Introduction En France, la vaccination contre le papillomavirus humain (HPV) est remboursée chez les 11-14 ans (vaccination de routine) et les 15-19 ans (rattrapage) chez les filles depuis 2007 et chez les garçons depuis 2021. La pandémie de COVID-19 a entraîné d'importantes perturbations dans la prestation des soins de santé. Notre objectif était d'explorer les implications de la pandémie sur la vaccination anti-HPV, en particulier pour les adolescents dont l’âge se situe à la limite supérieure des recommandations, et qui n'ont pas été vaccinés en raison du manque d'accès au vaccin pendant cette période. Méthodes Nous avons étudié les jeunes filles éligibles au vaccin anti-HPV en 2017-2021 en utilisant les données médico-administratives françaises (SNDS). Elles ont été considérées comme ayant initié la vaccination HPV si au moins une dose de vaccin HPV leur avait été délivrée. Les garçons n'ont pas été inclus dans l'analyse en raison d'un recul insuffisant sur les données de vaccination les concernant. L'effet de la pandémie de COVID-19 a été étudié en estimant la proportion mensuelle d'initiation (mPI) au cours d'une année donnée. Le taux annuel d'initiation de la vaccination contre le HPV a été décrit de 2017 à 2021, et comparé entre 2019, 2020 et 2021 à l'âge de 19 ans. Résultats Au total, 1 408 645 filles ont initié la vaccination contre le HPV entre 2017 et 2021. Des mPI plus faibles ont été observés au moment du premier confinement (mars-mai 2020) par rapport à la même période des années précédentes. Si le taux annuel d'initiation à la vaccination HPV a augmenté de 1,3 % en 2017 à 3,4 % 2019 chez les 19 ans, il était significativement plus faible en 2020 (3,1 %) par rapport à 2019 (p<0,0001), montrant que certaines jeunes filles de 19 ans sont sorties des recommandations sans avoir pu se faire vacciner. Les taux annuels d'initiation au HPV étaient significativement plus élevés en 2021 (3,9%) qu'en 2020 chez les jeunes de 19 ans (p<0,0001). Conclusion La pandémie de COVID-19 a ralenti la progression de la couverture vaccinale HPV en 2020, et a certainement exclu de la vaccination certaines jeunes filles ciblées par les recommandations. Nos résultats soutiennent l'importance de l'extension des recommandations aux adolescentes de 19 ans et plus en France pour leur donner une opportunité supplémentaire de se faire vacciner.

L'étude PAPILLON a utilisé des données médico-administratives pour suivre l'évolution du taux de couverture vaccinale (CV) HPV chez les 11-19 ans. Méthodes La CV annuelle a été décrite de 2017 à 2021. La vaccination partielle a été définie comme la délivrance d'au moins une dose de vaccin anti-HPV. Le schéma complet de vaccination a été défini selon les recommandations françaises actuelles comme la délivrance de deux ou trois doses de vaccin HPV sur une période de 18 mois. Les taux annuels d'initiation à la vaccination anti-HPV ont été estimés chez les 11-14 ans et les 15-19 ans. La CV cumulée a été estimé chez les adolescents âgés de 11 à 19 ans au moment de la première vaccination. Résultats Globalement, 1 408 645 femmes et 280 553 hommes ont initié une vaccination contre le HPV entre 2017 et 2021. Les initiations ont eu lieu entre 11 et 14 ans pour 65,9 % des femmes et 60,8 % des hommes, avec un délai médian entre les deux premières doses de 192 jours et 180 jours, respectivement. En 2021, la CV cumulée pour le schéma partiel à 15 ans était de 46,6 % pour les femmes et de 9,4 % pour les hommes, tandis qu'à 20 ans, elle était de 40,7 % et de 1,1 %, respectivement. Chez les filles, la CV cumulée pour le schéma complet de vaccination à 16 ans est passé de 14,1 % en 2016 à 30,7 % en 2021. En 2021, les taux d'initiation étaient de 9,3 % à l'âge de 14 ans et de 1,4 % à l'âge de 19 ans. Conclusion La couverture vaccinale contre le HPV a augmenté entre 2017 et 2021 chez les femmes, mais reste inférieure à l'objectif de 80 %. Les résultats de cette étude montrent un démarrage timide mais prometteur de la vaccination chez les garçons. Cette étude permettra de suivre les effets des mesures prises pour améliorer la vaccination, notamment l'extension des compétences en matière de vaccination aux pharmaciens d'officine depuis 2022.

