Introduction : La maîtrise de l’antibiothérapie et le traitement arthroscopique constituent deux avancées majeures dans le traitement des arthrites septiques. L’objectif de cette étude était d’évaluer les résultats cliniques et fonctionnels du traitement arthroscopique de l’arthrite septique du genou sur articulation native dans un hôpital camerounais. Méthodes : Il s’agissait d’une étude transversale avec une collecte prospective des variables. Les patients ont été opérés entre janvier 2022 et décembre 2024. Les adultes ayant une arthrite septique du genou étaient inclus. Tous les patients ont été traités par irrigation abondante et débridement/synovectomie par voie arthroscopique. Une antibiothérapie systémique adaptée aux résultats bactériologiques était associée. Les paramètres évalués étaient la douleur, l’amplitude articulaire, la vitesse de sédimentation (VS), la protéine C-réactive (CRP), le taux de réintervention, et les caractéristiques microbiologiques. Résultats : La population était composée de 22 patients (22 genoux). L’âge moyen des patients était de 28 ans. Six patients étaient drépanocytaires. Au recul moyen de 6 mois, les genoux étaient indolores (n=20), avec une flexion moyenne de 118°. La CRP et la VS s’étaient normalisées au bout de la quatrième semaine (n=14). Une reprise arthroscopique a été faite chez cinq patients. Un seul patient a nécessité une conversion en arthrotomie. Le Staphylococcus aureus a été isolé chez dix patients. Conclusion : Cette étude montre que l’arthroscopie associée à une antibiothérapie précoce permet un contrôle de l’infection, une bonne récupération fonctionnelle et des résultats radiologiques satisfaisants. The incidence of total hip replacement (THR) surgery is on the rise in Cameroon, attributed to the growing number of practitioners, increased availability of implants, and continuous improvement in surgeons' skills. However, challenges persist, particularly in the operating environment, equipment availability, financial policies, and prevailing cultural beliefs regarding traditional medicine. This study offers a short-term review of primary hip arthroplasty activities in an austere setting of the Far North region of Cameroon. A total of 168 hip arthroplasties were performed on 158 patients at the Maroua Military Hospital over 72 months. All procedures were done by a single surgeon using the anterior Hueter approach. Data on socio-demographics, clinical and radiological findings, and intraoperative details were collected. Complications and functional outcomes were assessed at a mean follow-up of 16 months using the Postel-Merle d'Aubigné (PMA)score. The study included 111 men and 47 women, with an average age of 49.39 years. Most patients (85.45%) had low to moderate incomes, and 79.75% of surgeries were financed in two or three installments. The main indications for surgery were neglected trauma and end-stage avascular necrosis, often delayed by traditional medical practices. Intraoperative complications, mainly nerve damage, occurred in 8% of cases, while postoperative infections were seen in 8.8%. At the final evaluation, 65% of patients had good or excellent outcomes according to the PMA score. This study highlights the challenges of performing total hip replacement surgery in resource-limited settings. It suggests improving technical infrastructure and introducing progressive funding options to enhance outcomes and reduce complication rates.
Open leg fractures frequently result in loss of substance, which can compromise the functional outcome of the lower limb. The resulting bone exposure presents a number of problems, including the need for coverage, which necessitates the utilisation of flap techniques. The medial gastrocnemius muscle flap with a proximal pedicle has an arc of rotation that restricts its use to covering the knee and the proximal epiphysis of the tibia. In this work, the authors propose a step-by-step technique for lengthening the medial gastrocnemius flap by scarification. This increases the flap's arc of rotation, compensating for the loss of bone substance and exposure of the tibia beyond its proximal epiphysis.
