Objectives: To study delayed primary closure in ballistic and explosive device injuries by describing the clinical, therapeutic and evolutionary aspects. Methodology: This was a prospective study running from August 1, 2022 to May 31, 2023 including all patients of all ages presenting a ballistic or explosive device injury. Results: In 10 months, we collected 190 cases of ballistic and explosive device trauma, or 35.9%. The average age was 31.89 ± 14.026 years with a sex ratio of 12. Intercommunity conflicts were the most frequent causes (61.1%). Limb injuries represented 78.9 %, of which isolated wounds without fracture were the most frequent (59.5%). Standard radiography was ’the most used medical imaging (94%). DPC (Deferred Primary Closure) was performed in 71.2% followed by DPC + external fixator in 19%. We recorded 2 cases of death. Conclusion: Well-conducted delayed primary closure helps reduce the risk of complications, give the patient every chance of survival and shorten the hospital stay.
Morel-Lavallée syndrome corresponds to a serolymphatic effusion secondary to tangential trauma in relation to a richly vascularized tissue. It is a pathology little mentioned in the medical literature. We report the case of a female patient, victim of a road traffic accident which caused a Morel-lavallée lesion and who was successfully treated by sclerotherapy with corticosteroid (triamcinolone). There is currently no universally accepted treatment algorithm for the management of Morel-Lavallée lesions. Corticosteroids constitute a valid therapeutic option as a sclerosing agent in the treatment of this pathology. Conclusion: Corticosteroids constitute a valid therapeutic option as a sclerosing agent in the treatment of Morel-lavallée syndrome with satisfactory results.
Introduction: Tuberculosis is an infectious disease that can affect different organs. The spinal disorder is called tuberculous spondylodiscitis (Pott's disease). The objective: Was to elucidate the aspects taken into account by caregivers in the diagnosis of tuberculosis spondylodiscitis in patients referred to the reference health center of commune III of Bamako for anti-tuberculosis treatment. Methodology: This was a descriptive, retrospective study covering all patients referred to CSRef CIII from different health structures with the diagnosis of Pott's disease for treatment during the study period. Results: During the study period, we collected 156 patients. The male sex represented 54% with a sex ratio of 1.18. The average age was 39.3 years with extremes of 12 and 92 years. All patients presented with spinal pain, fever was present in 11.11% of patients and 2.7% had respiratory symptoms. Tuberculin IDR was positive in 95.8% and 2 patients were AFB sputum positive. X-ray (of the lumbar spine) was negative in 18%; there were images of degenerative lesions in 41.6% and only 40.2% of patients had presumptive radiological signs. CT was carried out in all patients revealing 33.33% of sequelae of fractures and degenerative lesions and 55.55% of presumptive signs of tuberculous spondylodiscitis. Only 41.66 of the cases were referred by specialist doctors. Conclusion: In our context, the diagnosis of Pott's disease seems to be based mainly on chronic spinal pain and positive IDR and is not often made by appropriate personnel.
Objectives: To study acute surgical abdomens by describing the clinical, paraclinical and therapeutic aspects. Methodology: This was a retrospective descriptive study from July 2019 to June 2021 including all patients taken in the surgical unit for acute surgical abdomens. Results: In 24 months, we collected 117 cases of acute surgical abdomens, or 19.1% of surgical interventions. The mean age was 30.6 ± 16.95 years. The sex ratio was 1.78. The reason for consultation was abdominal pain (100% of cases). The stopping of materials and gases represented 10.3% of cases. Intraperitoneal collection was the most common ultrasound sign found in 47.1% of cases. Acute appendicitis was the most frequent intraoperative diagnosis in 60.7% of cases. The surgical procedure performed was appendectomy (71.8%). The average length of hospitalization was four days. Morbidity was 6.8%. We recorded two deaths. Conclusion: Acute surgical abdomens occupy an important place in surgery due to their high frequency. The care is multidisciplinary.