La proportion de schémas complets a été comparée en fonction de l'affiliation à la CMU-C (marqueur de déprivation sociale) au moment de l'initiation de la vaccination.RésultatsL'initiation à la vaccination anti-HPV a été retrouvée pour 1 408 645 filles entre 2017 et 2021 et 233 835 garçons en 2021. En 2021, les taux d'initiation pour les filles étaient de 16,4% à 14 ans et 3,9% à 19 ans. Ils étaient de 9,3% et 1,4% pour les garçons, respectivement. La même année, la CV cumulée pour le schéma partiel (1 dose) à 15 ans était de 46,6% pour les filles (40,7% à 20 ans) et de 9,4% pour les garçons (1,1% à 20 ans). La CV cumulée pour le schéma complet à 16 ans était de 30,7% pour les filles contre 15,5% en 2017. En 2019, 21,0% des filles âgées de 11 à 14 ans avaient reçu une dose de vaccin anti-HPV et 69,6% en avaient reçu 2 tandis que 19,6% des filles âgées de 15 à 19 ans avaient reçu 1 dose, 21,2% 2 doses, et 55,6% 3 doses. La proportion de schémas complets était plus faible et significativement différente chez les filles et les garçons bénéficiaires de la CMU-C que chez ceux qui ne l'étaient pas (tous p<0.0001).ConclusionCette étude montre une progression de la CV des filles entre 2017 et 2021, et un début prometteur de la vaccination anti-HPV chez les garçons. Les disparités socio-économiques jouent un rôle dans la complétion du schéma vaccinal. Dans les prochaines vagues, cette étude permettra de suivre les résultats des mesures visant à améliorer la vaccination (nouveaux vaccinateurs depuis 2022 et vaccination dans les collèges depuis 2023) et d'étudier les déterminants de la vaccination.Liens d'intérêts déclarés :M. BACKGROUND:The French cancer control strategy 2021-2030 aims to achieve 80 % human papillomavirus (HPV) vaccination coverage. Since 2021, HPV vaccination is also recommended for boys aged 11-14 years, with a catch-up vaccination recommended for unvaccinated adolescents aged ≤19 years. The PAPILLON study used claims data to monitor the evolution of HPV Vaccination Coverage Rate (VCR) in the French population. METHODS:The annual HPV VCR was described from 2017 to 2022. Partial vaccination was defined as the dispensing of at least one dose of HPV vaccination. Full scheme vaccination was defined according to the current French recommendations as two or three doses of HPV vaccine over an 18-month period. Annual HPV vaccine initiation rates were estimated on 11-14 and 15-19-year-olds adolescents. Cumulative VCR were estimated on adolescents aged between 11 and 19 years at the time of first vaccination. RESULTS:Overall, 1,773,900 females and 592,167 males initiated HPV vaccination between 2017 and 2022. Initiations occurred between 11 and 14 years for 67.3 % of females and 62.4 % of males with a median time between the first two doses of 195 days and 190 days, respectively. In girls, the cumulative vaccination rate for the partial scheme vaccination at 15 y.o. increased from 28.1 % in 2017 to 50.9 % in 2022. Similarly, the cumulative vaccination rate for the full scheme vaccination at 16 y.o. increased from 15.5 % in 2017 to 33.8 % in 2022. In 2022, the initiation rates for males were 12.6 % at age 14 and 1.9 % at age 19. CONCLUSIONS:HPV vaccination coverage increased between 2017 and 2022 among girls targeted by the recommendation but remains insufficient. The results of this study show a tentative but promising start to vaccination in boys. This study will monitor the effects of actions taken to improve vaccination, including the extension of vaccination competencies to community pharmacists since end of 2022.
