ISSN 0001-5555 A 68-year-old Italian man presented with a 7-month history of multiple erosions on the trunk as well as lips and oral cavity. Before admission to our department, he was treated in another institution with systemic corticosteroids (oral prednisone at the initial dose of 1 mg/kg/day, with progressively tapering dosages for 2 months) and other immunosuppressants, such as azathioprine (1.25 mg/kg/ day for 2 months), intravenous immunoglobulins (one cycle consisting of one infusion of 400 mg/kg/day for 5 consecutive days), and rituximab (one cycle consisting of one infusion of 375 mg/m2/every week for 4 weeks), achieving resolution of the cutaneous manifestations but without control of the oral involvement. Upon admission, he had multiple lip and oral erosions (Fig. 1a). In the following days, he developed erythematous-violaceous vegetating nodules that coalesced into ulcerated plaques on the lips and tongue (Fig. 1b). The patient’s general condition was compromised; he was asthenic and, due to dysphagia, had lost about 15 kg in the last 3 months. Staging fibroscopy revealed nodules that induced marked stenosis of the larynx and pharynx (Fig. 1c). Within a few days, an emergency tracheotomy was required because of acute dyspnea. Biopsy specimens from a nodule were submitted for histology (Fig. 1d).
The presence of hypovitaminosis D in patients with autoimmune bullous skin diseases, such as pemphigus vulgaris (PV) and bullous pemphigoid (BP), is debated. In a previous study we found an increased prevalence of vertebral fractures (VFx) and hypovitaminosis D in PV and BP patients. The present study extends the sample size of the previous one, for investigating the 25-hydroxyvitamin D (25OHVitD) levels in relation with the skeletal health and disease intensity in these patients.
All drugs currently used for treating patients with inflammatory bowel disease (IBD - including Crohn's disease and ulcerative colitis) have the potential to induce skin lesions ranging from mild eruptions to more serious and widespread clinical presentations. The number of cutaneous adverse reactions due to IBD therapies is progressively increasing and the most frequently involved drugs are thiopurines and biologics like tumor necrosis factor (TNF)-alpha antagonists. The main drug-induced cutaneous manifestations are non-melanoma skin cancer (NMSC), notably basal cell and squamous cell carcinomas, and viral skin infections for thiopurines and psoriasiform, eczematoid and lichenoid eruptions as well as skin infections and cutaneous lupus erythematosus for biologics. Cutaneous manifestations should be promptly recognized and correctly diagnosed in order to quickly establish an adequate therapy. The main treatment for NMSC is surgical excision whereas the management of immune-mediated inflammatory skin reactions varies from topical therapy for mild presentations to the shift to another drug alone or in combination with corticosteroids for extensive eruptions.
The skin is one of the most common extraintestinal organ system affected in patients with inflammatory bowel disease (IBD), including both Crohn's disease and ulcerative colitis. The skin manifestations associated with IBD are polymorphic and can be classified into 4 categories according to their pathophysiology: (1) specific, (2) reactive, (3) associated, and (4) induced by IBD treatment. Cutaneous manifestations are regarded as specific if they share with IBD the same granulomatous histopathological pattern: perianal or metastatic Crohn's disease (commonly presenting with abscesses, fistulas or hidradenitis suppurativa-like features) is the prototype of this setting. Reactive cutaneous manifestations are different from IBD in the histopathology but have close physiopathological links: pyoderma gangrenosum, a neutrophil-mediated autoinflammatory skin disease typically manifesting as painful ulcers, is the paradigm of this group. Among the cutaneous diseases associated with IBD, the most commonly seen are erythema nodosum, a form of panniculitis most commonly involving bilateral pretibial areas, and psoriasis, a T helper 1/T helper 17-mediated erythematous squamous inflammatory disease. Finally, the number of cutaneous adverse reactions because of IBD therapies is progressively increasing. The most frequent drug-induced cutaneous manifestations are psoriasis-like, eczema-like, and lichenoid eruptions, as well as cutaneous lupus erythematosus for biologics, and nonmelanoma skin cancer, mainly basal cell and squamous cell carcinomas for thiopurines.
