Purpose Thumb carpometacarpal osteoarthritis is a common condition in older adults and may severely impair hand function and quality of life. When conservative treatment fails, trapeziometacarpal arthroplasty is a widely accepted surgical option, with well-documented clinical and functional outcomes. However, the subjective perioperative experience of patients remains poorly explored, despite its growing importance in value-based health care. Methods This retrospective, multicenter observational study included patients who underwent primary implant trapeziometacarpal arthroplasty between November 2007 and December 2023 in two tertiary centers. A self-administered questionnaire specifically designed to assess perioperative experience was sent to all eligible patients. Demographic data, radiographic characteristics, patient satisfaction, perioperative discomfort, postoperative pain resolution, and time to “forgetting the thumb” were analyzed descriptively. Results A total of 119 prostheses in 95 patients were analyzed, corresponding to a 42.6% response rate (95/223 eligible patients), with a mean follow-up of 6.4 ± 4.7 years. Overall satisfaction was high, with 83.2% of patients reporting being satisfied or very satisfied. Locoregional anesthesia was identified as the most unpleasant perioperative element by 22.7% of patients. Postoperative pain resolved rapidly in most cases, and 33.7% of patients reported “forgetting their thumb” within 4 months after surgery. Despite postoperative pain being frequently cited as unpleasant, only 2.5% of patients expressed a desire for improved analgesic protocols. Conclusions Trapeziometacarpal arthroplasty is associated with high patient satisfaction and rapid perceived recovery. Beyond functional outcomes, perioperative experience particularly anesthesia-related discomfort and preoperative information plays a critical role in patient satisfaction. Integrating patient-reported experience into outcome assessment may help optimize perioperative care pathways. Type of study/level of evidence Prognosis IV.
Introduction Pantrapezial osteoarthritis (OA), combining trapeziometacarpal (TMC) and scaphotrapeziotrapezoid (STT) joint involvement, has long been considered a relative contraindication to TMC arthroplasty. The aim of this study was to evaluate the functional and radiological outcomes of TMC prostheses in patients with or without associated STT OA. Materials and methods This retrospective, observational, non-randomized, multicentre study included patients treated with a single mobility TMC prosthesis (ISIS® or HORUS®) between November 2007 and December 2023 in two centres. At final follow-up, patients were assessed using validated self-reported questionnaires (QuickDASH, Buffalo score, TASD), a self-administered Kapandji score, and an indirect estimation of pinch strength (A4 paper-folding test). Preoperative and final follow-up radiographs were analyzed using the Dell, Crosby, and SCFM classifications described by Obert et al. Results One hundred and thirteen prostheses in 94 patients were analyzed, with a mean follow-up of 6.4 years. Forty prostheses were implanted in a context of pantrapezial OA. No statistically significant differences were observed between groups (isolated rhizarthrosis vs pantrapezial OA) regarding functional scores or radiological complications. Radiological progression of STT OA was observed in 20% of cases, without significant functional impact. Conclusion The presence of associated STT OA did not appear to impair clinical outcomes after TMC arthroplasty at mid- and long-term follow-up. Surgical decision-making should primarily rely on patient symptoms and expectations rather than radiographic findings alone.
The aim of our study was to assess the anatomical relationships between wrist arthroscopic portals and the periscaphoid vascular network. Fourteen fresh human cadaver wrists were perfused. Needles were placed in the 3–4, 6R, midcarpal radial (MCR), midcarpal radial scaphoid (MCRS), and scaphotrapezium-trapezoid (STT) portals. Contrast-enhanced CT scans were performed to analyze the constant arteries, their anatomical relationships, and their distance from the needles. Scaphoid vascularization mainly consisted of a dorsal branch along the dorsal ridge and a palmar branch opposite the waist. Our study highlights the probable and more frequent-than-expected presence of an anterior artery also located at the level of the waist. The STT portal showed the closest proximity to the dorsal branch of the scaphoid. The safe distances from this branch were smaller when using the STT portal compared to the MCRS portal. The dorsal arthroscopic portals commonly used to treat scaphoid nonunions carried a low risk of damaging these branches. The periscaphoid vascular network appears to be more complex and potentially more variable than classically described in the literature.