IntroductionLa prise en charge et l'espérance de vie des patients atteints de mucoviscidose se sont considérablement améliorées au cours des dernières décennies, ce qui a conduit à un nombre croissant de diagnostics de comorbidités, notamment de cancers, chez ces patients.ObjectifsLes objectifs de l'étude étaient de caractériser l'épidémiologie des cancers entre 2006 et 2017 chez les patients atteints de mucoviscidose, et d'évaluer le délai entre la transplantation pulmonaire (pour les patients concernés) et la survenue du premier cancer.MéthodesLes dossiers médicaux des patients présents entre 2006 et 2016 dans le registre français de la mucoviscidose ont été chainées à leurs données de remboursement de l'Assurance maladie (SNDS). Les patients ayant reçu un diagnostic de cancer ont été identifiés parmi les patients avec et sans transplantation pulmonaire. Les taux de prévalence et d'incidence annuels ont été estimés de 2006 à 2017.RésultatsSur les 7671 patients inclus dans le registre français de la mucoviscidose, 6187 patients (80,7 %) ont pu être chainés au SNDS (51,9 % d'hommes, âge moyen=24,7 ans).Parmi eux, 1006 (16,3 %) patients avaient bénéficié d'une transplantation pulmonaire. La proportion de cas prévalents de tous types de cancer a augmenté entre 2006 et 2017, passant de 0,3 % à 1,0 % chez les patients non transplantés, et de 1,3 % à 6,3 % chez les patients transplantés. Par rapport à la population générale, l'incidence des cancers était significativement plus élevée chez les patients non transplantés (ratio d'incidence standardisé (SIR=2,57, 95% IC [2,05-3,17]) et chez les patients transplantés (SIR=19,76, 95% IC [16,45-23,55]). Chez les patients qui n'avaient pas de cancer avant leur transplantation pulmonaire (n=982), le délai médian entre la transplantation et l'apparition d'un cancer était de 3,9 ans. Dix ans après la transplantation, la probabilité d'avoir un cancer était de 13,7 %. Les sites les plus fréquents de néoplasmes malins étaient hématologiques, cutanés et digestifs.ConclusionLe fardeau global du cancer chez les patients atteints de mucoviscidose est élevé, en particulier après une transplantation pulmonaire. To estimate and compare costs associated with all-cause healthcare resource use (HCRU), stroke/systemic embolism (SE) and major bleedings (MB) between patients with non-valvular atrial fibrillation (NVAF) initiating apixaban and patients initiating other oral anticoagulants (OAC). An observational retrospective cohort generated from the French National Health System healthcare claims database (SNDS). Patients were included between 2014/01/01 and 2016/12/31 and followed until 2016/12/31. We used 4 sub-cohorts of AC-naive patients with NVAF, respectively initiating apixaban, dabigatran, rivaroxaban or VKAs. We matched patients initiating apixaban with patients initiating each of the other OACs using 1:n propensity score matching. We estimated all-cause HCRU costs and events-related costs from a medical care perspective by OAC treatment (euros) per patient per month (PPPM). We compared costs between matched patients initiating apixaban and those initiating each of the OAC using generalised linear models with gamma distribution and two-part models. There were 175,766 patients in the apixaban (n=68,208)-VKA (n=107,558) matched cohort, 42,490 in the apixaban (n=21,245)-dabigatran (n=21,245) matched cohort, and 181,809 in the apixaban (n=81,759)-rivaroxaban (n=100,050) matched cohort. Patients initiating apixaban had lower all-cause HCRU costs than patients initiating VKA (€1,105 vs. €1,578; p<0.0001), dabigatran (€993 vs. €1,140; p<0.0001) and rivaroxaban (€1,013 vs. €1,088; p<0.0001). Patients initiating apixaban also had lower costs related to stroke/SE and MB than patients initiating VKA (respectively €183 vs.€449; p<0.0001 and €147 vs.€413; p<0.0001), rivaroxaban (respectively €145 vs.€197; p<0.0001 and €129 vs.€193; p<0.0001), and lower costs related to stroke/SE than patients initiating dabigatran (€135 vs.€192; p<0.02). Costs related to MB were not significantly different in patients initiating apixaban versus patients initiating dabigatran (€119 vs.€149; p=0.07). All-cause HCRU and most events-related costs were lower in patients initiating apixaban compared to patients initiating other OAC. These findings suggest that apixaban may be cost-saving (all-cause HCRU costs) compared to all therapeutical alternatives.