The association of pyoderma gangrenosum, acne, and suppurative hidradenitis (PASH) has recently been described and suggested to be a new entity within the spectrum of autoinflammatory syndromes, which are characterized by recurrent episodes of sterile inflammation, without circulating autoantibodies and autoreactive T-cells. We conducted an observational study on 5 patients with PASH syndrome, analyzing their clinical features, genetic profile of 10 genes already known to be involved in autoinflammatory diseases (AIDs), and cytokine expression pattern both in lesional skin and serum. In tissue skin samples, the expressions of interleukin (IL)-1β and its receptors I and II were significantly higher in PASH (P = 0.028, 0.047, and 0.050, respectively) than in controls. In PASH patients, chemokines such as IL-8 (P = 0.004), C-X-C motif ligand (CXCL) 1/2/3 (P = 0.028), CXCL 16 (P = 0.008), and regulated on activation, normal T cell expressed and secreted (RANTES) (P = 0.005) were overexpressed. Fas/Fas ligand and cluster of differentiation (CD)40/CD40 ligand systems were also overexpressed (P = 0.016 for Fas, P = 0.006 for Fas ligand, P = 0.005 for CD40, and P = 0.004 for CD40 ligand), contributing to tissue damage and inflammation. In peripheral blood, serum levels of the main proinflammatory cytokines, that is, IL-1β, tumor necrosis factor-α, and IL-17, were within the normal range, suggesting that in PASH syndrome, the inflammatory process is mainly localized into the skin. Four out of our 5 PASH patients presented genetic alterations typical of well-known AIDs, including inflammatory bowel diseases, and the only patient lacking genetic changes had clinically evident Crohn disease. In conclusion, overexpression of cytokines/chemokines and molecules amplifying the inflammatory network, along with the genetic changes, supports the view that PASH syndrome is autoinflammatory in origin.
Hintergrund: Im Jahr 2008 wurde in Deutschland das Hautkrebs-Screening in die gesetzlichen Gesundheitsleistungen für Personen ≥35 Jahre aufgenommen. Bisher nehmen 95% der ca. 3500 niedergelassenen Dermatologen an diesem Programm teil, das von den gesetzlichen Krankenkassen finanziert wird. Ziel: Bewertung des deutschen Hautkrebs-Screenings aus der Sicht der Dermatologen. Methoden: Seit 2009 wurden an rund 2000 Dermatologen landesweit einmal jährlich standardisierte Fragebögen verschickt, in denen es um die jeweiligen Patienten, Screenings und Behandlungen sowie um die persönlichen Erfahrungen und Einstellungen der Dermatologen beim Hautkrebs-Screening geht. Für die Jahre 2009, 2010 und 2011 wurde eine deskriptive Datenanalyse durchgeführt. Ergebnisse: In den Jahren 2009, 2010 bzw. 2011 führten Dermatologen 1380, 1364 bzw. 1348 Screenings mit einem durchschnittlichen Honorar von 21,50, 22,10 bzw. 21,93 EUR durch. 32,9, 46,6 bzw. 53,3% der Dermatologen waren mit dem Hautkrebs-Screening ziemlich bzw. sehr zufrieden, und eine steigende Anzahl an Dermatologen (69,4, 80,0 bzw. 83,1%) stellte eine bessere Qualität der Gesundheitsversorgung bei Hautkrebs seit 2008 fest. Schlussfolgerung: Das Hautkrebs-Screening wird von deutschen Dermatologen weithin akzeptiert und zunehmend häufiger durchgeführt.

Hintergrund: Demodex-Milben sind an verschiedenen Hautkrankheiten beteiligt und werden im Allgemeinen mittels Schabetest oder oberflächlichen Hautbiopsien nachgewiesen. Eine neue hochauflösende optische Kohärenztomographie (HD-OCT) mit hoher lateraler und axialer Auflösung in einem horizontalen (en-face) und vertikalen (slice) Bildgebungsmodus könnte die Möglichkeit einer nichtinvasiven und schnellen In-vivo-Untersuchung auf Demodex-Milben bieten. Methoden: 20 Patienten mit Demodex-bedingten Hautkrankheiten und 20 nach Alter und Geschlecht vergleichbare Kontrollen wurden mittels HD-OCT untersucht. Die Milben pro Haarfollikel und Haarfollikel pro Gesichtsfeld wurden gezählt und mit den Ergebnissen des Schabetests verglichen. Ergebnisse: In den HD-OCT-Bildern erschienen die Milben als helle runde Flecken, die in Gruppen von 3-5 Milben pro Haarfollikel angeordnet waren. Bei den Patienten mit Demodex-bedingter Hauterkrankung war eine mittlere Anzahl von 3,4 Milben pro Follikel nachweisbar bei einer mittleren Anzahl von 2,9 befallenen Follikeln pro Gesichtsfeld gegenüber einer mittleren Anzahl von 0,6 Milben in 0,4 befallenen Follikeln bei den Kontrollen. Der Schabetest war bei 21% der Patienten negativ. Schlussfolgerung: Die innovative HD-OCT ermöglicht einen schnellen und nichtinvasiven In-vivo-Nachweis von Demodex-Milben und könnte zu einem wichtigen Hilfsmittel in der Diagnose und Behandlungskontrolle von Demodex-bedingten Hauterkrankungen werden.