Introduction : Les fractures de jambe sont des solutions de la continuité diaphysaire d’un ou des deux os de la jambe. Le diagnostic est le plus souvent évident. L’évolution peut être émaillée de complications d’où l’importance de faire une prise en charge précoce et adaptée. Notre étude visait à évaluer l’ampleur de cette pathologie à l’Hôpital Laquintinie de Douala (HLD). Matériels et méthodes : Nous avons mené une étude rétrospective descriptive transversale à l’HLD sur une période allant du 1er janvier 2018 au 31 Décembre 2022. Était inclus, les patients âgés d’au moins 15 ans, admis et traités pour fracture de jambe. Les données ont été recueillies grâce à une fiche d’enquête. L’analyse statistique a été faite avec le logiciel SPSS version 22.0. Résultats : Au total 163 patients ont été retenus dans notre étude. La fréquence par rapport aux fractures des membres pelviens était de 30 %. L’âge moyen était 33,7 ans. Une prédominance masculine soit 72,3%. Le secteur informel était la couche socioprofessionnelle la plus touchée soit 30%. Les A.V.P. étaient la première étiologie soit 77,9%. Soixante-un pourcent des cas étaient des fractures ouvertes. Le traitement par fixateur externe était effectué dans 47,8%. La récupération fonctionnelle a été bonne chez 90,1%. Conclusion : Les fractures de jambe sont fréquentes. Elles surviennent le plus souvent chez les jeunes adultes de sexe masculin. L’étiologie est dominée par les accidents de la voie publique.
La luxation tibio-talienne pure est une lésion très rare en pratique sportive. Elle survient généralement après un traumatisme à haute énergie. Son traitement est chirurgical ou orthopédique selon les cas. Les auteurs rapportent un cas rare de luxation tibio-talienne pure chez un footballeur traité orthopédiquement après réduction, ainsi que son évolution jusqu’à la reprise sportive.
Background: Costal fracture surgical is still a debate, therefore we shall select between early and delay surgical management.Case Report: We are reporting two cases of post road traffic clash delay ribs fractures osteosynthesis involving a 63-year-old man with multistage fractures on the left and pulmonary pinning of one of the costal arches, complicated by a homolateral haemothorax and a 41-year-old man with a bilateral flail chest.Conclusion: The simple postoperative course and the immediate postoperative improvement in the patient's clinical respiratory condition enabled us to discuss the time frame for management, in this case the indication for early or later surgery.
Background: Despite the conservative treatment of tibio-femoral osteoarthritis through realignment osteotomies, the rate of total knee replacements following an osteotomy is increasing. The aim of this study was to identify the factors associated with the progression of knee osteoarthritis after a medial closing-wedge distal femoral osteotomy. Methods: Hospital-based observational study on 20 patients who underwent a medial closing-wedge distal femoral osteotomy evaluating the progression of osteoarthritis using the Kellgren and Laurence classification. The Wilcoxon test was used to compare the variation in the progressive stage of the Kellgren and Laurence classification of knee osteoarthritis preoperatively and at the final follow up. Univariate analysis made it possible to determine the factors associated with progression. The final significance threshold for statistical tests was set at 5% (p Results: Overall, the mean follow-up of 46 months ± 6.6 months, with a mean age of 43 years (range: 27 - 69 years) and a female predominance (M: F = 3/7). The progression of tibiofemoral osteoarthritis following a medial closing-wedge distal femoral osteotomy is associated with valgus or varum malalignment been a moderate valgus (OR 6.2 [1.5 - 42.7] at 95% CI; p-value = 0.02), a correction of the mechanical deviation angle with a valgus alignment (OR 2.7 [0.9 - 8.3] at 95% CI), and loss of correction (OR 3.8 [1.3 - 11.6] at 95% CI; p -value) for the lateral compartment while varus alignment (OR 1.7 [0.9 - 8.3] 95% CI, p-value = 0.05) and with rupture of the lateral cortex (OR 2.8 [1.7 - 11.5] 95% CI, p-value = 0.02) were those of the medial compartment. Conclusion: Distal femur closing wedge osteotomy does not definitively interrupt the progression of valgus knee osteoarthritis. The factors associated with the progression of this pathology are modifiable. Taking them into account when performing this surgical technique could improve the osteotomy survival curve.