Ibrutinib is a Bruton's Tyrosine Kinase Inhibitor indicated for the treatment of several B-cell malignancies: Chronic Lymphocytic Leukemia (CLL), Mantle Cell Lymphoma (MCL) and Waldenström's Macroglobulinemia (WM). Medical resource utilization (MRU) and associated costs for patients treated with ibrutinib in France in real-life settings are unknown and need to be examined. All patients affiliated with the general scheme (around 80% of the French population) with a first dispensing of ibrutinib from August 1, 2017 (date of reimbursement in France) to December 31, 2018 were identified in the French national health insurance database (SNDS). Indications for which ibrutinib was supposed to be used were defined by an algorithm based on ICD-10 codes to identify the disease (CLL, MCL or WM), and on codes of previous and concomitant dispensed drugs to understand treatment line (first line –L1– or second or later line –L2+) and therapeutic combination (monotherapy or combination). Costs were described per month from a collective perspective for each indication and by categories (medications, hospitalizations and others). The population consisted of 2,235 patients who initiated ibrutinib in the first 18 months of the Marketing Authorization in France. The total monthly costs were respectively €6,203, €6,526, €8,579, €6,432 in CLL L1 monotherapy (N=121), CLL L2+ monotherapy (N=853), MCL L2+ monotherapy (N=215) and WM L2+ monotherapy (N=243). These costs were mainly driven by the costs of disease-specific cancer treatment (between 87.3% and 92.2% of total cost) and hospitalization costs. The most frequent MRU will be presented in the poster. This is the first study estimating the costs for patients with CLL, MCL and WM, treated with ibrutinib in France. However, no distinction could be made between costs specific to the management of these diseases and those related to comorbidities. Future analyses using data from 2019 and 2020 will be made to update Results.
Describe and compare healthcare resource utilization (HCRU) among non-valvular atrial fibrillation (NVAF) patients newly treated with apixaban vs. VKA (vitamin K antagonist), rivaroxaban or dabigatran. All adult patients with NVAF newly initiating apixaban, VKA, rivaroxaban or dabigatran between January 2014 and December 2016 (absence of OAC use in the previous 24 months) were identified using the SNIIRAM database. Different types of HCRU were described: medical visits, nurse visits, concomitant drugs, biology dosings, medical procedures and hospital stays. HCRU were compared between groups with generalized linear model adjusted on propensity score, taking account of the follow-up time of patients as offset. Final cohorts included 87,565 apixaban, 112,628 VKA, 100,063 rivaroxaban, and 21,245 dabigatran patients. Over the follow-up period, apixaban patients had an average of 1.8 medical visits per month (SD: 1.9), mostly with general practitioners (89.9%) and cardiologists (52.2%). Mean number of nurse visits was 4.5 per month (SD: 8.5) for 66.3% of patients treated with apixaban. Mean number of concomitant drugs packages was 13.4 per month (SD: 18.0), and most frequent drug classes were antiarrhythmics, beta-blocking agents, analgesics in 68.2%, 64.1%, 60.5% respectively. Half of patients treated with apixaban had monthly dosing of creatinine, glucose and transaminases (56.5%, 55.5%, and 53.1%). Most frequent medical procedures were Doppler, thorax radiography and electrocardiography, 48.5%, 27.6%, and 26.8% and 47.7% of patients had at least one hospitalization. For all HCRU types, apixaban patients had significantly fewer HCRU than VKA patients. For drug claims, biology, and medical procedures, apixaban patients had significantly fewer HCRU than rivaroxaban and dabigatran patients. This large real-world study found lower adjusted HCRU for apixaban compared to VKA and, to a lesser extent, compared to rivaroxaban and dabigatran.