Spraying water upstream of an air-cooled condenser allows to reduce the temperature of the inlet air flow and to obtain additional heat transfer gains when the water droplets impinge on the condenser outer walls. It is therefore a very effective method for improving the energetic performance of refrigeration systems. In this work, a prediction model of the wetted outer surface, well suited to finned-tubes air-cooled condensers and based on purely geometric arguments, is investigated. The wetted outer surface is expressed, in case of a spray normal to the impact area, as a function of the nozzle-condenser parameters. It is found that the condenser wetted outer surface increases with the spray angle, the spray-wall distance and the collected water flow rate. On the other hand, when the latter reaches a value of 0.009kg.s-1, the wetted area no longer increases with the collected water flow and becomes a constant at 0.07m2. It is also found that the condenser wetted outer surface is always greater with finned-tubes than with smooth tubes.
An improved method for the sizing of a stand-alone photovoltaic system in Ngoundiane site is investigated in this paper. From the numerical model, proposed in Sadio et al. (2018), the optimal combination between PV and battery capacities is found out, by taking into account the annual monthly irradiation variation and the simultaneity coefficient. So, all average monthly values of solar irradiation are considered instead of one average value and the maximal value of load demand is used and determined from energy balance and consumers behavior. After generating all PV/batteries combinations for each value of solar irradiation, the best configuration is chosen by using the TLCC. It is shown that the PV and battery capacities decrease by 25.13 and 88.39%, respectively, when compared to the intuitive method. However, the obtained LPSP, assessed to 0.3, is considered as high and is due to the lack of complete data.
Cette contribution interroge les différentes formes d’instrumentalisation de la randonnée pédestre par des organisations professionnelles. Il a été question d’identifier les modes de justifications exposés dans le discours de responsables de clubs d’entreprise de randonnée pédestre pour rendre compte des modalités d’usage de cette activité sportive en milieu professionnel au Sénégal. Grace à l’observation participante et à une trentaine d’entretiens individuels semi directifs menés auprès des acteurs de la randonnée pédestre au Sénégal (employés participants, responsables d’entreprise, organisateurs), les différentes catégories d’organismes professionnels (entreprises commerciales, administrations publiques, organisations internationales, représentations diplomatiques…) qui ont recouru à cette activité physique et sportive ont été identifiés. Aussi, divers enjeux (sanitaire, managériale, communicationnel…) ont motivé l’usage de la randonnée pédestre par ces organisations dont certaines espèrent améliorer la productivité leurs travailleurs. This contribution questions the different forms of instrumentalization of hiking by professional organizations. It was a question of identifying the modes of justifications exposed in the speeches of managers of corporate hiking clubs to account for the modalities of use of this sporting activity in a professional environment in Senegal. Thanks to the participant observation and about thirty semi-structured individual interviews conducted with the actors of hiking in Senegal (participating employees, company managers, organizers), the different categories of professional organizations (commercial companies, administrations organizations, international organizations, diplomatic representations, etc.) who have used this physical and sporting activity have been identified. Also, various issues (health, managerial, communicational...) have motivated the use of hiking by these organizations, some of which hope to improve the productivity of their workers.
Heart disease causes millions of deaths worldwide. Many approaches have been proposed for the prediction of heart disease. Several machine learning, deep learning, and data mining algorithms are used in the detection and diagnosis of heart disease based on parameters or risk factors. The most used algorithms are Naïve Bayes, Machine Vector Support, decision tree, KNNs, and artificial neural networks. The most frequently used parameters or risk factors are the 14 attributes of the UCI Cleveland standard. In this article, a study on these different approaches is carried out. This study shows diversity in relation to the choices and the use of different attributes in the prediction of cardiovascular diseases.
At the level of the National Statistical Institutes (NSI), there is a large volume of data which is generally in a format which conditions the method of publication of the information they contain. Data collection projects for households or businesses are increasingly integrating into their implementation phase, a platform for results dissemination. If not, it is the NSI Web portals that are often used for dissemination. Thus, these dissemination methods previously used, and do not promote rapid access to information and especially does not offer the option of being able to link data for in-depth processing. In this paper, we present an approach to modeling these data to publish them in a format intended for the Semantic Web. Our objective is to be able to publish all this data in a single platform and offer the option to link with other external data sources. An application of the approach will be made on data from major national surveys such as the one on employment, poverty, child labor, and the general census of the population of Senegal.
