ festgelegt wird, wird meist der N. suralis als Transplantat verwendet. Die sekundäre Operation nach mehr als 12 Monaten ist nur in Ausnahmefällen mittels primärer Nervenrekonstruktion möglich, da die Zielmuskeln in dieser Zeit meist atrophiert sind und ihre Funktion verloren haben. In diesen Fällen ist eine Muskel- und Sehnentransplantation indiziert. In der Regel wird für die Insuffizienz des M. biceps brachii ein M. latissimus dorsi Transplantat verwendet.
Germanium nanocrystals embedded in high- k dielectric matrices are of main interest for infrared sensing application, as a role model for Ge-based nanoelectronics passivation or for nonvolatile memory devices. The capability of the size control of those nanocrystals via rapid thermal processing of superlattice structures is shown for the [Ge–TaZrO x /TaZrO x ] n , [Ge–TaZrO x /SiO 2 /TaZrO x ] 6 , and [TaZrO x /Ge–SiO 2 ] n superlattice systems. All superlattices were deposited by radiofrequency magnetron sputtering. Transmission electron microscopy (TEM) imaging confirms the formation of spherically shaped nanocrystals. Raman scattering proved the crystallization of Ge above 700°C. The TaZrO x crystallizes above 770°C, associated with a phase separation of Ta 2 O 5 and ZrO 2 as confirmed by x-ray diffraction. For the composite layers having 3 nm and 6 nm thickness, the size of the Ge nanocrystals correlates with the deposited layer thickness. Thicker composite layers (above 9 nm) form two fractions of nanocrystals with different sizes. An additional SiO 2 layer in the [Ge–TaZrO x /SiO 2 /TaZrO x ] 6 superlattice stacks facilitates the formation of larger and better separated Ge nanocrystals. The deposition of Ge-SiO 2 composite layers separated by pure TaZrO x illustrates the barrier effect of TaZrO x against Ge diffusion. All three material systems allow the controlled formation of Ge nanocrystals in amorphous matrices at temperatures above 700 and below 770°C. Graphical abstract
An open decompression is the most common treatment for lumbar spinal canal stenosis (LSS), even in the elderly. However, it is not clear whether the treatment outcome is age dependent. The main purpose of this study was to evaluate the improvement in quality of life (QoL) and pain relief, after open decompression for LSS in relation to patient age.
Ein 64-jähriger Patient stellte sich mit stechenden, atemabhängigen rechtsthorakalen Schmerzen und Ruhedyspnoe in unserer Notaufnahme vor. An Vorerkrankungen bestanden eine Adipositas (BMI 43), ein Diabetes mellitus Typ 2 (insulinpflichtig), ein Schlafapnoe-Hypopnoe-Syndrom mit nCPAP-Therapie, eine essentielle arterielle Hypertonie, Z.n. TVT mit oraler Antikoagulation, eine chronische Gastritis sowie ein primäres Parkinsonsyndrom. Eine KHK wurde im Vorfeld ausgeschlossen. Im Labor fielen ein erhöhtes CRP (32 mg/l), eine Leukozytose (10,68 gpt/l) sowie elevierte D-Dimere auf. Die kapilläre Blutgasanalyse zeigte eine respiratorische Partialinsuffizienz. Eine Thorax-Angio-CT konnte keinen Hinweis für eine Lungenembolie erbringen, aber zeigte kleine pulmonale Rundherde. Bei V.a. intrapulmonale Metastasen erfolgten eine Ösophago-Gastro-Duodenoskopie, Koloskopie, Abdomen-CT, sowie Sonografie der Schilddrüse und des Abdomens ohne Hinweise für ein Malignom. Die bronchoalveoläre Lavage ergab ein neutrophiles lymphozytäres Zellbild bei normaler CD4/CD8-Ratio sowie einen hohen Anteil immunseneszenter Zellen (CD57+) mit Nachweis von DNA für Candida spp.. Zur Klärung der Ätiologie der pulmonalen Rundherde erfolgte eine atypische Resektion aus dem linken Lungenunterlappen. Histologisch zeigte sich eine epitheloidzellig-granulomatöse Entzündung. Molekularpathologisch wurde ein myzetales Genom, im Speziellen Pichia guillermondii (PG) nachgewiesen. Wir therapierten mittels Fluconazol 200 mg für 6 Wochen. Infektionen mit PG sind selten und treten in der Regel bei immunsupprimierten Patienten auf. Dies ist u.E. in Europa der erste beschriebene Fall einer PG-Infektion bei einem Patienten, der abgesehen von einem Diabetes Typ 2 keine Prädisposition für eine Infektion mit opportunistischen Erregern aufwies.