En raison de leur utilisation intensive, les effets des anticoagulants oraux directs (AOD) sur la fibrillation auriculaire non valvulaire (FANV) doivent être évalués dans les conditions réelles d'utilisation. L'objectif était de comparer en vraie vie la sécurité, l'efficacité et la mortalité chez des patients atteints de FANV et initiant apixaban versus les antagonistes de la vitamine K (AVK), rivaroxaban et dabigatran. Étude observationnelle utilisant les données du Système national des données de santé (SNDS) et incluant tous les adultes atteints de FANV ayant initié un AOD entre 2014 et 2016. Trois comparaisons ont été réalisées : apixaban versus AVK, apixaban versus rivaroxaban et apixaban versus dabigatran, sur les critères suivants : saignements majeurs (sécurité), accidents vasculaires cérébraux (AVC) et événements thromboemboliques systémiques (STE, efficacité) et mortalité toutes causes. Un appariement sur score de propension (1 : n) a été utilisé comme méthode principale et un ajustement sur facteurs de confusion comme analyse de sensibilité. Au total, 321 501 patients ont été étudiés, dont 35,0 %, 27,2 %, 31,1 % et 6,6 % ont initié respectivement AVK, apixaban, rivaroxaban et dabigatran. Après appariement sur score de propension, l'utilisation d'apixaban était associée à un risque de saignement plus faible par rapport aux AVK (HR = 0,43, CI 95 % : 0,40–0,46) et au rivaroxaban (HR = 0,67, IC 95 % : 0,63–0,72), mais pas par rapport au dabigatran (HR = 0,93, IC 95 % : 0,81–1,08). apixaban était associé à un risque plus faible d'AVC et de STE que les AVK (HR = 0,60, IC 95 % : 0,56–0,65), mais pas que le rivaroxaban (HR = 1,05, IC 95 % : 0,97–1,15) ou le dabigatran (HR = 0,93, IC95 % : 0,78–1,11). La mortalité toutes causes était plus faible avec apixaban qu'avec les AVK, mais pas comparé au rivaroxaban ou au dabigatran. Apixaban était associé à une meilleure sécurité, efficacité et mortalité comparé aux AVK. Apixaban présentait une sécurité supérieure à celle du rivaroxaban et à une sécurité similaire à celle du dabigatran, ainsi qu'une efficacité similaire à celle du rivaroxaban ou du dabigatran.
The number of CER continues to increase, but the different approaches used make difficult to compare study results. We conducted a CER using multiple statistical approaches, and reflected on strengths and limitations. These analyses were conducted as part of the NAXOS study, a cohort (2014-2016) of patients with non-valvular atrial fibrillation initiating an oral anticoagulant (OAC), identified in the SNDS French database. Safety, effectiveness and all-cause mortality endpoints were compared between apixaban and other OACs (VKAs, rivaroxaban and dabigatran), using four approaches: propensity scores (PS) adjustment (main analysis), simple adjustment, PS matching, and high dimensional (hd)PS matching. Overall, 321,501 patients were analyzed. Apixaban showed significant superiority vs. VKAs for all outcomes with consistent results across approaches. Main analyses also showed statistical superiority of apixaban vs. dabigatran in safety; and vs. rivaroxaban in safety and all-cause mortality. For others CER approaches, adjustment on PS results were globally confirmed, even if minor discrepancies were identified between adjustment and matching methods, with loss of significance. Indeed, for safety, compared with dabigatran, apixaban was associated with less bleeding according to adjustment on confounders and on PS (HR=0.85 [0.76-0.95] and HR=0.86 [0.76-0.96], respectively), whereas matching on PS or hdPS showed no difference (HR=0.93 [0.81-1.08] and 0.87 [0.75-1.00], respectively). Likewise, for all-cause mortality, adjustment on confounders and on PS showed superiority of apixaban over rivaroxaban (HR=0.89 [0.85-0.94] in both analyses) while matching on PS or hdPS suggested similarity (HR=0.97 [0.93-1.02] in both instances). Otherwise, in all analyses, high dimensional PS did not optimize PS performance. The four methods produced consistent results, even in matching approaches, which lowered sample sizes and statistical power. This consistency underlines the robustness of the comparisons in a large population, which confirmed clinical trial results. Future research should continue to investigate comparative approaches to increase external validity.