Introduction: Road traffic accidents pose a significant public health issue, resulting in 1.2 million deaths annually and ranking as the 8th leading cause of mortality worldwide. Limb trauma is a common traumatic emergency, often occurring as a result of road traffic accidents. This study aims to describe the epidemiological, clinical, and therapeutic aspects of limb trauma related to road traffic accidents at Bonassama District Hospital.Methods: This was a descriptive cross-sectional study that included all patients admitted and managed for limb trauma at Bonassama District Hospital between February and June 2023 (five months). Sociodemographic, clinical, and therapeutic parameters were described and compared with existing literature.Results: Overall, the frequency of limb trauma due to road traffic accidents was 94.9%. A total of 241 patients were included in the study. The mean age was 32.5 ± 6 years, ranging from 5 to 70 years. Males accounted for 58% of the population, with a sex ratio of 1.39. Motorcycle taxi riders were the most affected victims (28.22%), and motorcycle-to-motorcycle collision was the most common injury mechanism (48.54%). Lower limbs were the most affected (62.66%). Pain was a functional sign present in all patients, and fractures were the most frequently encountered injuries (63.9% of cases). Traumatic brain injury was the most commonly associated injury (86.67%). Transverse fractures were the most frequent type (45.45%), with the tibia being the most commonly fractured bone (34%). Sixty-five point fifty-six percent (65.56%) of patients were admitted within the first 6 hours after the accident, and 39.83% received treatment 12 hours after admission. All patients received medical treatment, with osteosynthesis being the most commonly associated surgical treatment (72.58%), while circular plaster cast was the most frequent orthopedic treatment (65.67%). Sixty-two percent (62%) of admitted patients had a hospital stay exceeding 24 hours.Conclusion: Road traffic accidents remain a major issue in our country. Raising awareness and protecting vulnerable road users such as pedestrians and motorcyclists are urgent priorities.
Introduction L’objectif de ce travail était de rapporter les résultats de la butée osseuse à ciel ouvert selon Bristow-Latarjet dans un environnement où l’arthroscopie est peu développée. Matériel et méthode Nous avons réalisé une étude prospective sur trois années sur une série de 20 sportifs présentant une luxation récidivante de l’épaule et opérés par la technique de Bristow-Latarjet à ciel ouvert. Résultats Il y avait une prédominance masculine avec un sex-ratio de 4. La prise en charge de la luxation initiale était inadaptée dans 100 % des cas. Le délai moyen de prise en charge des formes récidivantes était de 54,8 mois. Le score ISIS était - supérieur à 6 dans 75 % des cas. La butée était affleurante et sous-équatoriale dans 85 % des cas. Nous avons recensé un seul cas d’infection du site opératoire, un cas de lyse de la butée avec démontage de vis associé. Les scores fonctionnels de Rowe et Constant qui étaient mauvais en préopératoire sont devenus bons à excellents après chirurgie. Nous n’avons enregistré aucune récidive après un recul de 3 ans. Au total, 95 % des patients ont été satisfaits de leur intervention avec un taux de reprise sportive de 80 %. Conclusion La technique de Bristow-Latarjet à ciel ouvert reste une méthode efficace de traitement des luxations récidivantes de l’épaule dans les pays en voie de développement où l’arthroscopie est peu développée.
Background: Elbow dislocations in pediatric patients are rare injuries.This is a therapeutic emergency because a delay in treatment can have disastrous consequences on the functional level of the upper limb.Objective: To present the management of elbow dislocation in children in cases of limited resources.Presentation of the Cases: These were 2 older male children aged 7 and 9 years old, admitted to the emergency room for painful functional impotence of the right elbow after a fall and landing on the right hand.Clinical and radiological examinations were in favor of posterior elbow dislocations.The reductions were carried out under sedation and immobilization in Jersey.According to Robert's criteria, the functional result was excellent in both patients.Conclusion: Emergency reduction and immobilization whatever the means ensure an excellent functional prognosis even in cases of limited resources.