In developing countries, such as Uganda, skin problems are among the most common ailments seen in primary healthcare settings (1).Due to the dire lack of trained dermatologists, the vast majority of patients with skin diseases in these countries are treated by substitute auxiliary health workers with a limited education in skin disease management (1).To bridge this gap in access to dermatology services, we established a mobile teledermatology service and evaluated its applicability with regard to the impact of remote diagnoses on patient outcomes, as well as local health workers' perception concerning this mode of dermatology consultation.
Hintergrund: Zur Wirksamkeit der Corticosteroid-Pulstherapie bei Alopecia areata (AA) liegen nur wenige Daten vor. Ziel: Mit dieser Studie sollten die Langzeitergebnisse von Patienten unter Methylprednisolon-Bolustherapie untersucht werden. Methoden: In diese Studie wurden 60 Patienten eingeschlossen, die zwischen 1995 und 2000 eine Behandlung erhalten hatten. Die Kurzzeitergebnisse wurden im Jahr 2000 analysiert. Die Untersuchung der Langzeitergebnisse von 30 Patienten erfolgte 2010 mithilfe eines telefonischen Fragebogens. Ergebnisse: Bei 10/30 Patienten war 6 Monate nach der Bolusgabe ein signifikantes Haarwachstum zu beobachten. Die Hälfte der Patienten mit multifokaler AA hatte nach 6 Monaten auf die Therapie angesprochen, aber weniger als ein Viertel der Patienten mit Alopecia totalis (AT) und Alopecia universalis (AU) waren Responder. Die Untersuchung der Langzeitergebnisse erfolgte nach durchschnittlich 12,3 Jahren; 8/10 der initialen Responder hatten eine leichte oder keine Erkrankung und 14/20 initialen Non-Responder litten an schwerer AA. Schlussfolgerungen: Diese Studie bestätigte die geringe kurz- und langfristige Wirksamkeit dieser Behandlung bei AT und AU.
Die klinische Triade von Pyoderma gangraenosum (PG), Akne und suppurativer Hidradenitis (PASH) wurde kürzlich als neue Krankheitsentität innerhalb des Spektrums autoinflammatorischer Syndrome beschrieben. Dabei handelt es sich um eine neue Gruppe entzündlicher Erkrankungen, die sich von allergischen, infektiösen und autoimmunbedingten Krankheiten unterscheiden. Das PASH-Syndrom ähnelt dem PAPA(pyogene Arthritis, Akne und Pyoderma gangraenosum)-Syndrom, unterscheidet sich von diesem jedoch durch die fehlende assoziierte Arthritis und die fehlende genetische Grundlage. Das PAPA-Syndrom wird durch Mutationen in einem Gen verursacht, das an der Regulation der angeborenen Immunreaktion beteiligt ist, das PSTPIP1, wohingegen bei Patienten mit PASH-Syndrom bislang keine Mutationen nachgewiesen wurden. Wir berichten über einen jungen Mann, der nach einer Darm-Bypass-Operation wegen Adipositas gleichzeitig ein disseminiertes PG, eine typische Hidradenitis suppurativa und akneiforme Eruptionen im Gesicht entwickelte. Die kutanen Manifestationen im Zusammenhang mit einem Darm-Bypass-Syndrom gleichen oft einem PG oder anderen neutrophilen Dermatosen, was dafür spricht, dass die Pathogenese dieser Erkrankung mit einer Neutrophilen-vermittelten Entzündung in Zusammenhang steht. Dies ist der erste Bericht über das PASH-Syndrom nach einem bariatrischen Eingriff; wir schlagen vor, solche neutrophilen Dermatosen in die Liste der möglichen Komplikationen nach Darm-Bypass-Operationen aufzunehmen. Umfangreiche genetische Untersuchungen können helfen, die Ätiopathogenese des PASH-Syndroms sowie der autoinflammatorischen Erkrankungen im Allgemeinen zu klären.