Si la trapézectomie reste le « gold standard » chirurgical de la rhizarthrose, l’avènement des prothèses. trapézométacarpienne et l’innovation technologique les concernant a changé la donne. Leur principal avantage est une récupération fonctionnelle plus rapide. La pose de prothèse chez le sujet jeune reste débattue, mais aucun autre traitement n’a actuellement prouvé de supériorité globale à l’arthroplastie totale. Le but de cette étude était d’évaluer les résultats fonctionnels et les complications des prothèses posées avant 50 ans. Tous les patients opérés avant l’âge de 50 ans entre 2009 et 2023 d’une arthroplastie totale (prothèse à cupule vissée ou cimentée – semi contrainte) en première intention après traitement médical bien conduit ont été revus en consultation de façon prospective. Une évaluation fonctionnelle (mobilités – QDASH PRWE) et radiologique étaient réalisées avec recherche de complications (luxation, descellement, fracture) au plus grand recul. 16 prothèses chez 15 patients ont pu être évalués avec un recul de 68 mois (10–172). 3 patients ont été perdus de vue. Le QDASH moyen était de 24 (9,09–54,55), le PRWE de 34 (1–103). Tous les patients étaient satisfaits ou très satisfaits de l’intervention. Le score de Kapandji moyen était de 9 (6–10). L’opération permettait de retrouver de la force par rapport à l’état préopératoire, mais celle-ci restait inférieure à la force du coté controlatéral dans 80 % des mains dominantes opérées et 62 % des mains non dominantes opérées. Cinq prothèses (38 %) présentaient des anomalies radiologiques non symptomatiques (ossifications, nouveaux ostéophytes, liserés périprothétiques). Il n’existait aucun cas de luxation ou de descellement. Deux prothèses (15 %) ont nécessité une reprise pour ossifications péri prothétique (patients hyper uricémiques ou goutteux). Quatre patients (33 %) avaient changé de travail ou eu un arrêt important. Les résultats fonctionnels à moyen et long terme de l’arthroplastie totale trapézométacarpienne avant 50 ans sont élevés avec un taux de satisfaction important des patients et concordent avec les résultats dans la population générale. Néanmoins on observe un taux de reprise plus élevé secondaire à l’apparition d’ossification. Ces ossifications étaient associées à la présence de goutte ou d’hyperuricémie, facteur intrinsèque connu au niveau du coude. La prothèse trapézométacarpienne reste une solution satisfaisante chez les moins de 50 ans à un moment de leur vie où ils ont « besoin du pouce ». Le suivi précis de ce groupe de patients à haute demande nécessite la mise en place de registres.
Nous évaluons dans cette étude l’amélioration de la symptomatologie sensitive des reprises de canaux carpiens par neurotube en chitosan, comparativement aux reprises sans chitosan.Nous avons interrogé les chirurgiens de la main de deux centres hospitaliers régionaux : un centre hospitalier régional universitaire et un centre hospitalier afin de recenser les cas de reprise chirurgicale de canaux carpiens sur les 7 dernières années.Les variables épidémiologiques et pré opératoires ont été répertoriées à partir des dossiers médicaux.Nous avons recensé la symptomatologie clinique et les scores d’évaluation post opératoire de reprise des patients soit en consultation sur les centres hospitaliers soit par le biais d’une téléconsultation.Toutes les reprises étaient réalisées à ciel ouvert par neurolyse itérative associée ou non à un neurotube ou à un lambeau graisseux de protection.Les analyses statistiques sur les variables qualitatives ont été réalisées avec des tests du Khi2 avec un seuil de significativité fixé préalablement à 5 % et sur les variables quantitatives avec des tests t de Student avec également un seuil de significativité fixé à 5 %.Entre janvier 2015 au et août 2021, 18 patients ont dû être réopérés devant une symptomatologie de canal carpien récidivant ou récalcitrant. 5 patients ont bénéficié de la mise en place d’une protection du nerf médian par neurotube en chitosan.Dans le groupe Neuro T+ les signes sensitifs rapportés par le patient sont améliorés à 65 % et l’ensemble de la symptomatologie clinique est améliorée à presque 60 %.Le total de la symptomatologie clinique sensitive est significativement amélioré pour le groupe Neuro T+ comparativement au groupe Neuro T- avec lambeau à plus de 39 %.C’est également le critère de nombreuses études réalisées sur ce sujet comme nous avons pu le constater lors des recherches bibliographiques, avec notamment l’article de - G. Raimbeau en 2008 6- qui le mentionne très clairement. Nous pouvons comparer nos résultats à ceux obtenus par les équipes Strasbourgeoises d’A Bilasy en 2012 7 - et d’A Carmona en 2019 8 - qui eux évaluent l’implant Canaletto pour la couverture du nerf médian dans ces chirurgies de reprise du syndrome du canal carpien.Dans ce travail et avec ce recul la neurolyse itérative avec ou sans neurotube améliore les canaux carpiens réopérés pour symptomatologie « récalcitrante ou récidivante ».
