Correlations between self-reported and clinician-rated severity of depression are often only moderate, but studies in adults have shown that they increase over time. This study explored whether a similar effect occurs in adolescents and whether initial agreement vs. disagreement between self- and clinician ratings predicts differential outcomes. The analyzed data stem from a randomized controlled trial (DeLight) that explored the effect of bright light therapy as an add-on to treatment as usual in an inpatient sample of adolescents (n = 224). Depression was assessed at four time points (baseline, 4 weeks, 16 weeks, 28 weeks) using the Beck Depression Inventory (BDI-II) as a self-report measure and the Children’s Depression Rating Scale - Revised (CDRS-R) as a blinded clinician rating. The correlation between self- and clinician ratings was only moderate at baseline (r = .40) but increased considerably to a strong correlation at four weeks (r = .72), which was maintained thereafter. The predictive value of initial rater agreement / disagreement for the outcome was small (BDI-II) or non-significant (CDRS-R). Further analyses revealed that the reliability of both instruments increased over time and the factor structure became simpler, with fewer factors and higher factor loadings. These findings indicate increasing homogeneity within self- and clinician ratings over time and suggest that some type of response shift occurred, with adolescents appearing to increasingly view their depression as a unified concept. A consideration of response shifts could lead to more accurate assessments of treatment effectiveness.
AbstractBackgroundNon-suicidal self-injury (NSSI) is of high clinical relevance due to its high prevalence and negative long-term implications. In 2016, the German consensus-based clinical guidelines for diagnostic, assessment and treatment of NSSI in childhood and adolescence were published. However, research indicates that clinical guidelines are often poorly implemented in clinical practice. One crucial part of this process is the training of healthcare professionals to transfer knowledge and capacities to bring guideline recommendations into clinical practice.MethodsThe effect of three different dissemination strategies (printed educational material, e-learning, and blended-learning) on the NSSI guidelines´ recommendations was examined among 671 physicians and psychotherapists via an online-survey. The quasi-experimental study included three measurement points (before the training, after the training, 3-month follow-up) and mixed effects models were used to test for changes in knowledge, competences and attitudes toward NSSI and treatment. Moreover, the transfer of gained competences to practical work and user satisfaction were reviewed.ResultsWith all three training formats, the intended changes of the outcome variables could be observed. Hereby, the printed educational material condition showed the lowest improvement in the scores for the ‘negative attitudes toward NSSI and those who self-injure’. The training effect remained stable throughout the follow-up measurement. The highest application rate of acquired intervention techniques in clinical practice was reported for the blended-learning condition. For all three training strategies, user satisfaction was high and evaluation of training quality was positive, with printed educational material receiving the lowest and blended-learning the highest evaluations.ConclusionsIn summary, all three training formats were perceived to be of high quality and seem to be suited to cover the needs of a heterogeneous group of physicians and psychotherapists. The choice of training method could be driven by considering which training goals are desired to be achieved and by the benefit-cost ratio allowing for tailored training approaches.
Trends in Adolescents' Satisfaction with Their Inpatient Psychiatric Treatment: A Panel Analysis over 7 Years Abstract. Because the German legislature is increasingly enabling adolescents' participation in their psychiatric treatment, we traced changes in satisfaction of 6,893 patients at the end of their stay in 10 hospitals using BEST-J. The treatments resulted in a fair overall satisfaction without any improvement over the 7 years studied. In seven hospitals, we calculated deviation from the overall satisfaction based on annual surveys. Satisfaction with the psychotherapeutic relationship was good from the beginning, whereas the relationship with caregivers improved during the sample period. When new legal standards were introduced, we measured an improvement in some items concerning patient participation (coercive measures information, goals of treatment discussed). Patients were less satisfied with the information about their illness than with medication information. Initially poor satisfaction with leave regulations significantly improved over time. Respect for privacy also improved. Satisfaction with the environment and service organization scored substantially worse than the other items. Peer relations within the ward were rated close to overall satisfaction. The results point to benefits from training in psychotherapy and a growing awareness of closeness-distance issues and patients' rights. Nevertheless, psychoeducation and leave regulations require improvement. Furthermore, patients stated unmet needs for refurbishing and renovating buildings.