In previous works, we have proposed the use of a megamodel which is a language-based system for the maintenance of complex systems through a modeling perspective. We have called such a language LAMEME (a LAnguage for the Management and Evolution of MEgamodels and its Semantics). It is about a Domain Specific Language (DSL) which exclusively makes use of Higher-Order Functions (HOF) called Global Operation Models (GOMs) to update the megamodel content by adding or removing Component Models (CMs). The LAMEME semantics is given by specifying requires/ensures predicates that are checked at runtime. The paper presents a case study aiming to highlight how LAMEME can be used to describe a simple evolution of a complex software system such as Apache Tomcat.
Introduction: The shoulder is an exceptional location for osteoarticular tuberculosis particularly in children. We are reporting two observations of male children's shoulder tuberculosis. Observation 1 :E.M.D., aged 14, was examined on June 28, 2001 for a three-week shoulder suppuration which was related to a trauma caused by a fall. The clinical examination revealed a 37.4° C temperature, a pain associated with a deformation of the shoulder and pus producing axillary fistula. The standard radiography showed lesions of osteoarthritis of the shoulder. The bacteriological examination of the pus was negative. Iterative arthrotomies and an antibiotherapy did not result in a healing. Intradermal tuberculin test was highlly positive. The shoulder's biopsy showed caseous necrosis and an epithelio-gigantocellular follicle. The antituberculosis treatment, which just started 21 months after the beginning of the symptoms made the healing possible but at the cost of functional after-effects. Observation 2 :I. D, aged 10, was examined on April 7, 2005 for a painful five-month tumefaction of his right shoulder and left hand related to a trauma resulting from a fall. The clinical examination showed a 37.8 ° C febricule, an abscess on the left hand, a pain and tumefaction on the left shoulder the punction of which drained pus out. The bacteriological examination of these different samples did not isolate any germ. The standard radiography showed osteoarthritic lesions on the left shoulder and lesions on the third left metacarpal. Repeated arthrotomies, a fattening of the hand abscess and a non specific antibiotherapy did not improve the symptoms. Intradermal tuberculin test was highly positive. The biopsy of the hand and shoulder showed caseous necrosis and an epithelio-gigantocellular follicle. The antituberculosis treatment which had been started six months after the first consultation did not cure him. He died of meningo-encephalitis. Conclusion: The diagnosis of shoulder tuberculosis among children was always late. One has to be able to identify it in front of a chronic suppuration of a shoulder which does not heal despite the non specific antibiotic treatment and vainly repeated arthrotomies.
Model Driven Engineering (MDE) provides the concept of a runtime megamodel to represent the dynamic structure of a given system, to which it is causally connected. A system changes at runtime therefore implies frequent and dynamic changes of its related megamodel. In a previous work we have proposed to automate change management through a runtime megamodel evolution management approach. In such an approach, a megamodel manipulation, a kind of programming in-the-large activity, is considered as a mega-program which is modified throughout Global Operation Models (GOMs). Then we proposed a safe execution of GOMs as the solution for megamodel consistency preserving during evolution. In this work, we propose LAMEME, a domain-specific language for the management and the evolution of megamodels, and its axiomatic semantics. LAMEME gives the possibility to express an evolving megamodel as a mega-program and therefore defines a framework that supports our previously proposed approach.
In previous research [1] we proposed a causal model of performance measurement system by combining quantitative and qualitative models (CMPMS) that enhances the richness and validity of the analysis results then answer in several question (Who?, What?, Why ?, Or? How? Etc.). So for the validation of the CMPMS we opted for a case study of a hospital information system (HIS). In this phase of in-depth analysis and design, quantitative models will be proposed and qualitative analyzes will be carried out to conduct verification tests and practical implementation of the model for future work.