The humeral bone is subject to torsional forces. In case of displaced shaft fractures, internal fixation remains the standard of care. This retrospective two-center study assessed the fracture union rate and complications after dual 3.5 mm locking compression plate (LCP) fixation using an anterolateral approach. Over a 9-year period, 38 patients underwent surgery in two centers. They had a mean age of 53.7 years (15-97, +/- standard deviation (SD) 26) and there were three open fracture cases (7.9 %). The dominant side was affected in 21 cases (55.3 %) and there were 11 polytrauma patients (29 %). Mean operative time was 78 min (40-124, +/- 19.8 SD). Patients were treated with dual 3.5 mm LCP fixation (6 screws on either side of the fracture line, anterolateral approach without a tourniquet). The first two orthogonal views showing at least 3 cortical bridges out of 4 determined fracture healing, as assessed by two independent raters. Pre- and postoperative complications were tabulated. Clinical outcomes included range of motion (ROM) and return to activities, while functional outcomes were assessed with the Disability of the Arm Shoulder and Hand (DASH), the Constant score, the Subjective Shoulder Score (SSV) and the Mayo Elbow Performance Score (MEPS). Minimum follow-up was 1 year. Four patients were given a shoulder immobilizer to wear for 3 weeks; immediate mobilization was the standard of care for the other patients. Fracture union was achieved in all cases within a mean of 11.7 weeks (6-28 +/- 7.1 SD) without any heterotopic ossification of the brachialis muscle. There were eight patients with preoperative radial nerve palsy and two cases of postoperative palsy. There was one surgical site infection (2.6 %). Return to work for active patients was possible in 87 % of cases within a mean of 23 weeks (6-72 +/- 11 SD). The Constant score was 84.6 (35-100, +/- 13.4 SD), the SSV score was 80.7 (60-100, +/- 8.2 SD), the DASH score was 13.5 (0-38.3, +/- 8.8 SD) and the MEPS score was 85 (55-100, +/- 11.9 SD). Traditional fixation methods provide little control over torsional forces, leading to non-union rates between 3 % and 12 % and delayed union (12 to 20 weeks). The simplicity of the technique described here, and the short operative time, may help explain the low infection rate. Dual plate fixation makes it possible to use more screws and allows nerve exploration and decompression in case of preoperative nerve palsy. Dual plate fixation to treat humeral shaft fractures is a simple and reliable technique.