Objectives Study of the diagnostic performance and validity of the German Beck Depression Inventory (BDI-II) in a large data set from multiple adult samples. Methods The BDI-II was assessed together with the SCID-I as external criterion in 638 individuals (385 currently or remitted depressed individuals, 253 controls). The screening performance of the BDI-II was calculated for suggested cut-offs from the BDI-II manual and for optimal cut-offs derived from ROC-analyses. Results The internal consistency of the BDI-II was high (> 0,90) and it showed plausible associations with construct-related scales. Optimal cut-off scores of 16+ resulted for MDE (Youden's J = 0.838) and of 14+ for DD (J = 0.814). Conclusions The German BDI-II is a reliable und valid screening tool for detecting depressive episodes and depressive disorders in the adult population. Depending on prioritized goals different cut-off-values can be used.
Zusammenfassung. Da Partizipation in der eigenen psychiatrischen Behandlung Jugendlicher sich mit zunehmender Bedeutung auch in der deutschen Gesetzgebung widerspiegelt, sollten Patientenzufriedenheitsbefragungen zunehmend eine Rolle spielen. Es wurden deswegen Veränderungen in Patientenzufriedenheitsbefragungen an insgesamt 6893 Patient_innen bei Entlassung aus zehn kinder- und jugendpsychiatrischen Kliniken mittels des Fragebogens zur Behandlungseinschätzung stationärer Therapie für Jugendliche (BesT-J) ausgewertet. Die Behandlungen erzielten eine gute Gesamtzufriedenheit, ohne dass über 7 Jahre eine Verbesserung eintrat. An sieben Kliniken wurden jährlich Befragungen durchgeführt und Differenzmaße zur Gesamtzufriedenheit bestimmt. Bei bereits guter Zufriedenheit mit der therapeutischen Beziehung verbesserte sich die Beziehung zu pädagogisch-pflegerischen Kräften im Untersuchungszeitraum. Entsprechend geänderter rechtlicher Vorgaben zu Patientenrechten und Freiheitsentziehung waren Verbesserungen in einigen Items der Partizipation und der Aufklärung (Zwangsmaßnahmen, abgesprochene Therapieziele) zu verzeichnen. Die Patient_innen waren mit der Aufklärung über ihre Erkrankung weniger zufrieden als mit der Aufklärung über die Medikation. Die mäßige Zufriedenheit mit Ausgangsregelungen verbesserte sich signifikant. Anders als die verbesserte Respektierung der Privatsphäre schnitten Umgebungsvariablen, Milieugestaltung sowie die Essensqualität wesentlich schlechter ab. Peerbeziehungen auf der Station erzielten Zufriedenheitswerte nahe der Gesamtzufriedenheit. Die Ergebnisse lassen Erfolge von Weiterbildungen in Psychotherapie und Sensibilisierung für Grenzüberschreitungen sowie hinsichtlich der Patientenrechte erkennen. Sie legen aber Verbesserungspotenziale hinsichtlich der Psychoedukation und der Ausgangsregelungen nahe. Ferner besteht aus Patientensicht ein erheblicher gestalterischer und baulicher Nachholbedarf.
Objectives: Malignant catatonia (MC) is a rare, yet potentially life-threatening neuropsychiatric condition. Evidence on its therapy is weak, treatment recommendations are scarce and predominantly unprecise. The aim of this study was to compare the effectiveness of different MC treatment approaches regarding outcome and severity of MC. Methods: We conducted systematic searches for MC case reports in biomedical databases and the psychiatric archive of University Hospital Ulm. Treatments were compared considering MC severity and temporal aspects. Results: A total of 117 cases were included. Treatment had a significant influence on outcome: treatment with both benzodiazepines and electroconvulsive therapy (ECT) entailed the most favourable, purely supportive therapy the least favourable outcome. Earlier application of benzodiazepines was significantly associated with a favourable outcome. A classification of MC severity was developed. Patients with severe MC were significantly more often subject to intensive care treatment and had a 78% higher risk of dying than in moderate MC. Conclusions: This is the first study to introduce a severity classification for MC, and the largest to compare outcomes of MC treatments with clear distinction from neuroleptic malignant syndrome (NMS). Preferable MC treatment should include early initiation of benzodiazepines and ECT. MC severity could serve as a prognostic instrument.