Einleitung: Q-Fieber ist eine Zoonose. Häufige Organmanifestationen sind eine interstitielle Pneumonie oder eine Hepatitis, seltener sind Meningoenzephalitiden oder Myokarditiden. Der Erreger Coxiella burnetii bildet Dauerformen und hat somit eine hohe Resistenz gegenüber chemischen und physikalischen Reizen (RKI, 2009). Etwa 1% aller Erkrankungsfälle chronifiziert. Hierbei ist die häufigste Organmanifestation eine Endokarditis.
Mukoviszidose wird verursacht durch eine Fehlfunktion des CFTR (Cystic Fibrosis Transmembrane Conductance Regulator)-Proteinkanals aufgrund verschiedener Mutationen, die in 6 Mutationsklassen unterteilt werden. Seit Oktober 2012 steht für Patienten mit der Mutation G551D, einer sogenannten Gating-Mutation der Klasse III, das Medikament Ivacaftor zur Verfügung, welches das erste Beispiel einer mutationsspezifischen Therapie beim Menschen darstellt.
UNLABELLED:A patient presented himself with pungent, breath-dependent right chest pain and dyspnea at rest in our emergency department. The physical examination and the ECG revealed no relevant findings. The laboratory results showed an increased CRP, leukocytosis, elevated D-dimers and a respiratory partial insufficiency. In the thoracic CT angiography unclear pulmonary nodules (PN) were seen. The bronchoscopy was macroscopically normal. In the BAL yeasts and a high proportion of immune senescence cells (CD57+) were identified. After a pulmonary wedge resection resulted histologically an epithelioid cell-granulomatous inflammation. Molecular pathological a mycelium genome, in particular Pichia guilliermondii (PC) was detected. The therapy with fluconazole was successful. PC rarely causes candidemia, increased in immunocompromised patients. In our judgement this is in Europe the first described case of PC-infection in a patient, which presented no predisposition to infection with opportunistic pathogens apart from type 2 diabetes. CONCLUSION:It should be thought of fungal infection by these pathogens group in case of unclear PN, especially in combination with possibly predisposing factors.
Dynamic stabilization of the degenerated spine was invented to overcome the negative side effects of fusion surgery like adjacent segment degeneration. Amongst various different implants DSS® is a pedicle-based dynamic device for stabilizing the spine and preserving motion. Nearly no clinical data of the implant have been reported so far. The current analysis presents results from a single spine surgeon who has been using DSS® for the past 5 years and recorded all treatment and outcome data in the international Spine Tango registry. From the prospectively documented overall patient pool 436 cases treated with DSS® could be identified. The analysis was enhanced with a mailing of COMI patient questionnaires for generating longer-term follow-ups up to 4 years. 387 patients (189 male, 198 female; mean age 67.3 years) with degenerative lumbar spinal disease including degenerative spondylolisthesis (6.1 %) could be evaluated. The type of degeneration was mainly spinal stenosis (89.9 %). After a mean follow-up of 1.94 years, the COMI score and NRS back and leg pain improved significantly and to a clinically relevant extent. The postoperative trend analysis could not determine a relevant deterioration of these outcomes until 4 years postoperative. 10 patients were revised (2.6 %) and the implant was removed; in most cases, a fusion was performed. Another 5 cases (1.3 %) had an extension of the dynamic stabilization system to the adjacent level. 84.2 % of patients rated that the surgery had helped a lot or had helped. The results of this large consecutive series with a follow-up up to 4 years could demonstrate a good and stable clinical outcome after posterior dynamic stabilization with DSS®. For degenerative diseases of the lumbar spine, this treatment seems to be a valid alternative to fusion surgery.