IntroductionLa classification des fractures doit permettre une communication fiable et reproductible entre différents intervenants. Il doit s’agir d’un système logique et compréhensible ne conduisant pas à un trop grand nombre de catégories à maîtriser. L’objectif de cette étude est de déterminer la fiabilité intra- et inter-examinateur des classifications de Schatzker et Mayo dans l’analyse des fractures de l’ulna proximal.Matériel et méthodeNous avons réalisé une étude intra-observateur et inter-observateur sur 39 radiographies de coudes lésés tirés au sort parmi 74 cas utilisés dans une série pour l’analyse des facteurs prédictifs d’arthrose huméro-ulnaire dans les fractures de l’ulna proximal. Dix relecteurs ont réalisé chacun 2 lectures de ces examens espacées de trois mois. À chaque lecture, nous avons analysé le type lésionnel de la fracture selon les classifications de Schatzker et Mayo. Les fiabilités inter- et intra-observateur ont été mesurées à l’aide des coefficients Kappa de Cohen et Fleiss.RésultatsLa classification de Schatzker pour le premier temps d’évaluation possède une concordance inter-observateur passable (Schatzker T1, к Fleiss : 0,394), tout comme le deuxième temps d’évaluation (Schatzker T2, к Fleiss : 0,351). Le coefficient de concordance intra-observateur moyen entre les 10 évaluateurs pour la classification de Schatzker est évalué comme bon (0,61). La classification de Mayo pour le premier temps d’évaluation possède une concordance inter-observateur passable (Mayo T1, к Fleiss : 0,278), tout comme le deuxième temps d’évaluation (Mayo T2, к Fleiss : 0,292). Le coefficient de concordance intra-observateur moyen entre les 10 évaluateurs pour la classification de Mayo est évalué comme moyen (0,52).DiscussionLes classifications des fractures de l’ulna proximal semblent être peu reproductibles entre différents observateurs puisqu’elles possèdent un coefficient de concordance inter-observateur peu élevé. INTRODUCTION:A fracture classification system should provide a reliable and reproducible means of communication between different parties. It should be logical and understandable, with few categories to memorize. The aim of this study was to determine the intra- and interobserver reliability of the Schatzker and Mayo classification systems for the assessment of proximal ulna fractures. MATERIALS AND METHODS:Intra- and interobserver reliability studies were conducted on 39 X-rays of injured elbows drawn randomly from 74 cases previously used in a series on predictors of ulnohumeral osteoarthritis in proximal ulna fractures. Ten observers independently reviewed these X-rays on 2 separate occasions 3 months apart. The fracture type was assessed according to the Schatzker and Mayo classification systems during each reading session. Cohen's and Fleiss' kappa were used to measure the intra- and interobserver reliability. RESULTS:The Schatzker classification had a fair interobserver reliability for the first (Schatzker R1, Fleiss' κ: 0.394) and second (Schatzker R2, Fleiss' κ: 0.351) readings. The mean intraobserver reliability value between the 10 reviewers for the Schatzker classification was rated as substantial (0.61). The Mayo classification had a fair interobserver reliability for the first (Mayo R1, Fleiss' κ: 0.278) and second (Mayo R2, Fleiss' κ: 0.292) readings. The mean intraobserver reliability value between the 10 reviewers for the Mayo classification was rated as fair (0.52). DISCUSSION:The classification systems for proximal ulna fractures showed poor reproducibility between the different observers since they had low interobserver agreement values. Nevertheless, their use remained reliable since the measured intraobserver agreement value was deemed substantial. LEVEL OF EVIDENCE:IV; retrospective.
Upper limb chronic exertional compartment syndrome (CECS) has been described in amateur and professional motorcycle racers, but there is no published data about its prevalence. The purpose of this study was to define the awareness, prevention and prevalence of this syndrome in licensed motorcycle racers in competition in France. Secondary purposes were to determine the functional impact of CECS and post-treatment outcomes. The 20,641 licensed racers in competition of the French Motorcycle Federation were sent a self-assessment questionnaire about upper limb pain and CECS physical examination findings, functional impact and treatment outcomes. The satisfaction level was assessed after each type of treatment. Acceptability rate was 6.35% with 1311 racers responding. CECS was unknown by 29% of racers. Prevention methods were unknown by 10% of racers. Less than 50% of racers modified their bikes. The prevalence of upper limb CECS in competitive racers was 9%: 8.7% forearm, 0.2% thenar, 0.1% hypothenar and 0.4% first dorsal interosseus compartments. The prevalence was 16% in international level racers, 11% in national level racers and 7.3% in regional level racers. A quarter of racers were satisfied or very satisfied with the outcomes of conservative therapy and rehabilitation. Only 67 racers underwent surgical treatment for their upper limb CECS: 31 by open fasciotomy, 23 by minimally invasive fasciotomy and 13 by endoscopy-assisted compartment release. In these 67 racers, the mean visual analog score for pain improved significantly (p < 0.001 95% CI [3.1-4.5]) with 81% satisfied or very satisfied with surgery outcomes. This epidemiologic self-assessment questionnaire for upper limb CECS is a new concept. This study screened for CECS and offer information regarding evaluation, treatment, and management.