Zusammenfassung. Theoretischer Hintergrund: Das BDI-II ist ein Selbstbeurteilungsinstrument zur Erfassung des Schweregrads einer Depression. Es liegen kaum Analysen mit Modellen aus der Item-Response-Theorie (IRT) vor. Fragestellung: Wie hoch ist die Messgenauigkeit des BDI-II über die unterschiedlichen Ausprägungen des latenten Traits (Depressivität) hinweg und sind die Kategorien der Items jeweils aufsteigend geordnet? Methode: Anhand von sechs großen Datensätzen aus verschiedenen klinischen Bereichen wurden psychometrische Analysen mit dem Graded Response Model durchgeführt. Ergebnisse: In allen Stichproben fand sich eine hohe interne Konsistenz. Die Schwellenwerte waren mit Ausnahme von Item 6 („Bestrafungsgefühle“) geordnet. Gemäß Testinformationsfunktion misst das BDI-II im mittleren bis hohen Depressionsbereich sehr gut (Reliabilität > .90) und im unteren Bereich gut. Schlussfolgerung: Für das BDI-II ergibt sich eine hohe und relativ gleichbleibende Messpräzision über einen weiten Bereich des latenten Traits, weshalb es insbesondere im klinischen, aber auch im nicht klinisch relevanten Wertebereich zur Erhebung des Schweregrades einer Depression gut geeignet ist.
Development of Mental Health Problems of Girls and Boys in Residential Care Between 2008 and 2020 Abstract. Objective: Studies show a high prevalence of mental health problems in children and adolescents in youth-welfare facilities compared to the general population. However, to date, studies on this development over time are lacking. The present study examines the development of mental health problems and psychosocial functioning in the years 2008 to 2020. Method: Mental health problems were assessed with the CBCL, psychosocial functioning with Axis VI of the MAS. In total, data from 3,269 children and adolescents from residential-care facilities located throughout Germany were included in the analysis. Results: a decrease in externalizing behaviour was observed regarding the frequency of mental health problem in children and adolescents in youth-welfare facilities from 2008 to 2020. Particularly boys aged 12 and older show less deviant behavior. Older girls show high internalizing behavior problem scores over time, but there is no consistent trend. More than a quarter of the institutionalized children in youth-welfare institutions show severely impaired psychosocial functioning at a consistent level over time. Conclusions: In light of the age and gender effects associated with the development of mental health problems over time, there is a need to regard the care situation in youth-welfare institutions.
BACKGROUND:Quantitative (e.g. increasing recreational cannabinoid use) and qualitative (e.g. increasing availability and use of synthetic cannabinoids and cannabis preparations with increased tetrahydrocannabinol content) changes in cannabinoid use may be associated with changes in the prevalence of cannabinoid-related mental and behavioural disorders and, accordingly, changes in the need for medical care. We aimed to investigate if there are changes in the number of inpatient cases (ICs) due to cannabinoid-related disorders in Germany. METHODS:Data were obtained from the Federal Statistical Office of Germany (Destatis) and comprised type and number of hospital main diagnoses (according to ICD-10) of all ICs in Germany in the period 2000-18. Linear trend analysis of absolute and relative annual frequencies (AFs) of ICs with diagnoses related to the use of cannabinoids (DRUCs), and, as controls, alcohol-related psychiatric disorders and schizophrenia-spectrum disorders was performed. RESULTS:Absolute AFs of ICs with DRUCs increased statistically significantly (P<0.0001, trend analysis) in Germany between 2000 and 2018 and corresponding relative AFs increased considerably (4.8-fold increase when comparing 2000 and 2018). Specifically, absolute AFs of ICs with cannabinoid intoxications (P<0.0001), harmful use (P=0.0005), dependence syndrome (P< 0.0001), withdrawal state (P<0.0001), psychotic disorders (P< 0.0001) and residual and late-onset psychotic disorder (P<0.0001) statistically significantly increased. Absolute AFs of schizophrenia-spectrum disorders slightly, but statistically significantly decreased (P=0.008), and alcohol dependence did not statistically significantly change (P=0.844). CONCLUSIONS:Our evaluation demonstrates increasing numbers of ICs with mental and behavioural disorders due to use of cannabinoids in Germany and emphasizes the need for adequate prevention of such disorders.
