Le cancer du sein chez la femme de moins de 40 ans est un diagnostic rare, mais d’évolution généralement moins favorable que les cancers de femmes plus âgées. Il se manifeste généralement sous la forme d’une masse palpable, et sa présentation en radiologie peut être parfois trompeuse, sous forme de masse faussement bénigne. Le bilan diagnostique du cancer du sein chez la femme jeune doit être complet, associant une échographie mammaire et une mammographie, généralement complétées par une IRM mammaire à la recherche de lésion additionnelle. Dans les cas particuliers des femmes enceintes ou allaitantes, le Centre de référence des agents tératogènes (Crat), l’American College of Radiology (ACR), l’European Society of Urogenital Radiology (ESUR), l’International Commission on Radiological Protection (ICRP) et la Société française de radiologie (SFR) ont émis des recommandations assurant la sécurité de la mère et de l’enfant lors de la réalisation des examens complémentaires.
Brain metastases or primary brain tumours had poor prognosis until the use of high dose radiotherapy. However, radionecrosis is a complex challenge in the post-radiotherapy management of these patients due to the difficulty of distinguishing this complication from local tumour recurrence. MRI alone has a variable specificity and sensibility, as does PET-CT imaging. We aimed to investigate the diagnostic performance of dual-phase 18F-FDG PET-mpMRI to distinguish cerebral radionecrosis from local tumour recurrence after cranial radiotherapy. A retrospective analysis was conducted between May 2021 and September 2022. Inclusion criteria encompassed patients with inconclusive MRI findings post-radiotherapy and history of cerebral radiotherapy for primary or metastatic brain lesions. Lesions are assessed qualitatively and semi-quantitatively. The gold standard to assess radionecrosis was histopathology or a composite criterion at three months. The study evaluated 24 lesions in 23 patients. Qualitative analysis yielded 85.7% sensitivity and 75% specificity. Semi-quantitative analysis, based on contralateral background noise, achieved 100% sensitivity and 50% specificity. Moreover, using contralateral frontal lobe background noise resulted in higher performances with 92% sensitivity and 63% specificity. Stratification by lesion type demonstrated 100% sensitivity and specificity rates for metastatic lesions. The diagnostic performance of dual-phase 18F-FDG PET-mpMRI shows promising results for metastatic lesions.
Breast cancer in women under the age of 40 is a rare diagnosis, but its course is generally less favourable than that of older women. It generally appears as a palpable mass, and its radiological presentation can sometimes be misleading, in the form of a falsely benign mass. The diagnostic work -up for breast cancer in young women should be complete, combining breast ultrasound and mammography, generally supplemented by breast MRI to look for additional lesions. Ce cas clinique illustre la possibilité de réaliser une angiomammographie pour les deux indications validées par la Haute Autorité de santé en l’absence de contre-indication d’une IRM : i) dans le cadre d’un bilan d’extension locorégionale d’un cancer du sein pour réévaluer la taille d’une lésion multifocale ; et ii) pour une évaluation avant et après une chimiothérapie néoadjuvante en vue d’un traitement conservateur en cas de bonne réponse à la chimiothérapie néoadjuvante. Les avantages principaux pour les patientes sont la diminution du délai d’accès à l’examen, la durée plus courte de sa réalisation et sa meilleure acceptabilité. Pour l’équipe radiochirurgicale, l’angiomammographie présente l’intérêt de sa simplicité de lecture et d’interprétation.
Après que la Food and Drug Adminsitration (FDA) ait approuvé l’utilisation de l’angiomammographie en 2011, l’American College of Radiology (ACR) a publié en 2022 un addendum à l’atlas du Breast Imaging-Reporting and Data System (BI-RADS) de 2013 portant sur cette technique d’imagerie mammaire. L’objectif de cet article est de proposer une synthèse de ce document qui vise à améliorer la cohérence des comptes rendus et valider les termes à utiliser pour de futures études. Il y est détaillé le lexique à utiliser, similaire sans être identique au BI-RADS de l’IRM. Il y est ensuite rappelé la structure élémentaire du compte rendu. Enfin des situations inédites de l’angiomammographie y sont détaillées en proposant des pistes de réflexion.

Le rehaussement matriciel de fond en lien avec l’imprégnation hormonale existe en angiomammographie tout comme en IRM mammaire, et peut altérer la sensibilité de détection des lésions tumorales. Il serait cependant moins marqué en IRM en raison de la différence de captation mammaire des produits de contraste utilisés entre les deux techniques et serait un facteur de risque indépendant de cancer du sein.