The scapholunate joint is one of the keystones of the wrist kinematics, and its study is difficult due to the carpal bones size and the richness of surrounding ligaments. We propose a new method of quantitative assessment of scapholunate kinematics through bone motion tracking in order to investigate scapholunate ligament lesion as well as repair techniques. On 6 intact wrists, steel beads were inserted into the bones of interest to track their motions. Experimental set up allowed wrist flexion extension and radio-ulnar deviation motions. Low-dose bi-planar radiographs were performed each 10° of movement for different configurations: 1) intact wrist, 2) scapholunate ligament division, 3) repair by soft anchors at the posterior then 4) anterior part. Beads' 3D coordinates were computed at each position from biplanar X-Rays, allowing accurate registration of each wrist bone. The Monte Carlo sensitivity study showed accuracy between 0.2° and 1.6 ° for the scaphoid and the lunate in motions studied. The maximum flexion-extension range of motion of the scaphoid significantly decreased after anterior repair from 73° in injured wrist to 62.7°. The proposed protocol appears robust, and the tracking allowed to quantify the anchor's influence on the wrist kinematics.
Le ligament scapholunaire (SL) est une structure anatomique essentielle du carpe et sa lésion est préjudiciable à moyen et long termes. L’objectif est une analyse comparée de la cinématique scapholunaire sur des pièces anatomiques dont le SL est sain, puis lésé et enfin réparé par ancres souples. Sur 6 poignets sains, 3 billes en acier étaient insérées dans le scaphoïde, lunatum, radius et 3e métacarpe afin de définir des repères locaux. La reconstruction 3D du radius à l’aide d’un TDM préalable a permis de définir les axes du poignet — flexion-extension (FE), radio-ulnaire (RU) et prono-supination. Le banc d’essai permettait des mouvements passifs du poignet de 30° de flexion à 60° d’extension et de 20° d’inclinaison radiale à 30° ulnaire. Des radiographies bi-planaires à faible dose étaient réalisées tous les 10° pour chaque configuration — SL sain, lésé, après réparation par des ancres souples à la partie postérieure (I1) puis antérieure (I2) du ligament. Les coordonnées des billes étaient récupérées sur les radiographies. À partir des coordonnées et du repère d’interprétation du poignet, des routines Matlab ont permis de calculer la cinématique des os. La mesure des amplitudes de mouvements a pu être réalisée pour tous les axes, en rotation et translation, et pour chaque configuration du poignet. Lors des mouvements FE du poignet, les amplitudes moyennes (degré) pour les 6 sujets, du scaphoïde suivant FE pour les configurations saines, lésées, I1 et I2 étaient respectivement de 63,82+, 72,97+, 65,61 et 62,72. Pour le lunatum de 47,05+, 47,31+, 49,63 et 52,90. L’amplitude moyenne du déplacement transversal (mm) entre scaphoïde et lunatum — 0,85+, 1,07+, 0,71 et 0,86. Les résultats sont concordants avec la littérature, en termes de différences d’amplitudes entre les poignets sains et lésés et de variabilité interindividuelle. Malgré le faible effectif de notre population, nos résultats mettent en évidence, à l’instar d’autres équipes, une amélioration de la cinématique du scaphoïde par la réparation ligamentaire combinée, antérieure et postérieure. Par contre cela semble moins évident pour la cinématique du lunatum. Cette étude cadavérique ouvre la voie à une technique d’évaluation des moyens de réparation du SL. Les perspectives d’avenir sont l’évaluation de la cinématique in vivo et la personnalisation des traitements par la modélisation en éléments finis.