Zusammenfassung Ziel der Studie Untersuchung der diagnostischen Klassifikationsgüte und Validität des deutschsprachigen Beck-Depressions-Inventars (BDI-II) in einem kombinierten Datensatz aus Erwachsenenstichproben. Methode Das BDI-II und das SKID-I als externes Kriterium wurden an 638 Personen erhoben (385 mit akuter oder remittierter Depression, 253 Kontrollen). Die Screeningperformanz des BDI-II wurde für die im Manual vorgeschlagenen Cut-offs und für optimale Cut-offs anhand von ROC-Analysen berechnet. Ergebnisse Die interne Konsistenz des BDI-II war hoch (> 0,90) und es zeigten sich plausible Zusammenhänge mit konstruktnahen Skalen. Es resultierten optimale Cut-offs mit guter Sensitivität und Spezifität von 16+ für Depressive Episoden (Youden’s J = 0,838) und von 14+ für Depressive Störungen (J = 0,814). Diskussion Das deutschsprachige BDI-II stellt ein reliables und valides Screeninginstrument für Depressive Episoden und Depressive Störungen in der Erwachsenenbevölkerung dar. Je nach Priorisierung können unterschiedliche Cut-offs herangezogen werden.
Background Covid-19 pandemic has been profoundly affecting people around the world. While contact restrictions, school closures and economic shutdown were effective to reduce infection rates, these measures go along with high stress for many individuals. Persons who have experienced adverse childhood experiences (ACEs) have an increased risk for mental health problems already under normal conditions. As ACEs can be associated with a higher vulnerability to stress we aimed to assess the role of ACEs on depressive symptoms during the Covid-19 pandemic. Methods In a cross-sectional online survey, 1399 participants above the age of 18 years were included during the first lockdown in Germany. Via two-way repeated measures ANOVA, differences in depressive symptoms before (retrospectively assessed) and during the pandemic were analyzed. Linear regression analyses were performed in order to identify predictors for increase of depressive symptoms. Results Compared to prior to the Covid-19 pandemic, depressive symptoms increased among all participants. Participants with ACEs and income loss reported about a stronger increase of depressive symptoms. Other predictors for increased depressive symptoms were young age and a lack of social support. Conclusions Based on these results, ACEs are a significant predictor for an increase in depressive symptoms during the pandemic, indicating that personss with ACEs may be a risk group for mental health problems during the current and potential later pandemics. These findings underline the relevance of support for persons who have experienced ACEs and may help to provide more targeted support in possible scenarios due to the current or possible other pandemics. Besides, economic stability seems to be of prior importance for mental health.
Zusammenfassung. Fragestellung: Studien zeigen eine höhere Prävalenz von psychischen Auffälligkeiten bei Kindern und Jugendlichen in stationären Jugendhilfeeinrichtungen im Vergleich zur altersentsprechenden Allgemeinbevölkerung. Es fehlen bislang Untersuchungen zur Entwicklung über die Zeit. In der vorliegenden Studie wurden psychische Auffälligkeiten von Kindern und Jugendlichen in diesen Einrichtungen und die Entwicklung des psychosozialen Funktionsniveaus über die Jahre 2008 bis 2020 untersucht. Methodik: Psychische Auffälligkeiten wurden mittels der Child Behavior Checklist (CBCL) erfasst, das psychosoziale Funktionsniveau mittels der Achse VI des Multiaxialen Klassifikationsschemas (MAS). Insgesamt wurden die Daten von 3269 Kindern und Jugendlichen in Jugendhilfeeinrichtungen analysiert. Ergebnisse: Bei der Betrachtung der Häufigkeit von psychischen Auffälligkeiten bei Kindern und Jugendlichen in stationären Jugendhilfeeinrichtungen über die Jahre 2008 bis 2020 zeigt sich eine Abnahme externalisierender Verhaltensauffälligkeiten, wobei speziell Jungen ab 12 Jahren über die Jahre verglichen weniger auffällig sind. Bei internalisierenden Verhaltensauffälligkeiten gehen die hohen Problemwerte auf die Werte von älteren Mädchen zurück, wobei sich hier kein einheitlicher Trend über die Zeit zeigt. Bei über einem Viertel der Kinder und Jugendlichen ist das psychosoziale Funktionsniveau über die Jahre in etwa gleichbleibend deutlich beeinträchtigt. Schlussfolgerungen: In Anbetracht der Alters- und Geschlechtseffekte im Zusammenhang mit der Entwicklung psychischer Auffälligkeiten über die Zeit ist eine Überprüfung der Betreuungssituation in stationären Jugendhilfeeinrichtungen zu empfehlen.