Le retard diagnostic du mal de Pott est fréquemment observé dans notre environnement, à l’origine des complications neurologiques et des déformations rachidiennes malgré l’amélioration du plateau technique. Le but de cette étude était de décrire le profil clinique actuel, de préciser la place de la chirurgie du mal de Pott et évaluer les résultats. Il s’agissait d’une étude descriptive rétrospective multicentrique menée dans trois centres de référence de Yaoundé sur une période de 10 ans. Étaient inclus tous les patients admis pour mal de Pott confirmé par un faisceau d’arguments. Quatre-vingt-six cas ont été retenu avec une incidence de 10 cas par an. Les hommes représentaient 54,7 %. La moyenne d’âge des patients était de 41 ± 13,2 ans. La douleur rachidienne était le principal motif de consultation (47,7 %). Les patients présentaient une altération de l’état général (81,4 %), un syndrome rachidien (93,3 % des cas), des déficits moteurs (59,3 %). La localisation était dorsolombaire dans 96,6 %. Le diagnostic anatomopathologique a été posé dans 71,9 % des cas. L’indication chirurgicale était posée devant un déficit moteur, des signes d’instabilité du rachis à l’imagerie ou par une biopsie en cas de doute. Cinquante-sept patients (66,3 %) ont bénéficié d’une chirurgie. L’évaluation des patients après un recul de un à cinq a montré une amélioration de l’état général (94,3 %) et une récupération motrice (76,5 %) Le mal de Pott reste une affection fréquente dans notre environnement. La chirurgie apporte de bons résultats malgré la présentation clinique compliquée au moment du diagnostic. III.

Introduction: The management of open tibial fractures (OTF) is challenging in low and middle-income countries (LMICs) where appropriate human resources and infrastructure (including equipment, implants and surgical supplies) are not readily available and medical care is not readily accessible. OTF are not rarely associated with a subsequent fracture-related infection (FRI), which is one of the most devastating and difficult to cure compli-cations in orthopaedic trauma care. The aim of this study was to determine the rate and the predictive factors of FRI in OTF in a limited-resource setting of sub-Saharan Africa.Methods: Patients with OTF who underwent surgery from July 2015 to December 2020 and followed-up for at least 12 months in a tertiary care teaching hospital in Yaounde & PRIME; (Cameroon) were retrospectively investigated. Diagnosis of FRI was based on the confirmatory criteria of the International FRI Consensus definition. All patients with bone infections, occurring at any time point during follow-up, were included. Logistic regression was used to determine the predictive factors for FRI.Results: One hundred and five patients with OTF were studied. With a mean follow-up period of 29.5 & PLUSMN; 16.6 months, 33 patients (31.4%) presented with FRI. Gustilo-Anderson type of OTF, compliance with antibiotics, blood transfusion, time to first washing of the wounds and method of bone fixation were factors associated with the occurrence of FRI. In multivariable logistic regression, 6-hours delay to first washing of the wounds (OR=8.07, 95% CI: 1.43-45.31, p = 0.01), and compliance with antibiotics (OR=11.33, 95%CI: 1.11-115.6, p = 0.04) were the only independent predictors of FRI.Conclusion: The overall rate of FRI in open tibial fracture is still high in the sub-Saharan African context. For similar low-resources settings, this study supports the recommendations (1) to perform a very early washing -dressing-splinting of OTF on admission of the patient, (2) to administer antibiotics early, and (3) to perform surgery as soon as reasonably possible, once appropriate personnel, equipment, implants and surgical supplies are available.