Hintergrund: Psoriasis vulgaris (PV) ist mit einer niedriggradigen systemischen Entzündung verbunden. Ziel: Die Serummuster von Zytokinen und Wachstumsfaktoren wurden bei Patienten mit PV und allergischer Kontaktdermatitis (ACD) sowie bei gesunden Probanden untersucht, um systemische Entzündungsreaktionen bei diesen Erkrankungen zu beschreiben und zu vergleichen. Methoden: Insgesamt 12 entzündungssensitive Biomarker wurden mit Hilfe der Evidence Investigator™ Biochip-Technologie gleichzeitig analysiert. Ergebnisse: Bei PV waren der proinflammatorische Tumornekrosefaktor-α (TNF-α), Interferon-γ (IFN-γ), die Interleukine IL-1β, -2, -6, -8 und das Monozyten-Chemotaktische Protein-1 (MCP-1) erhöht. Bei ACD waren 2 Marker (TNF-α und MCP-1) erhöht und das regulatorische Zytokin IL-10 wies einen niedrigeren Wert auf. Das proinflammatorische IL-2 hatte bei PV und ACD die stärksten Korrelationen mit anderen pro- bzw. antiinflammatorischen Zytokinen, während IL-6 positiv mit dem Psoriasis Area and Severity Index korrelierte. Die Konzentrationen der Wachstumsfaktoren korrelierten mit MCP-1, jedoch nur bei PV. Schlussfolgerung: Obwohl PV eine vielfältigere proinflammatorische systemische Reaktion induziert, ist auch ACD mit einem systemischen Anstieg von Entzündungsmediatoren verbunden.
Background: Basosquamous carcinoma (BSC) is a rare epithelial tumor with a still confusing terminology. Since 2005 a more comprehensive and broader classification has existed. Aim: To retrospectively review our cases of BSC according to the new WHO definition and to re-evaluate their clinical and demographic characteristics and the margin involvement after traditional surgical excision. The data were compared with the same results obtained by basal cell carcinomas (BCCs) and squamous cell carcinomas (SCCs). Patients and Methods: Histologically confirmed carcinomas observed in our Department during a sixteen-year period (1994-2011) were studied. Surgical excision was evaluated following the international guidelines. Histopathologic subtypes of BSC were classified in accordance with accepted criteria. Results: Seventy-six patients had a BSC, 305 a SCC, 3,643 a BCC. There were significant differences among the median age of BSCs, the total BCCs and Non-Aggressive BCCs (74.7, 68.8 and 68.3 years respectively; p<0.05). BSC was more significantly located on head-neck region than Non-Aggressive BCC (p<0.04), and less on trunk than Mixed Histology BCC (p<0.01) and Non-Aggressive BCC (p<0.005). BSC has higher prevalence of positive margins after excision than total (p<0.03) and Non-Aggressive BCC (p<0.001). Conclusion: Basosquamous carcinoma fits to a tumor type with a different behavior pattern from non-aggressive basal cell carcinoma and more similar to squamous cell carcinoma or aggressive variants of basal cell carcinoma. Its infiltrative growth and the stromal reaction patterns give enough evidence to support the notion of considering basosquamous carcinoma as a relatively aggressive tumor.

Chronic urticaria (CU) is a cutaneous disease characterized by the recurrent eruption of short-lived wheals for at least six weeks. The wheals, which are surface swelling of the dermis accompanied by itching, may be elicited by different stimuli, or occur spontaneously. The pathogenesis of spontaneous CU has long remained a mystery, with food allergy, intolerance to food additives and chronic infections having been indicated as the main causes of the disease. The demonstration of histamine releasing autoantibodies in the serum of CU patients led to consider it of autoimmune origin in about 45% of cases, this condition being "idiopathic" in the remaining 55%. We recently evaluated the possible involvement of blood coagulation in its pathophysiology by measuring in CU patients the plasma levels of prothrombin fragment F1+ 2, a polypeptide released into the circulation during the activation of prothrombin to thrombin, and D-dimer, a marker of fibrin lysis. Interestingly, both F1+ 2 and D-dimer plasma levels were extremely high during the acute phase of disease, but were completely normal after remission, paralleling urticaria severity. It is noteworthy that thrombin causes an increase in vascular permeability, thus amplifying the CU inflammatory network. Moreover, immunohistochemical studies showed expression of tissue factor, the main initiator of coagulation, in CU skin lesions, and co-localization experiments demonstrated that tissue factor is expressed by eosinophils in the inflammatory infiltrate. All these findings support that coagulation activation contributes in the pathophysiology of spontaneous CU, and provide the rationale for using also anticoagulant and antifibrinolytic agents to treat it.