Oral anticoagulants (OAC: vitamin K antagonists [VKAs], dabigatran, rivaroxaban, and apixaban) are used for prevention of stroke and systemic embolism in patients with non-valvular atrial fibrillation (NVAF). While results from pivotal randomized trials are available, French-specific, “real-life” data pertaining to the four OAC treatments available are awaited. This analysis of the NAXOS study aimed to describe demographic and clinical characteristics of NVAF patients initiating an OAC. All patients aged ≥ 18 years with NVAF and newly initiating one of the study OACs (i.e. no dispensation of the same OAC in the 24 prior months) between 2014 and 2016 were identified in the French national health insurance database (SNIIRAM–a comprehensive in- and outpatient healthcare consumption database). Patients were allocated to 4 different treatment cohorts. To maximise the comparability of populations, only OAC-naive patients (i.e. no dispensation of any OAC during the 24 months prior) were studied. Overall, 321,501 AC-naive NVAF patients aged 18+ were identified. Of these, 112,628 (35.0%) initiated VKAs, while 87,565 (27.2%), 100,063 (31.1%), and 21,245 (6.6%) initiated apixaban, rivaroxaban and dabigatran, respectively. Median follow-up durations were 218, 213, 205 and 186 days for VKA, apixaban, rivaroxaban and dabigatran cohorts, respectively. Males represented 48.8% of patients in VKA cohort (mean age: 78.5), 51.2% in apixaban cohort (mean age: 74.7), 55.1% in rivarobaxan cohort (mean age: 72) and 54.1% in dabigatran cohort (mean age: 72.7). Median age-adjusted Charlson score was 6 in the VKA cohort, and 4 in the other three cohorts. The NAXOS study confirms the widespread use of OAC treatments in France, suggesting differences between cohorts for age, gender and comorbidities. Thanks to large cohort sizes and prolonged follow-up, longitudinal analyses will allow assessment of real-life effects of OAC therapy in NVAF.
Abstract Background Real-world data comparing all available oral anticoagulants (OAC) on a nationwide scale (i.e. in France: apixaban, rivaroxaban, dabigatran and vitamin K antagonists – VKAs) are lacking. In everyday practice, oral anticoagulants are often underdosed, which may render comparisons between agents difficult. Purpose and methods NAXOS is a French real-world study comparing the safety (major bleeding), effectiveness (stroke, systemic thromboembolic events (STE)) and all-cause mortality for apixaban, dabigatran, rivaroxaban, and VKAs, in adult patients with non-valvular atrial fibrillation (NVAF) initiating a given OAC between 2014 and 2016. The French national health insurance data (SNIIRAM) were used. Analyses were performed with adjustment on propensity scores. To avoid bias potentially related to underdosing, the present analysis included only patients receiving standard doses of apixaban (5mg bid), rivaroxaban (20mg od), and dabigatran (150 mg bid), or VKAs. Only OAC naïve patients were included. Results In the OAC-naive cohorts treated with apixaban, rivaroxaban, and dabigatran, 54,575 (62.3%), 65,208 (65.2%), and 9,000 (42.4%), respectively, had the standard dose at the index dispensation, and 112,628 patients received VKAs. After adjustment on propensity scores, apixaban 5 mg was associated with a lower risk of major bleeding, compared to VKAs (Hazard Ratio: 0.47; 95% CI: 0.43–0.51) and rivaroxaban 20mg (HR: 0.64; 0.59–0.71), but not to dabigatran 150 mg (HR: 0.97; 0.79–1.18). Apixaban was associated with a lower risk of stroke and STE, compared to VKAs (HR: 0.62; 0.56–0.69) but not to rivaroxaban (HR: 1.03; 0.92–1.16), and dabigatran (HR: 0.96; 0.76–1.21). Apixaban showed a lower risk of