A 27-year-old patient with traumatic brain injury and neuropsychiatric symptoms fitting the obsessive-compulsive disorder was investigated. Brain CT-scan revealed left temporal and bilateral fronto-basal parenchymal contusions. Main Outcome Measure was the Yale-Brown Obsessive Compulsive Scale at pre- and post-treatment and at 6 months follow-up. The combination of pharmacotherapy and psychotherapy resulted in lower intensity and frequency of symptoms. Our case illustrates the importance of a detailed diagnostic procedure in order to provide appropriate therapeutic interventions. Further studies are needed to guide the clinician in determining which patients are likely to benefit from a psychotherapeutic intervention in combination with pharmacotherapy.
Fazit für die Praxis Ressourcenaktivierung ist eine effektive Methode, um die störungsspezifische Sichtweise zu erweitern und dem Patienten dazu zu verhelfen, eigene Stärken zu erkennen und zu aktivieren. Im Sinne des Selbstmanagementansatzes von Kanfer [13] sollen die Patienten dabei unterstützt werden, eigene Strategien zu entwickeln, um mit den Widrigkeiten des Lebens aktiv, selbstständig und effizient umgehen zu können.
Understanding the dropout rates of efficacious forms of psychotherapy for patients with binge eating disorder (BED) is an unsolved problem within this increasing population. Up until now the role of psychotherapy process characteristics as predictors of premature termination has not been investigated in the BED literature. Within a randomized controlled trial (N = 78) we investigated the degree to which early psychological process characteristics, such as components of the therapeutic relationship and the experiences of mastery and motivational clarification, predicted premature termination of treatment. We statistically controlled for the influences of covariates such as rapid response of treatment, treatment group, body mass index, Axis II disorder, and patients’ preexisting generalized self-efficacy at baseline. Patients’ postsession reports from Sessions 1 to 5 indicated that low self-esteem in-session experiences was a stable predictor of premature termination. Its predictive value persisted after controlling for the above-mentioned covariates. Exploratory analyses further revealed low self-esteem experiences, low global alliance, and low mastery and clarification experiences as predictors in those patients who explicitly specified discontentment with therapy as reason for premature termination. These results indicate that patients’ self-esteem experiences may not be an epiphenomenon of their specific psychopathology but may represent general mechanisms on which remaining or withdrawing from psychotherapeutic treatment depends. Early psychotherapy process characteristics should therefore be considered in training and evaluation of psychotherapists carrying through BED treatments.

Im vorliegenden Kapitel werden zentrale Charakteristika körperorientierten Arbeitens auf dem Hintergrund dieser allgemeinen Wirkfaktoren reflektiert, insbesondere das therapeutische Beziehungsangebot und die Arbeit mit Grenzen und Emotionen. Bezüge zum Vorgehen in zielorientierter Gesprächs- und kognitiv-behavioraler Therapie sowie zu Erkenntnissen der Neurowissenschaften werden hergestellt.

In der Schweiz beklagen 15–20% der Patienten Einschlafstörungen, ebenso 15–20% Durchschlafstörungen. Entsprechend gehören Schlafstörungen zu den Krankheitsbildern, die in der Praxis von Grundversorgern, jedoch auch von Psychologen und Psychiatern am häufigsten angetroffen werden. Müdigkeit, Antriebsstörung und Energiemangel werden von vielen weiteren Patienten beklagt und führen zu Konsultationen von Ärzten und Psychologen und Inanspruchnahme des Gesundheitssystems. Auch diese Symptome können häufig mit einer gestörten Erholungsfunktion in der Nacht in Verbindung gebracht werden, wobei häufiger eine Korrelation als eine kausale Beziehung besteht. Die Relevanz von Schlafstörungen lässt auch erkennen, wer den Konsum an Hypnotika als Gradmesser heranzieht. Hypnotika werden von einem Drittel bis zur Hälfte der Bevölkerung konsumiert. Früher stammten die Hypnotika vor allem aus der Gruppe der Benzodiazepine. Heute stammen sie zusätzlich aus verwandten Stoffgruppen oder auch aus pflanzlichen und homöopathischen Präparaten.
Using the Consistency Theory Micro-Process Analysis (CMP), the relation between two general change mechanisms, problem activation and resource activation, was studied with the focus on (1) patient behaviour and (2) therapist intervention. The unit of analysis was one minute. The results show that problem and resource activation play different roles in the process of change: problem activation alone did not reliably lead to therapeutic progress; only when combined with thorough resource activation could it unfold its therapeutic potential. Successful therapists chose different degrees of and different timing in applying the two change mechanisms than unsuccessful ones. The results indicate that clearer conceptualizations and specific therapist training are necessary to make better use of resource activation. Copyright (c) 2006 John Wiley & Sons, Ltd.