Background: The Beck Depression Inventory (BDI-II) is a self-report instrument for assessing the severity of depression. To date, publications on psychometric properties based on item response theory (IRT) are largely missing. Objective: To determine how high the measurement precision is across the latent trait and whether the item categories are in ascending order. Methods: Using six large data sets from different clinical settings, we performed psychometric analyses using the graded response model. Results: We identified high internal consistencies in all samples. Apart from item 6 ("penalty feelings"), all categories were ordered. According to the Test Information Function, the BDI-II shows very good measurement precision (reliability > .90) in the moderate to high depression range, and good precision in the lower range. Conclusions: Our data revealed high and relatively stable measurement precision across a broad range of the depression construct. We consider the BDI-II to be welt suited for assessing depression severity levels, particularly in clinical but also in nonclinical settings.
Zusammenfassung Hintergrund Die durch die „coronavirus disease 2019“ (COVID-19) ausgelöste Pandemie hat das Leben von Familien in beispielloser Weise verändert. Während des ersten Lockdowns wurden außerfamiliäre Kontakte erheblich reduziert. Viele Eltern mussten parallel ihre Kinder betreuen und von zu Hause aus arbeiten, während der ökonomische Druck zunahm. Ziel der Arbeit (Fragestellung) Das Ziel der vorliegenden Studie war es, den Zusammenhang von Gewalt in intimen Partnerschaften und psychischen Problemen bei Kindern und Jugendlichen vor und während der COVID-19-Pandemie zu untersuchen. Material und Methoden Mithilfe einer Online-Querschnittsumfrage wurden die Daten von 687 Eltern minderjähriger Kinder in Deutschland erhoben. Demografische und psychosoziale Prädiktoren für Gewalt in intimen Partnerschaften und Assoziationen mit psychischen Gesundheitsproblemen von Kindern vor, während und nach dem ersten Lockdown aufgrund der COVID-19-Pandemie wurden analysiert. Die Umfrage fand vom 18.05.2020 bis zum 21.07.2020 statt. Der Lockdown in Deutschland begann am 23.03.2020 und endete über schrittweise Lockerungen – die ersten Schulen öffneten am 22.04.2020 wieder; die Öffnungen von Schulen, Kindergärten und Kitas zog sich bis Ende Juni 2020. Ergebnisse Ein geringeres Haushaltseinkommen und das Erleben von eigenen belastenden Kindheitserfahrungen erhöhen das Risiko, dass Studienteilnehmende Gewalt in ihrer Beziehung erfahren haben und darüber berichten. Kinder und Jugendliche, die in Familien leben, in denen Gewalt in intimen Partnerschaften vorkommt, wiesen nach Angaben der teilnehmenden Eltern vor und während der Pandemie häufiger höhere Werte für externalisierende Probleme auf, hinsichtlich emotionaler Probleme zeigten sich keine signifikanten Unterschiede. Diskussion Belastende Kindheitserlebnisse erhöhen das Risiko für Gewalt in intimen Partnerschaften – und diese wiederum das Risiko für psychische Probleme der eigenen Kinder. Insofern sollte auch in der psychotherapeutischen Praxis systematisch nach entsprechenden Erfahrungen gefragt und entsprechende Behandlungsangebote sollte empfohlen werden.