RÉSUMÉ Introduction. La fracture de jambe est une lésion fréquente, souvent due aux accidents de la voie publiques (AVP), mais dont les caractéristiques varient selon la situation géographique de survenue. L’objectif de ce travail est de comparer les caractéristiques épidémiologiques, cliniques et thérapeutiques des fractures des os de la jambe associée aux AVP dans des hôpitaux du Cameroun et de France. Méthodologie. Il s’agit d’une étude multicentrique transversale rétrospective à visée prospective. Elle a eu lieu dans quatre structures hospitalières dont l’hôpital Général de Douala, l’hôpital Laquintinie de Douala au Cameroun et l’hôpital de Meaux, l’hôpital de Marne la vallée en France. Nous avons inclus tous les patients âgés de 18 ans et plus, victimes d’AVP avec fractures des os de la jambe et un dossier complet. Résultats. Sur une période de deux ans, nous avons recruté 220 cas dont 164 au Cameroun et 56 en France. L’âge moyen des sujets était de 37,3±13,3 ans au Cameroun et 45,1 ± 18,8 ans en France. Les patients étaient de sexe masculin dans 77,4% des cas au Cameroun et dans 69,6% en France. Les mototaxis étaient la principale activité au Cameroun (25,6%) et en France, il s’agissait des ouvriers (30,4%). Au Cameroun, on notait un retard à l’admission (70,9%) et à la prise en charge (79,2%). Toujours au Cameroun, la prise en charge sur le lieu de l’accident était absente et le transport était non médicalisé. Les fractures ouvertes étaient significativement plus fréquentes au Cameroun (61%) par rapport à la France (12.5%). Le traitement chirurgical était le moyen thérapeutique le plus employé dans les deux pays, mais alors que le fixateur externe était la modalité principale au Cameroun (44,6%), c’est la plaque vissée qui était la principale en France (49,1%). Au Cameroun, 33,5% des patients ont eu des complications contre 10,7% en France. Conclusion. La fracture de jambe pose des problèmes plus sérieux au Cameroun qu’en France. En effet, au Cameroun, il y a absence de prise en charge préhospitalière, les fractures sont plus souvent ouvertes et les complications sont trois plus fréquentes. ABSTRACT Introduction. The leg fracture is a common injury, often due to road traffic accidents (RTA), but whose characteristics vary according to the geographical situation of occurrence. The objective of this work is to compare the epidemiological, clinical and therapeutic characteristics of PVA-associated leg bone fractures in hospitals in Cameroon and France. Methodology. This is a retrospective cross-sectional multicentre prospective study. It took place in four hospital structures including the General Hospital of Douala, the Laquintinie Hospital of Douala in Cameroon and the Meaux Hospital, the Marne la Vallée Hospital in France. We included all patients aged 18 years and older with AVP with broken leg bones and a complete record. Results. Over a two-year period, we recruited 220 cases, 164 in Cameroon and 56 in France. The average age of subjects was 37.3 years in Cameroon and 45.1 years in France. The patients were male in 77.4% of cases in Cameroon and in 69.6% in France. Mototaxis were the main activity in Cameroon (25.6%) while in France, it was workers (30.4%). In Cameroon, there was a delay in admission (70.9%) and treatment (79.2%). Also in Cameroon, coverage at the accident site was absent and transportation was not medicalized. Open fractures were significantly more frequent in Cameroon (61%) compared to France (12.5%). Surgical treatment was the most common treatment modality in both countries, but while the external fixative was the main modality in Cameroon (44.6%), it was the screwed plate that was the main in France (49.1%). In Cameroon, 33.5% of patients had complications compared to 10.7% in France. Conclusion. Leg fracture raises more serious issues in Cameroon than in France. Indeed, in Cameroon, there is no pre-hospital management, fractures are more often open and complications are three times more common.
L’enclouage centromédullaire (ECM) du fémur est majoritairement réalisé à foyer ouvert dans les pays à revenus limités, avec un risque accru de complications. Une technique originale de réduction à foyer fermé par manipulation percutanée et sous contrôle scopique est décrite à partir d’une série monocentrique. L’intervention a consisté en un ECM verrouillé sur table ordinaire avec fluoroscopie monoplan, en utilisant la technique dite du « double Joy stick » assurant la réduction percutanée du foyer de fracture tant pour la technique antérograde en décubitus latéral que rétrograde en décubitus dorsal. Deux vis de Schanz placées en percutanée proches du foyer et de part et d’autre font office d’instrument réducteur. Deux aides assurent la réduction, l’un du foyer par les joysticks, l’autre par traction sur le membre. Sous contrôle scopique, foyer réduit, le guide clou est enfoncé, le fémur systématiquement alésé. Le verrouillage est effectué à « main levée ». Appliquée chez 32 patients, cette technique (24 antérogrades et 8 rétrogrades) a été réussie 28 (87,5 %) fois ; 4 (12,5 %) patients ont nécessité réduction à foyer ouvert. La durée moyenne de la chirurgie était de 96,2±18,4min et les pertes sanguines de 80±15,1mL, un délai préopératoire>15 jours (OR=21,0, IC95 % [2,8–156,1], p=0,003) était associé à l’échec de réduction à foyer fermé. Le délai médian de reprise des activités était de 3,5 mois. Le taux de consolidation à 6 mois était de 94 % et 27 (84,4 %) patients avaient un score fonctionnel bon à excellent. La technique du double joystick réductionnel sur table ordinaire dans les fractures diaphysaires fémorales est une technique simple, efficace et reproductible après une courte courbe d’apprentissage.