BACKGROUND:Acute intestinal intussusception (AII) is defined as the telescoping of part of the intestine into the intestinal segment beneath it. The consequence is an obstruction and strangulation which can lead to necrosis of the intestinal tract that has telescoped. The aim of our work is, on the one hand, to assess our management style throughout analysis of clinical different aspects and on the other hand to set up a strategy for early diagnosis and treatment.METHODS:It is a retrospective study over 5 years from 2010 to 2015 including infants and children who had been treated for AII in Albert Royer Children's Hospital. The following variables such as age, sex, past time before the surgical consultation, initial diagnosis, imaging data view, treatment technique and results were itemised to bear comparison. Sixty-six records were reviewed.RESULTS:The mean age was 5.73 months with a range of 2 and 144 months. Boys constituted the majority of patients with a ratio of 2:1. Children were referred to us with the diagnosis of intestinal obstruction or gastroenteritis in 50% of cases. Past time average before consultation was 5.3 days with a range of 14 h and 30 days. Most of the patients underwent surgery within 48 h after the beginning of the clinical picture. An abdominal ultrasound scan confirmed the diagnosis of AII in 57 cases, whereas in 9 cases, it was normal or not contributive. Non-operative reduction by retrograde pneumatic pressure or barium enema represented 33.4% of patients against 56.6% for surgical treatment. We registered 5 deaths and 2 recurrences. AII remains the main cause of intestinal obstruction in infants.CONCLUSION:Authors stressed on delay in diagnosis with the treatment consequences belonging to that status and put emphasis in the importance of close collaboration between paediatricians, surgeons and radiologist for early diagnosis and nonoperative systematic reduction.
This paper addresses the problem of ensuring consistency, correctness and other properties in dynamically changing software systems. The approach uses a Megamodel that represents the current state of the system at runtime including some rules. These rules are formulated as Hoare-Triples and allow to check whether modifications to the software system result in a consistent state, otherwise to fix changes that are likely to violate the megamodel integrity.
Recent research often suggests ideas about quantitative or qualitative causal models of performance measurement systems. We also rely on some works that develop ideas on causal models of SMP. This research has highlighted two approaches in the study of causal models of performance measurement systems: the quantitative and qualitative approach. Indeed, the qualitative models lack precision and the qualitative models are confronted with problems of data collection in hierarchical level deployment. Therefore, it should be noted that the combination of these two methods is very rare or non-existent for the SMPs. Hence the idea of proposing a model that combines the two, because these approach also have certain limits. According to our studies we note a complementarity because to combine these two methods reinforces the richness and the validity of the results.
We report the study of the temperature dependance of the performance electronic parameters of an N-P solar cell by considering as model, the columnar cylindrical orientation associated to the dynamic junction velocity (SF) concept. We presented the photocurrent-photovoltage (I-V) and Power-photovoltage (P-V) characteristic curves. The short-circuit photocurrent (Isc), the open circuit photovoltage (Uoc), the fill factor (FF) and the efficiency (η) are linearly dependent on the temperature. The temperature coefficients (T-coefficient) relative to the short-circuit, open-circuit photovoltage and efficiency are calculated and the comparison with data from the literature showed the accuracy of the considered model.
Software architectures are often made to evolve due to changing user needs and/or to execution environment. The paper addresses the problem of ensuring consistency in dynamically changing software systems. The proposed approach uses a runtime megamodel which represents the architectural model of the running system and is used as a basis for model management and for changes representation. For this purpose, it should then be considered as an execution environment or more simply as a (mega-) program and thus should have to exhibit some self-∗ properties such as self-management and self-healing. And for that, we use inference rules for reasoning about, specifying, and representing change operations. Thereby preserving the megamodel's structural consistency through a logic-based approach, by defining a safe execution and an execution semantic for each operation likely to modify the megamodel contents. An initial implementation of a DSL suite for validating the approach, called MOME, is briefly presented.
In their mission of collecting, analyzing and disseminating statistic data, the National Institutes of Statistics are more and more confronted with personal data management. With the advent of the “Open Data” [1], the need for data diffusion and to establish a link between databases created during the surveys and censuses, are necessary for planning, monitoring and activities evaluation, in a context marked by a will of the official authorities to accelerate the economic growth [2]. For that matter, this article describes a methodology to anonymize database collected from households or companies, limiting the risk of information disclosure while preserving as much as possible the quality of the data resulting from the anonymization process.