Introduction: Non-union of the humeral shaft is a rare condition. In these patients, more stable fixation with or without autograft results in nearly 95% union rate. The purpose of this study was to evaluate bone union following use of recombinant human bone morphogenetic protein-7 (rhBMP7) for treating humeral non-union.Material and methods: This was a prospective study of resistant non-union cases treated by repeated fixation and addition of rhBMP7. The case series consisted of 16 patients with an average age of 53.8 years (24-71). Patients presented with non-union of the humeral shaft and had experienced at least one failed attempt at surgical repair. Osigraft® (rhBMP7) was added to the non-union site after decortication, medullary canal reaming and fixation with one or two plates. The average time elapsed between the initial fracture and the second revision was 31 months (5-103). The patients had undergone an average of 2.3 procedures (1-6).Results: All patients were reviewed after at least 24 months of follow-up. No neurological complications were reported. One failure occurred in a non-compliant patient with septic non-union who had undergone four previous procedures. The other patients experienced bone union after an average of 12.4 months (6-14), with no further procedures required. The patients were able to return to their normal daily activities.Conclusion: Failure of the initial fracture treatment (unstable fixation, postoperative bone defect) is the primary cause of non-union. Although autograft is the gold standard treatment for non-union cases, the course of action to take if this primary strategy fails has not been defined. During secondary use (due to failed autograft procedure) and when there is no requirement for a structural graft (humerus can be shortened), providing stable fixation and adding a growth factor leads to bone union even in a septic environment.
Internal fixation with volar locking plates has revolutionized the treatment of distal radius fractures. Manufacturers have introduced plate designs that closely follow the anatomy of the distal radius. However, use of volar plates has also led to the emergence of new types of complications. While the use of monoaxial or polyaxial locking screws and of minimally invasive techniques (arthroscopy, preservation of pronator quadratus) increases the cost of the surgical procedure, it results in a tangible benefit for patients, allowing them to move their wrist almost immediately after surgery and to quickly regain their autonomy. We reviewed the literature to analyze the level of proof.
Les fractures de l’articulation inter phalangienne proximale (IPP) sont fréquentes, avec souvent des lésions associées des parties molles. Parfois, la destruction articulaire ne permet pas de reconstruire l’articulation du fait de la comminution. Nous avons évalué les patients ayant présenté une fracture de l’IPP non reconstructible traitée chirurgicalement, avec ou sans lésions associées des parties molles. Il s’agissait d’une évaluation rétrospective monocentrique des patients ayant présenté une fracture articulaire de l’IPP touchant la tête de P1. Étaient inclus les patients opérés en urgence, avec ou sans lésions associées. Huit patients ont été inclus (12 IPP) d’âge moyen 50,6 ans (18–76), tous des hommes. Il s’agissait d’un traumatisme par scie circulaire chez 5 patients (9 fractures). Les lésions étaient pluritissulaires chez 6 patients 8 (10–12 IPP). Toutes ces fractures ont été traitées par prothèse de Swanson avec réparation des lésions associées si nécessaire. Nous avons évalué cliniquement la fonction de chaque articulation et la fonction globale (TAM), le degré de gêne quotidienne ainsi que la douleur sur EVA ainsi qu’avec es radiographies de face et de profil. Les complications ont aussi été relevées. Au dernier recul, 8 patients (12 doigts) ont été revu avec un recul moyen de 41,25 mois (12–108). L’arc moyen de mobilité active de l’IPP était de 47,5° ± 18,5 (20–70). L’arc d’amplitude total (TAM) était de pour les doigts longs 174,4° ± 31,6 (130–215). Aucun patient n’a présenté d’infection, un patient a présenté un syndrome douloureux régional complexe de type 1, un patient une nécrose de son lambeau de couverture, 2 patients une clinodactylie (dont une sur fracture de prothèse). Aucun doigt n’a été amputé secondairement. La plupart des techniques évaluées dans la littérature concernent des fractures isolées, reconstructibles. Les lésions pluritissulaires sont de moins bon pronostic, avec une réparation souvent complexe et des complications associées. Les fractures de l’IPP sont un challenge thérapeutique, notamment lorsque des lésions y sont associées. Près d’un patient sur deux présentera une complication. En cas de défect touchant la tête de la phalange proximale, l’arthroplastie en urgence permet une réintégration rapide du doigt.