all-cause mortality compared to VKAs (HR: 0.44; 0.41–0.47) and rivaroxaban (HR: 0.87; 0.81–0.94) but not to dabigatran (HR: 1.10; 0.92–1.32). Figure 1. Conclusions The NAXOS population-based country-wide observational study shows that 42% to 65% of patients were treated with standard doses of OACs. Analyses of standard doses confirmed the superiority of apixaban compared with VKAs for the three studied outcomes and suggests better safety profile of apixaban compared to rivaroxaban but similar to dabigatran. Background: The French Cancer Plan 2014-2019 had a target of 60% HPV vaccine coverage. The PAPILLON study investigated the annual age-specific vaccination initiation rates and cumulative partial and complete vaccination rates in France from 2017 to 2022. It also identified the factors associated with vaccination in different age groups and those associated with the type of completion of the vaccination scheme (partial vs full vaccination). Methods: For this publication, all females recorded in the French National Claims database who initiated HPV vaccination between 1 July 2007 and 31 December 2018 and were aged between 11 and 19 years at initiation were included. Annual HPV vaccination initiation rates were estimated in 11-to 14-year-old (target population) and 15-to 19-year-old females (catch-up). Cumulative vaccine coverage rates (VCRs) were estimated among those who were 15, 16, 20 and 21 years old. Partial vaccination was defined by dispensing of at least one dose of HPV vaccine by the pharmacy, while full vaccination was defined by two or three doses dispensed by a pharmacy over an 18-month period, according to current French recommendations based on the age at vaccination initiation. Results: Among the 465,629 females who initiated HPV vaccination in 2017 or 2018, the initiation rate increased from 7.7 to 11.1% in 11-to 14-year-old girls and from 4.5 to 6.5% in 15-to 19-year-old females. In 2017 and 2018, the cumulative VCRs for partial vaccination by age 15 were 28.2% and 32.8%, respectively, while by age 20, they were 41.6% and 38.8%. The cumulative VCRs for full vaccination were 15.6% and 18.6% by age 16, while they were 25.9 and 23.6% by age 20. HPV vaccination initiation and completion were strongly associated with the use of health services. Conclusion: Overall, the HPV VCR substantively increased between 2017 and 2018, which is positive evidence of the resumption of vaccination. Updates in 2022 should confirm these results. La sécurité des ß-agonistes à longue durée d’action (BALDA) reste une question dans l’asthme. Le projet européen Astrolab visait à évaluer le risque d’exacerbation de l’asthme (SAEx) chez les patients recevant des BALDA en association fixe (AF) avec des corticoïdes inhalés (CI), et chez les patients recevant des BALDA en association libre ou en monothérapie. Des patients asthmatiques persistants âgés de 6 à 40 ans ont été inclus en France et au Royaume-Uni, sur base d’une prescription d’au moins 6 mois de l’un des 4 traitements suivants : CI sans BALDA, BALCA sans CI, BALDA et CI en association libre, et BALDA et CI en AF. Les patients ont été suivis de façon prospective sur un minimum de 12 mois, avec une collecte de données sur l’exposition aux traitements et sur la survenue de SAEx (prise de corticoïdes oraux et/ou contacts médicaux non programmés pour asthme) par des interviews téléphoniques tous les 4 mois et des sms mensuels. Des modèles mixtes linéaires généralisés ont été utilisés pour évaluer le risque de SAEx au cours du suivi, selon l’exposition en fonction du temps aux BALDA seuls ou aux BALDA associés au CI en AF, après ajustement sur la sévérité de l’asthme et sur les