Background: In recent years, the scientific discussion has focused on new strategies to enable a torn anterior cruciate ligament (ACL) to heal into mechanically stable scar tissue. Dynamic intraligamentary stabilization (DIS) with LigamysTM was first performed in a pilot study of 10 patients. The purpose of the current study was to evaluate the five year results of this group.Methods: Inclusion criteria were an ACL rupture not older than 14 days, patient age < 45 years, no previous surgery on the injured knee, and regular participation in sports requiring pivoting of the knee joint. Ten consecutive patients (eight males, two females) underwent surgery between August 2009 and February 2010. They were treated by DIS employing an internal stabilizer to keep the unstable knee in a posterior translation, combined with microfracturing and platelet-rich fibrin induction at the rupture site to promote self-healing. Postoperative clinical outcome [Tegner, Lysholm, International Knee Documentation Committee (IKDC), visual analogue scale patient satisfaction score] and assessment of knee laxity was performed at 3, 6, 12, 24 and 60 months.Results: Median patient age at time of surgery was 23.3 years (range 19-41 years). The median time to surgery was 10 days (range 5-13 days). The rupture was located in the middle third of the ligament in seven patients and in the proximal third in three patients. Eight patients showed additional meniscal lesions, which were surgically treated in six patients. Eight of the ten patients reached the five-years follow-up. Median Lysholm score was 100 (range 90-100); the IKDC score was 98.9 (range 79.3-100); Tegner score was 5.5 (range 5-7); median Lachman difference to the other side was 2 mm (range 0-4 mm). Median patient satisfaction was 10 points (range 8-10 pts.). Four of the ten patients underwent metal removal (tibial implant component) after ACL healing and a consequently stable knee joint. Two patients suffered from a re-rupture at 5 months and 4.2 years after surgery and were treated with a bone-tendon-bone ACL graft.Conclusions: Dynamic intraligamentary stabilization in ten active patients with a fresh ACL rupture showed a 5-years survival rate of 80 %. At the last follow-up all patients with a functionally healed ACL showed excellent outcomes and satisfaction with regards to the treatment result.
Die ambulante erworbene Pneumonie (CAP) ist eine häufige Erkrankung. Differentialdiagnosen und Komplikationen sollten stets in Betracht gezogen werden. Insbesondere bei älteren Patienten mit Raucheranamnese sollte auch an ein Lungenkarzinom gedacht werden. Bei 9,2% der über 65-Jährigen mit CAP wird im Verlauf eines Jahres ein pulmonales Malignom festgestellt. Weiterhin wird vermutet, dass bis zu 15% der CAP mit Aspirationen vergesellschaftet sind.
High-temperature vapor phase epitaxy (HTVPE) is a physical vapor transport technology for a deposition of gallium nitride (GaN) layers. However, little is known about the influence of the deposition parameters on the microstructure of the layers. In order to fill this gap, the influence of the ammonia (NH 3 ) flow applied during the HTVPE growth on the microstructure of the deposited GaN layers is investigated in this work. Although the HTVPE technology is intended to grow GaN layers on foreign substrates, the GaN layers under study were grown on GaN templates produced by metal organic vapor phase epitaxy in order to be able to separate the growth defects from the defects induced by the lattice misfit between the foreign substrate and the GaN layer. The microstructure of the layers is characterized by means of high-resolution x-ray diffraction (XRD), transmission electron microscopy and photoluminescence. In samples deposited at low ammonia flow, planar defects were detected, along which the nitrogen atoms are found to be substituted by impurity atoms. The interplay between these planar defects and the threading dislocations is discussed. A combination of XRD and micro-Raman spectroscopy reveals the presence of compressive residual stress in the samples.
Einleitung: Die Sarkoidose ist eine "häufig" seltene granulomatöse Erkrankung mit vielen Gesichtern. Neben dem primären Befall der Lunge und Lymphknoten kann im Prinzip jedes Organsystem betroffen sein. In den meisten Fällen zeigt sich ein kurzer, selbstlimitierender Verlauf. In bis zu 10% der Patienten aber kommt es zu Komplikationen. Ein kardialer Befall stellt nach dem Lungenversagen die zweithäufigste mit der Sarkoidose assoziierte Todesursache dar. Im Folgenden möchten wir den Fall einer kardialen Sarkoidose mit schwerstem klinischem Verlauf vorstellen.