Introduction: Used routinely in maxillofacial reconstructive surgery, the chondrocostal graft is also used in hand surgery. The purpose of this overview was to analyze at long follow-up the radiological and histological evolution of this autograft, in the hand and wrist surgery. Materials and methods: Since 1992, 144 patients have benefitted from a chondrocostal autograft: 116 osteoarthritis of the thumb carpometacarpal joint, 18 radioscaphoid arthritis, six articular malunions of the distal radius, four kienbock, and four traumatic loss of cartilage of the PIP joint. Magnetic Resonance Imaging (MRI) was performed in 19 patients and histological study in 12 patients with a mean follow-up of 68 months (4–159). Results: Whatever the indication, the reconstruction by a chondrocostal or ostochondrocostal graft has allowed us to obtain satisfactory clinical results at long follow-up. The main question was the viability of the graft. The radiological study has shown the non-wear of the graft and a certain degree of ossification. The MRI confirmed a very small degree of osseous metaplasia but its viability. The biopsies showed a neo-vascularization of the cartilage. Conclusion: Despite the strong mechanical strain in the hand and wrist, the chondrocostal graft is a biological arthroplasty, trustworthy and secure at long time even if it can cause infrequent complications inherent to this type of surgery. Despite the inevitable histological modification, the cartilage remains alive and is of satisfactory quality at long term follow-up and fulfilling the requirements for interposition and reconstruction of an articular surface.
Few studies on the anatomy of the radial epiphysis have been published in the past 10 years. However, with the availability of new intra- and extra-medullary implants and the recent rash of avoidable iatrogenic injuries, now is the time for a more detailed description of the metaphyseal-epiphyseal regions in the distal radius. Published studies on distal radius anatomy in recent years have focused on three aspects: distal limit and watershed line, dorsal tubercle, and wrist columns. Furthermore, a fresh look at distal radius biomechanics shows that the loads experienced by the distal radius vary greatly. This information should be taken into account during volar plating of distal radius fractures.
Les lésions ostéocartilagineuses limitées du coude ont un impact fonctionnel important car elles surviennent chez des patients souvent de moins de 40 ans avec peu de solutions thérapeutiques définitives. Nous rapportons 5 cas de greffe de cartilage costal au coude. Trois cas de nécrose du condyle latéral (2 ostéochondrites chez 2 adolescents de 16 ans et une nécrose post-chimiothérapie chez une patiente de 26 ans, et 2 cas d’arthrose localisée, chez 2 patients de 28 et 39 ans) ont été opérés pour des douleurs et une raideur en flexion ou en prono-supination. Dans les cas de nécrose condylienne une greffe ostéochondrale prélevée au niveau de la 8e cote homolatérale a été réalisée et fixée par 2 vis après résection de la nécrose par voie latérale. Dans les cas d’arthrose localisée de l’ulna, un resurfacage par un lambeau de périchondre fixé par 4 ancres prélevé sans interruption de la continuité costale au niveau de la même 8e cote a été réalisé par voie postérieure. Les patients ont été à revus à 3, 6, 12 mois et au plus rand recul par un opérateur indépendant avec évaluation fonctionnelle du coude (amplitudes, scores MEPS, QDASH) et radiologique ainsi qu’IRM du greffon cartilagineux. Tous les patients ont été revus avec un recul de 2,4 ans (1–4,8 ans). L’amélioration de la douleur, de la mobilité en flexion–extension ou en prono-supination, du MEPS et du QDASH était significative entre le préopératoire et le postopératoire. Seuls les 2 adolescents présentaient une fonction du coude normale et comparable au plus grand recul de 4 ans. L’évaluation radiologique et IRM retrouvait un greffon viable au plus grand recul. Aucune complication au niveau du coude ou au niveau du site donneur n’ont été relevées. Si les perforations de la zone nécrotique (sous arthroscopie ou à ciel ouvert) demeurent un traitement classique de l’ostéochondrite, l’arthrose localisée huméro-ulnaire chez un adulte actif n’a pas de solution idéale. La greffe de cartilage costal, rapportée par Sato en 2003 puis par Shimada en 2012 peut être utile dans ces situations de destruction ostéocartilagineuse localisée.