ZUSAMMENFASSUNG Hintergrund Der erste Lockdown der Corona-Pandemie im Frühjahr 2020 erforderte die zeitnahe Etablierung der Videotherapie als digitales Angebot, um Kinder und Jugendliche mit psychotherapeutischen Maßnahmen regelmäßig versorgen zu können. Methode Kinder und Jugendliche, Sorgeberechtigte und Therapeuten einer verhaltenstherapeutischen Ausbildungsambulanz wurden im Zeitraum von April bis August 2020 gebeten, im Anschluss an jede videobasierte Psychotherapiesitzung einen eigens konstruierten „Fragebogen zur Akzeptanz der Videotherapie“ (FAV) auszufüllen. Der Fragebogen enthält die Skalen „Therapeutische Beziehung“, „Therapeutische Methoden“, „Umgebungsfaktoren“ sowie „Akzeptanz“. Ergebnis Der Etablierbarkeit einer „therapeutischen Beziehung“ in der Videotherapie stimmten alle 3 Gruppen „völlig zu“. Der Möglichkeit der Umsetzung von Methoden („Therapeutische Technik“), Herstellbarkeit einer konzentrierten Arbeitsatmosphäre („Umgebungsfaktoren“) und Zufriedenheit mit dem Medium Videotherapie („Akzeptanz“) wurde „eher zugestimmt“. Die kritischste Position nahmen die Therapeuten hinsichtlich der Vermittelbarkeit verhaltenstherapeutischer Methoden ein. Prädiktorenanalysen zeigten einen Einfluss von Alter und Geschlecht von Patienten auf die Akzeptanzeinschätzung der Therapeuten. Schlussfolgerung Es scheint möglich zu sein, im Rahmen der Videotherapie eine therapeutische Beziehung zu etablieren und diese – vor allem auch auf Patientenseite – zufriedenstellend zu gestalten. Die Zufriedenheit der Therapeuten schien von weiteren Faktoren abhängig zu sein, wie der Komplexität des behandelten Störungsbildes oder der Definition eines ausreichenden Sicherheits- und Notfallmanagements. Je routinierter der Umgang mit der Videotherapie, desto kreativer gestaltete sich der Einsatz therapeutischer Methoden. Die Videotherapie kann daher als wertvolle Ergänzung zur herkömmlichen „Face-to-face“-Therapie und als zukunftsweisendes Tool in der Kinder- und Jugendlichenpsychotherapie betrachtet werden.
Background Interest in the assessment of patient satisfaction with in-patient psychiatric treatment has steadily increased and several measurement tools are available for the quantification of patients’ experience. However, they are often uni-dimensional or focus mainly on therapeutic relationship and environment, and neglect other important issues such as information about treatment and participation. The BEST questionnaires were developed as comprehensive instruments that include items on all of the mentioned topics. The present study evaluates the psychometric properties of the BEST in a version for adolescents and for parents. Furthermore, the dimensionality of the satisfaction ratings is analyzed. Method Descriptive statistics were applied to data of 1582 adolescents (mean age = 15.0 years, SD = 1.65; 62.4% female) and 1998 parents/guardians assessed in seven in-patient units across Germany. The factorial structure of the BEST questionnaires was determined by exploratory and confirmatory factor analyses, including a bifactor model. Results The psychometric quality of the scales was strong. Correlations with another assessment instrument of patient satisfaction were good to high, indicating good convergent validity. Exploratory factor analyses revealed three factors in adolescents that were labelled as: Therapeutic relationship, environment, and general satisfaction and treatment success. For parents, the same three factors could be distinguished. Confirmatory bifactor models suggested that the vast majority of variance was accounted for by the general factor; the three specific factors provided some additional information. Agreement between the subscales of adolescents and their parents was only moderate. Parents were usually more satisfied. Conclusions The BEST questionnaires can be considered as reliable and valid instruments to not only assess the “classical” aspects of patient satisfaction, but to also assess newer fundamental topics such as children’s rights and treatment participation. For scientific usage, the total score seems superior because of the high explained variance by the general factor, but the subscale scores provide further information. The use of single items seems advantageous for quality management purposes.