Bone problems such as angular deformities of the knee are common in children in Africa. The aim of this survey was to study epidemiologic aspects of frontal plane angular knee deformities in school children in Kribi. A total of 860 school children in Kribi aged 3 to 18 years were surveyed in a cross-sectional descriptive study from December 2019 to March 2020. Each child was examined. Intercodylar distances, intermalleolar distances and the tibiofemoral angles were assessed. The type of knee deformity in the frontal plane was determined from the children`s tibiofemoral angles and compared with reference values of normal children in the same age ranges. One hundred and fourty two (142, 16.5%) children surveyed presented with frontal plane knee deformities, with genu varum representing 68.0% (96 cases) of the deformities. The prevalence of these deformities in school children in Kribi varied significantly with age. We did not find any significant difference in the variation of these deformities with gender or ethnic groups. We identified some frontal plane angular knee deformities, including bilateral deformities being predominant 90.71% (127 cases). The mean body mass index was higher than those of normal children. 15.5% (22) of them presented with associated deformity in the sagital plane, dominated by bilateral genu recurvatum and 33.8% (48) of them presented with associated rotational knee deformities, dominated by bilateral medial rotation. Frontal plane knee angular deformities are common amongst school children in Kribi. Their prevalence is 16.51% (142 cases). This prevalence varies with ages. Introduction:Scoliosis is a pediatric pathology characterized by a deformation of the spine exerting a twist around its axis.Its expression is variable and multifactorial.When they are severe, they can become disabling and therefore justify appropriate care.However, the African literature is scarce on the subject.Objective: The objective of our work was to draw up the epidemiological, radio-clinical and evolutionary profile of scoliosis in black children and to highlight their singularities. Patients and Methods:We conducted a retrospective descriptive and analytical study over a 10-year period including all patients aged 0 to 17 years with scoliosis at the NCRPD. Results:We collected 69 cases with a female predominance.The prevalence of scoliosis in children was 0.4% and that of adolescents was 4.2%.The average age was 4.88 years for children and 15.16 years for adolescents.The average time of the 1st consultation was one year for children and 3 years for adolescents.They were severe and very severe (Cobb angle > 35°) in 76.8% of cases and symptomatic in 78.3% of cases with gibbosities of up to 6 cm in children and 11 cm in adolescents.5 risk factors for severity were identified in the black subject. Conclusion:Despite much work on the subject, more targeted epidemiological research is needed to improve the prevention and management of this disabling condition.
Introduction: Patella tendon rupture (PTR) is a cause of handicap in athletic patients. Surgery remains the actual therapeutic modality with a variety of techniques that permits a return to sport. Our objective was to evaluate the return to sporting activities after surgical repair of a PTR. Method: We conducted a retrospective cohort study over five years from January 2016 to April 2021. We included athletes from 18 to 65 years of operating for a PTR. We evaluated: the knee functional score, the level of satisfaction, Lysholm score, VISA-P questionnaire, and return to sports. Results: A total of twenty-one cases of PTR were identified. The median age was 36 years (18 - 63). After surgery, the median follow-up was 17 months (12 - 55). All patients were able to walk at 3 months. A total of 17 patients (85%) returned to sports. The median return to sports was 6 months (5 - 11). Patients who return to a similar sport with a similar initial level of sporting activity were 45% (9 patients). The median time to return to sports for patients at a similar level of sport was 7.5 months (6 - 13). Functional scores were satisfactory; with a VISA-P score of 90/100 (25 - 100) and a Lysholm score of 98/100 (40 - 100). Conclusion: Surgical repair of PTR in athletes offers good results regarding return to sports in terms of proportion and timing whatsoever the technique. The use of protection band wires is reserved for patients with relative satisfaction during suture repair.