Molekularpathologisch wurde ein myzetales Genom, im Speziellen Pichia guilliermondii (PC) nachgewiesen. Die Therapie mit Fluconazol war erfolgreich. PC verursacht selten Candidämien, bei immunsupprimierten Patienten vermehrt. Dies ist u. E. in Europa der erste beschriebene Fall einer PC-Infektion bei einem Patienten, der abgesehen von einem Diabetes Typ 2 keine Prädisposition für eine Infektion mit opportunistischen Erregern aufwies. Fazit: Bei unklaren pulmonalen RH, insbesondere bei prädisponierenden Faktoren, sollte auch an eine Pilzinfektion durch diese Erregergruppe gedacht werden.
Thin films of pure Ge and ZrO2, composite Ge-ZrO2 layers and [Ge-ZrO2/ZrO2]40 multilayers were fabricated by confocal radio frequency magnetron sputtering. The structural and optical properties of these samples were studied by spectroscopic ellipsometry, Raman scattering and Fourier transform infrared spectroscopy in dependence of the chemical composition and the annealing treatment. It could be shown that rapid thermal annealing stimulates a phase separation process within the Ge-ZrO2 composite films leading to a formation of Ge nanocrystals and tetragonal ZrO2. The starting point of this process depends significantly on the Ge content: the higher the Ge content, the lower is the crystallization temperature. Compared to the monoclinic phase, the tetragonal ZrO2 phase offers an enhanced permittivity.
Surgical decompression for lumbar spinal stenosis (LSS) has been associated with poorer outcomes in patients with pronounced low back pain (LBP) as compared to patients with predominant leg pain. This cross registry study assessed potential benefits of the interlaminar coflex® device as an add-on to bony decompression alone.
Background: In recent years, the scientific discussion has focused on new strategies to enable a torn anterior cruciate ligament (ACL) to heal into mechanically stable scar tissue. Dynamic intraligamentary stabilization (DIS) was first performed in a pilot study of 10 patients. The purpose of the current study was to evaluate whether DIS would lead to similarly sufficient stability and good clinical function in a larger case series.Methods: Acute ACL ruptures were treated by using an internal stabilizer, combined with anatomical repositioning of torn bundles and microfracturing to promote self-healing. Clinical assessment (Tegner, Lysholm, IKDC, and visual analogue scale [VAS] for patient satisfaction scores) and assessment of knee laxity was performed at 3, 6, 12, and 24 months. A one-sample design with a non-inferiority margin was chosen to compare the preoperative and postoperative IKDS and Lysholm scores.Results: 278 patients with a 6:4 male to female ratio were included. Average patient age was 31 years. Preoperative mean IKDC, Lysholm, and Tegner scores were 98.8, 99.3, and 5.1 points, respectively. The mean anteroposterior (AP) translation difference from the healthy contralateral knee was 4.7 mm preoperatively. After DIS treatment, the mean 12-month IKDC, Lysholm, and Tegner scores were 93.6, 96.2, and 4.9 points, respectively, and the mean AP translation difference was 2.3 mm. All these outcomes were significantly non-inferior to the preoperative or healthy contralateral values (p < 0.0001). Mean patient satisfaction was 8.8 (VAS 0-10). Eight ACL reruptures occurred and 3 patients reported insufficient subjective stability of the knee at the end of the study period.Conclusions: Anatomical repositioning, along with DIS and microfracturing, leads to clinically stable healing of the torn ACL in the large majority of patients. Most patients exhibited almost normal knee function, reported excellent satisfaction, and were able to return to their previous levels of sporting activity. Moreover, this strategy resulted in stable healing of all sutured menisci, which could lower the rate of osteoarthritic changes in future. The present findings support the discussion of a new paradigm in ACL treatment based on preservation and self-healing of the torn ligament.