The background parenchymal enhancement related to hormonal impregnation exists in contrast-enhanced mammography as well as in breast MRI and may alter the sensitivity of tumour detection. However, it is less pronounced in MRI because of the difference in breast uptake of the contrast products between the two techniques and is an independent risk factor for breast cancer.(c) 2023 Elsevier Masson SAS. This case illustrates the possibility of performing contrast-enhanced mammography in the two indications validated by the French national authority for health (Haute Autorite de sante) in the absence of MRI contraindication: locoregional extension assessment of breast cancer to re-evaluate the lesion size of a multifocal lesion, and evaluation before and after neoadjuvant chemotherapy for conservative treatment in case of good response to neoadjuvant chemotherapy. The main advantages for the patients are the reduction of the delay of access to the examination, the shorter time of realization and the better acceptability of the examination. Distinguer une récidive après radiothérapie intracrânienne (RI) d’une radionécrose (RN) est un challenge diagnostique ayant un impact pronostique majeur. Cette situation est fréquemment retrouvée après traitement des glioblastomes mais aussi des métastases cérébrales, surtout depuis l’avènement de la radiothérapie stéréotaxique pour ces dernières. L’IRM est l’examen de référence après RI, mais sa spécificité est limitée. Le but de notre étude est d’évaluer l’impact de la TEP-IRM cérébrale au FDG en cas de doute diagnostique de l’IRM. Nous avons inclus (mai 2021–sept 2022), les patients traités par RI dont l’IRM ne permettait pas de distinguer une récidive d’une RN. Une TEP-IRM (Siemens Biograph mMR 3T) a été réalisée 0,5 et 4 heures après l’injection du FDG. Les séquences IRM étaient identiques à celles réalisées en IRM seule. Une co-analyse TEP et IRM (radiologue et médecin nucléaire) a été réalisée. Une analyse visuelle et semi-quantitative (SUVmax) a été réalisée sur les 2 séquences d’images pour la lésion cible et le bruit de fond cérébral pour calculer l’indice rétentionnel. Le gold standard a été soit l’anatomopathologie des lésions, soit le suivi (absence de traitement au moins 3 mois après la TEP-IRM). Onze patients présentaient des métastases de tumeurs solides, 3 de mélanomes et 9 d’une lésion primitive cérébrale. La TEP-IRM est en faveur d’une RN chez 13 patients et d’une récidive pour 10 patients, dont 2 ont été confirmés par l’anatomopathologie. La TEP-IRM a permis de reclasser en RN 4 patients (17,4 %) considérés plutôt en rechute à l’IRM. Pour les 11 patients (48 %) dont l’IRM seule était douteuse, la TEP-IRM a été en faveur d’une récidive pour 5 patients et d’une RN pour 6. Les résultats anatomopathologiques et/ou le suivi ont confirmé le diagnostic de la TEP-IRM pour 19/23 (83 %) patients, 2 ayant été perdu de vue, 1 ayant un suivi < 3 mois et 1 ayant rechuté après RN. Ces résultats préliminaires confirment l’intérêt de la TEP-IRM biphasique au FDG pour la distinction récidive/RN chez des patients lourdement traités. De manière robuste, la TEP-IRM a reclassé en RN plus de 1/6 des patients décrits en rechute à l’IRM seule. Près de la moitié des patients avec une IRM douteuse ont été correctement classés par la TEP-IRM confirmée par le gold standard. Une analyse des données semi-quantitative et de soustraction des images TEP et post-injection de gadolinium sont en cours.
After the Food and Drug Administration (FDA) approved the use of contrast enhanced mammography in 2011, the American College of Radiology published in 2022 an addendum to the 2013 Breast Imaging-Reporting and Data System (BI-RADS) atlas about this technique of breast imaging. The purpose of this article is to provide a summary of this document that aims to improve the consistency of reporting and validate the terms to be used for future studies. It details the lexicon to be used, which is similar but not identical to the MRI BI-RADS. The basic structure of the report is then recalled. ABSTRACT Digestive peristalsis generates many artifacts that limit the abdominal and pelvic MRI interpretation. Apart from the hypoglycemia treatment in patients with diabetes, glucagon analog is also indicated for the digestive peristalsis reduction to reduce MRI artifacts. However, its use in PET/MRI is not described, given the risk of interaction with the metabolism of FDG. To assess the importance of this interaction on the FDG PET images, we report FDG PET/MRI images obtained with and without glucagon analog injection.