traitements concomitants. À l’inclusion, parmi les 908 patients inclus (âge moyen = 21,8 ans ; 46,6 % de femmes ; 44 % d’enfants/adolescents ; 82 % de patients français), 59,0 % étaient sous AF, 28,9 % sous CI sans BALDA, 9,1 % sous BALDA et CI en association libre, et 3,0 % sous BALDA sans CI. Après ajustement, une augmentation non significative du risque de SAEx a été observée pour les patients exposés aux BALDA en association libre ou en monothérapie dans les 7 derniers jours (OR = 1,22 ; IC95 % = [0,73–2,03]). Le risque pour les AF était très proche de 1 (OR = 1,02 ; IC95 % = [0,78–1,33]). Le risque de SAEx était plus élevé chez les patients français que chez les patients anglais, (OR = 1,82 ; IC95 % = [1,22–2,94]), ainsi que chez les femmes (OR = 1,41 ; IC95 % = [1,01–1,80]), et les jeunes enfants (OR = 1,42 ; IC95 % = [1,07–1,88]). Une augmentation de 24 % du risque de SAEx a été observée pour les patients exposés aux BALDA en association libre ou en monothérapie (augmentation non statistiquement significative du fait des effectifs limités dans les groupes BALDA). Les données Astrolab suggèrent que, en vie réelle, les AF ont un profil de sécurité satisfaisant, alors que la sécurité des BALDA en monothérapie ou en association libre reste à vérifier sur un plus grand nombre de patients.
La sécurité des β–agonistes à longue durée d’action (BALDA) dans l’asthme reste débattue. L’objectif du projet européen (FP7) ASTROLAB est d’apporter de nouvelles connaissances sur cette question chez les adultes et les enfants atteints d’asthme, en condition réelle d’utilisation des traitements. La cohorte ASTROLAB comprend des patients (6–40 ans), recrutés en France et au Royaume-Uni, avec au moins six mois de couverture sur les 12 mois de la présélection par des prescriptions d’un des quatre types de traitements de fond suivants : corticoïdes inhalés (CI) sans BALDA, BALDA sans CI, BALDA et CI en flacons séparés (BALDA + CI) et en association fixe BALDA/CI (AF BALDA/CI). Les patients étaient suivis prospectivement pendant une durée maximale de 24 mois au moyen d’appels téléphoniques tous les quatre mois ainsi que de SMS mensuels pour détecter la survenue d’exacerbations sévères (EXA-S). Les EXA-S étaient définies par la prise de cures de corticoïdes oraux et/ou la survenue de contacts médicaux non programmés pour asthme. L’utilisation des traitements BALDA et CI, ainsi que la survenue d’EXA-S au cours du suivi étaient recueillies lors des appels programmés ou déclenchés par les SMS mensuels. Pour cette analyse, l’exposition aux traitements BALDA et CI était mesurée sur les sept jours précédents l’interview ou l’EXA-S (utilisation quotidienne ou non). Nous avons vérifié si les variations d’exposition aux BALDA déclarée par les patients durant le suivi étaient positivement corrélées à la survenue d’EXA-S (modèles multiniveaux généralisés). Sur 908 patients (âge moyen = 21,8 ans, 46,6 % de femmes), les patients sous CI sans BALDA, LABA sans CI, associations libre BALDA et CI et AF BALDA/CI étaient respectivement 28,9 %, 3,0 %, 9,1 % et 59,0 %. Un tiers des patients (29,2 %) a présenté au moins une EXA-S lors du suivi. Après ajustement sur la sévérité de l’asthme et l’exposition aux CIs, le risque d’EXA-S lié à l’utilisation des BALDA était OR = 1,2, IC95 % = (0,8–1,7). Aucune augmentation significative de risque d’EXA-S liée aux BALDA n’a été observée. Les conclusions étaient inchangées lorsque l’on se limitait aux patients sous associations fixes. L’étude ASTROLAB ne peut cependant exclure formellement un risque lié aux BALDA dans l’asthme, en raison des effectifs disponibles et de la quasi-absence de patients avec des BALDA sans CI.
